Abstract:Background: Dysphagia is one common symptom of post-stroke. Early identification and management can help to reduce the incidence of complications, lower medical cost, improve patients’ quality of life. Foreign guidelines relating to post-stroke dysphagia identification and management are abundant and mature, while they may not be totally suitable for China's situation and should be localized. There is no systematic scientific research on the localization and implementation of foreign guidelines in China. It's urgent to explore the way of implementing foreign high quality guidelines into China local context. Objective: To evaluate the quality of evidence-based guidelines related to post-stroke dysphagia identification and management, describe and analysis current situation of clinical practice, understand the gap between evidence and clinical practice, and improve patient's overall health condition, nursing quality, nursing operation standardization through local implementation. Methods: Iowa model is used as the guide model. Refer to ADAPTE procedures and JBI's evidence application procedures. We establish a practice team, retrieve related guidelines, utilize AGREE II to guide the strict appraisal of guidelines, recommend high quality guideline as the basis for implementation, form the initial recommendation items pool, complete the guideline applicability assessment through experts meetings and understand the baseline clinical practice status by observation, interview and questionnaire, identify facilitators and barriers on implementation according to the audit results, formulate the corresponding action strategies, develop appropriate clinical practice plan and put the plan into the ward. Results: We include 5 guidelines. The overall qualities are good. We have screened 6 recommendations that can be used in the pilot unit. The scores of nurses’ behavior dimension, attitude dimension, knowledge and skills of EBPQ are 4.08 ± 1.50, 4.82 ± 1.25 and 3.81 ± 1.23. The main barriers in the aspect of evidence appraisal is “lack of sufficient time” “do not know how”, the biggest obstacle in the aspect of evidence application is “don’t have enough time to practice“, next one is “lack resources”, lack of knowledge and standardized nursing procedures are also obstacles. The biggest facilitator is nursing leadership support. The implementation rates of recommendation items are from 0% to 25%. We select a pilot ward in a class 3 hospital in Beijing for practice. It shows no statistical significance (P > 0.05) in the general data of nurses and patients. The overall score and scores in activity dimension, role emotional dimension and mental health dimension in QOL of patients are higher than before (P < 0.05). There is also statistical significance in patient's satisfaction score and hospitalization time (P < 0.05). The same with nurses’ satisfaction scores and the related knowledge score (P < 0.05). The implementation rates of those audit recommendations reach 100% mostly. Nurses are in good compliance with this program. Discussion/Conclusion: The overall qualities of the included guidelines are good, and the contents of those guidelines are basically consistent. We can adapt these high-quality guidelines to our local context so as to guide and standardize the clinical nursing practice. The current situation is not optimistic, there is still a large gap between practice and guideline. Through the combination of best evidence with clinical experience and patient's preference, patients’ quality of life and satisfaction can be improved. Lack of knowledge, skills and time are the principal problems on evidence-based practice. Evidence-based guideline implementation, to a large extent, solves the problems and improves staff satisfaction, practical ability, sense of professional values, induce their evidence-based practice interest.
