Abstract:Diabetes mellitus is now fast emerging as one of the biggest health catastrophes the world has ever witnessed. It has huge global and societal implications, particularly in developing countries such as China and India [1]. China is now bring with it potential massive increase in type 1 diabetes (2-5% per annum increases in incidence in the world's most populous countries) and childhood obesity (with its associated insulin resistance and type 2 diabetes)[2]. A nine-year prospective study on the incidence of childhood type 1 diabetes mellitus in China by the WHO DiaMond Project China Participating Center and Chinese Academy of Preventive Medicine (CAPM) showed that between 1988 and 1996, the overall incidence rate (IR) was 0.59 per 100,000 person-year. The IR was 0.52/100,000 (95% CI: 0.50-0.54) for males and 0.66/100,000 (95% CI: 0.64-0.68) for females[3]. We recently conducted a nationwide study to evaluate the state and the trend of diabetes based on hospital inpatient data from China’s 14 medical centers and pre-diabetes among obese children from October 1995 through September 2010. We found that in the past 15 years, the prevalence of Chinese childhood diabetes increased dramatically and the growth of T2DM has exceeded T1DM. T1DM has occurrence rate of 89.6% of all diabetes and is still the dominant form of diabetes in children. The prevalence of T1DM was relatively stable from the year of 1995 to 2005, but increased obviously in the recent 5 years according to the hospital records in China. The clear increasing trend from Southwest to East and North disclosed strong regional differences (T1DM from 59.76 to 80.02 and 120.45, T2DM from 2.52 to 3.77 and 15.64 (1/100,000) (p all < 0.0001). Well developed areas had a higher prevalence compared to less developed areas {T1DM: 151.51 vs. 32.2; T2DM: 15.16 vs. 1.64 and other types: 7.54 vs. 0.42 (1/100,000)}. An important finding in this study is that the prevalence of childhood T2DM in China doubled from 4.1/100,000 in the first 5 years to 10.0/100,000 in the recent 5 years, which was 7.44 % of total diabetics. Though the ratio is still lower than that of America (8%-46%)[4] , the trend is clear and the consequences are serious because China has the largest population in the world. Another important finding in this study showed obese children are potential pools of T2DM. Of the 3153 obese children, 18.24% had IFG alone, 5.99% had IGT, 4% had combined IFG and IGT.

Abstract:Background The worldwide geographical and temporal variation in the prevalence of diabetes represents a challenge, but also an opportunity for gaining etiological insights. Encompassing the bulk of East Asians, a large and distinct proportion of the world population, China can be a source of valuable epidemiological insights for diabetes, especially in early life, when pathophysiology begins. We carried out a nationwide, epidemiological survey of Prevalence and Risk of Obesity and Diabetes in Youth (PRODY) in China, from 2017 to 2019, to estimate the population-based prevalence of diagnosed pediatric diabetes and screen for undiagnosed pediatric type 2 diabetes (T2D). Methods PRODY was a nation-wide, school population-based, cross-sectional, multicenter survey by questionnaire, fasting urine glucose test and simple oral glucose tolerance test (s-OGTT), among a total number of 193,801 general-population children and adolescents (covered a pediatric population of more than 96.8 million), aged 3-18, - 18, from twelve provinces across China. The prevalence of the self-reported pediatric diabetes, the proportion of subtypes, the crude prevalence of undiagnosed T2D and prediabetes in general juvenile population and the main risk factors of type 1 (T1D) and type 2 (T2D) diabetes had been analyzed in the study. Findings The prevalence of all self-reported pediatric diabetes was estimated at 0.62/1000 (95% CI: 0.51-0.74), - 0.74), with T1D at 0.44/1000 (95% CI: 0.35-0.54) - 0.54) and T2D at 0.18/1000 (95% CI: 0.13-0.25). - 0.25). For undiagnosed T2D, the crude prevalence was almost ten-fold higher, at 1.59/1000, with an estimated extra 28.45/1000 of undiagnosed impaired glucose tolerance (IGT) and 53.74/1000 of undiagnosed impaired fasting glucose (IFG) by s-OGTT screening. Maternal diabetes history is the major risk factors for all subtypes of pediatric diabetes in China. Interpretation The PRODY study provides the fi rst population-based estimate of the prevalence of pediatric diabetes China and reveals a magnitude of the problem of undiagnosed pediatric T2D. We propose a practical screening strategy by s-OGTT to address this serious gap.

Abstract:China is gradually taking its place as one of the world’s economic giants and concurrently learning to understand how to bear the burdens of diseases that are more common in the fully developed world, such as pediatric obesity, metabolic syndrome, and type 2 diabetes mellitus. The purpose of this review is to consolidate the available information regarding these and draw the focus toward their sequential progression and increasing prevalence in Chinese children. Studies were collected in both English and Chinese, and the data were reviewed on the basis of disease prevalence and risk factors that are known from scientific literature that has been published to date. The majority of studies with appropriate content for inclusion here have been conducted within the last 15 years and up to date information from recent local and international research has also been included. Several factors have been implicated for the rise in obesity, most notably, the progressing economic expansion and exposure of local Chinese populations to Western influences. With this, metabolic syndrome has become a growing concern, as it is a precursor to cardiovascular disease and type 2 diabetes, leading to the alarmingly rapid development of deleterious consequences in children. The International Diabetes Federation proposed a definition for metabolic syndrome in 2007 (MS-IDF2007) worldwide, but whether it is also suitable for the Chinese population remains uncertain, so we have created the Chinese definition of metabolic syndrome upon the IDF framework. This MS-CHN2012 definition is based on multicenter studies to simplify and standardize primary care screening methods and is the first of its kind in China. Juvenile type 2 diabetes is the most worrisome result of obesity and metabolic syndrome, and studies have shown that the prevalence has doubled within 5 years—surpassing the prevalence of juvenile type 1 diabetes. Because of the extremely low number of studies currently published on these topics in China, emphasis needs to be placed on the assessment of the health status of the population. Screening methods are imperative because lifestyle interventions can reduce and even reverse the pathologic consequences of this disease, if detected early.

Abstract:BACKGROUND:Childhood diabetes has become a growing concern. We conducted a study to evaluate the status and trend of diabetes from 14 medical centers in China. Pre-diabetic status among obese children was also noted.METHODS:Hospital medical records were reviewed, and data of diabetes were collected from 1995 through 2010. We took every five years as a calculation unit to analyze the trend of new-onset diabetes. Data on obesity were collected in the recent five years.RESULTS:A total of 4 337 836 patients aged 0-18 years were discharged from the 14 centers. The prevalence (per 100 000 persons) of new-onset type 1 diabetes, type 2 diabetes and other types of diabetes were 96.8, 8.0, and 3.3, respectively. The prevalence of type 1 diabetes increased from 90.9 to 92.9 and 101.4, while type 2 diabetes increased from 4.1 to 7.1 and 10.0 in every five years (P<0.0001). The increasing trend was significant from Southwest to East and North China (type 1 diabetes from 59.76 to 80.02 and 120.45, type 2 diabetes from 2.52 to 3.77 and 15.64 (per 100 000 persons) (all P<0.0001). Well developed areas in China had a higher prevalence compared to less developed areas [type 1 diabetes: 151.51 vs. 32.2 (per 100 000 persons); type 2 diabetes: 15.16 vs. 1.64 and others: 7.54 vs. 0.42 (per 100 000 persons)]. Of the 3153 obese children, 18.24% had impaired fasting glucose (IFG), 5.99% had impaired gulose tolerance (IGT), and 4% had combined IFG and IGT.CONCLUSIONS:The prevalence of childhood diabetes in China has increased dramatically, with type 2 diabetes exceeding type 1 diabetes. The incidence rate of abnormal glucose metabolism in obese children has reached 28.26%.

Abstract:OBJECTIVE-Our objective was to determine the secular trend in prevalence of type 2 diabetes in Shanghai, China.RESEARCH DESIGN AND METHODS-Two consecutive population-based surveys for type 2 diabetes were conducted in randomly selected adults aged 35-74 years in Shanghai in 2002-2003 (n = 12,329) and in 2009 (n = 7,423). Diagnosed type 2 diabetes was determined based on self-report, whereas those undiagnosed were identified by measured fasting and post-load glucose according to 2009 American Diabetes Association criteria.RESULTS-Age-standardized prevalence of diagnosed and undiagnosed type 2 diabetes increased from 5.1 and 4.6% in 2002-2003 to 7.4 and 5.2% in 2009. The prevalence of type 2 diabetes increased with age and was higher among men and in urban residents in both surveys (P < 0.001). Between the two surveys, the increase in the prevalence was more evident in the rural population (P < 0.001) and appeared more rapid in younger birth cohorts (P < 0.05).CONCLUSIONS-Our results suggest that Shanghai has experienced an increasing burden of type 2 diabetes.

Abstract:Background: To estimate secular trends and disease burden of diabetes and prediabetes among Chinese adults. Methods: Three population-based surveys were performed among Chinese adults in Shanghai in 2002-2003 (n = 12 302), 2009 (n = 7414), and 2017 ( n = 18 960). Diabetes and prediabetes were defined using the 1999 World Health Organization ( WHO) criteria. Cochran-Armitage trend test was used to examine the trends in prevalence, awareness, and glycemic control status. Disability adjusted life years (DALYs) were estimated to evaluate the disease burden of diabetes- related complications using the population attribution fraction approach based on published data. Results: The age-adjusted prevalence of diabetes increased during the 15-year period (p for trend <.001) and reached 23.0% (95% CI: 22.1 similar to 24.0%) in men and 15.7% ( 95% CI: 15.1 similar to 16.4%) among women in 2017. The prevalence of impaired glucose tolerance peaked in 2009, whereas that of impaired fasting glucose increased continuously (p for trend <.001). The awareness of diabetes was found to increase and the glycemic control rates decreased over the three surveys. The estimated DALYs of diabetes complications were found to have increased rapidly due to the increasing prevalence of diabetes and the decreasing glycemic control rates. Conclusions: Prediabetes and diabetes affect a considerable proportion of Chinese adults in Shanghai. Our results highlight the necessary to strengthen the community healthcare system in China to guarantee extensive management of diabetes and prediabetes.
Changes in the prevalence of diabetes and control of risk factors for diabetes among Chinese adults from 2007 to 2017: An analysis of repeated national cross-sectional surveys

Abstract:Introduction: To examine changes in the prevalence of diabetes and the control of risk factors for diabetes over 10 years among adults in China. Methods: Two population-based cross-sectional surveys were used to obtain a nationally representative sample of adults aged 20 years and older in mainland China in 2007 (n = 46 239) and 2017 (n = 73 340). Changes in the prevalence of diabetes, impaired fasting glucose, impaired glucose tolerance, and prediabetes, as diagnosed by the World Health Organization criteria, were assessed over time. Results: The weighted prevalence of diagnosed diabetes (3.8% vs 6.3%, p = .0001) and total diabetes (9.7% vs 11.7%, p = .005) increased among the overall population between 2007 and 2017. The weighted prevalence of undiagnosed diabetes (5.9% vs 5.4%, p = .7), impaired fasting glucose (2.7% vs 2.6%, p = .68), impaired glucose tolerance (12.7% vs 12.5%, p = .95), prediabetes (15.4% vs 15.1%, p = .79), the treatment of diabetes (34.1% vs 32.5%, p = .44), and the control of diabetes (31.1% vs 32.8%, p = .73) did not significantly change over this period. The awareness of diabetes (39.4% vs 53.6%, p = .0004) increased over 10 years among the overall population. The proportion of achieved high-density lipoprotein cholesterol targets increased (p = .005), but the proportion of achieved body mass index (p = .01) and waist circumference (p = .0002) targets decreased significantly. Conclusions: Between 2007 and 2017, the prevalence of total diabetes (diagnosed by the World Health Organization criteria), especially diagnosed diabetes, increased among adults in China. Although awareness of diabetes improved, effective interventions and clinical strategies are urgently required.

Abstract:Objective To evaluate the prevalence and trend of diabetes mellitus among Chinese adults during the past thirty years.Methods Papers,published before October 1,2017 and related to the prevalence of diabetes mellitus among Chinese adults,were searched through PubMed,China Knowledge Resource Integrated Database,Wanfang Digital Database and VIP Citation Databases.Stata 13.0 software was used to estimate the prevalence of diabetes mellitus,with pooled prevalence calculated based on random effects.Subgroup analysis was conducted based on time,sex,areas and body mass index groups of investigation.Continuous fractional polynomial regression model on the midpoint of each survey period,weighted by the number of participants in each study,was used to estimate and illustrate the trends of prevalence of diabetes over the years.Results In total,15 studies were included and two of them were excluded in the primary analysis with the age limitation of participants as ≥40 years old,for recruitment.The average prevalence of diabetes among Chinese adults was 6.3％ (95％CI:4.6％-8.0％),during the past thirty years.The pooled prevalence appeared higher in urban than in rural areas and higher in men than in women.Between 1980 and 2013,the increase of Chinese diabetes prevalence did not follow the linear trend.Before 2000,the average prevalence showed as 3.5％ (95％CI:2.0％-4.9％),with an annual increase rate as 0.17％.Since 2000,the average annual prevalence of diabetes mellitus had appeared around 8.0％ (95％CI:6.0％-10.1％),with an annual growth rate of 0.72％ (95％CI:0.34％-1.10％).Conclusion The prevalence of diabetes in Chinese adults had been rapidly increasing since the year 2000,indicating that efforts should be strengthened for diabetes prevention,in China.

Abstract:Aims To systematically review trends in diabetes mellitus (DM) prevalence in adults in China over the last 10 years and to identify the determinants of these trends. Methods A systematic search was conducted for studies published between 2000 and 2010. Studies reporting DM prevalence were included if they met the pre-determined criteria. The prevalence estimates and reported determinants of these studies were compared. Results Twenty-five manuscripts, reporting on 22 studies, were selected for inclusion in the review. There has been an increase in DM prevalence from 2.6% to 9.7% in China over the past decade. DM prevalence is strongly associated with age and is higher in urban residents compared with rural populations. Some studies found a difference in DM prevalence between males and females, but this finding was not consistent. Other commonly reported associations with DM included family history, obesity and hypertension. Conclusion Over the period of 2000–2010, we identify a significant increase in DM prevalence at the national level. It is important for all levels of government to develop more effective strategies to prevent and manage this rising diabetes epidemic. There is also an important need for more large-scale studies of diabetes in the western and central regions of China.

Abstract:Background: The incidence of type 1 diabetes mellitus (T1DM) is increasing rapidly worldwide for children of every age group. Materials and Methods: A retrospective analysis of hospital registration data from five tertiary hospitals in Beijing, China, identified 485 patients (210 boys, 275 girls) younger than 15 years of age with newly diagnosed T1DM between 1995 and 2010. Results: From 1995 to 2010, the incidence of T1DM in patients <15 years of age ranged from 0.935-3.26 per 100,000 per year. During this period, the average age-standardized incidence rate was 1.7 per 100,000 per year, and the average annual increase in T1DM incidence was 4.36%. The highest rate increases occurred in boys and in patients <5 years of age. For both genders and all age groups, T1DM incidence increased at an accelerated rate after 2006. By 2020, the incidence of T1DM among children in Beijing is projected to increase by a factor of 1.69. Conclusions: The incidence of T1DM among patients younger than 15 years of age in Beijing increased from 1995 to 2010, with rates growing at an accelerated pace since 2006. Based on recent trends, we project continued rapid growth in the number of new childhood T1DM cases in Beijing.

Abstract:Abstract Background Studies of type 1 diabetes incidence in China have conflicting findings, and the epidemiology of type 1 diabetes in China is unclear. We aimed to assess the incidence and epidemiology of type 1 diabetes in relation to age, sex, and geographical area in mainland China. Methods In this systematic review and meta-analysis, we searched Pubmed (Jan 1, 1980, to May 18, 2016), Embase (Jan 1, 1980 to May 18, 2016), China National Knowledge Infrastructure (Jan 1, 1999, to May 20, 2016), and Wanfang Medical Database (Jan 1, 1998, to May 19, 2016) to identify population-based studies of type 1 diabetes incidence in mainland China. Only population-based studies with data on the incidence of type 1 diabetes in mainland China were included, and studies with overlapping data from included studies were excluded. We extracted data from the published reports and used random-effects meta-analysis to pool the incidence and incidence rate ratios (IRRs) with 95% CIs. We used the Q statistic and I 2 method to assess heterogeneity, and also did subgroup analyses by sex, age, and geographical area. Additionally, we used time trends to explore the difference in incidence over time. This study is registered with PROSPERO, number CRD42016039284. Findings We identified 816 reports from the search and included 19 reports of population-based studies published between 1994 and 2015 in the analysis. Overall incidence of type 1 diabetes in mainland China was 0·74 (95% CI 0·55–1·00) per 100 000 person-years between 1980 and 2013. Abstract:BACKGROUND:Because of the rapid change in lifestyle in China, there is concern that diabetes may become epidemic. We conducted a national study from June 2007 through May 2008 to estimate the prevalence of diabetes among Chinese adults. METHODS:A nationally representative sample of 46,239 adults, 20 years of age or older, from 14 provinces and municipalities participated in the study. After an overnight fast, participants underwent an oral glucose-tolerance test, and fasting and 2-hour glucose levels were measured to identify undiagnosed diabetes and prediabetes (i.e., impaired fasting glucose or impaired glucose tolerance). Previously diagnosed diabetes was determined on the basis of self-report. RESULTS:The age-standardized prevalences of total diabetes (which included both previously diagnosed diabetes and previously undiagnosed diabetes) and prediabetes were 9.7% (10.6% among men and 8.8% among women) and 15.5% (16.1% among men and 14.9% among women), respectively, accounting for 92.4 million adults with diabetes (50.2 million men and 42.2 million women) and 148.2 million adults with prediabetes (76.1 million men and 72.1 million women). The prevalence of diabetes increased with increasing age (3.2%, 11.5%, and 20.4% among persons who were 20 to 39, 40 to 59, and > or = 60 years of age, respectively) and with increasing weight (4.5%, 7.6%, 12.8%, and 18.5% among persons with a body-mass index [the weight in kilograms divided by the square of the height in meters] of < 18.5, 18.5 to 24.9, 25.0 to 29.9, and > or = 30.0, respectively). The prevalence of diabetes was higher among urban residents than among rural residents (11.4% vs. 8.2%). The prevalence of isolated impaired glucose tolerance was higher than that of isolated impaired fasting glucose (11.0% vs. 3.2% among men and 10.9% vs. 2.2% among women). CONCLUSIONS:These results indicate that diabetes has become a major public health problem in China and that strategies aimed at the prevention and treatment of diabetes are needed.
Sex Differences in the Association Between Socioeconomic Status and Diabetes Prevalence and Incidence in China: Cross-Sectional and Prospective Studies of 0.5 Million Adults

Abstract:China has undergone rapid socioeconomic transition accompanied by lifestyle changes that are expected to have a profound impact on the health of its population. However, there is limited evidence from large nationwide studies about the relevance of socioeconomic status (SES) to risk of diabetes. We describe the associations of two key measures of SES with prevalent and incident diabetes in Chinese men and women. The China Kadoorie Biobank study included 0.5 million adults aged 30–79 years recruited from ten diverse areas in China during 2004–2008. SES was assessed using the highest educational level attained and annual household income. Prevalent diabetes was identified from self-report and plasma glucose measurements. Incident diabetes was identified from linkage to disease and death registries and national health insurance claim databases. We estimated adjusted ORs and HRs for prevalent and incident diabetes associated with SES using logistic and Cox regression models, respectively. At baseline, 30,066 (5.9%) participants had previously diagnosed (3.1%) or screen-detected (2.8%) diabetes among 510,219 participants included for cross-sectional analyses. There were 480,153 people without prevalent diabetes at baseline, of whom 9544 (2.0%) had new-onset diabetes during follow-up (median 7 years). Adjusted ORs (95% CIs) for prevalent diabetes, comparing highest vs lowest educational level, were 1.21 (1.09, 1.35) in men and 0.69 (0.63, 0.76) in women; for incident diabetes, the corresponding HRs were 1.27 (1.07, 1.51) and 0.80 (0.67, 0.95), respectively. For household income, the adjusted ORs for prevalent diabetes, comparing highest vs lowest categories, were 1.45 (1.34, 1.56) in men and 1.26 (1.19, 1.34) in women; for incident diabetes, the HRs were 1.36 (1.19, 1.55) and 1.06 (0.95, 1.17), respectively. Among Chinese adults, the associations between education and diabetes prevalence and incidence differed qualitatively between men and women, whereas higher household income was positively associated with diabetes prevalence and incidence in both sexes, with a stronger relationship in men than in women.

Abstract:Objective: To review trends in the prevalence and incidence of diabetes mellitus (DM) and related risk factors in China.Methods: We searched the literature using PubMed, China Knowledge Resource Integrated Database, and China Wanfang Digital Database for large epidemiologic studies and national surveys.Results: During the past 30 years (1980-2010), 7 national diabetes mellitus surveys were conducted in China mainland, indicating that the prevalence of DM has increased 17-fold, from 0.67 to 11.6% of the population. The prevalence of impaired glucose regulation (IGR, including impaired fasting glucose and impaired glucose tolerance) also increased, from 2.09 in 1994 to 27.2% in 2010. There was no national representative study of the incidence of diabetes to date; the reported incidence of type 2 diabetes during past 25 years in several cohort studies varied (2.7 to 15.8 per 1,000 person-years). Potential risk factors which could have contributed to the increasing prevalence and incidence of DM and IGR in the Chinese population include social and economic development, urbanization, dietary pattern, and Westernized lifestyle. Further, genetic studies have suggested that unique inheritable risk factors in the Chinese population may increase the risk for DM when compared to Caucasians.Conclusion: DM and IGR have become epidemic in China. Public health strategies should focus on modifying lifestyle and dietary factors, particularly among those with a susceptible genetic background.

Abstract:Abstract Objective To assess the prevalence of diabetes and its risk factors. Design Population based, cross sectional study. Setting 31 provinces in mainland China with nationally representative cross sectional data from 2015 to 2017. Participants 75 880 participants aged 18 and older—a nationally representative sample of the mainland Chinese population. Main outcome measures Prevalence of diabetes among adults living in China, and the prevalence by sex, regions, and ethnic groups, estimated by the 2018 American Diabetes Association (ADA) and the World Health Organization diagnostic criteria. Demographic characteristics, lifestyle, and history of disease were recorded by participants on a questionnaire. Anthropometric and clinical assessments were made of serum concentrations of fasting plasma glucose (one measurement), two hour plasma glucose, and glycated haemoglobin (HbA 1c ). Results The weighted prevalence of total diabetes (n=9772), self-reported diabetes (n=4464), newly diagnosed diabetes (n=5308), and prediabetes (n=27 230) diagnosed by the ADA criteria were 12.8% (95% confidence interval 12.0% to 13.6%), 6.0% (5.4% to 6.7%), 6.8% (6.1% to 7.4%), and 35.2% (33.5% to 37.0%), respectively, among adults living in China. The weighted prevalence of total diabetes was higher among adults aged 50 and older and among men. The prevalence of total diabetes in 31 provinces ranged from 6.2% in Guizhou to 19.9% in Inner Mongolia. Han ethnicity had the highest prevalence of diabetes (12.8%) and Hui ethnicity had the lowest (6.3%) among five investigated ethnicities. The weighted prevalence of total diabetes (n=8385) using the WHO criteria was 11.2% (95% confidence interval 10.5% to 11.9%). Conclusion The prevalence of diabetes has increased slightly from 2007 to 2017 among adults living in China. The findings indicate that diabetes is an important public health problem in China.

Abstract:IMPORTANCE Previous studies have shown increasing prevalence of diabetes in China, which now has the world's largest diabetes epidemic.OBJECTIVES To estimate the recent prevalence and to investigate the ethnic variation of diabetes and prediabetes in the Chinese adult population.DESIGN, SETTING, AND PARTICIPANTS A nationally representative cross-sectional survey in 2013 in mainland China, which consisted of 170 287 participants.EXPOSURES Fasting plasma glucose and hemoglobin A(1c) levels were measured for all participants. A 2-hour oral glucose tolerance test was conducted for all participants without diagnosed diabetes.MAIN OUTCOMES AND MEASURES Primary outcomes were total diabetes and prediabetes defined according to the 2010 American Diabetes Association criteria. Awareness and treatment were also evaluated. Hemoglobin A(1c) concentration of less than 7.0% among treated diabetes patients was considered adequate glycemic control. Minority ethnic groups in China with at least 1000 participants (Tibetan, Zhuang, Manchu, Uyghur, and Muslim) were compared with Han participants.RESULTS Among the Chinese adult population, the estimated standardized prevalence of total diagnosed and undiagnosed diabetes was 10.9%(95% CI, 10.4%-11.5%); that of diagnosed diabetes, 4.0% (95% CI, 3.6%-4.3%); and that of prediabetes, 35.7%(95% CI, 34.1%-37.4%). Among persons with diabetes, 36.5%(95% CI, 34.3%-38.6%) were aware of their diagnosis and 32.2%(95% CI, 30.1%-34.2%) were treated; 49.2%(95% CI, 46.9%-51.5%) of patients treated had adequate glycemic control. Tibetan and Muslim Chinese had significantly lower crude prevalence of diabetes than Han participants (14.7% [95% CI, 14.6%-14.9%] for Han, 4.3%[95% CI, 3.5%-5.0%] for Tibetan, and 10.6%[95% CI, 9.3%-11.9%] for Muslim; P<.001 for Tibetan and Muslim compared with Han). In the multivariable logistic models, the adjusted odds ratios compared with Han participants were 0.42 (95% CI, 0.35-0.50) for diabetes and 0.77 (95% CI, 0.71-0.84) for prediabetes for Tibetan Chinese and 0.73 (95% CI, 0.63-0.85) for diabetes and 0.78 (95% CI, 0.71-0.86) for prediabetes in Muslim Chinese.CONCLUSIONS AND RELEVANCE Among adults in China, the estimated overall prevalence of diabetes was 10.9%, and that for prediabetes was 35.7%. Differences from previous estimates for 2010 may be due to an alternate method of measuring hemoglobin A(1c).

Abstract:Objective To evaluate the changes in body mass index (BMI) and waist circumference (WC) and their associations with the prevalence of hypertension and type 2 diabetes mellitus (T2DM) in Chinese adults.Design 2 consecutive population-based cross-sectional surveys.Setting A total of 12 districts and seven counties in Shanghai, China.Participants 12329 randomly selected participants of the survey in 2002-2003, and 7423 randomly selected participants of the survey in 2009. All participants were residents of Shanghai aged 35-74years.Outcome measures Measured BMI and WC. Previously diagnosed and newly identified hypertension and T2DM by measured blood pressure, fasting and postload glucose.Results While the participants of the two surveys were comparable in BMI in each age group, the participants of the 2009 survey had significantly larger WC than those of the 2002-2003 survey, with an annual percentage change being higher among participants aged 45-49years in men and women. The increase in prevalence of T2DM was observed in all age groups and also appeared more evident in participants aged 45-49years. The prevalence of hypertension was observed to increase more rapidly in elderly men and middle-aged women. Obesity, both overt and central, was associated with the risk of the two diseases, but BMI was more strongly linked to hypertension while WC appeared more evidently related with T2DM.Conclusions The prevalence of central obesity and related chronic diseases has been increasing in Shanghai, China. Our findings provide useful information for the projection of the growing burden of T2DM and hypertension in Chinese adults.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aim</h3><p>This study aims to evaluate the trends and geographic variations of incident diabetes as well as the corresponding sex differences in China.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>The open cohort study derived data of 16,610 individuals from the China Health and Nutrition Survey 1997 ‐ 2015. Direct standardisation was employed to calculate the age‐standardised diabetes incidence. Mixed effects logistic regression models with interaction terms were performed to examine variations in incident diabetes. Socio‐demographic (age, sex, marital status, racial compositions, and educational attainment) and lifestyle attributes (smoking history, BMI, and waist circumference) were sequentially included as covariates.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Overall age‐standardised diabetes incidence increased from 2.94 per 1,000 person‐years (95% CI, 2.44 to 3.44) in 1997 ‐ 2004 to 5.54 (95% CI, 4.94 to 6.14) in 2009 ‐ 2015. Models with interaction terms suggest that the increase among men was higher than that among women (wave 2006 ‐ 2009 x Female: OR = 0.45, 95% CI 0.28 to 0.72). Age‐standardised incidence of diabetes varied across regions, ranging from 5.67 (95%CI, 4.95 to 6.40) in Eastern China to 2.69 (95%CI 2.19 to 3.19) in Western China. Subsequent modelling analyses suggest that the geographic variations could be mostly explained by the variations in the BMI and waist circumference across regions.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Results suggest that the incidence of self‐reported diabetes approximately doubled during the study period. The increase among men was steeper than that among women. Public interventions reducing the population's obesity level hold promise to alleviate geographic variations and flatten the growth curve.</p></section>

Abstract:OBJECTIVE To describe the prevalence of type 2 diabetes mellitus( T2 DM) in rural Deqing County, Zhejiang Province, and explore its risk factors. METHODS Random cluster sampling was used to select 8 rural communities in Deqing County, Zhejiang Province, and a cross-sectional survey was conducted in 2006-2008, 2011-2012 and 2013-2014, respectively. Totally, 6562 eligible subjects were investigated in 2006-2008, 11 763 eligible subjects in 2011-2012, and 10 904 eligible subjects in 2013-2014. Data on demography, life style and disease history, and physical examination such as height, weight and blood pressure were collected. Also, fasting plasma glucose was tested. T2 DM was defined as who having fasting plasma glucose ≥7. 0 mmol/L or was diagnosed by doctors or receiving diabetic treatment. RESULTS In 2006-2008, 2011-2012 and 2013-2014, the standardized prevalences of T2 DM were 1. 2%, 1. 6% and 2. 1% and those of impaired fasting glucose( IFG) were 7. 2%, 18. 7% and22. 8%, respectively. There were increasing trends over years for both. After adjustment for other covariates in logistic regression, age( compared with age between 18 to 39. 9 years old: aOR_(40-59. 9)= 5. 20, 95% CI 2. 74-9. 89; aOR_(60-)= 8. 02, 95% CI 4. 19-15. 35), female( aOR = 1. 23, 95% CI 1. 02-1. 49), family history of T2 DM( aOR =4. 86, 95% CI 3. 49-6. 77), overweight/obesity( overweight: aOR = 1. 88, 95% CI1. 61-2. 20; obesity: aOR = 5. 10, 95% CI 4. 06-6. 41), hypertension( aOR = 2. 84, 95% CI 2. 44-3. 31), having meat mainly( aOR = 2. 25, 95% CI 1. 89-2. 69) and tea drinking( aOR = 0. 81, 95% CI 0. 68-0. 95) were significantly correlated with T2 DM. CONCLUSION The prevalence of T2 DM increased over time in rural Deqing County, China. Age, gender, family history of T2 DM, overweight or obesity, hypertension, diet and drinking tea were significantly related to T2 DM.

Abstract:Objective To estimate the trend in the coexistence of hypertension and diabetes mellitus(HTN-DM)among residents aged 35-74 years in Shanghai during 2002-2017 for the prevention and control of the comorbidity.Methods The study data were from four waves of surveillance on diabetes conducted in 2002,2009,2017 and on non-communicable chronic diseases in 2013 among totally 56 927 residents aged 35-74 years in Shanghai.The prevalence of HTN-DM and its annual percentage change(APC)among the residents were calculated to analyze the trend of HTN-DM during the period.Results The detected prevalence rates of HTN-DM were 4.61％,9.93％,13.41％and 18.97％in the participants separately surveyed in 2002,2009,2013 and 2017,with a significantly increasing trend over the 16-year period(APC=9.51％,P=0.003).Further analyses revealed that the increasing trend in HTN-DM prevalence was significant in the participants of all subgroups stratified by gender,five-year age grouping,ethnicity,education,marital status,and residential area and the increasing trend was greater for the participants of male gender,aged 35-44 years,being Han ethnicity,with the education of high school and above,being married,and not residing in central urban area,with the APC of 9.66％,11.49％,9.53％,10.56％,9.94％,and 10.09％,respectively.Conclusion The prevalence of HTN-DM showed an increasing trend among 35-74 years old residents in Shanghai over the period from 2002 to 2017,especially among those aged 35-44 years,being male,Han ethnicity,with the education of high school education and above,being married and not residing in central urban areas.
