Abstract:BACKGROUND Extragastrointestinal stromal tumors (EGIST) and gastrointestinal stromal tumors are of similar pathological type and form. Here we report a rare case of EGIST diffusely distributed in membranous tissue in abdominal cavity, the feature of which included diffuse tumors at membranous tissue in entire abdominal cavity and spontaneous bleeding of the tumors. CASE SUMMARY The patient was a 71-year man and hospitalized due to continuous pain at lower abdomen for more than 10 days. Upon physical examination, the patient had flat and tough abdomen with mild pressing pain at lower abdomen, no obvious abdominal mass was touchable, and shifting dullness was positive. Positron emission tomography-computed tomography (CT) showed that in his peritoneal cavity, there were multiple nodules of various sizes, seroperitoneum, multiple enlarged lymph nodes in abdominal/pelvic cavity and right external ilium as well as pulmonary nodules. Plain CT scanning at epigastrium/hypogastrium/pelvic cavity + enhanced three-dimensional reconstruction revealed multiple soft tissue nodules in abdominal/pelvic cavity, peritoneum and right groin. Tumor marker of carbohydrate antigen 125 was 808 U/mL, diffuse tuberous tumor was seen in abdominal/pelvic cavity during operation with hematocelia, and postoperative pathological examination confirmed EGIST. Imatinib was administered with better therapeutic effect. CONCLUSION Gene testing showed breast cancer susceptibility gene 1 interacting protein C-terminal helicase 1 and KIT genovariation, and the patient was treated with imatinib follow-up visit found that his clinical symptoms disappeared and the tumor load alleviated obviously via imageological examination.

Extragastrointestinal stromal tumors in soft tissues of abdominal cavity:a clinicopathologic analysis of 8 cases

Abstract:Objective To analyze clinicopathologic features of extragastrointestinal stromal tumors (EGISTs) in the soft tissues of abdominal cavity. Methods Morphologic characteristics of 8 cases of EGISTs in abdominal cavity were studied under light microscope, and the expression of CD117 and CD34 was detected with immunohistochemical staining. The cases with follow up data were analyzed. Results The patients with abdominal EGISTs ranged in age from 45 to 80 years (mean, 63 5 years). 8 tumors occurred in 3 women and 5 men. 5 tumors arose from mesentery and three from omentum. Most of the patients presented with an enlarging abdominal mass often accompanied by variable abdominal pain, but no overt gastrointestinal symptoms. Their diameters ranged from 5 0 to 25 0?cm (mean 17 4?cm). The histopathologic features and immunophenotypes of the tumors was similar to GISTs, but microscopically high cellularity were relatively common. Most cases were consisted predominantly of short fusiform cells which closely resembled a conventional smooth muscle tumors, but long well oriented fascicles like a leiomyosarcoma or skeinoid fibers were less noted in the tumors; All were positive for CD117, 6 for CD34 (75 0%), 2 for S 100 (25 0%), 3 for α SMA (37 5%), 3 for MSA (37 5%) and 3 for NSE (37 5%). 5 out of 6 patients with follow up data presented with adverse outcome in which the tumor in displayed 5?cm tumor in diameter, 2/50HPF mitosis activity, high cellularity and necrosis. Conclusion Patients with EGISTs in abdominal cavity predominantly are the elderly. The tumors are similar histopathologically and immunophenotypically to their gastrointestinal counterpart, but display high potential malignancy and adverse outcome; The tumor diameter 5?cm, mitosis 2/50HPF, high cellularity and necrosis in the tumor tissue tend to be the diagnostic parameters of malignancy.

Abstract:GIST-type stromal tumors can also occur in the abdomen, most cases were borderline or malignant, tumor coagulative necrosis, mitoses >or= 5 per 50 high-power fields and obvious nuclear atypia indicating malignancy. Differential diagnosis of EGIST including benign or malignant smooth muscle tumors, benign or malignant nerve sheath tumors etc.

Abstract:Gastrointestinal stromal tumors (GISTs) are the dominant mesenchymal tumors of the digestive tract. Extragastrointestinal stromal tumors (EGISTs) usually originate outside the gastrointestinal tract without connection to the gastric or intestinal wall. However, EGISTs arising from the vaginal wall are very rare. Here, we report a case of EGIST that occurred in the vagina of a 60-year-old woman. The tumor was present in the posterior vaginal wall. It was surgically excised, and histological examination revealed spindle cell morphology with up to 14 mitoses per 50 high power field (HPF) and necrosis with the tumor-negative margins. Immunohistochemical analyses showed strongly positive CD34, CD117, and DOG-1 expression, but negative SMA, S-100, CD10, desmin, and actin expression. The patient underwent surgery and is currently being followed up. A literature review of EGSTs and treatments is also discussed in this report.

Abstract:The occurrence of de novo malignant neoplasms has been shown in post-transplant recipients receiving immunosuppressive treatment. We present a case of a rare extragastrointestinal stromal tumor (EGIST) located in the pelvic cavity of a kidney transplant patient. A 57-year-old female patient was admitted to our department because of non-specific lower abdominal pain 6 months after renal transplantation. An abdominal computed tomography scan showed a 4.5 cm diameter pelvic tumor mass. The tumor was resected en bloc and confirmed as not being connected to the gastrointestinal wall. Microscopically, the tumor consisted of typical spindle cells with 2−3 mitotic figures per 50 high-power fields. Immunohistochemically, the tumor cells were strongly positive for CD117 (c-kit), and negative for CD34, SMA, s-100 protein, and desmin. Genetically, the tumor showed a silent mutation in exon 18 of the PDGFRA gene at codon 824 GTC > GTT (V824V) [rs2228230]. No recurrence was noted 24 months after the operation. This case draws our attention to the importance of considering EGISTs (including GISTs), even though they are extremely uncommon, in the differential diagnosis of mesenchymal neoplasms, especially in transplant patients.

Abstract:Background Solitary fibrous tumor (SFT) is a rare soft tissue tumor originating from mesenchymal cells. Thus far, there have been no reported cases of SFT closely related to the iliac vessels. Case presentation An elderly woman was found to have had a lower abdominal mass for more than 20 years. The enhanced computerized tomography (CT) showed a progressively enhanced hypervascular mass. The external iliac blood vessels were closely related to the mass, which was misdiagnosed as an ovarian tumor. During laparotomy, the external iliac vein was seen to penetrate the tumor, and the external iliac artery was seen to penetrate the tumor capsule. The retroperitoneal tumor was diagnosed during the operation. The surgical plan of complete tumor resection, severing of the external iliac arteries and veins, and blood vessel replacement was implemented. Pathological immunohistochemistry showed positive results for STAT6 and CD34, confirming the diagnosis of giant retroperitoneal SFT. The risk is classified as high and requires long-term follow-up. There has been no local recurrence or distant metastasis almost 1 year after surgery. Conclusion The incidence of giant retroperitoneal SFT is rare, and the diagnosis can be confirmed through preoperative imaging examination and pathological examination. If the SFT capsule is intact, there is a chance of surgical resection. For SFTs that are penetrated by the iliac blood vessels, adequate preparation must be made before the surgery is performed. Removing the tumor and the iliac blood vessels at the corresponding site and then replacing it with artificial blood vessels is a feasible method with less risk of bleeding. In this case, imaging showed a progressively enhancing hypervascular mass in the lower abdomen, which was related to blood vessels. Preoperative biopsy and pathological testing can confirm the diagnosis. Neoadjuvant therapy or interventional therapy before surgery can shrink the tumor, making the surgical procedure relatively easy with less risk of bleeding.

Abstract:Ewing's sarcoma (ES) is the second most common primary bone malignancy of childhood, whereas extraosseous ES (EES) is more common in older patients. EES of the small intestine is an extremely rare disease. We present a case of ES of the small intestine with liver metastasis mimicking a gastrointestinal stromal tumor. The diagnosis of ES requires a combination of histological, immunohistochemical, and molecular techniques. Further studies to investigate whether multimodality treatments can improve the survival of metastatic EES are needed.

Abstract:Objective To review the CT features of extragastrointestinal stromal tumor (EGIST) in the mesentery. Methods The clinical records and CT of 15 patients with histologically confirmed EGIST in the mesentery were retrospectively studied. Results Of the 11 high-risk EGISTs and 4 intermediate-risk EGISTs, metastasis was observed in the liver (1), abdominal cavity (1), right lower abdomen and pelvis (2), around the descending duodenum (3), mesentery root (4), left lower abdominal mesentery (4), or right pelvis (2). The mean tumor size was 11.5 cm (5.6-22.0 cm). The tumor was round with well-defined boundary (6) or irregular with poor-de-fined boundary (9). Unenhanced CT showed heterogeneous density with cystic-necrotic components in all 15 masses with calcification (2) or hemorrhage (1). The solid components showed progressive mild to moderate enhancement with CT values of 22-32 HU in the ar-terial phase and 38-40 HU in the venous phase. The cystic-necrotic components did not enhance. Maximum intensity projection (MIP) helped to clarify tumor blood supply by the superior mesenteric artery in 6 tumors with stripe-like and clustered vessels. Conclusion EGIST in the mesentery has characteristic CT features. MIP can help clarify the tumor blood supply for preoperative evaluation.
Inguinal Solitary Fibrous Tumor: A Case Report

Abstract:Introduction: A solitary fibrous tumor (SFT) is a rare mesenchymal neoplasm that may occur in a wide range of sites, more commonly in the pleura. We report a well-documented case of an SFT with malignant histology arising in the inguinal region. Case presentation: A 59-year-old man, with an unremarkable medical history, presented with a slow-growing painless mass in the left side of the inguinal region, which he had ignored for 3 years. On clinical examination, there was an indurated, irreducible, mobile, and painless mass in the left side of the inguinal area. Magnetic resonance imaging and computed tomography results revealed a 15-cm mass made up of soft tissue and with nonspecific radiologic aspect. The tumor was totally excised, and the results of the pathologic examination concluded it was an SFT. Discussion: The aim of our case report is to present a rare extrapleural localization of an SFT. Although its radiologic features are not specific, SFT should be considered in the differential diagnosis of inguinal masses.

Abstract:Various pathologies could lead to occult gastrointestinal (GI) bleeding. Here we report the case of a 73-year-old woman who presented with hematochezia and syncope, and was found to have a large bleeding GI stromal tumor incidentally from 99m Tc-RBC scintigraphy. This study was done after negative workup with CT angiography, colonoscopy, and capsule endoscopy for the source of GI bleeding. Final pathology confirmed the mass being a low-grade GI stromal tumor after exploratory laparotomy.

Abstract:A 40-year-old man presented with a 3-year duration of intermittent upper abdominal pain. Gastroscopy and endoscopic ultrasonography revealed a tumor in the bottom of the stomach with a homogenous low echogenicity that originated from the gastric serosa. An abdominal contrast-enhanced computed tomography (CT) scan showed that a well-marginated ovoid mass was obviously enhanced homogenously in the arterial period and weakened slightly in the venous period. The diagnosis of a gastric stromal tumor was made. Laparoscopic resection of the gastric tumor was subsequently performed. Surprisingly, postoperative pathological examination of the mass showed the typical structure of a normal splenic tissue. To our knowledge, ectopic splenic tissue originating from the gastric serosa is reported for the first time, showing important clinical significance for the identification of gastric malignant tumors.
Giant Paratesticular Liposarcoma Mimicking a Left-Sided Groin Hernia: A Case Report

Abstract:Giant paratesticular liposarcoma (PLS) is an uncommon tumour, often misdiagnosed pre-operatively, which presents as a painless scrotal mass. Early detection and prompt surgical management provide the best outcome. We present an 87-year-old patient with gradually enlarging, painless left scrotal swelling. Ultrasound on initial presentation suggested a benign hernia, resulting in an 11-month treatment delay. Computed tomography (CT) thereafter showed paratesticular scrotal mass measuring 14 x 8 x 7cm. Radical inguinal orchidectomy with high ligation of the spermatic cord was performed. Histopathology and cytogenetics confirmed PLS with both de-differentiated and well-differentiated features involving the spermatic cord margin. The patient had rapid progression to fatal lung metastasis within three months of surgery. Our case highlights that any suspicious fat swelling should be investigated thoroughly and excised promptly if paratesticular liposarcoma is suspected, as delayed management gives poor outcomes.
Aggressive gastrointestinal stromal tumor with spinal metastases: a case report

Abstract:We report a case of a 56-year-old male who presented with several month history of severe low back pain. Physical examination revealed generalized tenderness at his thoracolumbar spine without notable neuromuscular findings. Radiographs revealed a chronic compression fracture of T10 and T11 with anterior height loss. Subsequent magnetic resonance imaging demonstrated multiple lytic lesions in the thoracolumbar spine without canal compromise. During his hospital stay, he developed acute cord compression with loss of motor and sensory levels below T12 and an absence of sphincter tone. The patient was taken for emergent multilevel, posterior decompression and fusion with biopsy of the lesion. Microscopic examination of the tissue in addition to immunohistochemical analysis utilizing CD117-antibody/c-kit revealed gastrointestinal stromal tumor. Further workup revealed the primary tumor to be intra-abdominal and the patient was subsequently begun on adjuvant chemotherapy. Gastrointestinal stromal tumors should be considered in the workup of patients with bone metastasis with an unknown primary malignancy.
CT Diagnosis of Extra-Gastrointestinal Stromal Tumors

Abstract:Objective To analyze CT findings of extra-gastrointestinal stromal tumors(EGIST),and to evaluate the CT diagnostic value of EGIST.Materials and MethodsBoth plain and enhanced CT findings of 25 cases with EGIST proved by pathology were retrospectively analyzed,CT angiography was performed in 8 cases.ResultsAmong 25 cases,17 cases were high risk,5 mid risk,3 lower risk,and metastases were found in 5 cases.The lesions located at omentum in 6,mesentery in 10,posterior peritonium in 5,pelvic cavity in 2,multiple locations in 2 cases which originated from posterior peritonium and mesentery.The mean diameter of tumors was 13.2 cm.The tumor was lobulated in shape(n=8),irregular in shape(n=4),round or oval in shape(n=13),1 case was accompanied with calcification,low density necrosis within the lesions(n=23).There were two types of contrast enhancement patterns.Type Ⅰ was almost slightly homogeneous enhancement in the arterial phase,the tumors had slightly enhancement in venous phase than that in arterial phase.Type Ⅱ was moderate inhomogeneous enhancement in the arterial phase,the tumor enhancement was also seen during venous phase.There was no enhancement of cystic degeneration and necrosis in tumors.Among 15 cases,there were a few small vessels in intraabdominal masses.The dilated tumoral feeding artery and draining venous could be found on CTA.ConclusionEGIST have some characteristic CT features which are of great value in diagnosing and following up.

Abstract:Extragastrointestinal stromal tumor (EGIST) in the posterior mediastinum is very rare. Herein, we report FDG PET/CT findings of posterior mediastinum EGIST in a 77-year-old man. On FDG PET/CT, it manifested as a solitary posterior mediastinum mass with heterogeneous necrosis and intense FDG uptake. The final pathology supported a diagnosis of EGIST. This case hints us when we encounter a soft tissue mass with intense FDG uptake in the posterior mediastinum; EGIST should be regarded as a differential diagnosis.

Abstract:Abstract We present the case of a 66-year-old woman with a huge gastrointestinal stromal tumor of the stomach that traversed her upper abdomen. The predominant abdominal sign was a huge, palpable mass, but there were no other distinctive findings in her physical examination or her routine blood workup, including biochemical markers. It was difficult to judge the origin of the mass upon imaging. Furthermore, radiological findings revealed that the mass had a complex relationship with many major blood vessels. An exploratory laparotomy revealed a huge tumor protruding from the anterior wall of the stomach fundus, on the lesser curvature of the stomach, measuring approximately 21 × 34 × 11 cm in diameter and weighing 5.5 kg. A complete resection was performed and the tumor was characterized on immunohistochemistry as a gastrointestinal stromal tumor of the stomach. Preoperative diagnosis of gastrointestinal stromal tumors can be difficult, and we hope that the presentation of this rare case and literature review will benefit other diagnosing clinicians having similar problems.
