Abstract:Objective Interstitial lung disease in children represents a heterogeneous group of disorders of both known and unknown causes.This study aimed to understand better the causes of the disease in children and to provide information on the current approach to diagnosis and management of the disease.Method Through the Pediatric Diffuse Parenchymal Lung Disease/Pediatric Interstitial Lung Disease Cooperative Group of China,data of 93 cases of interstitial lung disease of children from 11 hospitals werecollected with the same questionnaire in 2009.Respiratory tract secretions were obtained for bacterial culture.Respiratory virus antigen examination,mycoplasma antibody,EB virus,cytomegalovirus,and herpes simplex viruses antibody detection were performed.Cells in the sputum,gastric juice and bronchoalveolar lavage fluid(BALF) were tested for hemosiderin.The CT or high resolution CT (HRCT) of the lung and blood- gas analysis were also performed.Fourteen cases underwent lung biopsy and 25 cases underwent bronchomicrocopy.Data were then pooled and discussed through a series of meetings.Result Fifty-three cases were male,40 were female and their age ranged from 8 months to 14 years.Thirty-nine cases were diagnosed as bronchiolitis obliterans (BO) ;39 as idiopathic pulmonary hemosiderosis (IPH) ;7 as idiopathic interstitial pneumonia (IIP) of unknown causes,of whom 4 cases had non specific interstitial pneumonia,1 case as acute interstitial pneumonia and 1 case as lymphocytic interstitial pneumonia,1 case as idiopathic pulmonary fibrosis;2 cases as secondary interstitial lung disease,one was secondary to SLE,one to human immunodeficiency virus (HIV) infection; 2 cases had hypersensitive pneumonitis; 2 cases had pulmonary alveolar proteinosis; 1 case had bronchiolitis obliterans organizing pneumonia; 1 case had lipoid pneumonia ; 1 case of diffuse panbronchiolitis; 1 case of microlithiasis alveolaris pulmonum.Forty two cases had cough,24 of them also had tachypnea,8 cases had clubbing.HRCT showed that 56 cases had groundglass-like opacification;30 cases had mosaic appearance;1case had diffuse micronodular opacities,1 case had diffuse reticulonodular opacities and cysts.Conclusion Interstitial lung disease in children is a heterogeneous group of disorders.In this study diagnoses of 11 diseases were made,the top third diagnoses were BO,IPH and IIP,respectively.

Abstract:Abstract BackgroundMutation in the surfactant protein C gene (SFTPC) is a cause of interstitial lung disease (ILD). Our objective was to investigate the clinical characteristics, outcome and influencing factors of ILD in Chinese children with SFTPC mutations.MethodA total of 8 Chinese children with ILD heterozygous for SFTPC mutations that were treated in our hospital from January 2014 to December 2020 were included in our study. Candidate genes responsible for surfactant dysfunction were sequenced by next-generation sequencing. The clinical and genetic data were reviewed retrospectively.ResultsThe children’s onset age was before the age of 2 years, and one case was just after birth. The most significant clinical manifestations were cough, tachypnea, hypoxemia and failure to thrive. The most common mutation was p. lle73Thr, which accounted for 87.5% (7/8) of our patients. Four patients whose onset was within 3 months, including 3 children with CMV infection, died. Conclusionp. lle73Thr mutation of SFTPC was an important and common cause of ILD in the Chinese children. The clinical manifestations of ILD associated with this mutation are not specific. The severity and outcome of the disease may be affected by factors such as onset age and viral infection.

Abstract:Purpose: To assess the high-resolution computed tomography (HRCT) features of childhood interstitial lung disease (ChILD) and the differences from adult interstitial lung disease. Methods and Materials: 14 patients with ChILD proven histopathologically were enrolled in the current study. Among them, there were 10 boys and 4 girls, aged from 1y3m to 13y1m (the median age was 6y). The final diagnoses of this series included nonspecific interstitial pneumonia, NSIP (NSIP) (n=6), lymphocytic interstitial pneumonia (LIP) (n=3), pulmonary alveolar proteinosis (PAP) (n=2), respiratory bronchiolitis interstitial lung disease (RB-ILD) (n=1), acute interstitial pneumonia (AIP, histopathologic pattern as known as diffuse alveolar damage) (n=1), and Eosinophilic Pneumonia (EP) (n=1). Results: The most common HRCT patterns of NSIP included reticulation, ground-glass opacity (GGO) and peribronchiolar thickness (6/6, 100%). Nodules were always present on the HRCT of children with LIP (3/3, 100%). In the two PAP patients, there was the specific sign, named ”crazy-paving sign”, (2/2, 100%) could be found. The nodules with hazy margin in EP were different from that in LIP. Nodule, reticulation and GGO were present in the boy with AIP. The reticulation, GGO, consolidation and peribronchiolar thickening were present in RB-ILD patient. By the analysis with logistic regression, the nodule was regarded as the specific negativepattern for NSIP (P=0.047＜0.05, OR=-2.71) in our study. There was no other elementary lesion was considered as specific pattern of any subset of ChILD. Conclusions: HRCT scanning is the primary imaging modality for ChILD. HRCT findings contribute to detection, evaluation of pulmonary damage prognosis and screening for complications. So far, however, there is no sign would be identified as the specific pattern for any subsets of childhood interstitial lung disease.

Abstract:Objectives To enhance the recognization of interstitial lung disease(ILD)associated with connective tissue disease(CTD)in children in order to diagnose early,treat timely,and improve therapeutical effect. Methods Clinical data of ILD associated with CTD (Henoch-Schonlein purpura HSP,systemic lupus erythematosus SLE,juvenile idiopathic athritis So-JIA,Weber-Christian disease)were analyzed in 7 children admitted during Dec. 2005 to Apr. 2007,and relevant literatures were reviewed. Results ILD was pointed out by high resolution computer tomography (HRCT)in all 7 children although no obvious respiratory symptoms were presented in these cases. There were 4 cases with shortness of breath after activity,2 cases with agnogenic increased heart rates,and other 2 cases with decreased PaO2,respectively. No special changes of chest X-ray were observed in all 7 cases with ILD. After the impulse therapy of high-dose methylprednisolone combinated with CTX,symptoms were quickly alleviated in 2 cases (with SLE or So-JIA)of 7 patinets with ILD,and their HRCT became normal or improved after 2 -3 weeks. Conclusions It is usually no respiratory symptoms in children with ILD associated with CTD at early. The occurrence of ILD should be considered if there is agnogenic increased heart rates after the parimary diseases is controlled. HRCT is sensitive to ILD. The most common therapeutic approaches for ILD associated with CTD involves the use of oral steroids combination with immunosuppressive drugs(cyclophosphamide,azathioprine and cyclosporin). If ILD is presented with acute course,it will be feasible to use the impulse therapy of high-dose methylprednisolone combinated with CTX. High-dose intravenous injection of gamma globulin can also be given as an auxiliary therapy. However,infection must be excluded in children with acute ILD.

Abstract:Systemic juvenile idiopathic arthritis(sJIA) is one of the most serious critical illnesses in childhood, characterized by high fever, recurrent rash, and arthritis, etc.Children with sJIA associated-lung disease(sJIA-LD) are more severely ill and have a worse prognosis, the correlation between the mechanism and age, disease activity, anti-rheumatic drug therapy, applications of biologics, infection and other factors is worth exploring.This article reviews the research progress on the mechanism, risk factors, treatment methods and prognosis of sJIA-LD, so as to provide a theoretical basis for improving the diagnosis and treatment of sJIA and improving the prognosis.

Abstract:Objective:To investigate the clinical manifestations, characteristics of chest high-resolution computed tomography (HRCT), and prognosis of connective tissue disease (CTD) complicated with interstitial lung disease (ILD) in children.Methods:The clinical data of 53 children with CTD-ILD who were admitted to the Department of Rheumatology and Immunology, Affiliated Xi′an Children′s Hospital of Xi′an Jiaotong University from October 2013 to October 2019 were retrospectively analyzed, including clinical manifestations, blood gas analysis, chest HRCT and prognosis.Results:As for these 53 children with CTD-ILD, the ratio of male to female was 1.0∶1.4, the average age was (7.50±3.34) years, and the course of disease was 2.00 (0.85, 7.50) months.Among them, there were 25 cases (47.2%) of juvenile idiopathic arthritis (JIA), 15 cases (28.3%) of systemic lupus erythematosus (SLE), 11 cases of polymyositis / dermatomyositis (PM/DM) (20.7%), 1 case of overlap syndrome (OS) (1.9%), and 1 case of allergic granulomatosis with polyangiitis (AGPA) (1.9%). Although cough (39.6%) was the most common symptom of respiratory system in these children with CTD-ILD and fever(66.0%) was the most common symptom in the systemic features.Blood gas analysis appeared abnormal in 17 cases, including 10 cases of hypoxemia (18.9%) and 7 cases of type Ⅰ respiratory failure (13.2%). HRCT chest showed ground glass shadow, strip shadow, subpleural spot shadow, grid shadow, pleural thickening, consolidation shadow, nodular shadow and cystic low-density shadow, with the proportion of 52.8%, 26.4%, 22.6%, 18.9%, 11.3%, 7.5%, 1.9% and 1.9%, respectively; nonspecific interstitial pneumonia (NSIP)(39.6%) was the most common type of imaging classification.After the combined treatment with glucocorticoids, immunosuppressive agents and biological agents, HRCT chest showed remarkably improvement in 36 cases (67.9%), while no change in 8 cases (15.1%). A total of 75.0%(33 cases) of 44 cases were infected in the course of combined treatment.In addition, 9 cases (17.0%) died from acute respiratory distress syndrome (ARDS), among which 4 cases exacerbated to rapid progressive luge disease and 5 cases aggravated secondary ARDS due to infection.Conclusions:Only a small number of children with CTD-ILD have respiratory symptoms and signs.HRCT chest contributes to the early diagnosis of CTD-ILD, and its imaging manifestations are diverse.Blood gas analysis and HRCT chest play an important role in the disease evaluation and treatment planning.Moreover, it is the direction for further research to develop effective methods to prevent and control secondary infection so as to improve the survival rate and reduce the mortality rate during the active treatment of primary diseases.

Abstract:Objective To investigate the prevalence and clinical characters of systemic lupus erythematosus-interstitial lung disease(SLE-ILD)in children.Methods Fifty-two children with systemic lupus erythematosus(SLE)were studied by radiologic examinations including chest X radiographs and chest high resolution computed tomography(HRCT)that simultaneously read and described by more than two radiologists.Also,correlation factors with clinical manifestation and serologic examination were analyzed.Results The incidence of SLE-ILD was 26.9%,chest HRCT was more sensitive than chest radiography;According to clinical characters,chest plain radiographs and HRCT,we classified SLE-ILD into:preclinical(n=12,85.7%),acute pneumonitis(n=2,14.3%)and not found the patient of chronic interstitial disease;The incidence of oromeningitis in SLE-ILD obviously increased,but renal damages decreased significantly.The positive percentages of anti-SSA antibodies in SLE-ILD children were higher than those in non-ILD.Conclusions SLE-ILD in children is frequent.SLE-ILD is inclined to appear in those patients who are combined with oromeningitis,not have renal damages and anti-SSA antibo-dies positive.Chest HRCT is important in finding early ILD.The clinical classification may provide some help for the treatment and follow-up study.

Abstract:SESSION TITLE: Diffuse Lung Disease: Connective Tissue Disease SESSION TYPE: Original Investigation Poster PRESENTED ON: Saturday, April 16, 2016 at 11:45 AM - 12:45 PM PURPOSE: To analyze the clinical characteristics of connective tissue disease- associated interstitial lung disease (CTD-ILD) in Chinese patients. METHODS: The medical records of patients diagnosed with ILD and treated in our hospital from January 1999 to January 2013 were reviewed. From the records, patients who were also diagnosed with CTD were identified and their records of follow-up examinations for a minimum of 12 months till the end of December 2013 were reviewed. RESULTS: Of the 2678 patients diagnosed with ILD, 1798 (67%) were identified as CTD-ILD; 299 (11.2%) had idiopathic pulmonary fibrosis (IPF). Complete clinical data were available from 1044 patients with CTD-ILD and 178 patients with IPF. We found that 32% of the 1044 patients with CTD-ILD were not diagnosed accurately at the initial hospital admission; 19% showed persistent negative test results for autoantibodies; 25% had negative autoantibodies at initial hospital admission and then became positive at follow-up examinations. Of the 288 patients with confirmed CTD-ILD, 14% showed pulmonary symptoms as the initial clinical manifestation (PSIM) and 86% showed extra-pulmonary symptoms as the initial clinical manifestation (EPSIM). Of the 756 patients with undifferentiated CTD-ILD, the proportion of PSIM and EPSIM were 44% and 56%, respectively. CONCLUSIONS: Patients with CTD-ILD are not diagnosed accurately at the initial hospital admission possibly because of negative serologic test results for autoantibodies and absence of obvious extra-pulmonary symptoms. CLINICAL IMPLICATIONS: Thus, patients with ILD should be examined for extra-pulmonary symptoms and tested for autoantibodies at follow-up examinations. Abstract:Connective tissue diseases are a heterogeneous group of autoimmune diseases that can affect a variety of organ systems. Lung parenchymal involvement is an important contributor to morbidity and mortality in children with connective tissue disease. Connective tissue disease-associated lung disease in children often manifests as one of several radiologic-pathologic patterns of disease, with certain patterns having a propensity to occur in association with certain connective tissue diseases. In this article, key clinical, histopathologic, and computed tomography (CT) features of typical patterns of connective tissue disease-associated lung disease in children are reviewed, with an emphasis on radiologic-pathologic correlation, to improve recognition of these patterns of lung disease at CT and to empower the pediatric radiologist to more fully contribute to the care of pediatric patients with these conditions.

Abstract:Objective To explore the importance of lung biopsy in diagnosis of interstitial lung disease.Methods Two cases of interstitial lung disease in children,diagnosed by pathological examination,were reviewed and analyzed retrospectively.Their clinical features,chest radiology and pathology were studied.The final diagnosis was made based on the principle of clinico-radiologic-pathologic diagnosis(C-R-P).Results Two patients presented with cough and shortness of breath,no clubbing of fingers and toes observed.Ground-glass opacities and vesicles shadow were presented under high resolution CT.Neither the evidence of microbial infection nor connective tissue disease was found.In this case,lung biopsies were needed to identify the causes of the infection.The final diagnoses were made after lung biopsy that one child was lipid pneumonia and another one was desquamative interstitial pneumonia.Conclusions The respiratory symptoms of interstitial lung disease in children include cough,wheezing and shortness of breath.For those children who do not show obvious effect after conventional treatment physician should consider the possibility of interstitial lung disease.

Abstract:Objective To analyze the clinical features of connective tissue disease associated interstitial lung disease (CTD-ILD). Methods The clinical manifestation of 305 patients diagnosed CTD-ILD and chest high resolu-tion computed tomography features of rheumatoid arthritis associated interstitial lung disease ( RA-ILD) and Sjogren's syndrome associated interstitial lung disease ( pSS-ILD) were retrospectively analyzed. Results The total incidence of ILD in CTD was 11. 78%. The highest incidence of ILD was polymyositis ( 53. 13%) , then was anti-neutrophil cytoplasmic antibody associated vasculitis (40. 74%), mixed connective tissue disease (35. 14%) and scleroderma (29. 73%). pSS-ILD and RA-ILD were the major part of the total CTD-ILD with the proportion of 24. 92% and 23. 61% respectively and almost occupied the half of all CTD-ILD patients. Conclusion The incidence of CTD-ILD is very high. pSS-ILD and RA-ILD are the main diseases of CTD-ILD, and the early screening and treatment is very important.

Abstract:Objective:To explore the clinical features and risk factors of systemic lupus erythematosus(SLE) concomitant with interstitial lung disease(ILD) in children.Methods:A retrospective analysis was performed.A total of 111 hospitalized children diagnosed with SLE in the Department of Rheumatology and Immunology, Children′s Hospital of Soochow University from February 2016 to November 2018 were selected as the research subjects and divided into the SLE-ILD group(18 cases) and the SLE-non-ILD group(93 cases)according to the lung high-resolution CT manifestations. T-test and Wilcoxon rank sum test were used to compare and analyze the general situation, clinical manifestations and laboratory results.Multivariate Logistic regression was used to analyze the risk factors of SLE-ILD. Results:The prevalence of SLE-ILD was 16.2%(18/111 cases). There were significant differences between the SLE-ILD group and the SLE-non-ILD group in the course of disease [14.00 (12.00-24.25) months vs.1.00(1.00-2.00) months], the incidence of serositis [55.6%(10/18 cases) vs.8.6%(8/93 cases)], post-activity shortness of breath [83.3%(15/18 cases) vs.25.8%(24/93 cases)], nervous system damage [27.8%(5/18 cases) vs.6.5%(6/93 cases)], cardiovascular system damage [38.9%(7/18 cases) vs.9.7%(9/93 cases)], the occu-rrence of increased erythrocyte sedimentation rate [66.7%(12/18 cases) vs.31.2%(29/93 cases)], the decreased C 3[88.9%(16/18 cases) vs.62.4%(58/93 cases)], positive anti neutrophil cytoplasmic antibody (ANCA) [88.9%(16/18 cases) vs.18.3%(17/93 cases)], positive anti-Sm antibody [61.1%(11/18 cases) vs.15.1%(14/93 cases)] and anti ribonucleoprotein antibody (anti RNP antibody)[66.7%(12/18 cases) vs.16.1%(15/93 cases)](all P<0.05). Logistic regression analysis demonstrated that serositis( OR=30.535, 95% CI: 2.167-430.336, P=0.011), shortness of breath after exercise( OR=55.115, 95% CI: 1.117-2 579.852, P=0.041), positive ANCA( OR=65.090, 95% CI: 4.488-944.071, P=0.002) and positive anti-RNP antibody( OR=10.007, 95% CI: 1.362-73.500, P=0.024) were risk factors for SLE-ILD. Conclusions:The longer the course of SLE, the higher the incidence of ILD; serositis, shortness of breath after exercise, positive ANCA and positive anti RNP antibody may be risk factors for SLE-ILD.
Analysis of Pulmonary Function in Children with Connective Tissue Disease

Abstract:Objective To expore the pulmonary function in children with connective tissue disease(CTD),and evaluate lung complication and prognosis of children with CTD.Methods Thirty-five children with CTD(male 16 cases,female 19 cases) who visited children′s hospital of Fudan university during Oct.2006 to Jul.2007 and had non-respiratory symptoms were selected.The pulmonary function of them were measured by Master-Screen Paed and the data were compared with 32 children as the healthy control group(male 13 cases,female 19 cases).The data were analyzed by STATA 7.0 software.Results The abnormality ratio of lung functions were 57.14%,17 cases(48.57%)children with CTD displayed restrictive ventilatory disorder,9 cases(25.71%)children with CTD displayed dropped function of small airway,2 cases(5.71%)children displayed dominant restrictive ventilatory disorder and coexisted with obstructive ventilatory disorder.The parameters of pulmonary ventilation such as maximal vital capacity,forced vital capacity,forced expiratory volume in 1 second,in CTD group were significantly lower than those in healthy control group[(77.66±20.22)% vs(94.39±11.27)%;(79.05±21.45)% vs(94.89±11.46)%;(84.91±23.29)% vs(103.62±12.96)% Pa 0.01].The parameters of small airway function such as forced expiratory flow after exhaling 50% of vital capacity,forced expiratory flow after exhaling 75% of vital capacity in CTD group were significantly lower than those in healthy control group [(85.82±31.02)% vs(102.82±27.32)%;(74.52±32.59)% vs(98.75±40.59)% Pa0.05].There were no significant differences about the parameters of airway resistance(total airway resistance,center airway resistance,peripheral elastic resistance,resonance frequency) between 2 groups(Pa0.05).Conclusions The most characteristic pulmonary function changes in children with connective tissue disease are restrictive ventilatory disorder with some disorder in small airway function.Pulmonary function test shall become routine test in children with connective tissue disease.

Abstract:Objective: Childhood interstitial lung diseases (chILDs) are rare, heterogeneous group of disorder. The exact role of inflammation and underlying immune defects in their etiology unclear. Most data derived from in vitro or adult studies, with a noticeable absence of clinical studies in children. Innate lymphoid cells (ILCs) are recognized for their crucial role in lung immunity, extensively studied in asthma but not in chILD. Methods: Prospective study, including 20 children with chILD and 19 age-matched controls. Peripheral blood mononuclear cells were separated and analyzed via the flow cytometry method, measuring T regulatory cells (anti-CD4, anti-CD25, anti-CD127), IL-10, and TGFβ within the Treg cell. ILCs 1, 2, and 3 were characterized using anti-CRTH2, anti-CD161, anti-c-Kit, and anti-NKp44 antibodies. T helper cell-17 (TH-17) cells were identified using anti-CD4, anti-CCR6, and anti-IL17A antibodies. Results: In peripheral blood, no significant differences were observed in Treg, IL-10, TGFβ, ILCs, TH-17, and IL-17 between the patient and control. However, subgroup analysis revealed a significantly lower ILC2 value in the chILD subgroup than in the control (p=0.037). No significant clinical correlations were found between clinical characteristics, and the investigated immunological parameters. Conclusion: This study provides the first comprehensive analysis of extended inflammatory parameters in chILD, including ILCs, found ILC2 lower. Further studies are necessary to comprehend the role of ILCs, how diagnostic and therapeutic interventions may impact the ILCs'function. Additional studies in bronchoalveolar lavage are necessary to elucidate this relationship.

Abstract:The aim of this study is to investigate the clinical potential of immune microenvironment in peripheral blood for the severity and therapeutic efficacy of Langerhans cell histiocytosis (LCH). A total of 200 newly diagnosed children with LCH during 10 years was enrolled for analysis in this study. Peripheral blood samples were acquired from patients before treatment in our hospital and immune indicators were detected by a four-color flow cytometer. The levels of CD3 + CD8 + T cell, CD3 + CD4 + HLA-DR + T cell, CD3 + CD8 + HLA-DR + T cell, IL 4, IL-6, IL-10 and IFN-gamma in peripheral blood were markedly elevated in LCH patients vs. healthy controls. Patients with multiple system with risk organ involvement (MS-RO + ) exhibited higher levels in IL-6, IL-10 and IFN-gamma, CD3 + CD4 + HLA-DR + T cell and CD3 + CD8 + HLA-DR + T cell, compared to those in patients without risk organ involvement (RO-). Patients who responded effectively to initial chemotherapy showed significantly lower levels of IL-4, IL-10, IFN-gamma, CD3 + CD4 + HLA-DR + T cell and CD3 + CD8 + HLA-DR + T cell in peripheral blood, compared to those in patients who did not respond to initial chemotherapy. Furthermore, univariate analyses were performed that the higher levels of CD3 + CD4 + HLA-DR + T cells, CD3 + CD8 + HLA-DR + T cells and IL-10 in peripheral blood were related to non-response in LCH after initial chemotherapy. Immune microenvironment in peripheral blood may be associated with the severity and treatment response of LCH. The levels of CD3 + CD4 + HLA-DR + T cells, CD3 + CD8 + HLA-DR + T cells and IL-10 may be biomarkers to predict treatment response of LCH patients.
Clinical manifestations of connective tissue diseases complicated with interstitial lung disease:an analysis of 90 cases

Abstract:Objective To strengthen the understanding and knowledge of connective tissue diseases(CTD) complicated with interstitial lung disease(ILD) by reviewing the clinical manifestations,blood gas analysis,and pulmonary function data of patients.Methods The clinical manifestations,blood gas analysis,and pulmonary function data of 90 patients with CTD-ILD,who were treated from January 1997 to December 2007 in the in-patient department,were retrospectively analyzed.Results Thirty-nine patients were male and 51 were female,with an age range from 11 to 87 years old(average [62±15] years).The CTD included 13 cases with rheumatoid arthritis(RA),2 with systemic sclerosis(SSc),9 with dermatomyositis(DM)/polymyositis(PM),8 with sjogren syndrome(SS),6 with systemic lupus erythematosus(SLE),9 with overlap syndrome(OS),10 with undifferentiated connectived tissue disease(UD-CTD),and 33 with antineutrophil cytoplasmic antibody(ANCA) associated systemic vasculitis(AASV).The incidences of respiratory symptoms,such as cough,dyspnea,and haemoptysis,were higher than those of general symptoms,such as fever,fatigue,arthralgia and skin lesion.Among the 81 cases who received blood gas analysis,31 had hypoxemia,16 had respiratory failure type Ⅰ,and 2 had respiratory failure type Ⅱ.Among 45 cases who received pulmonary function test,25 had restrictive,2 had obstructive and 4 had mixed ventilation defect.Reduced diffusing capacity(DLCO decreased) was observed in 39 cases.Conclusion Patients with CTD-ILD have higher incidences of respiratory symptoms and lower incidences of general symptoms.The clinical manifestations of CTD may not be prominent when there are interstitial lung diseases;therefore the possibility of ILD second to CTD should be considered in these patients.The characteristics of pulmonary function of patients with CTD-ILD are mainly restrictive ventilation defect and reduced diffusing capacity.When combined with obstructive ventilation defects,the possibility of bronchiolitis should be considered.
