Modified Cantwell-Ransley Repair Via a Ventral Penile Approach for Preserving the Integrity of Corpus Spongiosum for Male Epispadias

Abstract:Objective To explore the clinical efficacy of modified Cantwell-Ransley repair via a ventral penile approach for preserving the integrity of corpus spongiosum for male epispadias.Methods Retrospective analysis was performed for 18 cases of isolated male epispadias since December 2015.The relevant clinical data included patient profiles,operative approaches and such outcomes as voiding condition,penile cosmesis and complications.Results Primary repair was performed in 15 cases (glanular,n =7;penile shaft,n =4;penopubic,n =4).Secondary salvage repair was performed (n =3).The median age was 21 (12-168) months.Kelly's plus Young-Dees-Leadbetter's procedure was performed for reconstructing bladder neck for incontinence.The average follow-up period was 13.1 (6-21) months.There were no such severe complications such as glanular dehiscence,loss of glans or corpora,urethral stricture or permanent urethrocutaneous fistula.All dorsal curvatures were corrected,yet penile length seemed shorter than normal boys.And 88.9% families were satisfied with penile cosmesis.Four incontinent cases achieved grade Ⅰ-Ⅱ urinary continence.Conclusions Modified Cantwell-Ransley repair may recover anatomical penile structure and avoid various complications by preserving the integrity of corpus spongiosum for male epispadias.
