Helicobacter Pylori Infection Status Among Health Check-Up Populations In China: A 3-Year Follow-Up Study

Abstract:Background: Currently, most health care centers in China perform detection of Helicobacter pylori (H. pylori) infection, but changes in H. pylori infection among these populations are rarely reported. Here, a 3-year follow-up study of H. pylori infection status was performed using C-13-urea breath tests (UBTs) performed in the health care center of our hospital from 2013 to 2015 to study changes in the H. pylori infection status among health check-up populations in China. Methods: A total of 5,779 subjects examined at our institution between January 2013 and December 2013 were enrolled in this study. These participants were tested for H. pylori infection and followed up for 3 years. The current infection rate, annual incidence, annual reinfection rate and spontaneous clearance rate were determined. Further univariate and multivariate logistic regression analyses were performed to determine the causes of eradication failure. Results: Among the entire cohort (3,806 (65.86%) male; 1,973 (34.14%) female; average age 54.28 years), the H. pylori current infection rate was 40.27%, with males presenting a significantly higher rate than females (p<0.001). A total of 2,587 H. pylori-positive subjects were diagnosed before 2015, and 1,370 underwent eradication therapy. The eradication rate of H. pylori among these 1,370 cases was 85.04% (1,165/1,370). The annual incidence, annual reinfection and spontaneous clearance rates of H. pylori infection were approximately 7.69%, 2.66% and 3.18%, respectively. Multivariate logistic regression analysis suggested that the following factors were related to eradication failure: male sex, smoking history, a low education level, no gastroscopy and poor treatment compliance. Conclusions: Efforts should be directed towards treating H. pylori-infected patients after screening, including implementation of standard initial treatments to improve the eradication rate and reduce the resistance rate.

Abstract:The association between Helicobacter pylori (H. pylori) infection and metabolic syndrome (MetS) determined in different cohorts from different countries is currently inconclusive. In the majority of previous studies, H. pylori infection was diagnosed based on the presence of H. pylori IgG antibody in the serum. However, to the best of our knowledge, only few studies have investigated the association between H. pylori infection and MetS using the urea breath test (UBT) as a diagnostic tool. The present study was performed with the aim of providing a detailed analysis of the association between H. pylori infection, as diagnosed by the UBT method, and MetS in a large community from Zhejiang province in eastern China. The results indicated that H. pylori infection increases the risk of MetS in the aged female population.

Abstract:Background and Aim.The infection of Helicobacter pylori (H.pylori) is acquired in childhood and the prevalence vary greatly in different countries and regions.The study aimed to investigate the characteristics of H. pylori infection among children with gastrointestinal symptoms in Hangzhou, a representative city of eastern China.Methods.A systematic surveillance of H. pylori infection according to the 13 C-urea breath test was conducted from January 2007 to December 2014 in the Children's hospital, Zhejiang University School of Medicine.The demographic information and main symptoms of every subject were recorded.Results.A total of 12796 subjects were recruited and 18.6% children evaluated as H. pylori positive.The annual positive rates decreased from 2007 to 2014 ( X 2 =20.461, p<0.01).The positive rates were 14.8%, 20.2% and 25.8% in 3-6, 7-11 and 12-17 years age group respectively, which increased with age ( X 2 =116.002,p<0.01).And it was significantly higher in boys than girls ( X 2 =15.090, p<0.01).Multivariate logistic regression identified possible risk factors for H. pylori infection.Age, gender, gastrointestinal symptoms and history of H. pylori infected family member were all significantly associated with H. pylori infection (all p<0.05).Conclusions.H. pylori infection rates in children with gastrointestinal symptoms were lower than most of those reported in mainland China.Further studies are required to determine the prevalence in the general population.Comprehensively understanding of the characteristics and the possible risk factors of H. pylori infection will be helpful to its management strategies in children in China.

Abstract:OBJECTIVE:To investigate the relationship between the serum anti-Müllerian hormone (AMH) level and semen parameters. METHODS:We collected the data about 726 outpatients at the Male Infertility Clinic of Jinling Hospital from September 2015 to November 2016, including 72 with non-obstructive azoospermia, 123 with oligospermia, and 531 with normal sperm concentration. We obtained the semen volume, total sperm count, sperm concentration, sperm motility, the percentages of progressively motile sperm (PMS) and morphologically normal sperm (MNS), and the levels of serum AMH, inhibin B (INH-B), total testosterone (TT) and follicle - stimulating hormone (FSH) of the patients, analyzed the correlation of the serum AMH level with the other parameters, and compared the AMH level among different groups. RESULTS:The serum AMH level was found to be correlated positively with the total sperm count (r = 0.227, P <0.001), sperm concentration (r = 0.215, P <0.001), sperm motility (r = 0.111, P = 0.003), the percentage of PMS (r = 0.120, P = 0.001), and the levels of INH-B (r = 0.399, P <0.001) and TT (r = 0.184, P = 0.002), negatively with the FSH level (r = －0.283, P <0.001), but insignificantly with age, time of abstinence, semen volume, and the percentage of MNS (P >0.05). There was a statistically significant difference in the serum AMH level among the patients with non-obstructive azoospermia, oligozoospermia, and normal sperm concentration (［6.33 ± 4.26］ vs ［8.26 ± 3.98］ vs ［9.8 ± 5.19］ ng/ml, P <0.001). CONCLUSIONS:Serum AMH is a biomarker reflecting the function of Sertoli cells and its level is significantly correlated with sperm concentration and motility, suggesting that AMH may be involved in spermatogenesis and sperm maturation.

Abstract:Objectives: Aim was to evaluate anti-Mullerian-hormone (AMH) in women with polycystic ovary syndrome (PCOS) comparing with normo-ovulatory women, and to assess for the first time AMH levels across reproductive lifespan in Chinese women.Methods: Within the prospective cross-sectional study in PCOS cases and in normal subjects, AMH and other hormone levels were measured in early follicular phase in five age groups. Additionally ovarian ultrasound parameters were assessed.Results: A total of 437 PCOS patients and 150 normo-ovulatory women matched by age were included. AMH levels were 2- to 3-fold higher in PCOS cases. In both groups AMH decreased with age in a non-linear pattern. In normo-ovulatory women AMH levels were relatively constant from 18 to 25 years and then declined with age. In contrast, AMH in PCOS cases declined with an increasing rate from 18 to 45 years, narrowing the difference in older women.Conclusions: AMH may be a useful parameter to assess the severity and prognosis of PCOS since certain differences exist to normo-ovulatory women. Further studies may improve the understanding of the relationship between AMH levels and age, providing more scientific basis for fertility counselling for both healthy women and PCOS cases.
