Abstract:Background: Transdifferentiation of stem cells is regulated by extracellular matrix and gene program. Thus, it is difficult to simulate the microenvironment of in vivo stem cell nests to induce stem cell differentiation in vitro. Stem cell orthotopic transplantation may be a feasible method. Objective: To observe the possibility of transdifferentiation of fetal rat pancreatic stem cells into islet-like cell cluster by transplanting fetal rat pancreatic stem cells into diabetic rats. Design, Time and Setting: The cytology in ivivo study was performed at the Central Laboratory, Shenzhen People's Hospital from March 2006 to April 2007. Materials: A total of 12 Sprague Dawley rats on embryonic day 16 and 50 10-week-old female Sprague Dawley rats, weighing 200-250 g, were supplied by Guangdong Animal Experimental Center. Methods: Fetal rat pancreatic stem cells (PSCs) were harvested and purified. Using fluorescence in situ hybridization, at the fifth passage, fetal rat PSCs were used for in vivo transplantation. 10 of 50 Sprague Dawley female rats served as the normal control group. Remaining 40 rats were utilized to establish diabetic models by intraperitneal injection of streptozotocin. After model establishment, rat models were equally assigned into 4 groups. In the pancreatic parenchyma transplantation group, 1×106 male fetal rat PSCs were injected into rat pancreatic parenchyma at various sites. In the liver transplantation group, an equal volume of cell suspension was infused into the portal vein by puncture. In the subrenal capsule transplantation group, the same volume of cell suspension was infused into the left renal capsule. In the model control group, an equal volume of phosphate buffered saline was injected into the pancreatic parenchyma. Cell culture was conducted for 8 weeks. Main Outcome Measures: Rat blood glucose and plasma insulin levels were regularly monitored. Histological and pathological changes were observed in the pancreatic gland, liver and kidney. Results: At passage 3, nestin-positive rate of male fetal rat PSCs was 74.1%. In the pancreatic parenchyma transplantation group, blood glucose decreased at 3 weeks following transplantation, but plasma insulin levels increased. At 5 weeks, blood glucose and plasma insulin decreased to a normal level. Pathological observation showed exogenous islet-like cell clusters in the pancreatic gland. A positive reaction to insulin immunohistochemistry staining was detected. A strong positive reaction to nestin immunohistochemistry staining was detected. Fluorescence in situ hybridication produced a positive reaction in Y chromosome. Hyperglycemia was found, but exogenous islet-like cell clusters were not seen in rats of the liver transplantation group and subrenal capsule transplantation group. Conclusion: Following orthotopic transplantation in the pancreatic gland, purified fetal rat PSCs can differentiate into islet-like cell cluster, and induce blood glucose to a normal level.
