CAR 10. Surgical Outcomes of Carotid Body Tumor Mainly With Shamblin III and Related Factors of Malignancy

Abstract:The objective of this study was to assess the correlation of surgical outcomes with Shamblin classification and size of carotid body tumor (CBT) and to evaluate the related factors of malignancy. Patients who underwent surgical resection of CBT from 2005 to 2017 in our center were retrospectively reviewed. The tumor size was measured as maximum transverse diameter through computed tomography. The correlation of surgical outcomes with Shamblin classification and tumor size was evaluated. Related factors of malignancy were further analyzed. There were 209 CBTs resected in 202 patients (mean age, 42 years; 64.9% female). The mean tumor size was 3.3 ± 1.2 cm (0.5-7.0 cm). The number of CBTs classified as Shamblin I, II, and III was 30 (14.4%), 40 (19.1%), and 139 (66.5%), respectively. Shamblin classification was related to tumor size (P < .001). Surgical outcomes, including procedural time, blood loss, vascular involvement, and neural complication, were associated with Shamblin classification and tumor size. Furthermore, logistic regression showed that CBT with Shamblin III was more related to carotid artery involvement (P < .001; odds ratio [OR], 18.11; 95% confidence interval [CI], 6.43-51.03), whereas tumor size was more related to neural complication (P = .001; OR, 1.60; 95% CI, 1.22-2.11; Table I). The rate of malignancy was 7.9% (16/202). Logistic analysis revealed that patients with bilateral CBT (P = .0010; OR, 0.09; 95% CI, 0.02-0.38) and symptomatic CBT (P = .0015; OR, 7.14, 95% CI, 2.11-25.13; Table II) have higher risk for malignant disease. Shamblin classification and size of CBT are both related to procedural outcomes. However, the correlation between Shamblin classification and carotid artery involvement is more closed, whereas tumor size is more related to neural complication.
