Prevention of Cytomegalovirus Reinfection after Liver Transplantation: Targeted Prophylaxis Versus Universal Prophylaxis

Abstract:Objective To compare the clinical efficacy between targeted prophylaxis and universal prophylaxis in prevention of cytomegalovirus(CMV) reinfection after liver transplantation.Methods The clinical data of 62 patients enrolled in this study were retrospectively analyzed.These 62 patients were divided into 2 groups according to different prophylactic strategies.The patients in group A received targeted prophylaxis,i.e.ganciclovir was administered to those who were at high risk for CMV reinfection.Those patients in group B received universal prophylaxis,i.e.ganciclovir was administered to all recipients.The incidence of CMV reinfection and CMV disease,mean postoperative hospitalization duration,1-year survival rate and cost of ganciclovir were statistically analyzed.Results The overall incidence of CMV reinfection was 14.5%(9/62).In group A(n=40),asymptomatic CMV reinfection occurred in 6 patients.Of the 6 patients,5 had full recovery and 1 died from massive gastrointestinal hemorrhage.In group B(n=22),CMV reinfection occurred in 3 patients.Of the 3 patients,2 suffered from asymptomatic CMV infection and had full recovery but the other 1 from CMV hepatitis and died from liver failure.The incidence of CMV reinfection was 15% in group A and 13.6% in group B(P0.05).There was no significant difference in mean postoperative hospitalization duration and 1-year survival rate between the 2 groups(P0.05).The mean cost of ganciclovir was significantly higher in group B than in group A(P0.01).Conclusions Targeted prophylaxis is equally effective as universal prophylaxis in prevention of CMV reinfection after liver transplantation.Therefore,targeted prophylaxis is a economic prophylactic strategy and suitable for the recipients in China.
