Abstract:No studies have examined endogenous insulin secretion in pediatric patients with type 1 diabetes in China using the gold-standard mixed-meal tolerance test. Because the latter is labor-intensive, we examined simpler surrogate markers of endogenous insulin secretion in Chinese youth, as previously reported for a European population. Participants were 57 children and adolescents with type 1 diabetes aged 4.4–16.8 years (56% females). We performed 120-minute mixed-meal tolerance tests with serum C-peptide (CP) measurements every 30 minutes. Severe insulin deficiency (SID) was defined as CP peak < 0.2 nmol/L. Urine CP and creatinine levels were measured at 0 and 120 minutes. Twenty-five (44%) patients had SID. Fasting CP levels missed one case (96% sensitivity) with no false positives (100% specificity). While the 120-minute urine CP/creatinine had 100% sensitivity, it yielded markedly lower specificity (63%). Every 1-year increase in diabetes duration and 1-year decrease in age at diagnosis were associated with 37% (P < 0.001) and 20% (P = 0.005) reductions in serum CP area-under-the-curve, respectively. Thus, 86% of children aged < 5 years had SID compared to none among patients aged ≥ 11 years. Simple fasting CP measurements could be used to detect most SID cases in Chinese youth with type 1 diabetes. Fasting CP is a far more reliable measure of endogenous insulin secretion than the more commonly used insulin dose. Therefore, it could more precisely determine insulin secretory capacity to target those who could benefit, if treatments to preserve residual insulin secretion are developed.

Abstract:Objective To evaluate beta-cell function in obese children and adolescents meeting clinical criteria for isolated obesity (iOB), isolated components of dysmetabolism (cMD), or metabolic syndrome (MS), and in obese children and adolescents with normal glucose tolerance (NGT), impaired glucose regulation (IGR), or type 2 diabetes (T2DM). Study Design We undertook a prospective study of Han Chinese children and adolescents aged 8-16 years (median 11 +/- 1.4) seen in an obesity clinic between May 2013 and 2018. Patients were classified as iOB (53), cMD (139), and MS (139) groups based on clinical criteria. The same patients were also classified as NGT (212), IGR (111), or T2DM (8) based on results of an oral glucose tolerance test (OGTT). The MS patients were classified as NGT [MS](59) and IGR [MS](72) for the further study. All participants also completed a mixed-meal tolerance test (MMTT). Results Compared with the iOB group, the MS group had significantly higher area under the curve of C-peptide up to the 2 hours (AUC CP) (P = .03) and peak C-peptide (P = .03), adjusted for BMI, age and Tanner stage, on MMTT. However, there was no difference in the insulinogenic index (Delta I30/Delta G30) or oral disposition index (oDI) derived from the OGTT among the three groups. However, 52% of participants with MS had IGR, compared to 28% in the cMD group. Compared with the NGT group, the individuals with IGR had significantly lower Delta I30/Delta G30 (P = .001) and oDI (P < .001). Compared with the iOB group, the NGT[MS] had significantly higher AUC CP (P = .004), peak C-peptide (P = .004) and Delta I30/Delta G30 (P = .007) adjusted for age, but no difference in oDI. Compared with the NGT[MS], the IGR[MS] had significantly lower Delta I30/Delta G30 (P = .005) and oDI (P < .001), but the AUC CP and peak C-peptide had no difference. Conclusion Although the MS youth have beta-cell hyperfunction as a whole, beta-cell dysfunction is present in the early stages of dysmetabolism in obese youth with cMD or MS and worsened across the spectrum from iOB to cMD and MS, contributing to development of T2DM.

Abstract:Islet β cell secretion deficiency and( or) the decreased insulin sensitivity of target tissue are the important pathophysiological mechanisms of diabetes. So,detection and assessment of isletβcell function in the early stages,could be of great significance for disease severity evaluation,early intervention and prognosis of the disease. At present,the main methods of the testing and assessment ofβcell function includeβcell function evaluating indexes,pulsatile insulin secretion,insulin secretion by glucose or non-glucose secretagogues and func-tion testing by other secretions of isletβcells. Among of them,βcell functional assessment methods by detecting C-peptide( especially aspects such as 90 minutes of C-peptide testing in mixed-meal tolerance test,urinary C-pep-tide creatinine ratio) have experienced some progress in recent years.
Clinical Incidence and Characteristics of Newly Diagnosed Type 1 Diabetes in Chinese Children and Adolescents: A Nationwide Registry Study of 34 Medical Centers.

Abstract:ObjectiveTo investigate the clinical incidence and characteristics of type 1 diabetes mellitus (T1DM) of children and adolescents at the time of initial diagnosis in China.MethodsData on all pediatric patients with newly diagnosed T1DM were retrospectively collected from 34 medical centers in 25 major cities in China from January 2015 to January 2020. Patients were classified into three age groups: <5 years, 5 to <10 years, and ≥10 years of age. The same patient population was also categorized into diabetic ketoacidosis (DKA) and non-DKA groups based on clinical criteria.ResultsThe mean annual clinical incidence of T1DM was 3.16/100,000 from the years 2015 to 2019. A total of 6,544 patients with newly diagnosed T1DM aged 0–16 years (median 7.84 ± 3.8) were studied [ages <5 years (29.3%), 5 to <10 years (38.7%), and ≥10 years (32%)], 52.4% of them were women. In total, 90.5% of the cases were occurred in individuals without a family history. Patients had lower C-peptide (CP) and body mass index (BMI) z scores when compared with healthy children, 41.8% of them had measurable T1DM-related antibodies and 52.7% had DKA. Among all three age groups, the <5 years group had the lowest BMI z score, CP, and glycated hemoglobin (HbA1c) on average, while it had the highest incidence rate of DKA (56.9%). Compared to the non-DKA group, the DKA group was significantly younger, with a lower BMI z score and CP, higher antibody positive rate, HbA1c, and the rate of insulin pump therapy.ConclusionThe clinical incidence of T1DM in children and adolescents in China was 3.16/100,000. Patients with DKA at the first diagnosis of T1DM have a worse β-cell function. Public health measures for the prevention and treatment of T1DM should focus on preschoolers (aged <5 years) in particular, considering the severity and the highest frequency of DKA in this age group. More efforts should be dedicated to early screening and diagnosis of the T1DM.
Analysis of the clinical characteristics and differential diagnosis in 186 cases with type 1 and type 2 diabetes in young adult and children

Abstract:Objective To analyze the clinical characteristics and key points in differential diagnosis for type 1 and type 2 diabetes in young adult and children.Methods The clinical data of the diabetic patients younger than 30 years,who were admitted to Peking Union College Hospital from 2000 through 2005 including 111 type 1 diabetic cases and 75 type 2 diabetic cases,were retrospectively analyzed,as did the differences in clinical symptoms,signs and laboratory parameters,the odds ratio(OR) and ROC curve. Results The infection rate(35.1%) before onset and diabetic ketoacidosis(71.2%) were higher in type 1 diabetes than in type 2 diabetes.Type 2 diabetic patients had higher incidence rates of fatty liver(70.7%), history of obes ity(95.7%)and acanthosis nigricans(54.7%).Levels of BMI(28.12±4.38 kg/m~2),fasting C peptide[2.80(0.5 to 8.2)ng/ml],HOMA2-IR[l.7(0.4 to 6.1)] were higher in type 2 diabetes than in type 1 diabetes (P<0.05).The maximum OR value is acanthosis nigricans (trend to infinity),next is fatty liver(78.6),history of obesity(8.5).Analysis of the area under the ROC curve showed BMI,HOMA2-IR, fasting C peptide were the most important factors,which could help us to discriminate the two types of diabetes.The cut off points of BMI,HOMA2-IR and fasting C-peptide were 23 kg/m~2,0.75 and 1.0ng/ ml,respectively.Conclusions The best indexes to discriminate the type 1 and 2 diabetes in young adult and children are acanthosis nigricans,fatty liver,the history of obesity,BMI,HOMA2-IR and fasting C peptide.
Residual Β-Cell Function after 10 Years of Autoimmune Type 1 Diabetes: Prevalence, Possible Determinants, and Implications for Metabolism

Abstract:Background: Type 1 diabetes (T1D) has long been considered a progressive autoimmune disease resulting in the failure of pancreatic beta-cell function and absolute endogenous insulin deficiency. However, several studies have demonstrated patients with T1D have detectable C-peptide levels long after diagnosis, which has remarkable clinical significance. Since this issue has not been systematically explored in non-Caucasian populations, we aimed to identify the prevalence of residual beta-cell function and its related clinical features in Chinese long-term T1D patients. Methods: We enrolled 109 patients with T1D for >= 10 years and administered a mixed-meal tolerance test (MMTT). Fasting and postprandial C-peptide (FCP/PCP) levels were measured to evaluate the insulin secretion function of beta-cells. Patients whose FCP and PCP levels were both below the lower detection limit (16.7 pmol/L) were grouped as 'beta-cell function depleted', while others were thought to have 'residual beta-cell function'. Demographic data, metabolic status, and diabetic complications were compared between patients with or without residual beta-cell function. Results: 38.5% of subjects retained residual beta-cell function, and among those, 33.3% responded to MMTT by a two-fold or greater rise of their FCP levels. Clinical features associated with residual beta-cell function were older age of diagnosis [27.5 (interquartile range:11.5-37.0) vs. 17.0 (interquartile range: 8.0-30.0) years, P=0.037], lower HbA1c (64.6 +/- 20.3 vs. 72.4 +/- 18.5 mmol/ mol, P=0.026), and reduced rate of hypoglycemia (23.8% vs. 52.2%, P=0.003). Age of diagnosis was positively correlated with detectable FCP level (r=0.393, P=0.020). Individuals diagnosed after 30 years of age tended to retain residual beta-cell function (OR =3.016, P=0.044). We found no association between residual beta-cell function and chronic diabetic complications. Conclusions: Residual beta-cell function can be found in nearly 40% of long-term patients with T1D in China and is associated with older age at diagnosis and better glucose control. The relationship between residual beta-cell function and chronic diabetic complications remains to be explored.
Metabolomics and Lipidomics Studies in Pediatric Type 1 Diabetes: Biomarker Discovery for the Early Diagnosis and Prognosis

Abstract:Aim. Type 1 diabetes (T1D) is an autoimmune disease with heterogeneous risk factors. Metabolic perturbations in the pathogenesis of the disease are remarkable to illuminate the interaction between genetic and environmental factors and how islet immunity and overt diabetes develop. This review aimed to integrate the metabolic changes of T1D to identify potential biomarkers for predicting disease progression based on recent metabolomics and lipidomics studies with parallel methodologies. Methods. A total of 18 metabolomics and lipidomics studies of childhood T1D during the last 15 years were reviewed. The metabolic fingerprints consisting of 41 lipids and/or metabolite classes of subjects with islet autoantibodies, progressors of T1D, and T1D children were mapped in four-time dimensions based on a tentative effect-score rule. Results. From birth, high-risk T1D subjects had decreased unsaturated triacylglycerols, unsaturated phosphatidylcholines (PCs), sphingomyelins (SMs), amino acids, and metabolites in the tricarboxylic acid (TCA) cycle. On the contrary, lysophosphatidylcholines (LPCs) and monosaccharides increased. And LPCs and branched-chain amino acids (BCAAs) were elevated before the appearance of islet autoantibodies but were lowered after seroconversion. Choline-related lipids (including PCs, SMs, and LPCs), BCAAs, and metabolites involved in the TCA cycle were identified as consensus biomarkers potentially predicting the development of islet autoimmunity and T1D. Decreased LPCs and amino acids indicated poor glycemic control of T1D, while elevated lysophosphatidylethanolamines and saturated PCs implied good glycemic control. Further pathway analysis revealed that biosynthesis of aminoacyl-tRNA, BCAAs, and alanine, aspartate, and glutamate metabolism were significantly enriched. Moreover, established cohort studies and predictive statistical models of pediatric T1D were also summarized. Conclusion. The metabolic profile of high-risk T1D subjects and patients demonstrated significant changes compared with healthy controls. This integrated analysis provides a comprehensive overview of metabolic features and potential biomarkers in the pathogenesis and progression of T1D.

Abstract:Purpose To investigate the features and treatment status of children with type 1 diabetes mellitus (T1DM) in China. Methods We recruited patients <14 years of age with T1DM from 33 medical centers in 25 major cities of China between January 2012 and March 2015. All patients completed a questionnaire that was conducted by their pediatric endocrinologists at all centers. Results A total of 1,603 children (755 males and 848 females) with T1DM participated in this survey. Of these, 834 (52.03%) of the patients exhibited diabetic ketoacidosis (DKA) at onset, while 769 patients (47.97%) did not exhibit DKA (non-DKA) at onset. There was a higher proportion of females (55.71%) in the cohort of patients exhibiting DKA at onset than in the non-DKA cohort (49.33%). The mean age of patients exhibiting DKA at presentation was 7.12 ± 0.14 years; this was significantly younger than that in non-DKA group (7.79 ± 0.15 years; P < 0.005). The frequency of DKA in 3 years old, 3-7 years old, and 7 years old or more was 77.21%, 26.17%, and 37.62%, respectively. Upon initial diagnosis, 29.4%, 15.2% and 11.8% of patients showed positivity for glutamic acid decarboxylase antibody (GADA), Insulin autoantibodies (IAA), or islet cell antibody (ICA), respectively. During six months follow-up, 244 patients (15.21%) reported receiving insulin pump therapy, and more than 60% of patients monitored their blood glucose levels less than 35 times per week. Although the majority of patients had no problems with obtaining insulin, 4.74% of the children surveyed were not able to receive insulin due to financial reasons, a shortage of insulin preparations, or the failure of the parents or guardians to acquire the appropriate medicine. Conclusion DKA is more common in very young children. Treatment and follow-up of T1DM in China still face very serious challenges.

Relationship Between the Level of Fasting Plasma Glucose and Beta Cell Functions in Chinese with or Without Diabetes.

Abstract:BACKGROUND:Type 2 diabetes is a chronic disease characterized by a progressive loss of beta cell functions. However, the evaluation of beta cell functions is either expensive or inconvenient for clinical practice. We aimed to elucidate the association between the changes of insulin responsiveness and the fasting plasma glucose (FPG) during the development of diabetes.METHODS:A total of 1192 Chinese individuals with normal blood glucose or hyperglycemia were enrolled for the analysis. The early insulinogenic index (DeltaI30/DeltaG30), the area under the curve of insulin (AUC-I), and homeostasis model assessment were applied to evaluate the early phase secretion, total insulin secretion, and insulin resistance respectively. Polynomial regression analysis was performed to estimate the fluctuation of beta cell functions.RESULTS:The DeltaI30/DeltaG30 decreased much more rapidly than the AUC-I accompanying with the elevation of FPG. At the FPG of 110 mg/dl (a pre-diabetic stage), the DeltaI30/DeltaG30 lost 50% of its maximum while the AUC-I was still at a compensated normal level. The AUC-I exhibited abnormal and decreased gradually at the FPG of from 130 mg/dl to higher (overt diabetes), while the DeltaI30/DeltaG30 almost remained at 25% of its maximum value. When hyperglycemia continuously existed at > 180 mg/dl, both the DeltaI30/DeltaG30 and AUC-I were totally lost.CONCLUSION:The increased fasting plasma glucose reflects progressive decompensation of beta cell functions, and could be used to guide the strategy of clinical treatments.
Comparison of beta-cell function and insulin sensitivity between normal-weight and obese Chinese with young-onset type 2 diabetes

Abstract:Normal-weight individuals with usual-onset type 2 diabetes had reduced beta-cell function and greater insulin sensitivity compared to their obese counterparts. The relative contribution of beta-cell dysfunction and insulin resistance to young-onset type 2 diabetes (YOD) among normal-weight individuals is not well established. In 44 individuals with YOD (24 normal-weight and 20 obese) and 24 healthy controls with normoglycemia (12 normal-weight and 12 obese), we conducted 2-h 12mmol/L hyperglycemic clamps to measure acute (0-10 minute) and steady-state (100-120 minute) insulin and C-peptide responses, and insulin sensitivity index. Normal-weight individuals with YOD had lower acute insulin response, steady-state insulin and C-peptide responses, and higher insulin sensitivity index compared to their obese counterparts with YOD. When compared to BMI-matched healthy controls, normal-weight individuals with YOD had lower acute and steady-state insulin and C-peptide responses but similar insulin sensitivity index. The impairment of steady-state beta-cell response relative to healthy controls was more pronounced in normal-weight versus obese individuals with YOD. In conclusion, normal-weight Chinese with YOD exhibited worse beta-cell function but preserved insulin sensitivity relative to obese individuals with YOD and BMI-matched healthy individuals with normoglycemia. The selection of glucose-lowering therapy should account for pathophysiological differences underlying YOD between normal-weight and obese individuals.

Abstract:The limited available studies have unveiled different natural histories and prognosis associated with pediatric type 2 diabetes (T2D) and adult T2D. To date, data on the clinical features, metabolic profiles and beta-cell function characteristics are still limited in the Chinese pediatric T2D population. A total of 56 children with T2D, 31 with prediabetes and 159 with obesity were recruited. Clinical characteristics, metabolic profiles, beta-cell function and insulin resistance were analyzed. The mean onset age of T2D was 12.35 ± 1.99 (7.9–17.8) years, and 7% of children were younger than 10 years; 55% of them were male, 57% had a family history of diabetes and 64% had classic symptoms, and 25% had a low or high birth weight. 89% of T2D patients were obese or overweight. A total of 58% of the patients with prediabetes were male. The fast serum C-peptide level was highest in the obesity group (P < 0.001), and there was no significant difference between the T2D and prediabetes groups. The mean homeostatic model of assessment of beta-cell function was the highest in the obesity group and was lowest in the T2D group (P < 0.001). The T2D group had the most serious lipid metabolism disorder, with the highest levels of total triglycerides, total cholesterol, and low density lipoprotein and the lowest high density lipoprotein level among the three groups. A younger onset age and greater male susceptibility were found in Chinese pediatric T2D patients, and there was a stepwise deterioration trend in beta-cell function among patients with obesity, prediabetes and T2D. Based on our results, together with the SEARCH study results, an early screening and intervention program for T2D is recommended in high-risk or obese Chinese pediatric populations starting at 7 years.
Worse Pancreatic Β-Cell Function and Better Insulin Sensitivity in Older Chinese Without Diabetes.

Abstract:BACKGROUND:Whether insulin secretion and insulin sensitivity in older Chinese differ from younger persons without diabetes has not been examined and is unknown to us.METHODS:Oral glucose tolerance test was performed in a total of 32,245 Chinese without diabetes history. Insulin sensitivity was estimated by Matsuda's insulin sensitivity index (Matsuda ISI), and insulin secretion was measured by the insulinogenic index. The insulin secretion and sensitivity were compared among three age groups: 20-39 (the young), 40-59 (the middle), and greater than or equal to 60 years (the older).RESULTS:A higher proportion of postchallenge hyperglycemia (PH), both in prediabetes and in diabetes stage, was associated with older age. In the young, middle, and older age groups, the insulinogenic index median was 10.64, 8.40, and 6.67, respectively, in individuals with normal glucose tolerance (NGT); it was 8.56, 6.12, and 4.98 in individuals with prediabetes, respectively (p < .001 vs the young group). In these three age groups, the Matsuda insulin sensitivity index was 8.20, 8.39, and 8.84 in individuals with normal glucose tolerance, and it was 5.23, 5.94, and 6.33 in individuals with prediabetes, respectively (p < .05 vs the young group). These differences were independent of body mass index and waist circumference.CONCLUSIONS:Older Chinese without diabetes are characterized with worse insulin secretion and better insulin sensitivity. Β Cell Function of Islets of Langerhans in 61 Patients of Old Aged Korean-Chinese with Type 2 Diabetes Mellitus

Abstract:OBJECTIVE To observe the insulin resistance(IR) and β cell function of islets of Langerhans in old aged Korean-Chinese with type 2 diabetes mellitus(T2DM).METHODSThe clinical indicator and cholesterol levels were detected in 61 patients of old age with T2DM,16 cases of impared glucose tolerance(IGR) and 19 cases of normal glucose tolerance(NGT),and the oral glucose insulin release test(OGIRT) was carried out.The insulin sensitivity index(SEN),HOMA-IR,HOMA-β,and ΔI30/ΔG30 were calculated.RESULTSIn the patients with T2DM,the ΔI30/ΔG30 had no significant difference(P=0.733,P=0.068) and HOMA-β had significant differences(P=0.000,P=0.014) as compared with the NGT and IGR,the HOMA-IR and SEN had significant differences(P=0.001,P=0.000) as compared with NGT,and the HOMA-IR and SEN had no significant differences(P=0.148,P=0.078) as compared with IGR.CONCLUSIONThe diagnosis for IR of T2DM in old aged Korean-Chinese is more obvious in the period of IGR,and the β cell dysfunction of islets of Langerhans is gradually aggravated from IGR to T2DM.

Abstract:Background The aim of this study was to investigate the annual decline of beta-cell function correlated with disease duration in patients with type 2 diabetes in China. Methods This cross-sectional study included 4792 adults with type 2 diabetes who were recruited from four university hospital diabetes clinics between April 2018 and November 2018. Baseline data were collected from electric medical records. Participants were divided into 21 groups with 1-year diabetes duration interval to assess the decline rate of beta-cell function. Homeostatic model assessment model (HOMA 2) model was applied to assess beta-cell function. Multiple linear regression model was used to evaluate the association between biochemical and clinical variables and beta-cell function. Results In Chinese patients with type 2 diabetes,beta-cell function declined by 2% annually. Using angiotensin receptor blockade (ARB) (beta= .048;P= .011), metformin (beta= .138;P= .021), or insulin (beta= .142;P= .018) was associated with increased beta-cell function. However, increased BMI (beta= -.215;P= .022), alcohol consumption (beta= -.331;P< .001), haemoglobin A1c (beta= -.104;P= .027), or increased diabetes duration (beta= -.183;P= .003) was significantly and negatively associated with beta-cell function. Conclusions We determined that the annual rate of the beta-cell function decline was 2% in patients with type 2 diabetes in China. Moreover, we confirmed a positive relationship between ARB treatment and beta-cell function, while BMI and alcohol consumption were significantly and negatively associated with the beta-cell function.

Abstract:To determine type II diabetes in association with alteration in beta-cell function, we studied 30 patients with newly diagnosed diabetes and 30 matched controls. The beta-cell 1st phase insulin response was measured by intravenous glucose tolerance test and the 2nd phase insulin response was measured by oral glucose tolerance test. The results revealed that in type II diabetes mellitus the pulse of 1st phase insulin secretion could not be found. The amount of 2nd phase insulin secretion of the patients was lower than that normal controls, and the patients' secretary pulse of insulin was not concomitant with glucose pulse. These findings again suggest it is the defect in beta-cell function, not the peripheral resistance an essential cause of overt type II diabetes.
Random C-Peptide and Islet Antibodies at Onset Predict β Cell Function Trajectory and Insulin Dependence in Pediatric Diabetes

Abstract:Context Identification of prognostic biomarkers in pediatric diabetes is important for precision medicine. Objective We assessed whether C-peptide and islet autoantibodies are useful to predict the natural history of children with new-onset diabetes. Methods We prospectively studied 72 children with new-onset diabetes (median follow-up: 8 months) by applying the Aβ classification system ("A+": islet autoantibody positive, "β+": random serum C-peptide≥1.3 ng/mL at diagnosis). Beta-cell function was assessed longitudinally with 2h post-prandial/stimulated urinary C-peptide-to-creatinine ratio (UCPCR) 3-12 weeks (V1) and 6-12 months after diagnosis (V2). We obtained a type 1 diabetes genetic risk score (T1D GRS2) for each participant, and compared characteristics at baseline, and clinical outcomes at V2. Results The cohort was 50% male. Racial distribution was 76.4% White, 20.8% Black and 2.8% Asian or other races. A total of 46.5% participants were Hispanic. Median age (Q1-Q3) was 12.4 (8.3-14.5) years. The Aβ subgroup frequencies were 46 A+β-(63.9%), 1 A-β-(1.4%), 4 A+β+(5.6%), and 21 A-β+(29.2%). Baseline serum C-peptide correlated with UCPCR at both V1 (r=0.36, p=0.002) and V2 (r=0.47, p<0.001). There were significant subgroup differences in age, race, frequency of diabetic ketoacidosis and T1D GRS2 (p<0.01). At V2, the two β- subgroups had lower UCPCR and higher HbA1c compared with the two β+ subgroups (p<0.001 and p=0.02, respectively). Thirty-eight percent of A-β+ but none of the other subgroups were insulin-independent at V2 (p<0.001). Conclusions C-peptide and islet autoimmunity at diagnosis define distinct phenotypes and predict beta-cell function and insulin dependence 6-12 months later in racially/ethnically diverse children with new-onset diabetes.

Abstract:Aims: The paucity of data on Type 1 diabetes in China hinders progress in care and policy-making. This study compares Type 1 diabetes care and clinical outcomes in Beijing and Shantou with current clinical guidelines. Methods: The 3C Study was a cross-sectional study of the clinical practices and outcomes of people with Type 1 diabetes. The study sequentially enrolled 849 participants from hospital records, inpatient wards, and outpatient clinics. Data were collected via face-to-face interviews with patients and health professionals, the Summary of Diabetes Self-Care Activities, medical records, and venous blood samples. Care was audited using ISPAD/IDF indicators. Data underwent descriptive analysis and tests for association. Results: The median age was 22years (IQR=13-34years), and 48.4% of the sample had diabetes less than six years. The median HbA1c was 8.5% (69mmol/mol) (IQR 7.2-10.5%), with significant regional variance (p=0.002). Insulin treatment was predominantly two injections/day (45% of patients). The highest incidence of diabetic ketoacidosis was 14.4 events/100 patient years among adolescents. Of the 57.3% of patients with LDL-C>2.6mmol/L, only 11.2% received treatment. Of the 10.6% considered hypertensive, 47.1% received treatment. Rates of documented screening for retinopathy, nephropathy, and peripheral neuropathy were 35.2%, 42.3%, and 25.0%, respectively. The median number of days of self-monitoring/week was 3.0 (IQR=1.0-7.0). There were significant differences in care practices across regions. Conclusions: The study documented an overall deficit in care with significant regional differences noted compared to practice guidelines. Modifications to treatment modalities and the structure of care may improve outcomes.

Abstract:Context: Type 1 diabetes (T1D) results from the autoimmune T-cell mediated destruction of pancreatic beta cells leading to insufficient insulin secretion. At the time of diagnosis of T1D, there is residual beta cell function that declines over the subsequent months to years. Recent interventions have been approved to preserve beta cell function in evolving T1D. Objective: The aim of this review is to summarise the approaches used to assess residual beta cell function in evolving T1D, and to highlight potential future directions. Methods: Studies including subjects aged 0 to 18 years were included in this review. The following search terms were used; “(type 1 diabetes) and (partial remission)” and “(type 1 diabetes) and (honeymoon)”. References of included studies were reviewed to determine if additional relevant studies were eligible. Results: There are numerous approaches to quantifying beta cell reserve in evolving T1D. These include c-peptide measurement after a mixed meal or glucagon stimuli, fasting c-peptide, the urinary c-peptide/creatinine ratio, insulin dose-adjusted haemoglobin A1c, and other clinical models to estimate beta cell function. Other biomarkers may have a role, including the proinsulin/c-peptide ratio, cytokines, and microRNA. Studies using thresholds to determine if residual beta cell function is present often differ in values used to define remission. Conclusions: As interventions are approved to preserve beta cell function, it will become increasingly necessary to quantify residual beta cell function in research and clinical contexts. In this report, we have highlighted the strengths and limitations of the current approaches.

Defective Insulin Maturation in Patients with Type 2 Diabetes

Abstract:Objective: Progressive beta-cell dysfunction is a hallmark of type 2 diabetes (T2D). Increasing evidence indicates that over-stimulating proinsulin synthesis causes proinsulin misfolding and impairs insulin maturation and storage in db/db mice. However, defective insulin maturation in patients with T2D remains unknown. Methods: We examined intra-islet and intra-cellular distributions of proinsulin and insulin and proinsulin to insulin ratio in the islets of patients with T2D. The expression of transcription factor NKX6.1 and dedifferentiation marker ALDH1A3, as well as glucagon, were detected by immunofluorescence. Results: We identified a novel subgroup of beta cells expressing only proinsulin but not insulin. Importantly, significantly increased proinsulin positive and insulin negative (PI+/INS-) cells were evident in T2D, and this increase was strongly correlated with levels of hemoglobin A1C (HbA1c) in T2D and prediabetes. The percentages of beta cells expressing prohormone convertase 1/3 and carboxypeptidase E were not reduced. Indeed, while proinsulin displayed a higher degree of co-localization with the golgi markers GM130/TGN46 in control beta cells, it appeared to be more diffused within the cytoplasm and less co-localized with GM130/TGN46 in PI+/INS- cells. Furthermore, the key functional transcription factor NKX6.1 markedly decreased in the islets of T2D, especially in the cells with PI+/INS-. The decreased NKX6.1+/PI+/INS+ was strongly correlated with levels of HbA1c in T2D. Almost all PI+/INS- cells showed absence of NKX6.1. Moreover, the percentages of PI+/INS- cells expressing ALDH1A3 were elevated along with an increased acquisition of glucagon immunostaining. Conclusion: Our data demonstrate defective insulin maturation in patients with T2D.

Abstract:The objective is to explore the effects of hyperlipidemia on beta cell function in newly diagnosed type 2 diabetes mellitus (T2DM). 208 patients were enrolled in the study and were divided into newly diagnosed T2DM with hyperlipidemia (132 patients) and without hyperlipidemia (76 patients). Demographic data, glucose levels, insulin levels, lipid profiles, homeostasis model assessment for.. cell function index (HOMA-beta), homeostasis model assessment for insulin resistance index (HOMA-IR), and quantitative insulin-sensitivity check index (QUICKI) were compared between the two groups. We found that comparing with those of normal lipid levels, the subjects of newly diagnosed T2DM with hyperlipidemia were younger, and had declined HOMA-beta. However, the levels of HOMA-beta were comparable regardless of different lipid profiles (combined hyperlipidemia, hypertriglyceridemia, and hypercholesterolemia). Multiple stepwise linear regression analysis showed that high fasting plasma glucose (FPG), decreased fasting insulin level (FINS), and high triglyceride (TG) were independent risk factors of beta cell dysfunction in newly diagnosed T2DM. Therefore, the management of dyslipidemia, together with glucose control, may be beneficial for T2DM with hyperlipidemia.
