Expert Review of Molecular Diagnostics

Abstract:Introduction: Acquired immunodeficiency syndrome (AIDS) is a kind of acquired disease that breaks down the immune system. Human immunodeficiency virus (HIV) is the causative agent of AIDS. By the end of 2016, there were 36.7 million people living with HIV worldwide. Early diagnosis can alert infected individuals to risk behaviors in order to control HIV transmission. Infected individuals are also benefited from proper treatment and management upon early diagnosis. Thanks to the public awareness of the disease, the annual increase of new HIV infections has been slowly declining over the past decades. The advent of molecular diagnostics has allowed early detection and better management of HIV infected patients.Areas covered: In this review, the authors summarized and discussed the current and future technologies in molecular diagnosis as well as the biomarkers developed for HIV infection.Expert Commentary: A simple and rapid detection of viral load is important for patients and doctors to monitor HIV progression and antiretroviral treatment efficiency. In the near future, it is expected that new technologies such as digital PCR and CRISPR-based technology will play more important role in HIV detection and patient management.

Abstract:Significant advances have been made in the molecular assays used for the detection of human immunodeficiency virus (HIV), which are crucial in preventing HIV transmission and monitoring disease progression. Molecular assays for HIV diagnosis have now reached a high degree of specificity, sensitivity and reproducibility, and have less operator involvement to minimize risk of contamination. Furthermore, analyses have been developed for the characterization of host gene polymorphisms and host responses to better identify and monitor HIV-1 infections in the clinic. Currently, molecular technologies including HIV quantitative and qualitative assays are mainly based on the polymerase chain reaction (PCR), transcription-mediated amplification (TMA), nucleic acid sequence-based amplification (NASBA), and branched chain (b) DNA methods and widely used for HIV detection and characterization, such as blood screening, point-of-care testing (POCT), pediatric diagnosis, acute HIV infection (AHI), HIV drug resistance testing, antiretroviral (AR) susceptibility testing, host genome polymorphism testing, and host response analysis. This review summarizes the development and the potential utility of molecular assays used to detect and characterize HIV infections.

Abstract:Objective To study the diagnostic techniques of HIV/AIDS complicated with mycobacterium tuberculosis infection,and to analysis the drug sensitivity and resistance phenotype of mycobacterium tuberculosis.Methods The blood samples of HIV/AIDS patients complicated with mycobacterium tuberculosis infection all were detected by T-SPOT.TB assay,MTD assay,acid-fast bacillus test of sputum smear,TB-DNA and mycobacterium tuberculosis culturing,and the positive rates of these several detection methods were compared.The mycobacterium tuberculosis positive patients were tested drug sensitivity(Isoniazid,Rifampin,Streptomycin,Ethambutol,Pyrazinamide).Results The positive rate of T-SPOT.TB detection was 68.1%(64/94),obviously higher than the other four kinds of detection methods(P<0.01).The positive rates of MTD,TB-DNA,AFB and mycobacterium tuberculosis culturing were 40.0%(30/75),35.5%(27/76),18.1%(17/94)and 26.6%(25/94)respectively.The positive rate of MTD detection was higher than TB-DNA,AFB test and mycobacterium tuberculosis culturing,and had statistically significant compared with AFB,but had no statistically significant differences with others.In 25 cases of HIV/AIDS complicated with mycobacterium tuberculosis infection,the sensitivity rates of five drug all were 76.0%(19/25),the multi-drug resistance rate(MDR)was 16.0%(4/25),the rate of Isoniazid-resistance was 8.0%(2/25).Conclusions T-SPOT.TB and MTD both were fast sensitive diagnostic method in HIV/AIDS complicated with mycobacterium tuberculosis infection,and these two detection methods could be used as assistant diagnosis methods for HIV/AIDS with tuberculosis.Most of HIV/AIDS patients with tuberculosis were still sensitive to first-line drugs,but drug resistance patients were multi-drug resistance.
Human immunodeficiency virus: 25 years of diagnostic and therapeutic strategies and their impact on hepatitis B and C virus

Abstract:The human immunodeficiency virus (HIV) had spread unrecognized in the human population as sexually transmitted disease and was finally identified by its disease AIDS in 1981. Even after the isolation of the causative agent in 1983, the burden and death rate of AIDS accelerated worldwide especially in young people despite the confection of new drugs capable to inhibit virus replication since 1997. However, at least in industrialised countries, this trend could be reversed by the introduction of combination therapy strategies. The design of new drugs is on going; besides the inhibition of the three enzymes of HIV for replication and maturation (reverse transcriptase, integrase and protease), further drugs inhibits fusion of viral and cellular membranes and virus maturation. On the other hand, viral diagnostics had been considerably improved since the emergence of HIV. There was a need to identify infected people correctly, to follow up the course of immune reconstitution of patients by measuring viral load and CD4 cells, and to analyse drug escape mutations leading to drug resistance. Both the development of drugs and the refined diagnostics have been transferred to the treatment of patients infected with hepatitis B virus (HBV) and hepatitis C virus (HCV). This progress is not completed; there are beneficial aspects in the response of the scientific community to the HIV burden for the management of other viral diseases. These aspects are described in this contribution. Further aspects as handling a stigmatising disease, education of self-responsiveness within sexual relationships, and ways for confection of a protective vaccine are not covered.

Abstract:Traditional phenotypic assays used to assess the susceptibility of mutant human immunodeficiency virus type-1 (HIV-1) obtained from infected patients or from resistance selection to antiviral agents in cell culture are rather tedious and time consuming. To improve the efficiency of this process, a novel method was developed in which mutant viruses are captured with magnetic nano-beads and used to infect gagGFP reporter cells to evaluate the extent of resistance conferred by the mutant viruses against antiviral agents. The optimal timing for measuring the inhibitory potencies of antiviral agents was found to be day 3 post-infection for integrase strand transfer inhibitors and protease inhibitors and day 4 for non-nucleoside reverse transcriptase inhibitors. Comparable EC50 values were obtained when bead-captured breakthrough virus from in vitro resistance selection experiments and its matched site-directed mutagenesis virus were tested side by side in this assay. This assay protocol was also employed to evaluate the inhibitor susceptibility of breakthrough viruses collected from resistance selections that were conducted in the presence of increasing concentrations of an HIV-1 protease inhibitor. Taken together, these findings suggest that a rapid, sensitive, non-invasive, and homogeneous phenotypic assay has been developed for assessing the antiviral agent susceptibility of mutant viruses that emerge from in vitro resistance selection studies.

Abstract:Human immunodeficiency virus (HIV) is a lentivirus that leads to acquired immunodeficiency syndrome (AIDS). With increasing awareness of AIDS emerging as a global public health threat, different HIV testing kits have been developed to detect antibodies (Ab) directed toward different parts of HIV. A great limitation of these tests is that they can not detect HIV antibodies during early virus infection. Therefore, to overcome this challenge, a wide range of biosensors have been developed for early diagnosis of HIV infection. A significant amount of these studies have been focused on the application of nanomaterials for improving the sensitivity and accuracy of the sensing methods. Following an introduction into this field, a first section of this review covers the synthesis and applicability of such nanomaterials as metal nanoparticles (NPs), quantum dots (QDs), carbon-based nanomaterials and metal nanoclusters (NCs). A second larger section covers the latest developments concerning nanomaterial-based biosensors for HIV diagnosis, with paying a special attention to the determination of CD4+ cells as a hall mark of HIV infection, HIV gene, HIV p24 core protein, HIV p17 peptide, HIV-1 virus-like particles (VLPs) and HIV related enzymes, particularly those that are passed on from the virus to the CD4+ T lymphocytes and are necessary for viral reproduction within the host cell. These studies are described in detail along with their diverse principles/mechanisms (e.g. electrochemistry, fluorescence, electromagnetic-piezoelectric, surface plasmon resonance (SPR), surface enhanced Raman spectroscopy (SERS) and colorimetry). Despite the significant progress in HIV biosensing in the last years, there is a great need for the development of point-of-care (POC) technologies which are affordable, robust, easy to use, portable, and possessing sufficient quantitative accuracy to enable clinical decision making. In the final section, the focus is on the portable sensing devices as a new standard of POC and personalized diagnostics.

Abstract:BACKGROUND:People living with HIV (PLWH) and receiving antiretroviral therapy (ART) have a goal of achieving and maintaining viral suppression; however, the existence of PLWH that show events of low-level viremia (LLV) between 50 and 1000 copies/mL and with different virological consequences have been observed. Moreover, some reports indicate that LLV status can lead to residual immune activation and inflammation, leading to a higher occurrence of non-AIDS-defining events (nADEs) and other adverse clinical outcomes. Until now, however, published data have shown controversial results that hinder understanding of this phenomenon's actual cause(s) and origin(s). Integrase strand transfer inhibitors (INSTIs)-based therapies could lead to lower LLV over time and, therefore, more effective virological control. OBJECTIVES:This review aims to assess recent findings to provide a view of the clinical significance and management of low-level HIV viremia in the era of INSTIs.

Abstract:Human immunodeficiency virus-1 (HIV-1) mainly relies on host factors to complete its life cycle. Hence, it is very important to identify HIV-regulated host proteins. Proteomics is an excellent technique for this purpose because of its high throughput and sensitivity. In this review, we summarized current technological advances in proteomics, including general isobaric tags for relative and absolute quantitation (iTRAQ) and stable isotope labeling by amino acids in cell culture (SILAC), as well as subcellular proteomics and investigation of posttranslational modifications. Furthermore, we reviewed the applications of proteomics in the discovery of HIV-related diseases and HIV infection mechanisms. Proteins identified by proteomic studies might offer new avenues for the diagnosis and treatment of HIV infection and the related diseases.

Abstract:Diagnostics of the HIV Infection The clinical picture of an acute HIV infection resembles that of mononucleosis with lymphadenopathy, fatigue and fever. In this phase, the infection can be diagnosed with certainty only through direct virus detection. During the subsequent latent phase, recurring or serious progressive skin manifestations from different illnesses, prominent candidiasis of the oral cavity and community acquired pneumonia frequently occur. Ulcerations in atypical locations of the gastrointestinal tract could also indicate an HIV infection. For cases of clear lymphopenia, an HIV infection should be definitely considered. Above all, the presence of non-Hodgkin's lymphoma is characteristic of the complete clinical picture of AIDS. An appropriate diagnostic test (antibody test or detection of HIV) is urgently indicated in situations that carry a high risk for HIV transmission. This applies, above all, to patients whose partner is HIV positive, to patients who frequently change sex partners, to prostitutes and to intravenous drug users.

Abstract:While significant progress has been made since 2003 when a comprehensive treatment, prevention and control program was implemented to combat the HIV/AIDS epidemic in China, new challenges are emerging. There have been limited direct case reports on profiles of HIV/AIDS patients under care at unique clinical settings in China despite significant differences between clinics in this part of the world and those in Western countries. In this report, characteristics of HIV/AIDS patients managed during a 12-month period from June 2007 to May 2008 at a major medical school-affiliated AIDS clinic in the center of Beijing are described. Our data confirm an alarming trend of increased rates of sexual transmission of HIV-1 in recent years and suggest that major improvements are needed for both the type of antiviral treatments being used and post-treatment monitoring of viral load. This information will be useful for the continued progress in the clinical management of HIV/AIDS patients in China. A human immunodeficiency virus-seronegative acquired immunodeficiency syndrome patient with opportunistic infections: A case report

Abstract:Background: In rare cases, people living with chronic human immunodeficiency virus (HIV) infection do not develop antibodies despite demonstrable infection. Delayed or missed diagnosis of HIV infection leads to a lack of timely therapy, resulting in rapid disease progression with opportunistic infections or malignancies. Case report: A 44-year-old Chinese man presented with sore throat, oral leukoplakia, fever, dyspnoea and diffuse ground glass-like lesions in both lungs. Serum cytomegalovirus DNA was detectable, and CD4+ T-cell count was low. The patient was suspected of being a person living with HIV despite of the repeatedly negative HIV antibody tests using enzyme-linked immunsorbent assay and Western blot. Subsequently, high-plasma HIV RNA viral load was found on two repeated tests, while HIV DNA was also positive. Thus, the patient was confirmed as presenting with HIV-seronegative acquired immunodeficiency syndrome (AIDS). The symptoms improved in response to effective anti-fungal and anti-retroviral therapy after diagnosis. Conclusion: This is the third reported case of an HIV-seronegative AIDS patient in China, which are also rarely reported globally. HIV nucleic acid testing is important to screen out HIV infection, especially in those who present with severe immunodeficiency but remain HIV serogenative.
New progress in antiretroviral therapy for HIV-1/AIDS

Abstract:HIV-1/AIDS has been transformed from a fatal disease to a treatable chronic one due to the implementation and wide use of antiretroviral therapy (ART). New progress has been made in ART in recent years along with the advances in clinical and basic research for HIV-1/AIDS, and many new ideas on the timing, regimens and follow-up of ART, and ART for special populations and patients with coinfections have been suggested. Nowadays people agree that all AIDS patients should receive ART as early as possible while some new choices of ART for HIV-1/AIDS can be available for patients with the marketing of some new antiviral drugs. The advances in the clinical research, especially a large-scale randomized clinical trial results, will constantly update and optimize ART regimens, and continuously improve the clinical curative effect of ART, which will make it possible to cure HIV-1/AIDS. Currently, the most realistic way to improve prognosis of patients with AIDS is to provide a high quality of ART for HIV-1/AIDS patients.

Abstract:More than 2 decades of developments in the field have led to a wide variety of test kits available for addressing the different needs of surveillance, diagnosis, and monitoring of human immunodeficiency virus (HIV) infection and antiretroviral therapy (20, 23). These test kits are based on different tech- nologies, including immunoassays and molecular testing. Typ- ically, enzyme-linked immunosorbent assays (ELISAs) and rapid tests for detecting anti-HIV antibodies are used for blood screening and/or surveillance purposes. The former in- clude third-generation ELISAs that utilize the antigen sand- wich design for improved sensitivity, while the latter term re- fers to immunochromatographic tests or other simple antibody tests requiring less than 30 min of assay time. Since the viral capsid (core) p24 protein can be detected earlier than anti- HIV antibodies (93), attempts had also been made to develop a new generation of ELISA capable of simultaneously detect- ing the p24 antigen and anti-HIV antibodies (34, 52, 76, 101, 103). Significant reductions of the window period were achieved, which is ideal for blood screening and surveillance (102), but users were also advised to be mindful of possible limitations when utilizing such a combination assay (52, 81, 83). With specific panels, a delay in detection by a fourth- generation ELISA was evident compared with a dedicated p24 antigen assay (81). This shortcoming of fourth-generation ELISAs is understandable, because the surface area of the solid phase for detection is constant and limited regardless of whether it is utilized for a single test or combined assays (101). It should also be noted that an additional procedure for the dissociation of immune complexes employed by dedicated p24 antigen assays but not by combined assays was found to facil- itate sensitive detection (85). However, this does not mean that using ELISA dedicated for detecting p24 antigen has no short- comings. The viral p24 protein is a transient marker that can fall to an undetectable level after initial acute infection and only reemerges at the advanced stage of AIDS in patients (93, 107). Abstract:Etiology, transmission and protection: Transmission of HIV, the causative agent of AIDS, occurs predominantly through bodily fluids. Factors that significantly alter the risk of HIV transmission include male circumcision, condom use, high viral load, and the presence of other sexually transmitted diseases. Pathology/Symptomatology: HIV infects preferentially CD4+ T lymphocytes, and Monocytes. Because of their central role in regulating the immune response, depletion of CD4+ T cells renders the infected individual incapable of adequately responding to microorganisms otherwise inconsequential. Epidemiology, incidence and prevalence: New HIV infections affect predominantly young heterosexual women and homosexual men. While the mortality rates of AIDS related causes have decreased globally in recent years due to the use of highly active antiretroviral therapy (HAART) treatment, a vaccine remains an elusive goal. Treatment and curability: For those afflicted HIV infection remains a serious illness. Nonetheless, the use of advanced therapeutics have transformed a dire scenario into a chronic condition with near average life spans. When to apply those remedies appears to be as important as the remedies themselves. The high rate of HIV replication and the ability to generate variants are central to the viral survival strategy and major barriers to be overcome. Molecular mechanisms of infection: In this review, we assemble new details on the molecular events from the attachment of the virus, to the assembly and release of the viral progeny. Yet, much remains to be learned as understanding of the molecular mechanisms used in viral replication and the measures engaged in the evasion of immune surveillance will be important to develop effective interventions to address the global HIV pandemic.
A New Pattern-Based Method for Identifying Recent HIV-1 Infections from the Viral Env Sequence

Abstract:The long asymptomatic stage of HIV infection poses a great challenge in identifying recent HIV infections. This is a bottleneck for monitoring HIV epidemic trends and evaluating the effectiveness of national AIDS control programs. Several serological methods were used to address this issue with some success. Because of high false-positive rates in patients with advanced infection or in ART treatment, UNAIDS still hesitates to recommend their use in routine surveillance. We developed a new pattern-based method for measuring intra-patient viral genetic diversity for determination of recent infections and estimation of population incidence. This method is verified by using several datasets (424 subtype B and 77 CRF07_BC samples) with clearly identified HIV-1 infection times. Pattern-based diversities of recent infections are significantly lower than that of chronic ones. With larger window periods varying from 200 to 350 days, a higher accuracy (90%–95%) not affected by advanced disease nor ART treatment could be obtained. The pattern-based genetic method is supplementary to the existing serology-based assays, both of which could be suitable for use in low and high epidemic regions, respectively.

A More Comprehensive Clinical and Laboratory Characterization of 61 Acute HIV Infection Patients in Southwest China

Abstract:Acute HIV infection (AHI), i.e., the early stage of HIV infection, plays an important role in immune system failure and HIV transmission, but most AHI patients are missed due to their non-specific symptoms. To facilitate the identification of patients with high AHI risk and reduction of missed diagnosis, we characterized 61 AHI patients in a Southwest China hospital with 4300 beds; specifically, we characterized their general clinical characteristics, evolution in results of a novel HIV screening assay called Elecsys® HIV Duo, and by programming, we analyzed the ability of all routine laboratory tests (e.g., routine blood analysis) to identify AHI patients. Among 61 AHI patients, 85.2% were male and the median age was 42 (interquartile range, 25–62) years. A total of 61.9% of patients visit the emergency department first during AHI. Clinical presentation of AHI patients included fever, fatigue, chills, rash, and various respiratory, digestive, and nervous system symptoms. Two of three results from Elecsys® HIV Duo show clear evolution trends: HIV P24 antigen decreased while HIV antibody increased in consecutive samples of nearly all patients. High fluorescence lymphocytes have a very high positive likelihood ratio (LR+) of 10.33 and a relatively high "rate of out-of-range tests" of 56.8% (21 in 37 patients who received this test had a result outside the reference range). In addition, we identified more than ten tests with LR+ greater than two. In summary, the emergency department is important for AHI screening. The evolution of HIV P24 Ag and HIV Ab and those laboratory tests with a high "rate of out-of-range tests" or high LR+ may aid the AHI identification and missed diagnosis reduction.

Abstract:In recent years, great achievements have been made in the therapy of viral hepatitis B and viral hepatitis C, as majority of hepatitis C patients can be clinically cured.Though current antiviral therapy is still unable to eradicate hepatitis B virus in infected hepatocyte and few patients could achieve HBsAg loss or seroconversion, end-stage liver disease like cirrhosis, liver failure and hepatocellular carcinoma have been dramatically prevented.The advances in treatments have prompted the progress in laboratory diagnosis of viral hepatitis.Here we review the progress in the field.

Abstract:Early diagnosis of HIV infection decreases the time from HIV diagnosis to viral suppression and reduces further HIV transmission. The Chinese Guidelines for the Diagnosis and Treatment of HIV/AIDS (2021 edition) state that an HIV RNA level > 5,000 copies/mL is the threshold for diagnosing HIV infection. The impact of low viral load values on HIV diagnosis needs to be investigated.
