Microsurgical Treatment of Solid Hemangioblastomas in Posterior Fossa: A Report of 21 Cases

Abstract:Objective To discuss microsurgical treatment and outcomes of solid hemangioblastomas in posterior fossa.Methods This retrospective analysis represented our experience with 21 cases of solid hemangioblastomas in posterior fossa admitted to Neurosurgery Department of Tianjin Huanhu Hospital from January 2008 to October 2016.Among them,4 patients received endovascular embolization within preoperative 72 hours whose digital subtraction angiography (DSA) results showed the feeding arteries located on the ventral side of tumors.Results Gross total resection (GTR) was achieved in 18 patients and subtotal resection (STR) in 3.At the early stage post operation,2 patients were in the state of coma (1 case died on the ninth day after operation),1 patient suffered from dyspnea,5 patients developed caudal cranial nerve palsy,2 patients developed dysacousis,1 patient reported facial paralysis and 3 patients developed hydrocephalus.Seventeen patients were followed up for about 3-96 months (average:43.6 ± 8.7 months).No recurrence was found in 15 patients with GTR.The MRI at 6 months post operation showed tumor growth in 1 patient undergoing STR and no change in 1 patient with multiple solid hemangioblastomas located in the medulla and thoracic spinal canal.Conclusions Microsurgical treatment seems to be safe and effective for solid hemangioblastomas in posterior fossa.Intraoperative bleeding could be reduced by preoperative endovascular embolization of tumor feeding artery.
