Complex Congenital Heart Disease in Children: a Comparison Between Lateral Tunnel and Extracardiac Conduit Fortran Procedure

Abstract:Objective To compare between lateral tunnel and extracardiac conduit Fontan procedure in complex congenitalheart disease in children. Methods 128 patients with complex congenital heart disease received the modified Fentan procedure. Theyaged from 1.9 to 16.5 years and weighted from 8 to 62 kg. The main abnormalities included single ventricle (SV) in 59 cases anddouble outlet right ventricle in 25.87 patinas were treated with one stage Fontan procedure and 41 patients with two stages Fontanprocedure. The time between the two operations was 0.8~7.3 years [ (3.9±2.8) years]. The first operations were Glenn in 12 cas-es, double-Glenn in 8 and other procedures in 21.89 patients were treated by lateral tunnel Fentan procedure (LT) and 39 patientsby extracardiac conduit Fontsn procedure (ECC). They were evahiated by ECHO, MRI for 11 cases and catheterizatiou for 30 casesbefore operation. Results The mortality was 13.3% (17/28). The age at operation in the LT group was youger thaninthe ECCgroup. Early postoperative outcome was low cardiac output in 32 patients, renal dysfunction in 11. The postoperative arrhythmias ratein the LT group was higher than in the ECC group. The time of follow-up was from 3 months to 2 years and the results were satisfying.Conclusion The postoperative arrhythmias rate in the LT group was higher than in the ECC group. The ECC Fontan procedure mayavoid the aortic cross-clamping, minimse the cardiopulmonary bypass and keep direct flow from inferior vena cava to superior vena ca-va. The superiority of fenestration is still uncertain.
