Abstract:Brexit will have profound implications for health and health policy yet, while much attention has focused on health professionals, medicines and health protection, the risk of food insecurity, and thus health, has received less attention. We identify five major threats to the availability and affordability of food supplies. These are a lack of regulatory alignment restricting ability to import foods from the EU and beyond, a shortage of agricultural labour in the UK, increased prices of imported foods due to tariffs, damage to supply chains, for example, due to customs delays and loss of interoperability of transportation, and damage to agricultural production and food flows in Ireland.

Abstract:BACKGROUND The continuing uncertainty around Brexit has caused concern in the pharmaceutical industry and among health care professionals and patients. The exact consequences of Brexit on the pharmaceutical supply chain in the United Kingdom will depend on whether a deal is reached and what it entails, but it is likely to be affected by the withdrawal of the United Kingdom from the European Union. Regulatory issues and delays in supply have the potential to negatively affect the ability of UK residents to receive an adequate and timely supply of necessary medicines. OBJECTIVE The purpose of this protocol is to provide an overview and critical analysis of current perspectives on the effect of Brexit on the UK pharmaceutical supply chain. METHODS The PRISMA-ScR (Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews) guidelines will be used to structure this protocol. A systematic search of MEDLINE, EMBASE, PsycINFO, Healthcare Management Information Consortium (HMIC), Cochrane, Web of Science, Business Source Complete, EconLit, and Economist Intelligence Unit will be conducted, as well as a Google and Nexis.UK search for grey literature such as reports, opinion pieces, and press releases. Two reviewers will independently screen the titles and abstracts of identified references and select studies according to the eligibility criteria. Any discrepancies will then be discussed and resolved. One reviewer will extract data from the included studies into a standardized form, which will be validated by a second reviewer. Risk of bias will be assessed using the Cochrane Collaboration Risk of Bias tool for any randomized controlled trials; quality will be assessed using the relevant Critical Appraisal Skills Programme (CASP) checklists; and grey literature will be assessed using the Authority, Accuracy, Coverage, Objectivity, Date, Significance (AACODS) checklist. Outcomes include the agreement between sources on the potential, likelihood, and severity of the consequences of Brexit on the UK pharmaceutical supply chain. RESULTS Results will be included in the scoping review, which will be published in 2020. Assessing the potential impact on health of the UK's future relationship agreement with the EU: analysis of the negotiating positions

Abstract:While policy attention is understandably diverted to COVID-19, the end of the UK's post-Brexit 'transition period' remains 31 December 2020. All forms of future EU-UK relationship are worse for health than EU membership, but analysis of the negotiating texts shows some forms are better than others. The likely outcomes involve major negative effects for NHS staffing, funding for health and social care, and capital financing for the NHS; and for UK global leadership and influence. We expect minor negative effects for cross border healthcare (except in Northern Ireland); research collaboration; and data sharing, such as the Early Warning and Response System for health threats. Despite political narratives, the legal texts show that the UK seeks de facto continuity in selected key areas for pharmaceuticals, medical devices, and equipment [including personal protective equipment (PPE)], especially clinical trials, pharmacovigilance, and batch-testing. The UK will be excluded from economies of scale of EU membership, e.g. joint procurement programmes as used recently for PPE. Above all, there is a major risk of reaching an agreement with significant adverse effects for health, without meaningful oversight by or input from the UK Parliament, or other health policy stakeholders.

Historically, medication unavailability has typically, if not exclusively, affected low- and middle-income countries [1]. More recently however, drug shortages have also been reported in high-income European countries [2]. Abstract:Health security in the European Union (EU) aims to protect citizens from serious threats to health such as biological agents and infectious disease outbreaks- whether natural, intentional or accidental. Threats may include established infections, emerging diseases or chemical and radiological agents. Co-ordinated international efforts attempt to minimize risks and mitigate the spread of infectious disease across borders.We review the current situation (March 2019) with respect to detection and management of serious human health threats across Irish borders- and what may change for Ireland if/when the United Kingdom (UK) withdraws from the EU (Brexit).Specifically, this paper reviews international legislation covering health threats, and its national transposition; and EU legislation and processes, especially the relevant European Decision No. 1082/2013/EU of the European Parliament and of the Council on serious cross border threats to health with repeal of Decision No 2119/98/EC. We enumerate European surveillance systems and agencies which relate to port health security; we consider consortia and academic arrangements within the EU framework and established collaboration with the World Health Organization. We describe current Health Services Executive port health structures in Ireland which address preparedness and management of human health threats at points of entry. We appraise risks which Brexit could bring, reviewing literature on shared concerns about these risks, and we evaluate post-Brexit challenges for the EU, and potential opportunities to remain within current structures in shared health threat preparedness and response.It is imperative that the UK, Ireland and the EU work together to mitigate these risks using some agreed joint coordination mechanisms for a robust, harmonised approach to global public health threats at points of entry.
Managing supply chain uncertainty arising from geopolitical disruptions: evidence from the pharmaceutical industry and brexit

Abstract:Purpose This paper examines how firms of different sizes formulate and implement strategies to achieve fit with an external environment disrupted by a geopolitical event. The context of the study is the pharmaceutical industry and how it managed the supply chain uncertainty created by the United Kingdom's decision to leave the European Union, or Brexit. Design/methodology/approach Data were collected longitudinally from the pro-Brexit vote on 23 June 2016, until the UK’s departure from the EU on 31 January 2020. Twenty-seven interviews were conducted in the pharmaceutical sector, including nineteen interviews with senior managers at eight case companies and eight interviews with experts working for trade associations and standards institutes. The interview findings were triangulated with Brexit policy and strategy documentation. Findings When formulating strategy, multi-national enterprises (MNEs) used worst case assumptions, while large firms, and small and medium sized enterprises (SMEs) gathered knowledge as part of a “wait-and-see” strategy, allowing them to reduce perceptions of heightened supply chain uncertainty. Firms then implemented reactive and/or proactive strategies to mitigate supply chain risks. Originality/value The study elaborates on strategic contingency theory by identifying two important conditions for achieving strategic fit: first, companies deploy intangible resources, such as management time, to gather information and reduce perceptions of heightened supply chain uncertainty. Second, companies deploy tangible resources (supply chain redundancies, new supply chain assets) to lessen the negative outcomes of supply chain risks. Managers are provided with an empirical framework for mitigating supply chain uncertainty and risk originating from geopolitical disruptions.

Abstract:After the European Union (EU) was left by the United Kingdom (UK), a free trade area was established between these economies. Although bilateral trade in all goods is tariff-free, regulatory requirements make exports more costly and burdensome. We used a partial equilibrium model to analyze the implications of Brexit for agricultural exports from Visegrad countries (Czechia, Hungary, Poland and Slovakia). We assess trade creation and trade diversion effects resulting from an increase in non-tariff measures and border costs for 4-digit agricultural products identified as sensitive in each of the Visegrad countries. The simulations reveal that exports of sensitive products from Visegrad countries to the UK could decrease by up to 20%. While the macroeconomic importance of this change is not significant, for the producers of the sensitive goods such export losses are substantial.

Abstract:Introduction Food insecurity is associated with increased risk for several health conditions and with poor chronic disease management. Key determinants for household food insecurity are income and food costs. Whereas short-term household incomes are likely to remain static, increased food prices would be a significant driver of food insecurity. Objectives To investigate food price drivers for household food security and its health consequences in the UK under scenarios of Deal and No-deal for Britain’s exit from the European Union. To estimate the 5% and 95% quantiles of the projected price distributions. Design Structured expert judgement elicitation, a well-established method for quantifying uncertainty, using experts. In July 2018, each expert estimated the median, 5% and 95% quantiles of changes in price for 10 food categories under Brexit Deal and No-deal to June 2020 assuming Brexit had taken place on 29 March 2019. These were aggregated based on the accuracy and informativeness of the experts on calibration questions. Participants Ten specialists with expertise in food procurement, retail, agriculture, economics, statistics and household food security. Results When combined in proportions used to calculate Consumer Price Index food basket costs, median food price change for Brexit with a Deal is expected to be +6.1% (90% credible interval −3% to +17%) and with No-deal +22.5% (90% credible interval +1% to +52%). Conclusions The number of households experiencing food insecurity and its severity is likely to increase because of expected sizeable increases in median food prices after Brexit. Higher increases are more likely than lower rises and towards the upper limits, these would entail severe impacts. Research showing a low food budget leads to increasingly poor diet suggests that demand for health services in both the short and longer terms is likely to increase due to the effects of food insecurity on the incidence and management of diet-sensitive conditions.

Key determinants for household food insecurity are income and food costs. Forecasts show household disposable income for 2020 expected to fall and for 2021 to rise only slightly. Prices are forecast to rise. Thus, future increased food prices would be a significant driver of greater food insecurity. Structured expert judgement elicitation, a well-established method for quantifying uncertainty, using experts. In July 2020, each expert estimated the median, 5th percentile and 95th percentile quantiles of changes in price to April 2022 for ten food categories under three end-2020 settlement Brexit scenarios: A: full WTO terms; B: a moderately disruptive trade agreement (better than WTO); C: a minimally disruptive trade agreement. When combined in proportions for calculate Consumer Prices Index food basket costs, the median food price change under full WTO terms is expected to be +17.9% [90% credible interval:+5.2%, +35.1%]; with moderately disruptive trade agreement: +13.2% [+2.6%, +26.4%] and with a minimally disruptive trade agreement +9.3% [+0.8%, +21.9%]. The number of households experiencing food insecurity and its severity are likely to increase because of expected sizeable increases in median food prices in the months after Brexit, whereas low income group spending on food is unlikely to increase, and may be further eroded by other factors not considered here (e.g. COVID-19). Abstract:Purpose: Health-system pharmacists play a crucial role in monitoring the pharmaceutical pipeline to manage formularies, allocate resources, and optimize clinical programs for new therapies. This article aims to support pharmacists by sharing new and anticipated novel drug approvals. Summary: Selected drug approvals anticipated in the 12-month period covering the third quarter of 2024 through the second quarter of 2025 are reviewed. The analysis emphasizes drugs expected to have significant clinical and financial impact in hospitals and clinics selected from 54 novel drugs awaiting US Food and Drug Administration approval. New cell therapies for treating cancers continue to enter the drug pipeline, while novel targeted therapies for biliary tract, gastric, pancreatic, and breast cancers, as well as 3 subcutaneous versions of already approved drugs given intravenously, are awaiting approval. Additionally, many novel drugs are being developed for treatment of rare and ultra-rare diseases such as hereditary angioedema, macular telangiectasia, congenital adrenal hyperplasia, and Barth syndrome. Two new subcutaneous drugs for hemophilia, a new oral medication for hereditary angioedema, a novel monoclonal antibody for atopic dermatitis, and the first oral penem antibiotic are also in the pipeline. Conclusion: New drugs with various indications for cancers and rare diseases continue to strengthen the drug pipeline.

Abstract:Medicine shortages are an ever increasing problem for the UK primary care system, with the Department of Health and Social Care (DHSC) logging an average of 137 shortage notifications per month from medication manufacturers in 2023, an increase of 67% from 2021.1 With shortages becoming more common, community pharmacies are spending more time attempting to source alternatives and on the phone to GP practices, with practice staff subsequently burdened with prescribing alternatives on an ad hoc basis.2 Patients may question the reason for or, more worryingly, be confused by changes, and medication regimens may be disrupted while waiting for a suitable alternative to be sourced. Supply shortages affect a wide range of therapies, including type 2 diabetes, hormone replacement therapy (HRT), epilepsy, and attention deficit hyperactivity disorder (ADHD) medications in recent years.3 The medicines supply network is a complex system, encompassing raw materials suppliers, manufacturers, wholesalers, prescribers, pharmacies, and, ultimately, patients. Different parts of this network are exposed to a variety of external factors beyond their immediate control, including market forces, regulatory policy, changes in guidelines or product licensing, and operational challenges. The system's many moving parts and different vulnerabilities means there can be numerous reasons for shortages of a particular drug. Lack of Certification of the WTO Goods Schedules of the United Kingdom: A Way for Frictionless Trade Under No-Deal Brexit?

Abstract:The United Kingdom (‘UK’), as a Member State of the European Union (‘EU’), does not have its own schedule of concessions under the World Trade Organization (‘WTO’) – for now – because the EU, as a single customs union, has consolidated schedules for goods and services. The UK is currently negotiating its schedules with other WTO Members, but time is running short ahead of the UK’s scheduled exit from the EU on 31 October 2019 (‘Brexit Date’). If the UK fails to certify its schedules before the Brexit Date, the question becomes whether the UK could unilaterally establish its new schedules and conduct trade based on ‘uncertified’ schedules that have not been agreed by all WTO Members. The panel in EU – Poultry (China) found that certification of a schedule is not a legal prerequisite to implement agreed-upon changes in negotiations under Article XXVIII of the General Agreement on Tariffs and Trade 1994 (‘GATT’) at the national level. Based on the panel’s finding and Article XXVIII:3 of the GATT, under a no-deal Brexit scenario, the UK would be able to unilaterally establish its goods schedule, even in the absence of agreement with other Members. Further, the UK could enforce the new schedules before completing the certification process and, thus, trade under the uncertified goods schedule. However, three caveats must be noted: (1) the UK must certify its new goods schedule eventually; (2) unlike the goods schedule, the completion of certification is necessary for the UK to give effect to its new services schedule; and (3) if another Member brings a complaint concerning the UK’s uncertified schedules, and the dispute results in an appeal, the Appellate Body may reach a different conclusion when applying the panel’s decision in the context of Brexit. In short, the UK must certify its goods schedule as soon as it concludes Article XXVIII negotiations with other WTO Members. The Poultry panel’s decision does not support a way for frictionless trade between the UK and other countries under no-deal Brexit.

Abstract:Summary The supply of blood components and products in sufficient quantities is key to any effective health care system. This report describes the challenges faced by the English blood service, NHS Blood and Transplant (NHSBT), towards the end of the COVID‐19 pandemic, which in October 2022 led to an Amber Alert being declared to hospitals indicating an impending blood shortage. The impact on the hospital transfusion services and clinical users is explained. The actions taken by NHSBT to mitigate the blood supply challenges and ensure equity of transfusion support for hospitals in England including revisions to the national blood shortage plans are described. This report focuses on the collaboration and communication between NHSBT, NHS England (NHSE), Department of Health and Social Care (DHSC), National Blood Transfusion Committee (NBTC), National Transfusion Laboratory Managers Advisory Group for NBTC (NTLM), National Transfusion Practitioners Network, the medical Royal Colleges and clinical colleagues across the NHS.

Proposal for Handling of Medicine Shortages Based on a Comparison of Retrospective Risk Analysis

Abstract:Introduction: We reviewed and compared current drug shortages and shortage management practices in six selected countries (Hungary, Belgium, Spain, Switzerland, Australia, United States) based on the most comprehensive national shortage databases for each country, for four Anatomical Therapeutic Chemical (ATC) groups, to analyze the criticality of drug shortages across countries and identify best practices in shortage management strategies. Materials and methods: Countries were selected to cover a wide geographical range of high-income nations where a lack of economic power as a potential source of drug shortages is not observable. ATC groups were selected based on a pre-examination of the databases to analyze groups most often in shortage, and groups where the absence of which could have a severe negative impact on treatment outcomes. The bias originating from the different reporting systems had to be reduced to gain comprehensive and comparable information. The first bias-reducing mechanism was transforming the raw number of shortages into proportion per million people. Secondly, critical cases were classified, and thirdly, critical cases were compared with the Word Health Organization (WHO) Essential Medicine Lists. Results: The results indicate that every European country studied reports significantly higher total and critical shortages per population compared to the US and Australia. Within Europe, Hungary reports the highest number of cases both for total and critical shortages, while Spain has the lowest results in both aspects. While in the US and Australia critical shortages were observable in similar proportions across all ATC groups, in European countries ATC groups of anti-infectives for systemic use (J) and the nervous system (N) were found to account for a notably higher proportion of critical shortages. Current shortage management practices were examined in each country and classified into five groups to identify common best practices. Conclusions: Due to the different characterization of reporting systems, several bias-reducing mechanisms should be applied to compare and evaluate shortages. In addition, European harmonization should be initiated to create mutually acknowledged definitions and reporting systems, which could be the basis of good drug shortage handling practices in Europe.

Abstract:The British people voted in a 2016 referendum to leave the European Union (EU). Brexit presents both threats and opportunities to animal protection in the United Kingdom (UK), EU and internationally. This paper discusses threats to animal protection in terms of five criteria. These are first, political context; second, regulatory changes; third, economic and trade factors; fourth, institutional and capacity-related factors; and fifth, EU and international considerations. The EU has the most progressive animal welfare laws in the world. The Conservative Government delivering Brexit has a mixed record on animal protection. Major time and resource constraints inherent in Brexit risk negatively impacting animal protection. Brexit is projected to have a negative economic impact, which is generally associated with lower animal welfare standards. The development of Brexit policy suggests there to be a substantial risk that the major threat of importing lower welfare products to the UK will materialise. Brexit will reduce the political influence of the progressive animal protection lobby in the EU. Post-Brexit, the politically and economically weakened EU and UK risks a detrimental impact on animal protection on an international scale. Brexit poses substantial threats to animal protection, with a high risk that many threats will materialise. Further research is needed to assess the opportunities presented by Brexit to judge whether Brexit will be overall positive or negative for animal protection.

Abstract:Introduction The COVID-19 pandemic has brought the long-standing vulnerability of the medical product supply chain into sharp focus. Global shortages of medical products accompanied the global spread of the disease, joined by high prices, the proliferation of suspect dealers and dramatic interventions by governments, philanthropy and industry in oftentimes-unsuccessful attempts to secure solutions. Much attention has focused on personal protective equipment (PPE). Abstract:Accelerating access to new drugs in the NHS will do more harm than good, asserts Karl Claxton, a health economist at the University of York.
