Abstract:Kawasaki disease is an acute, systemic vasculitis that easily injures coronary arteries and is the leading cause of acquired heart disease in children.Although the cause of Kawasaki disease remains unknown, it is widely believed that the pathogenesis of Kawasaki disease is an inflammatory cascade caused by a combination of infection and genetic predisposition.Regulatory T cells, which express Foxp3 + , CD4 + and CD25 + , are a T-cell subpopulation specialized in immune suppression.There are some correlations between regulatory T cells and Kawasaki disease in pathophysiology, treatment and prognosis.The dysfunction of regulatory T cells may be involved with the pathogenesis of Kawasaki disease, but there are few researches on it.This article reviews the progress of regulatory T cells in Kawasaki disease in recent years and summarizes the mechanism of regulatory T cells in the occurrence and repair of Kawasaki disease, prospecting the research future of targeted regulatory T cells therapy in the prevention of coronary artery lesions in Kawasaki disease.
Clinical features of incomplete Kawasaki disease

Abstract:In recent years,the understanding of KD has been deepened.Children with complete KD were timely diagnosed,but there was still a challenge for patients with incomplete KD.Full recognition of clinical features of incomplete KD would enable doctors to avoid delayed diagnosis and treatment,and reduce the incidence of coronary artery complications.Patients with 4 or fewer main criteria were considered with incomplete KD,and those with insufficient criteria and other atypical organ involved were considered with atypical KD.The main criteria for KD were in a clinically dynamic evolution,and some patients with incomplete KD present new main criteria and eventually meet the diagnostic criteria for complete KD.Early recognition of clinical features of patients with potential turning into complete KD was useful to improve the prognosis of children with KD.

Abstract:We report the case of a six-year-old boy who presented with cardiogenic shock due to Kawasaki disease (KD). He was misdiagnosed at first as septic shock. After careful examination, he was diagnosed as KD complicated with acute coronary syndrome, which leads to cardiogenic shock. Cardiogenic shock is often neglected as a complication of KD, and it tends to be misdiagnosed. We hereby call attention to KD, in some cases of which, it can lead to acute coronary syndrome in the acute phase.
[Clinical Features of Recurrent Kawasaki Disease in 20 Children].

Abstract:OBJECTIVE:To investigate the clinical features of recurrent Kawasaki disease (KD) and the relationship of recurrent KD with coronary artery lesions.METHODS:The medical data of 20 children with recurrent KD who were admitted to the Children's Hospital from January 1998 to May 2007 were retrospectively studied. Their clinical features were compared with those of children with initial KD.RESULTS:The incidence of recurrent KD was 1.34% (20/1489). KD relapsed 2 months to 4.6 years (average: 1.2 years) after the first episode in the 20 children. Compared with the initial KD group, the clinical symptoms in the recurrent KD group were incomplete, complicated and less severe. The period of fever, platelet count, C-reactive protein and ESR were remarkably reduced in the recurrent KD group, but the incidence of coronary artery lesions increased significantly compared with the initial KD group (40% vs 25%; P<0.05).CONCLUSIONS:The clinical symptoms of recurrent KD were incomplete in children. Recurrent KD was associated with an increased incidence of coronary artery lesions.

Abstract:Kawasaki disease is nonspecific systemic vasculitic syndrome that mainly afflicts children.The etiology of Kawasaki remains unknown.The disease involves systemic medium and small blood vessels.It is noted that coronary artery lesions are subject to coronary artery stenosis and thrombosis,further leading to ischemic heart disease,myocardial infarction,and sudden death.Kawasaki disease has been threatening the quality of life and life expectancy of children,whereas in some developed countries,it has been the leading cause of pediatric acquired heart disease,surpassing rheumatic fever.This article reviews the recent progress in the diagnosis and treatment of coronary artery lesion in Kawasaki disease.

Abstract:Kawasaki disease (KD) is an acute, self-limiting, and medium-sized vasculitis, which has been the commonest cause of acquired heart disease in children in developed countries.Without timely diagnosis and treatment, up-to 25% of the affected children may develop coronary artery abnormalities (CAA). Due to the lack of the specific diagnostic method, KD is mainly diagnosed according to the clinical criteria.As a result, typical KD is recognized easily, but it is a big challenge to diagnose KD patients with incomplete or atypical symptoms.The pandemic of novel coronavirus disease 2019 (COVID-19) around the world makes the diagnosis of KD even more complex.In this review, hot issues in diagnosing KD were discussed according to the 2017 guidelines for diagnosis and treatment of KD recently published by the American Heart Association (AHA), expecting to provide help for diagnosis of KD children.

Abstract:Aim To intensify the recognition of Kawasaki disease.Methods The clinical and laboratory data of 66 patients were analyzed.The diagnoses were confirmed from 2004 to 2006.Results 50 patients were boys and 16 patients were girls with the ages ranging from 6.5 months to 10 years old.Fever,fever with rash were common symptoms.Bilateral bulbar conjunctival injection was the most common positive sign and cervical lymphadenopathy was seldom.Continuiously high ESR was important for the diagnosis of Kawasaki disease and warning of coronary aneurysms.Conclusions The diagnosis could be confirmed with representative clinical pictures,laboratory examination and utrasound cardiogram.Reexamination of ultrasound cardiogram was recommended for the diagnostic confirmation.

Abstract:This review summarizes recent advances in understanding the development of coronary arteritis in Kawasaki disease. Kawasaki disease is the most common cause of acquired heart disease among children characterized with coronary artery abnormalities, which can cause myocardial ischemia, infarction, and even death. The pathogenic factors of Kawasaki disease and the pathological process of coronary artery disease are not clear at present, which brings challenges to the prevention and treatment of the disease. The treatment of Kawasaki disease focuses mainly on timely administration of intravenous high doses of immunoglobulin and aspirin. However, there are still some patients who do not respond well to this standard treatment, and its management remains a challenge. As a result, coronary artery lesions still occur in patients and affect their quality of life. In this review, we discuss updated research data of Kawasaki disease coronary artery lesions.

Atypical Manifestations in Children with Kawasaki Disease

Abstract:Objective: To explore the atypical manifestations and coronary artery disease in children with Kawasaki disease (KD) and discuss the diagnostic criteria of KD. Methods: The clinical and echocardiographic features of 75 children with KD were analyzed. Results: 42 (56%),23(30.7%),7(9.33%),2(2.67%) and 1(1.33%) cases had 6,5,4,3 and 1 major manifestations, respectively. Some cases showed atypical manifestations, such as suppurative lymphadenopathy, scabbed rashes, drop of the fingernails and no desquamation of the digits during the entire course. Aseptic meningitis, cholecystitis and myelitis were also found. Echocardiographic examinations suggested that 47 cases (66.2%) had coronary artery disease,in which 30 cases (42.3%) suffered from coronary artery dilation, 17 (23.9%) had coronary artery aneurysm (included 2 cases with big aneurysm ). Conclusion: Children with KD varied in clinical manifestations, and there were some exceptions in major manifestations, but coronary artery disease was often found.

Abstract:Kawasaki disease (KD) without proper treatment,the incidence of coronary artery lesions is up to 25％.Some of them develope of coronary artery aneurysms (CAA),obstructive coronary artery disease(stenosis,occlusion or thrombosis).KD with CAA in children need long term follow-up.Electrocardiogram(ECG) and two-dimensional echocardiography(2DE) are still the basic means of checking for follow-up.KD with CAA who should be preferred magnetic resonance imaging examination.Dobutamine stress echocardiography or coronary angiography should be done with suggestive of myocardial ischemia in ECG or 2DE.

Abstract:Objective To investigate the clinical features,laboratory examination,risk factors of coronary artery lesions and treatment for Kawasaki disease.Methods A total of 165 patients with Kawasaki disease hospitalized in Shandong Provincial Hospital during August 1995 and July 2005 were reviewed.Results Among 165 cases,136 were diagnosed with typical KD and 29 atypical(incomplete) KD;114 were male and 51 were female,male to female ratio is 2.2∶1.The age was between 23 days and 11 years old;41 cases was under 1 year old,68 between 1 to 3 years old,23 between 3~5 years old 33 over 5 years old.The mean age was 2 years and 8 months old.Totally 44 cases were complicated with coronary artery lesions,including 26 coronary artery dilations,13 coronary artery aneurysms and 5 giant coronary artery aneurysms.The patients with CA lesions were followed up from 6 months to 3 years,40 cases regressed completely,1 of 5 cases with giant CA aneurysm regressed completely,and other 4 cases didn't regress,1 case complicated with coronary thrombosis.Conclusion Atypical KD has higher prevalence of CA lesions.Children receiving IVIG 2g/kg in the early 10 days of KD has little chance of CAL.The single dose of 2g/kg IVIG administration has better outcome.The risk factors of CA lesions include high WBC count,low ALB level,male children and persistent fever.

Abstract:Aim: Coronary artery lesion (CAL) is a common yet serious complication in children with Kawasaki disease. The aim of the present study was to evaluate the influencing factors of CAL in children with Kawasaki disease, to provide reference for the clinical treatment and care of children with Kawasaki disease. Design: A retrospective cohort study. Methods: Children with Kawasaki disease treated in a tertiary hospital in China between 1 January 2021 and 31 December 2022 were selected. The characteristics and clinical data of children with Kawasaki disease were analyzed. Spearman's correlation analysis was conducted to evaluate the relationship between CAL and the characteristics of children with Kawasaki disease. A logistic regression analysis was used to analyze the influencing factors of CAL in children with Kawasaki disease. Results: In total, 185 children with Kawasaki disease were included; the incidence of CAL in children with Kawasaki disease was 18.38%. Pearson's correlation analysis showed that gender ( r = 0.504), age ( r = 0.611), duration of fever ≥10 days ( r = 0.579), hemoglobin (Hb) ( r = 0.623), and C-reactive protein (CRP) ( r = 0.558) were all correlated with the CAL in children with Kawasaki disease (all p < 0.05). Logistic regression analyses showed that male [odds ratio (OR) = 2.543, 95% confidence interval (CI): 1.801–3.077, p = 0.040], age ≤2 years (OR = 3.002, 95% CI: 2.744–3.641, p = 0.012), duration of fever ≥10 days (OR = 2.089, 95% CI: 1.624–2.515, p = 0.028), Hb ≤105 g/L (OR = 1.914, 95% CI: 1.431–2.406, p = 0.013), and CRP ≥100 mg/L (OR = 2.168, 95% CI: 1.893–2.531, p = 0.035) were the risk factors of CAL in children with Kawasaki disease (all p < 0.05). Conclusions: The incidence of CAL in children with Kawasaki disease is high and there are many related risk factors. Clinical medical workers should take early warning and carry out interventions and nursing care according to these risk factors to improve the prognosis of children with Kawasaki disease.

A clinical analysis of 82 cases of Kawasaki disease

Abstract:Objective To investigate clinical characteristics of Kawasaki disease(KD) and to explore diagnosis and treatment of the disease. Methods The clinical data of 82 patients with KD admitted to Shiquan County Hospital over a period from January, 1999 to December, 2006 were analyzed retrospectively. Results KD was commonly seen in children under 5 years old, and mainly in male children. Also, there was no seasonal differences in morbidity rate. Most of patients suffered from typical KD. The incidence of incomplete KD was low ( 6.097%). While the rate of misdiagnosis was high, which reached to 15.85%. The therapeutic effect of intravenous infusion of immunoglobulin combined with oral administration of aspirin was significant for KD. Conclusion The clinicians' recognition for KD should be raised in order to reduce rates of misdiagnosis and missed diagnosis. Early diagnosis and treatment can lower incidence of coronary artery lesions and improve prognosis of the children with KD.

Abstract:Objective To determine the role of echocardiography in diagnosing and following-up the coronary arterial lesion in Kawasaki disease(KD).Methods 157 cases of KD children's coronary arteries were detected by echocardiography.Among them,children who had coronary arterial lesions were followed-up.Results The incidence rate of coronary arterial lesions was 27.4%(43/157).Most of the incidence was coronary arterial dilatation.The followed-up results revealed that most coronary arterial lesions recovered.The febrile days before diagnosis in the giant coronary aneurysm group and in the coronary aneurysm group were longer than that in coronary arterial dilatation group(P0.01).Conclusion Echocardiography is capable of depicting dynamically the coronary arterial lesions,It can serve as the most valuable non-invasive means and follow-up for the diagnosis of KD with coronary arterial lesions.

Abstract:Background: Kawasaki disease (KD) is a major cause of childhood vasculitis and a common cause of acquired heart disease in children worldwide. This study sought to investigate the epidemiological characteristics of KD and to analyze the risk factors associated with KD complications, particularly coronary artery lesions (CALs). Methods: The medical records of children diagnosed with KD in Xuzhou from January 1, 2016, to December 31, 2020, were reviewed retrospectively. Categorical variables were analyzed using the Chi-square test, while unpaired Student's t-tests or the Mann–Whitney U test were employed for continuous data. Univariate and multivariable logistic regression analyses were conducted to identify risk factors of KD- associated CALs. Results: Of the 1082 children enrolled, the male-to-female ratio was 1.7:1. The age at diagnosis ranged from 1 to 127 months, with a median of 17 months. Seasonal variation was observed, with a peak incidence noted in summer (27.5%). CALs were identified in 292 cases (27.0%), detailed as follows: transient dilation (17.4%), mild dilation (4.0%), small aneurysms (2.9%), medium-sized aneurysms (1.9%), and giant aneurysms (0.8%). Independent risk factors for CALs included male gender, longer fever duration before intravenous immunoglobulin (IVIG), and elevated levels of leukocytes, C-reactive protein, creatine kinase-muscle/brain (CK-MB), and interleukin-6 (IL-6). Conclusions: Male gender, elevated levels of fever duration before IVIG, leukocytes, C-reactive protein, CK-MB, and IL-6 were predictive risk factors of CALs. These findings underscore the need for increased clinical awareness and targeted interventions to mitigate the risk of CALs in children affected by KD.

Clinical analysis of 93 children with the Kawasaki disease

Abstract:BACKGROUND To summarize clinical characteristics of the Kawasaki disease,and explore the diagnosis and treatment method.CASE REPORTS Clinical data of 93 children with the Kawasaki disease were retrospectively analyzed in hospital from January 1999 to December 2009.The Kawasaki disease occurred most under 6 years of age,boys more than girls,and the treatment effectiveness was good with intravenous drip with the gamma globulin and aspirin.DISCUSSION To improve clinical physicians for understanding the Kawasaki disease and reducing misdiagnosis and missed diagnosis,early diagnosis and treatment can reduce the incidence of coronary artery disease and improve the prognosis of Kawasaki disease.

Abstract:Kawasaki disease is an acute, self-limited vasculitis of childhood and has become the leading cause of acquired pediatric heart disease in the USA. Prompt treatment is essential in reducing cardiac-related morbidity and mortality. The underlying etiology remains unknown. The disease itself may be the characteristic manifestation of a common pathway of immune-mediated vascular inflammation in susceptible hosts. The characteristic clinical features of fever for at least 5 days with bilateral nonpurulent conjunctivitis, rash, changes in lips and oral cavity, changes in peripheral extremities, and cervical lymphadenopathy remain the mainstay of diagnosis. Supplementary laboratory criteria can aid in the diagnosis, particularly in cases of incomplete clinical presentation. Diagnosis of Kawasaki disease can be challenging as the clinical presentation can be mistaken for a variety of other pediatric illnesses. Standard of care consists of intravenous immune globulin and aspirin. Corticosteroids, infliximab, and cyclosporine A have been used as adjunct therapy for Kawasaki disease refractory to initial treatment. There is ongoing research into the use of these agents in the initial therapy of Kawasaki disease.

Abstract:Kawasaki Disease (KD) is a vasculitis of unknown etiology commonly seen in infants and children. Children with Kawasaki disease are at risk for serious cardiovascular sequelae, especially coronary artery abnormalities, which can lead to myocardial ischemia, myocardial infarction, and sudden death. Coronary artery lesions (CAL) caused by Kawasaki disease has become one of the common acquired heart diseases in children in some countries and regions. This article reviews the current research progress on the mechanism, risk factors and treatment of coronary artery damage in Kawasaki disease, aiming to improve the long-term prognosis of vascular health management in children with KD, in order to provide references for clinicians.
