Influencing Factors for Surgical Treatment of Severe Acute Pancreatitis

Abstract:Objective To investigate the influencing factors for surgical treatment of severe a-cute pancreatitis (SAP). Methods The clinical data of 90 patients with SAP receiving initial surgical management from January 2005 to July 2007 in our hospital were respectively analyzed to determine the influencing factors for surgical treatment of SAP. The same analysis was carried for patients with fulminant acute pancreatits(FAP) and non-fulminant severe acute pancreatitis (nF-SAP) receiving sur-gical treatment in our hospital in the same period of time. Results The survival rate was 81.11% and 75% for SAP and FAP patients, respectively. Logistic regression analysis showed that age, organ dysfunction at the moment of surgery were independent predictors of mortality, and it was better for those FAP patients to be treated operatively in the first two weeks from onset, vice versa. Conclusion Rationale selection of the operative indication and timing is the key factor for improving outcomes. However, for FAP patients, it could be determined by the trend of organ dysfunction development and intra-abdominal pressure increseasing, usually in the first two weeks. For nF-SAP patients, the decision of surgery should be made according to infected pancreatic necrosis and pancreatic necrosis associated organ dysfunction, usually after the first two weeks.
