Intraoperafive Microvascular Doppler Sonography in Aneurysm Surgery

Abstract:Objective To evaluate the reliability and efficacy of micmvasculur Doppler sonography (MDS)in intracranial aneurysm clipping.Methods 133 consecutive patients harboring 141 intracranial aneurysms were assessed by using MDS with a 20-MHZ miempmbe(1-mm diameter)before and after clipping to confirm the obliteration of aneurysms,patency of purent vessels and branching arteries,and to record the flow within the aneurysm sac,flow spectrum and acoustic signal of arteries.The data were analyzed and compared with those of operative MDS findings and postoperative angiography.Results Intraoperative MDS was applied in all patients.In 42 patients(31.6%),MDS wag helpful in understanding the microvagculur anatomy before clipping.MDS demonstrated unoceluded aneurysm in 11 out of 141 aneurysms,stenosis of parent and adjacent Vessels in 16 out 141.The clips were repositioned on the basis of the MDS results before completing occlusion of the aneurysms and normalized flow velocities.In middie cerebral artery aneurysms,significant findings occurred 40.9%,whereas with aneurysms of the posterior communicating artery,the internal carotid artery,and the anterior communicating artery,the results were 14%、15.2%and 14.3%respectively.The MDS results were correlated with the dale of 133 who underwent post-operative angiography.Conclusions Intmoperative MDS is a noninvagive,reliable and shortcut method,which play a significant role in locating the aneurysm.
