Abstract:Objective: To examine the influence of lumber disk degeneration on vertebral disc shape and lumbar lordosis. Methods: 121 cases were adopted and divided into three groups: control group of 33 asymptomatic volunteers, slight degeneration group of 53 cases and severe degeneration group of 35 cases. For all cases, sagittal magnetic resonance imaging of lumbar spine were inputted into personal computer. The anterior, middle and posterior heights of L4/5 intervertebral space, as well as the signal, the angle of the L4/5 disc and lumbar lordosis were measured. Results: The height of intervertebral space and the angle of L4/5 disc and lumbar lordosis gradually decreased among the control group, slight degeneration group and severe degeneration group (P 0.05). The angle of L4/5 disc had positive correlation with lumbar lordosis (P 0.05). Conclusions: For patients with lumbar disc degeneration, changes of the disc shape and lumbar lordosis present. To investigate those changes is very valuable for the therapy of lumbar disc degeneration.
Endplate Concavity Variation of Lumbar Motion Segments a Finite Element Analysis

Abstract:BACKGROUND: Histomorphological change of endplate may affect the nntritional transmission of intervertebral disk, eventually leading to intervertebral disc degeneration. OBJECTIVE: To observe the effects of endplate concavity variation on mechanical response of lumbar motion segment. DESIGN, TIME AND SETTING: Repeated measurement design, biomechanical analysis of finite element models, performed at the Laboratory of Biomechanics, Department of Orthopedics, the Second Affiliated Hospital of Medical College of Zhejiang University between January 2005 and January 2007. MATERIALS: SOMATOM SENSATION 16 spiral CT machine (Siemens, German) and ANSYS (Inc. Pennsylvania, USA).METHODS: A three-dimensional nonlinear geometrical and mechanical accurate finite element model of lumbar L<,4-5> segment was developed. Parametric studies were undertaken by studying endplate of three different concave angles, whereas disc angle, the gap of facet joint, finite element mesh density, and all other parameters were kept constant. Biomechanical parameters of three kinds of finite element models were tested under 5 different loading conditions, including vertical compression, flexion, extension, and anterior and posterior shearing loading. MAIN OUTCOME MEASURES: Endplate-intervertebral disc interface strain, intervertebral disc stiffness, nucleus pulposus pressure, annular fiber stress, radial disc bulge, stress in the annulus ground substance, stresses in posterior structure and facet contact force. RESULTS: The decrease in the endplate concavity stimulated by an increasing endplate concave angle would result in decreased strains of endplate-intervertebral disc interface, increased disc stiffness and nucleus pulposus pressure, decreased annular fiber stress, radial disc bulge and stress in the annulus ground substance, and simultaneously produce decreased facet contact force and stresses in posterior structure. CONCLUSION: The decrease of endplate concavity enhances the protective effects of the disc on vertebral body breakage. Small endplate deformation results from decreased endplate concavity would contribute to the reduction of nutritional diffusion to the intervertebral disc.

Abstract:Objectives: Discectomy is a conventional surgery for lumbar disc herniation. However, recurrence and residual back pain are the main postoperative complications. The contribution of endplate changes to the occurrence of these complications remains controversial. This study aimed to investigate the effect of endplate changes after discectomy. Patients and Methods: We conducted a retrospective evaluation of 128 patients who had endplate changes after undergoing discectomy in our hospital. The patients were divided into three groups according to severity of abnormality according the Weishaupt classification (mild, moderate, and severe). The Oswestry Disability Index (ODI) and visual analog scale (VAS) were used to evaluate the efficacy of different surgical methods. Results: Seventeen patients dropped out of the follow-up study. Satisfactory efficacy was observed in most patients, but 16 patients underwent reoperation. Significant differences were observed among the three groups in terms of low back pain severity and ODI (p < 0.05), but not radicular leg pain severity. The severe group had a higher recurrence rate of disc herniation (23.5%) than the mild and moderate groups (10.3% and 10.4%, respectively). Conclusion: Discectomy had a noticeable efficacy. However, severe endplate changes, which indicated fissures on the endplate, damaged the lumbar stability and resulted in a higher recurrence rate and residual back pain. For such cases, internal fixation surgery should be considered.
Analyzing the Influence of Modic Changes on Patients with Lower Back Pain Undergoing Conservative Treatment.

Abstract:Objective. This study aimed to investigate if the presence of Modic changes (MCs) was correlated with lower back pain (LBP) and LBP-related disability in patients who underwent nonsurgical treatment. Methods. In this study, 129 patients who experienced consecutive LBP and underwent lumbar spine magnetic resonance imaging in our institute were divided into three groups according to the presence or type of MCs. The Oswestry Disability Index (ODI) and visual analog scale (VAS) were used to assess the outcomes of the treatment. Results. Based on the achieved results, there was no significant difference between three groups before treatment (P>0.05). Three months after undergoing nonsurgical treatment, the rates of improved ODI and VAS scores were statistically significantly different (P=0.014,0.023). After an additional 3months of treatment, in patients with Modic type I changes, the symptoms significantly improved in comparison with those 3months prior (P=0.037,0.026), while that improvement did not occur in patients with Modic type II changes (P>0.05). Conclusions. The existence of MCs affects the outcomes of nonsurgical treatment in patients with LBP. However, symptoms can be improved after an additional round of treatment for Modic type I changes, while this is not confirmed for Modic type II changes.

Abstract:Study Design: This is a cross-sectional study. Objective: To determine the prevalence and distribution patterns of Modic changes (MCs) in the lumbar spine and their associations with disk degeneration in mainland Chinese using a sample of general population. Summary of Background Data: Previous studies reported that the prevalence of MCs in Hong Kong Chinese was much lower than in other populations. Moreover, their associations with disk degeneration need further study. Materials and Methods: The study sample consisted of 442 subjects (53.614.9y; range, 20-88y) randomly selected from a typical Chinese community. Lumbar spines were imaged using a 3.0T magnetic resonance scanner. Eleven endplates (L1-S1) in the lumbar spine were evaluated for the presence of MCs, type, location, and size to determine MCs prevalence and distribution patterns. Disk degeneration was graded using a Pfirrmann scale. Results: MCs were present in 209 (47.3%) subjects and 593 (12.2%) endplates. Among these endplates, 84.1% (499) were type II, 9.1% (54) were type I, and 6.4% (38) were mixed MCs. Approximately 2/3 MCs were present in the lower lumbar spine and 44.9% of MCs were at the L5/S1 disk level. Most MCs (73.9%) involved both endplates of a disk. Greater age [odds ratio (OR) = 2.44 for each 10-year increase, P < 0.001] and body mass index (OR = 1.07, P = 0.016) were associated with the presence of MCs, as was adjacent disk degeneration (OR = 6.00, P < 0.001), controlling for age and other covariates. Greater age, body mass index, and adjacent disk degeneration were also associated with greater MCs size. Conclusions: MCs are common in mainland Chinese, with type II predominating. MCs mainly present in the lower lumbar region and tend to occur in pairs. MCs were strongly associated with age and disk degeneration, suggesting MCs may be aging-related degenerative findings that parallel disk degeneration.
The Effect of Disc Height and Degeneration on the Mechanical Behavior of Lumbar Spine:A Finite Element Analysis

Abstract:Objective: To evaluate the effects of disc height and degeneration on mechanical behavior of human lumbar spine segments. Methods: A new effective CAD method was used to accurately establish three finite element models, including normal height disc (NHD) model, pure height decreased disc (PHDD) model and degeneration combined height decrease disc (DHDD) model. The biomechanical parameters of three finite element models were measured under axial compressive load. Results: The height and degeneration of disc had obviously effects on the mechanical behavior of lumbar spine segment in terms of axial displacement, posterolateral disc bulge, the maximum stress in the peripheral anulus fibers. The stress distribution differed between DHDD and non-DHDD model. Conclusions: The stiffness of disc obviously increases after height decreasing. The happening of disruption of inner anulus fibers is the lowest in DHDD model. The ratio between the gap of facet articulation and axial displacement are crucial to determine the load strength of disc. The degenerated disc plays an effective role for stress distribution.
Association Between Modic Changes and Endplate Sclerosis: Evidence from a Clinical Radiology Study and a Rabbit Model

Abstract:Purpose: To analyse the presence of endplate sclerosis in patients with various types of Modic changes (MCs) and to confirm the results using a rabbit model. Methods: Participants in the clinical study included 1023 consecutive inpatients with lumbar degenerative disease who attended the Department of Orthopaedics between August 2011 and August 2015. All patients underwent computed tomography (CT) and magnetic resonance imaging of the lumbar spine. In those patients with MCs, endplate sclerosis was evaluated from sagittally reconstructed CT images. In addition to the clinical study, MCs type I, II and III were initiated using a previously developed rabbit model of MCs. Specimens of MCs type I, II and III and normal endplates were harvested, bone mineral density and bone volume/tissue volume of "treated" vertebrae were evaluated using mCT and osteogenic protein expressions of run-related transcription factor 2 and osteocalcin were assessed using immunohistochemical staining. Measurements were compared between vertebrae with normal endplates and those with different types of MCs. Results: Of 1023 patients, 214 (20.9%) had MCs in one or more endplates; these changes affected 1044 (10.2%) of 10230 endplates. Type I, II and III MCs were seen in 164 (1.6%), 838(8.2%) and 40 (0.4%) endplates, respectively. Of 1044 endplates with MCs, 274 (26.2%) showed evidence of sclerosis on CT images including: 26/164 endplates (15.8%) with type I MCs, 208/838 (24.8%) with type II and 40/40 (100%) with type III. HU (CT value) ratios for sclerotic and nonsclerotic endplates with MCs were 2.0 +/- 0.3 and 1.1 +/- 0.1, respectively. In the animal study, the bone mineral density, bone volume/tissue volume and expression of runt-related transcription factor 2 and osteocalcin of endplates with type I and II MCs were higher than those of normal endplates and lower than those of endplates with type III MCs. Conclusion: Sclerosis can occur in endplates with any type of MCs. However, the clinical and animal study suggests that sclerosis is greatest in endplates showing type III MCs. The translational potential of this article: The study showed that sclerosis can occur in endplates with MCs type I, II and III. In patients with endplate sclerosis on plain radiographs or CT scans, the endplate can still represent an inflammatory process associated with chronic lower back pain. (C) 2018 The Authors. Intervertebral Disc Vacuum Phenomenon is Associated with Modic Changes in the Adjacent Endplate: Implications for Discogenic Back Pain

Abstract:Abstract Background Back pain often arises from degenerative changes in lumbar intervertebral discs and their adjacent endplates. A painful endplate is not easy to identify in patients, but could possibly be revealed by inflammatory-like ‘Modic’ changes and by a ‘vacuum phenomenon’ within the disc. We hypothesize that Modic changes and a VP often co-exist in those lumbar levels most closely associated with back pain Methods We scanned 1023 consecutive inpatients of the Department of Orthopaedics from 2015 August to 2018 August, all patients suffered from lumbar degenerative diseases, whether the patients had back pain or not were evaluated, and the prevalence and location of vacuum phenomenon(VP) and Modic changes were compared at each spinal level. Results 5115 discs were studied from 1023 patients. The number of discs showed to have a VP was 430 using CT, of the 430 discs with a CT-diagnosed VP, 116 were L4-5, and 171 were L5-S1. 522 of the 5115 discs exhibited Modic changes, with prevalence of type I, type II and type III Modic changes being 1.6%, 8.2% and 0.4% respectively. Modic changes usually occurred adjacent to L4-5 discs or L5-S1 discs. The prevalence of back pain was higher in the VP group than no-VP group, VP were significantly associated with Modic changes Type II at L4/5 and at L5/S1. Conclusion VP are closely associated with back pain and Modic changes in the lower lumbar spine. Further investigations may be warranted when radiographs or CT identify a VP in a lumbar disc.
Spontaneous Resorption of Lumbar Disc Herniation is Less Likely when Modic Changes Are Present.

Abstract:Study Design. A retrospective survey on 85 consecutive patients with primary single-level lumbar disc herniation (LDH). Objective. To investigate associations between Modic changes (MCs) and the likelihood of resorption of herniated lumbar intervertebral discs. Summary of Background Data. Spontaneous resorption of LDH has been demonstrated, whereas the mechanisms are unclear. MCs are closely associated with disc degeneration, but research focusing on their association with spontaneous resorption of LDH has not been specifically investigated. Methods. Eighty-five consecutive patients with LDH (52 males, 33 females, aged 20–66 yr) were included. Patients' diagnosis was based on clinical presentation, magnetic resonance imaging, and computed tomography. Patients were divided into surgical and conservative groups and further divided into MC and non-MC subgroups. Spontaneous resorption and clinical success in the conservative group were assessed by reduction in the herniated volume and Oswestry Disability Index. Disc tissues collected from the surgical group were examined histologically, and immunohistochemistry was used to identify endothelial cells and macrophages. Results. In total, 35 of 85 patients showed MC, mostly type II. Herniated tissue in MC group contained relatively more hyaline cartilage endplate than that in non-MC group (on average, 50% vs. 8%, P < 0.05) but less nucleus pulposus (18% vs. 55%, P < 0.05). Conservative treatment reduced Oswestry Disability Index scores in non-MC group from 29.4 to 23.5 on average (P < 0.05), but reductions in MC group (30.1–29.0) were nonsignificant. Herniated volumes reduced after conservative treatment in non-MC group (0.44–0.21 cm3, P < 0.05) but not in MC group (0.52–0.45 cm3, P > 0.05). More neovascularization and macrophage infiltration was observed in herniated tissue from non-MC group than from MC group (P < 0.001). Conclusion. MCs in patients with LDH are associated with cartilaginous herniations that resorb poorly, so that patients respond less well to conservative treatments. Loss of cartilaginous endplate may explain the origins of MCs and their association with disc infection. Level of Evidence: 3
Modic Changes (mcs) Associated with Endplate Sclerosis Can Prevent Cage Subsidence in Oblique Lumbar Interbody Fusion (OLIF) Stand -Alone

Abstract:OBJECTIVE:To investigate the association between Modic changes (MCs) and endplate sclerosis; and to investigate the effect of MCs and endplate sclerosis on cage subsidence in oblique lumbar interbody fusion (OLIF) stand-alone.METHODS:We included 78 inpatients who underwent OLIF stand-alone from August 2015 to August 2017. All patients underwent computed tomography (CT) and magnetic resonance imaging. The presence and the types of MCs were assessed. Endplate sclerosis was evaluated from sagittally reconstructed CT images. Cage subsidence was evaluated at the 1-, 3-, 6-, and 12-month follow-up.RESULTS:Of 78 patients, 92 discs underwent OLIF without posterior instrumentation; 32 of 92 (34.8%) had MCs. Type I, II, and III MCs were seen in 10 (10.9%), 19 (20.7%), and 3 (3.2%) endplates, respectively. Among 32 endplates with MCs, 10 (31.3%) showed evidence of sclerosis on CT images, including 2 of 10 endplates (20%) with type I MCs, 5 of 19 (26.3%) with type II, and 3 of 3 (100%) with type III. Among 60 endplates without MCs, 5 (8.3%) showed evidence of sclerosis. Cage subsidence rate in the no-MCs group was higher than that in the MCs group (P <0.01). We found only 1 case of cage subsidence in the MCs group (n = 32), which was classified as MCs type I, whereas 6 cases of cage subsidence were observed in the no-MCs group (n = 60). Hounsfield unit ratios for sclerotic and no-sclerotic endplates were 2.2 ± 0.3 and 1.1 ± 0.1, respectively. No subsidence of cage was found in the sclerotic endplates group (n = 15), whereas 7 cases of cage subsidence were found in the no-sclerotic group (n = 77).CONCLUSION:MCs associated with endplate sclerosis can prevent cage subsidence in OLIF stand-alone.

Abstract:Objective To evaluate the distribution of Modic changes of lumbar endplate in patients suffering low back pain or radiculopathy and its related factors. Methods 1287 patients suffering from low back pain without tumor,infection and degenerative scoliosis were examined by MRI scan from July 2012 to June 2013. According to the criteria of Modic changes,the features of distribution of Modic changes of lumbar endplate on age,segment and type of intervertebral disc degenemtion were analyzed retrospectively. Results Among 6435 intervertebral disc of the 1287 patients. 450(34. 97% )cases and 675(10. 49% )intervertebral discs were invo1ved with Modic changes. Among them,103(8. 0% )cases and 116(1. 80% )discs were type Ⅰ;313(24. 32% )cases and 519(8. 01% )discs were typeⅡ;34(2. 64% )cases and 40(0. 62% )discs were type Ⅲ. There were 182 male cases(28. 09% ),268 female cases(41. 94% ),the difference was statistically significant from men and women(P ﹤ 0. 05). There were 56 cases(14. 32% )less than 40 years old,394 cases(43. 97% )greater than or equal to 40 years old. The difference was statistically significant(P ﹤ 0. 05). There were 86(2. 29% )normal disc and 589(21. 97% )de-generative disc,the difference was statistically significant(P ﹤ 0. 05). There were 285(7. 38% )lumbar,390(15. 15% )lower lumbar. The difference was statistically significant(P ﹤ 0. 05). Conclusion Endplate Modic changes happen more in women patients with low back pain,overe 40 years old and multiple degeneration. And it correlates with the degree of disc degenera-tion.
Changes in the Flexion-Relaxation Response after Percutaneous Endoscopic Lumbar Discectomy in Patients with Disc Herniation

Abstract:BACKGROUND:Previous studies of percutaneous endoscopic lumbar discectomy (PELD) to treat lumbar disc herniation (LDH) have shown good clinical results. However, limited information has been reported regarding postoperative improvement in back muscle function. We aimed to determine whether changes in neuromuscular patterns, assessed using the flexion-relaxation phenomenon (FRP), could be observed after PELD.METHODS:A total of 30 patients participated in the present study. The patients' muscle functions were evaluated before and after PELD using surface electromyography and self-report measures. The evaluated action included 3 repetitions of a trunk flexion and extension movement. The association between the FRP measures and patient-reported outcomes was evaluated.RESULTS:At 4 weeks after PELD, the patients had significantly lower visual analog scale and Quebec Back Pain Disability Scale (QBPDS) scores and significantly increased gross range of motion. Also, we observed a reduction in erector spinae muscle activation at L3 in the maximum voluntary flexion phase. Normalized electromyographic root-mean-square values were significantly reduced in the maximum voluntary flexion phase and significantly increased in the extension phase. Postoperatively, 60% of the patients successfully achieved flexion-relaxation compared with 7% of the patients preoperatively. A significant correlation was found between the FRP measures and patient-reported outcomes, except for the Fear-Avoidance Beliefs Questionnaire scale.CONCLUSION:PELD for individuals with LDH appears to normalize paraspinal muscle activation during lumbar flexion-extension movement. The gross range of motion and disability-related limitations of physical activity also improved.
Modic Changes, Possible Causes and Promotion to Lumbar Intervertebral Disc Degeneration.

Abstract:Modic changes are bone marrow and endplate lesions visible in magnetic resonance imaging (MRI). They are regarded as a part of degenerative disc disease and associated with low back pain. And severe disc degeneration was occurred more in the patients with Modic changes. But there is still no study to analyze the relationship between Modic changes and intervertebral disc degeneration.We hypothesize that Modic changes are the possible causes and promotion of lumbar intervertebral disc degeneration. And there are three possible mechanisms for this hypothesis: a structural cause: Modic changes make cartilaginous material easier in extruded disc herniations, to destroy the structure of intervertebral disc and inhibit the absorption of the disc. A biomechanical cause: Modic changes alter the mechanical loading distribution on disc, to initiate a series of disc disruption and inhibit the self-recovery of the disc. A nutritional cause: Modic changes destroy the vascular architecture in vertebral endplate and block the most important metabolism pathway between vertebrae and disc.Perspectives: (1) Find out procedures to cure Modic changes may be an important breakthrough for disc degenerative disease. The distribution of Modic changes of cervical endplate in patients suffered neck pain and its related factors

Abstract:0bjective To evaluate the distribution of Modic changes of cervical endplate in patients suffered neck pain and its related factors.Methods 1023 cases of patients suffered from neck pain without any pathological lesions were examined by MRi scan and X-ray.According to the criteria of Modic changes,the feature of distribution of Modic changes of cervical endplate on age,segment and grade of intervertebral disc degeneration was analyzed retrospectively.Results Among 6138 intervertebral disc of 1023 cases of patients,90(8.8%)patients and 108(1.8%)intervertebral discs were involved with Modic changes.32(3.1%) cases and 34(0.6%)discs were type I;55(5.4%)cases and 71(1.2%)discs were typeⅡ;3(0.3%)cases and 3(0.06%)discs were typeⅢ.According to the segment,the lesions involved C2-3. disc for 0(0%),C3-4 for 10(0.2%),C4-5 for 18(0.4%),C5-6 for 52(1.O%),C6-7 for 23(0.4%),C7-T1 for 5(0.1%).Modic changes was distributed mainly over the age of 40 and correlated with disc degeneration,disc level,condition of eervical curve and ages,moreover,disc degeneration played the most important role in the occurrence of Modic changes.Conclusion Modic changes of cervical endplate mainly OCCULT in C5-6 segment,then the C4-5 and C6-7 segment,and type Ⅱ is the most and type Ⅲ is the least.Modic changes is distributed mainlv over the age of 40 and correlated with disc degeneration,disc level condition of eervieal curve and ages,moreover,disc degeneration plays an important role in the occurrence of Modic changes.

Abstract:Background Modic changes are the MRI signal changes of degenerative lumbar vertebral endplate and which lead to or accelerate intervertebral disc degeneration. NLRP3, caspase-1, and interleukin-1β (IL-1β) play a pivotal role in the pathogenesis of many inflammatory diseases, such as osteoarthritis. However, the roles of IL-1β and its activators caspase-1 and NLRP3 are unclear in the degenerative endplate. Questions/purposes We asked: (1) What are the degenerative changes of the histologic features and chondrogenic markers’ gene expressions between the cartilaginous endplates of patients with Modic changes and trauma (control)? (2) How does the NLRP3/caspase-1/IL-1β axis in the cartilaginous endplates of patients with Modic changes compare with control (trauma) specimens? Methods Surgical specimens of cartilaginous endplates were divided into Modic changes (n = 56) and the trauma control (n = 16) groups. Hematoxylin and eosin and safranin O staining of cartilaginous endplate tissues were done to evaluate the extracellular matrix. Reverse transcription-polymerase chain reaction was performed on these tissues to investigate mRNA expression of type II collagen (Col II), SOX-9, matrix metalloproteinase-3, and a disintegrin like and metalloproteinase thrombospondin type I motifs-5. NLRP3, caspase-1, and IL-1β were evaluated by reverse transcription-polymerase chain reaction and immunohistochemistry. Results Hematoxylin and eosin and safranin O staining showed the extracellular matrix degraded in the cartilaginous endplates of patients with Modic changes but not in the control cartilaginous endplates. Chondrogenic Col II (p = 0.024) and SOX9 (p = 0.053) were downregulated in the Modic changes group compared with the control group. In contrast to the control group, the transcriptional levels of NLRP3 (p < 0.001), caspase-1 (p = 0.054), and IL-1β (p = 0.001) were all upregulated in the Modic changes group. Conclusions The expression of NLRP3, caspase-1, and IL-1β was upregulated in the patients with low back pain and Modic changes on MRI compared with patients with vertebral burst fracture without degenerative changes on MRI. The data suggest the NLRP3/caspase-1/IL-1β axis may be implicated in lumbar cartilaginous endplate degeneration. Clinical Relevance The NLRP3/caspase-1/IL-1β axis is active in cartilaginous endplates of patients with Modic changes and inflammatory cascades can exacerbate the cartilaginous endplate degeneration which may act as a trigger for intervertebral disc degeneration and low back pain. If these findings can be confirmed by others, we hope that new and effective therapy could be developed directed against this target.
The Correlation Between the Lumbar Endplate Modic Degeneration and Degeneration of Lumbar Intervertebral Disc or Pain Provocation Observed on CT Discography

Abstract:Objective To determine the correlation between the Modic degeneration of lumbar endplate on MRI and the pain provocation or degeneration of lumbar intervertebral discs observed on CT discography. Methods One hundred and twenty lumbar intervertebral discs (40 L3-4, 40 L4-5, 40 L5S1 intervertebral discs) of 40 patients among 45 patients were examined. 120 intervertebral discs underwent discography guided by CT scan, according to Dallas Discography Description system, degeneration of lumbar intervertebral disc were divided into 0-3 grade, and pain provocation were graded into negative, indifferent and positive. On the basis of Modic classification system of the lumbar endplate changes and Pearce classification system of the degeneration of lumbar intervertebral disc, the lumbar endplate changes were divided into 0-3 grade, the degeneration of lumbar intervertebral disc were graded into Ⅰ-Ⅴ. All the results were analysed by Chi-Square test for the correlations. Results There was positive correlation between pain provocation test and the lumbar endplate Modic degeneration; There was a positive correlation between the Modic degeneration of lumbar endplate and the degeneration of lumbar intervertebral disc and so is the relation between the CT discography and MRI on discs. Conclusion There is positive correlation between pain provocation test and the Modic degeneration of lumbar endplate, it suggest the endplate might be one of the sources of low back pain. There is a positive correlation between the Modic degeneration of lumbar endplate and the degeneration of lumbar intervertebral disc.

Abstract:Background Context: There are few comparisons of Modic changes (MCs) in the lumbar and cervical spine.Purpose: Compare the prevalence of MCs in the lumbar and cervical spine, and determine how MC prevalence depends on spinal pain, age, disc degeneration, spinal level, and the presence or absence of kyphosis.Study Design: Retrospective clinical survey.Materials and Methods: Magnetic resonance images (MRIs) were compared from five patient groups: 1. 1223 patients with low-back pain/radiculopathy only; 2. 1023 patients with neck pain/radiculopathy only; 3. 497 patients with concurrent low-back and neck symptoms; 4. 304 asymptomatic subjects with lumbar MRIs; and 5. 120 asymptomatic subjects with cervical MRIs.Results: The prevalence of MCs was higher in those with spinal pain than in those without, both in the lumbar spine (21.0% vs 10.5%) and cervical spine (8.8% vs 3.3%). Type II MCs were most common and Type III were least common in all groups. The prevalence of lumbar MCs in people with back pain was little affected by the presence of concurrent neck pain, and the same was true for the prevalence of cervical MCs in people with neck pain with or without concurrent back pain. When symptomatic patients were reclassified into two groups (back pain, neck pain), the prevalence of lumbar MCs in people with back pain was greater than that of cervical MCs in people with neck pain. The prevalence of lumbar and cervical MCs increased with age, disc degeneration, (descending) spinal level, and increased kyphosis.Conclusions: There is a significantly higher prevalence of MCs in patients with back and neck pain. The reported association with increased kyphosis (flat back) is novel.

Abstract:How stiffness and strength of the human lumbar endplate vary with location, spinal level, and its correlation with MRI findings of lumbar degeneration, has not been reported in detail. 27 lumbar spines (16 male, 11 female, 31–49yrs) were harvested from cadavers without history of lumbar lesion or trauma. Disc and endplate degeneration was evaluated from MRI. Micro-CT was used to evaluate endplate microstructure. Indentation tests were performed to quantify stiffness and strength at 23 sites on each endplate from load-displacement graphs. Results showed that stiffness and strength increased from the centre of the endplate towards its periphery. There was no general age-related reduction in endplate stiffness or strength, although strength decreased slightly with age opposite the inner annulus. Disc degeneration was associated with a 39–46% decrease in stiffness, and a 21–30% decrease in strength, with effects being greatest near the endplate periphery. The presence of Modic changes had a similar effect. Strength and stiffness consistently increased at lower spinal levels, and were consistently greater in the inferior endplate (relative to the vertebra). Gender had little influence, although stiffness in the peripheral endplate was greater in males. BV/TV, SMI, Tb.Th and BMD were positively correlated with strength. We conclude that endplate properties reflect compressive stresses within adjacent intervertebral discs. Weaker and softer endplates may indicate reduced mechanical loading in decompressed discs that are stress-shielded by the neural arch. Preoperative MRI evaluation of endplate integrity could reduce the risk of implant subsidence following inter-body fusion.
Spontaneous regression of lumbar disc herniation: four cases report and review of the literature

Abstract:Spontaneous regression of lumbar disc herniation refers to shrinkage or disappearance of herniated nucleus pulposus without invasive surgical treatments. This phenomenon has been reported and is supported by improved clinical symptoms and radiographic after conservative treatment, but the underlying mechanism remains unclear. This article reports 4 cases of disc reabsorption and reviews the distribution of several clinical and radiographic factors of disc herniation reabsorption of total 46 patients, including the four from our study, gathered from 28 recent publications. Some of these factors are present with anomalous distributions. But some factors have similar deviations in patients with lumbar disc herniation. Therefore, more research is needed to explore the correlation between those factors and disc reabsorption.
The Influence of Direct Inoculation of Propionibacterium Acnes on Modic Changes in the Spine

Abstract:Background: Modic changes (inflammatory-like changes visible on magnetic resonance imaging [MRI] scans of a vertebral end plate) are common and are associated with low back pain, but their origin is unclear. To our knowledge, there have been no previous in vivo animal models of Modic changes. We hypothesized that Modic changes may be related to Propionibacterium acnes. Methods: Ten New Zealand White rabbits were injected percutaneously with 1 mL of P. acnes (1.6 × 107 colony forming units/mL) into the subchondral bone superior to the L4-L5 and L5-L6 discs; 10 other control rabbits received sham injections at L4-L5 and 1 mL of normal saline solution (vehicle) at L5-L6. The subchondral bone superior to L3-L4 discs was untreated (blank). Development of Modic changes was investigated with MRI studies before the operation and at 2 weeks and 1, 2, 3, and 6 months postoperatively. Following sacrifice of the rabbits, histological analysis and microcomputed tomography (micro-CT) were performed, and blood samples were analyzed. Cytokine expression in end-plate tissues was quantified using real-time polymerase chain reaction (PCR). Results: The group that received P. acnes showed significantly increased T1-weighted signal intensity at 6 months (mean and standard deviation, 3.43 ± 0.41 [range, 2.42 to 4.44] compared with 2.43 ± 0.66 [range, 1.98 to 2.87] before the injection) and higher T2-weighted signal intensity at 6 months. Positive culture results were obtained from 9 of 20 samples injected with P. acnes. Specimens with positive cultures had a higher prevalence of Modic changes (4 of 9 samples positive for P. acnes compared with 3 of 11 samples negative for P. acnes). Real-time PCR showed significantly increased expression of tumor necrosis factor-α, interleukin-1β, and interferon-γ following injection of P. acnes, but no changes were seen on histological analysis, micro-CT, or blood analysis. Conclusions: P. acnes can survive within the end-plate region and can initiate mild inflammatory-like responses from host cells, leading to signal intensity changes in MRI scans, which potentially resemble Modic changes. Clinical Relevance: Disc degeneration and low back pain are associated with Modic changes. Our results indicate that Modic changes can be associated with P. acnes in the conjunction area of the disc and subchondral bone. These results may be useful for understanding the underlying mechanisms of Modic changes and related pain.
