Abstract:Emergency and critical care medicine are specialties facing life-threatening emergencies such as stroke, acute myocardial infarction, and severe trauma. The management of these critical illnesses requires rapid responses and accurate triage. However, most practices in emergency room and intensive care unit (ICU) lack high-level evidence. As a result, most of the practices thought to be true in the present may be proven to be wrong decades later.

Abstract:Emergency medicine as an independent subject has developed for nearly 20 years in China and achieved satisfying progress in the process of exploring,controversy and using the international experience for reference.We have introduced up-to-date technologies and programs into emergency department of our hospital,presented fresh image in teaching and research and cultivated new pattern abilities of emergency medicine.Although current emergency medicine has entered a new development stage,its growth is unbalanced.This article discusses whether emergency medicine is common department or special department,branch subject or integrated subject,as well as probing into other aspects,such as its creation and conformity,its preclinic and specialty,first aid and disease prevention,confusion and future prospect.

Quantitative Imaging in Medicine and Surgery

Abstract:Following tremendous economic progress, society in China is also undergoing fundamental changes, as is the healthcare system. Currently the training of Chinese young doctors and their future work placement are all undergoing re-structuring. We compiled some thoughts and opinions on the topic of 'should clinicians in China engage in research?', and publish them as a special report in this issue of Quantitative Imaging in Medicine and Surgery (QIMS). The contributors included some editorial members of this journal, and a few personal friends. Besides a few minor linguistic corrections, opinions from the contributors have not been edited, as we want authors' to write their own independent views. However, it is possible there is a selection bias of the contributors of this paper; more likely those who are interested in the medical research are selected and therefore the views of the contributors may not be generalizable. To compare the structure and funding of China with other countries, authors from UK, The Netherlands, France, and USA are also invited.

Abstract:The medical specialty of critical care, or intensive care, provides emergency medical care to patients suffering from life-threatening complications and injuries. The medical specialty is featured by the generation of a huge amount of high-granularity data in routine practice. Currently, these data are well archived in the hospital information system for the primary purpose of routine clinical practice. However, data scientists have noticed that in-depth mining of such big data may provide insights into the pathophysiology of underlying diseases and healthcare practices. There have been several openly accessible critical care databases being established, which have generated hundreds of scientific outputs published in scientific journals. However, such work is still in its infancy in China. China is a large country with a huge patient population, contributing to the generation of large healthcare databases in hospitals. In this data descriptor article, we report the establishment of an openly accessible critical care database generated from the hospital information system.

International Collaboration in Taiwan Emergency Department Publications: A Social Network Analysis

Abstract:Background: Emergency medicine (EM) is a growing specialty both clinically and academically. Academic EM development can be measured by number of scientific publications. This study aimed to evaluate the academic international cooperation trend of Taiwan emergency departments (EDs) in the past two decades using social network analysis (SNA). Methods: The study population were publications with first author affiliated with Taiwan EDs and the study duration was publication year before 2021. The enrolled publications were categorized into two groups: Group one, all authors were affiliated with Taiwan (domestic publications) and Group two, authors were affiliated with Taiwan and other countries (international publications). The primary outcome measurement was the degree centrality of Taiwan before 2021. The secondary outcome measurements included the trend in annual publication number of Group one and Group two, the trend in country number of each year in Group two, the top five countries that collaborate with Taiwan, the difference between the median of citation numbers in Group one and Group two, and the difference between the median of author numbers in Group one and Group two. Results: A total 4,363 publications were enrolled, of which 4,046 publications were classified in Group one and 317 publications were in Group two. The annual publication number of both groups increased significantly. The annual country number of collaboration with Taiwan ED publications had also significantly increased. The median of citation number and author number in Group two were both significantly higher than Group one. The top five countries collaborating with Taiwan were the United States, China, Malaysia, Japan, and Australia. Conclusions: Taiwan EDs' growing international collaboration in the past two decades indicated a capacity to conduct multi-country research. International collaboration publications obtained higher citations compared to domestic publications. Researchers should enhance international collaborations for academic advancement.
Societies for Rejuvenating Asia and Sino-Japanese Non-Governmental Alliances, 1898—1900

Abstract:The Shanghai Asian Association, established in 1898, was the product of a Sino-Japanese non-governmental alliance formed against the backdrop of the gradual confluence of various Japanese groups concerned with Chinese affairs, and a search for unity on the part of all kinds of Chinese radical forces. As the views of Japanese concerned with Chinese affairs converged, the differences between organizations became blurred. Some important figures backed the formation of multiple societies, or even took positions of leadership in different organizations. For this reason, one sector of China's people and public opinion preferred to call these kinds of groups “Societies for Rejuvenating Asia," regarding them not as mere branches of particular Japanese organizations, but as alliances between the people of China and Japan uniting together to resolve the current crisis and rejuvenate East Asia. Because the Asian Association was already showing signs of decline, the establishment of the East Asian Society, and particularly of the Common Culture Association was intended largely to replace the Asian Association as the organizational framework of the Sino-Japanese alliance. For this reason, the associations naturally focused on[KMB][AM]Shanghai. The Shanghai 1895 Comrades' Society, built by Shiraiwa Ryuhei, Munakata Kotaro and others played a vital role in the transition process. Under their influence, the Chinese gradually turned towards the East Asian Common Culture Association as their ally. Taking advantage of the organizational and personnel resources of previous groups concerned with Chinese affairs, the Society was more comprehensively and deeply involved in anti-Qing plots in Southern China and worked to promote a united uprising.

Abstract:More than 4.2 billion inhabitants populate the Asia-Pacific region. Acute coronary syndrome (ACS) is now a major cause of death and disability in this region with in-hospital mortality typically exceeding 5%. Yet, the region still lacks consensus on the best approach to overcoming its specific challenges in reducing mortality from ACS. The Asia-Pacific Real world evIdenCe on Outcome and Treatment of ACS (APRICOT) project reviewed current published and unpublished registry data, unmet needs in ACS management and possible approaches towards improving ACS-related mortality in the region. There was striking heterogeneity in the use of invasive procedures, pharmacologic practice (hospitalization/post-discharge), and in short- and long-term clinical outcomes across healthcare systems; this heterogeneity was perceived to be far greater than in Western Europe or the United States. 'Benchmark' short-term clinical outcomes are preferred over long-term outcomes due to difficulties in follow-up, recording and maintenance of medication adherence in a geographically large and culturally diverse region. Key 'barriers' towards improving outcomes include patient education (pain awareness, consequences of missing medication and secondary prevention), geographical landscape (urban vs. metropolitan), limited long-term adherence to guideline-based management and widespread adoption of cost-based rather than value-based healthcare systems. Initiatives to overcome these barriers should include implementation of pre-hospital management strategies, toolkits to aid in-hospital treatment, greater community outreach with online patient/physician education and telemedicine, sustainable economic models to improve accessibility to effective pharmacotherapies and the acquisition of high-quality 'real-world' regional data to tailor secondary prevention initiatives that meet the unique needs of countries in this region.

To disseminate emerging knowledge regarding cardiovascular diseases, the central theme of OCC has been "Open, Creative, Cooperative." This year's gathering provided an outstanding platform for physicians and scientists from both China and abroad to share novel insights into disease mechanism, diagnosis, and therapeutics.During the 3.5-day scientific program, dozens of internationally renowned experts, including Professors J. Hill, P. Libby, I. Komuro, Ch. Hamm, J. Camm, and D. Wood and many domestic experts, delivered lectures. As a highlight of our international collaborative efforts, >50 members of the Chinese American Heart Association were invited to discuss cutting-edge research and cardiology training experiences in the United States.In his opening speech, the president of the Chinese Society of Cardiology, Professor J. Ge, announced a new index, the China Cardiovascular Index, which analyzes the distribution of cardiovascular disease burden in China. This novel epidemiological instrument incorporates >50 variables of risk factor exposure, premature cardiovascular death, risk factor control (eg, hypertension, smoking), and air quality. The China Cardiovascular Index is likely to emerge as a valuable tool for policymakers, healthcare professionals, and clinical investigators. Going forward, the China Cardiovascular Index will track the impact of novel initiatives (eg, chest pain centers, smoking cessation efforts), hopefully contributing to the control of cardiovascular diseases in China.Productive discussions were facilitated in carefully organized fora. At the Interventional Cardiology forum, in addition to lectures on antiplatelet therapies, live cases were performed and broadcasted from several Shanghai hospitals. Complex coronary interventions, including chronic total occlusion and left main stenosis, were performed. Dr S. Walsh conducted a live-case right coronary artery-chronic total occlusion from Belfast, and a complex transcatheter aortic valve replacement case was broadcasted live from Zhongshan Hospital, Fudan University. Another highlight of this year's OCC was the foundation of the Chinese College of Cardiology. The Committee of Health and Family Planning formulated a policy to build a National Cardiovascular Specialists Training Program. "The college is in charge of this program and cardiologists' continuous education," noted the immediate past president of the Chinese Society of Cardiology, Professor Huo. Furthermore, as another way to advance cardiovascular research in China, the OCC has established a number of continuing education and training programs, including satellite meetings in rural counties and competition for the Oriental Rising Star award for early career professionals.The percutaneous coronary intervention cinema was a new feature of OCC 2017. Different percutaneous coronary intervention procedures were performed, recorded, and composed as an action film. Each movie script depicted heroes defeating villains using a range of weapons. Typical cardiac enemies included chronic total occlusions, calcified lesions, and bifurcation lesions.In summary, the 11th OCC provided outstanding dissemination of state-of-the-art cardiovascular science and medicine. Abstract:Emergency and essential surgery is a critical component of universal health coverage. Session three of the three-part virtual meeting series on Strategic Planning to Improve Surgical, Obstetric, Anaesthesia, and Trauma Care in the Asia-Pacific Region focused on strategic partnerships. During this session, a range of partner organisations, including intergovernmental organisations, professional associations, academic and research institutions, non-governmental organisations, and the private sector provided an update on their work in surgical system strengthening in the Asia-Pacific region. Partner organisations could provide technical and implementation support for National Surgical, Obstetric, and Anaesthesia Planning (NSOAP) in a number of areas, including workforce strengthening, capacity building, guideline development, monitoring and evaluation, and service delivery. Participants emphasised the importance of several forms of strategic collaboration: 1) collaboration across the spectrum of care between emergency, critical, and surgical care, which share many common underlying health system requirements; 2) interprofessional collaboration between surgery, obstetrics, anaesthesia, diagnostics, nursing, midwifery among other professions; 3) regional collaboration, particularly between Pacific Island Countries, and 4) South-South collaboration between low- and middle-income countries (LMICs) in mutual knowledge sharing. Partnerships between high-income countries (HIC) and LMIC organisations must include LMIC participants at a governance level for shared decision-making. Areas for joint action that emerged in the discussion included coordinated advocacy efforts to generate political view, developing common monitoring and evaluation frameworks, and utilising remote technology for workforce development and service delivery.
Practice and discussion about one-stop compact emergency management mode in Shanghai Xin Hua Hospital

Abstract:The emergency care volume of Xin Hua Hospital affiliated to Shanghai Jiaotong University School of Medicine keeps ranking among the top in Shanghai all the year round.Xin Hua Hospital is one of the first hospitals in China to establish the integrated operation and management mode of first aid,trauma and ICU.With the change of medical model,the original loose management model of emergency medical service can no longer adapt to the needs of emergency patients and it also exposes many weak link at the same time.The emergency department of Xin Hua Hospital put forward to build a one-stop and compact emergency management model,which realized intelligent triage,specialized personnel,one-stop process,integrated first aid,field rescue,and informationization management through practice and exploration.This model embodies the characteristics of modern emergency medical treatment featuring convenience,high efficiency and safety,and reflected modern first aid concept of "patient centered".This mode has been preliminary achievements,making the Xin Hua Hospital emergency diagnosis and treatment level towards a new stage.
