Abstract:Objective The aim of this study was to investigate the incidence of hepatitis C virus(HCV) infection and the risk factors for the infection in maintenance hemodialysis(HD) patients from multiple centers in Jiangsu province.We also measured serum aminotransferase level in these patients.Methods Clinical data of these patients,including demographical information,history of blood transfusion and kidney transplantation,were recorded.Liver biochemistry tests including alanine aminotransferase(ALT) and aspartate aminotransferase(AST) were measured.Anti-HCV antibody was assayed by enzyme-linked immunosorbent assay(ELISA),and HCV-RNA was detected by real-time polymerase chain reaction(RT-PCR) method.Results(a) Positive anti-HCV antibody was found in 22.1%(176/796) HD patients,and HCV-RNA was found in 80.7%(142/176) patients with positive anti-HCV antibody.(b) Univariate analysis of HCV infected and non-HCV infected patients indicated that the HCV infection was associated with longer duration on hemodialysis(7.13±4.60 vs.2.73± 3.01 year,P0.05),blood transfusion(64.2% vs.45.5%,P0.01),dialyzer reuse(51.3% vs.7.9%,P 0.05) and kidney transplantation(11.0% vs.3.5%,P0.05).Moreover,logistic analysis revealed that longer duration on hemodialysis(OR=1.38,95% CI=1.24-1.53,P0.001) and dialyzer reuse history(OR=10.91,95% CI=5.52-21.55,P0.01) were the independent risk factors for HCV infection.(c) Despite the normal range of average aminotransferase level in HCV infected patients,the average aminotransferase level and the rate of abnormal aminotransferase level were significantly higher in HCV infected patients than in non-HCV infected patients.Conclusions(a) HCV-RNA detection is required in HD patients with positive anti-HCV antibody for the estimation of HCV activity.(b) Serum aminotransferase level may be an insensitive marker for the diagnosis of HCV infection in HD patients,but is useful for the evaluation of hepatitis activity and hepatocytic injury.(c) Comprehensive interventions should be taken to control HCV transmission in HD patients.
Hepatitis Gene Chip in Detecting HBV DNA, HCV RNA in Serum and Liver Tissue Samples of Hepatitis Patients.

Abstract:OBJECTIVE:To study the preparation of diagnostic gene chip for detecting hepatitis B virus (HBV) and hepatitis C virus (HCV) and its accuracy in detecting HBV DNA and HCV RNA in serum and liver tissues.METHODS:The probes, which depend on the conservative gene fragment of hepatitis virus, was designed, synthesized and spotted on the modified glass. The probes and some other control probes were assembled on the diagnostic microarray of hepatitis virus. The gene of hepatitis virus, purified from blood or tissue, was labeled with fluorescence and hybridized to the microarray. The hybridized microarry was scanned with microarray scanner and the diagnostic result was analyzed from the scanning data. Fourty patients with hepatitis B virus and 40 healthy people or 40 patients with hepatitis C virus were subjected to detection of HBV DNA and HCV RNA with the hepatitis virus gene chip by the double-blind method. Paraffin liver specimens obtained from 99 cases of posthepatitic cirrhosis were used to detect HBV DNA. The liver tissues and serum from 15 cases of chronic hepatitis B were used to detect HBV DNA. Simultaneously, HBsAg and HBcAg were detected in the serum by fluorescence microparticle quantitation, HBV DNA and HCV RNA in the serum by PCR, and HBcAg in liver tissues by immunocytochemistry or HBV DNA by in situ molecular hybridization.RESULTS:Chip detection of serum specimens showed that 30 patients were HBV DNA positive and 10 HBV DNA negative in the 40 patients with HBV positive, 25 patients were HCV RNA positive and 15 patients were HCV RNA negative in the 40 patients with HCV positive, and all were HBV and HCV negative in the 40 healthy people. In 15 patients with HBV marker positive who were subjected to liver biopsy, 15 patients were detected HBV DNA positive in serum by gene chip, 15 patients HBcAg positive in liver tissues by immunocytochemistry, 14 patients HBV DNA positive in liver tissues by in situ molecular hybridization, and 14 patients HBV DNA positive in liver tissues by gene chip. Paraffin liver tissues specimens from the 99 patients with posthepatitis B cirrhosis showed that 67 patients were detected HBcAg positive by immunocytochemistry, 53 patients HBV DNA positive by in situ molecular hybridization, and 46 patients HBV DNA positive by gene chip. In the 46 patients, 40 patients were detected HBV DNA and HBcAg positive by in situ molecular hybridization and immunocytochemistry, 6 patients only HBcAg positive, and 33 patients HBcAg negative.CONCLUSIONS:The designed diagnostic gene chip can be used to simultaneously detect serum HBV DNA and HCV RNA, but the positive rate of HCV RNA diagnosed by this chip is lower. The gene chip can detect HBV DNA in serum and in liver tissue.
Quality Control Measures for Lowering the Seroconversion Rate of Hemodialysis Patients with Hepatitis B or C Virus.

Abstract:BACKGROUND: Hemodialysis (HD) patients are at high risk of infection by hepatitis B virus (HBV) or hepatitis C virus (HCV). The present study was designed to determine the impact of quality control measures on the prevention of transmission of blood-borne viruses.METHODS: A total of 6182 adult maintenance HD patients from all HD units in Zhejiang Province were recruited on January 1, 2007. The baseline demographic and clinical characteristics were recorded and all patients were followed up until death or survival at 4 years later. The Quality Control Standards of Hemodialysis were gradually implemented in HD units. The HBV or HCV seroconversion rates of the recruited patients were calculated and compared every year during the observation period.RESULTS: The prevalence of HBV was 8.3% at the beginning of the study, and 6.6% for HCV. With the implementation of the HD quality control measures, the HBV seroconversion rate tended to decrease year by year (chi(2)=6.620, P=0.085), and the HCV seroconversion rate decreased significantly (chi(2)=10.41, P=0.015). Compared with the data in 2007, the HBV seroconversion rate (chi(2)=4.204, P=0.040, relative risk ratio 0.393, 95% CI 0.156-0.991) and the HCV seroconversion rate (chi(2)=7.373, P=0.007, relative risk ratio 0.386,95% CI 0.189-0.787) decreased significantly in 2010.CONCLUSION: Quality control measures for HD decreased the seroconversion rates of HBV or HCV in HD patients, showing that updated quality control measures reduce the risk for transmission of blood-borne viruses in the HD population.
Discrepancy between serological and virological analysis of viral hepatitis in hemodialysis patients

Abstract:Background and aim: Viral hepatitis is a health threat for hemodialysis (HD) patients and it may be transmitted during treatment. Some patients categorized to have viral hepatitis were found to be non-viremic. To clarify the discrepancy between the serological tests in HD patients, we conducted the study. Methods: A total of 1681 HD patients was included. Blood samples were analyzed for hepatitis B surface antigen (HBsAg) and anti-hepatitis C antibody (anti-HCV). Detection of hepatitis B virus (HBV) DNA and hepatitis C virus (HCV) RNA were performed in either HBsAg (+) or anti-HCV (+) samples. HBV DNA/HCV RNA was also measured in a subset of HBsAg (-) and anti-HCV (-) patients. Liver function tests were analyzed and compared with the serological and virological tests. Results: The serological tests showed that 230 patients (13.7%) were HBsAg (+) and 290 (17.3%) were anti-HCV (+). We were unable to detect HBV DNA in 97 of 230 (42.2%) HBsAg (+) patients, and HCV RNA could not be found in 76 of 290 (26.2%) anti-HCV (+) patients. In 167 HBsAg (-) patients, only one showed a trace amount of HBV DNA. None of 151 anti-HCV (-) patients showed detectable HCV RNA. The prevalence rate of viral hepatitis remains high in Taiwanese HD patients: 13.7% for HBV and 17.3% for HCV. However, virological analysis showed 42.2% non-viremic rate for HBsAg and 26.2% non-viremic rate for anti-HCV. Conclusions: The findings might challenge the presently suggested principles of bed and machine dedication and the diagnosis of viral hepatitis in HD patients.

Abstract:Objective To evaluate the sensitivity of the enzyme immunoassay(EIA)kits of Hunan Jynda Gene Co.Ltd in detecting hepatitis C virus core antigen.Methods Blood samples from 2808 blood donors,1909 outpatients,and 244 anti-HCV(+)cases were randomly selected and hepatitis C virus core antigen(HCV-cAg)was detected with EIA kit.Samples found out to be HCV-cAg(+)were further tested for HCV-RNA and anti-HCV antibody.Results In 2808 blood donors,one case(0.04%)with both HCV-cAg(+)and HCV-RNA(+)was found.In 1099 outpatients,8 cases(0.73%)of HCV-cAg(+)was detected,and 5 of them were anti-HCV(+).Of these 8 HCV-cAg(+)cases,6 were tested for HCV-RNA and 5 were found positive.The other 2 cases of HCV-cAg(+)were not tested for HCV-RAN because of inadequate specimen.Of the 244 anti-HCV(+)cases,HCV-cAg(+)was found in 52,and HCV-RNA(+)in 49.The other 192 HCV-cAg(-)were also tested for HCV-RNA,and 68 of them were HCV-RNA(+).Conclusion EIA kit for the detection of HCV-cAg in human serum or plasma can be used clinically in prevention of the HCV transmission.
The difference of ELISA and LABScreen in detecting HLA antibodies

Abstract:Objective To compare the difference of ELISA and LABScreen in detecting HLA antibodies and evaluate their effects on allograft rejection.Methods Consecutive patients undergoing kidney transplantion from November,2008 to December,2009 in the First Affiliated Hospital and the following up patients during the same period,with abnormal Scr who had completed kidney biopsies,were included in this study.Patients' HLA antibodies were detected by ELISA (Lambda antigen tray,LATTM) and LABScreen Mix Beads (LABScreen MIX,One Lambda,Canoga Park,CA,USA) or LABScreen Single antigen beads (LABScreenTM single antigen beads,One Lambda,Canoga Park,CA,USA).Patients' Scr were also detected at different time potints.Results There were 277 patietns included.Among them 145 (52.3％) cases were HLA antibody positive detected by LABScreen,which including 118 cases ELISA negative but LABScreen positive,and 27 cases both ELISA and LABScreen positive.No case was ELISA positive but LABScreen negative.Among 118 cases which were LABScreen positive but ELISA negative,41 (34.7％) cases happened acute or chronic rejection.However,only 24 cases happened rejection in 132 double negative cases (18.2％,P =0.003).There were 31％ patients in rejection group while only 12.8％ patients (P=0.01) in non-rejection group whose HLA antibody fluorescence intensity detected by LABScreen single antigen beads still increased two weeks after transplantation.Conclusion LABScreen is more sensitive than ELISA in detecting HLA antibodies,and its result highly correlates with the incidence of allograft rejection.
Clinical Significance of Joint Detection of HCV-RNA and Anti-Hcv in Diagnosis of Hepatitis C

Abstract:Objective: To analyze the clinical significance of joint detection of HCV-RNA and anti-HCV in diagnosis of hepatitis C.Methods: We detected anti-HCV by enzyme linked immunosorbent assay and HCV-RNA by real-time fluorescent quantitation PCR.Results: The rate of positive reaction(29.8%) to anti-HCV and HCV-RNA in acute hepatitis C group was lower than that(62.3%) in chronic hepatitis C group(P 0.05);The rate(58.3%)of positive in HCV-RNA and negtive in anti-HCV in acute hepatitis C group was higher than that(24.6%)in chronic hepatitis C group(P 0.05).The correlation analysis showed that HCV-RNA and ALT levels were positively correlated(r =0.725,P0.05).Conclusions: Clinical joint detection of HCV-RNA and anti-HCV contributes to the early diagnosis of hepatitis C.
Screening for hepatitis C virus infection in Peking University People's Hospital in 2015-2018

Abstract:Objective To investigate missed detection in the screening for hepatitis C virus (HCV) infection in a general hospital in 2015-2018. Methods A retrospective analysis was performed for the data of 3659 patients who attended People’s Hospital, Peking University, from 2015 to 2018 and underwent the detection of anti-HCV and HCV RNA. Architect I2000 by Abbott and Vitros 3600 by Johnson and Johnson were used to measure anti-HCV, and the Roche Cobas AmpliPrep/Cobas Taqman 96 automatic virus quantification system was used to measure HCV RNA. The specimens were collected from the patients with positive HCV RNA and negative anti-HCV, and Sanger sequencing was used to determine HCV genotype. The patients were followed up to observe the status of HCV infection and clinical conditions. The signal-to-cut-off (S/CO) ratio was used to express the results of anti-HCV detection. GraphPad Prism 5.0 was used to plot the distribution map of the S/CO ratio of anti-HCV. Results Of all 3659 patients, 6 (0.16%) had negative anti-HCV based on at least one reagent and positive HCV RNA, with a mean level of (6.40±1.86)log10 IU/ml. Among these 6 patients, 5 (83%) had acute leukemia and 1 had respiratory disease; among these patients, 1 had good prognosis, 3 had poor prognosis, and 2 died. Conclusion When antibody is used as the primary screening method for HCV infection, the rate of missed detection reaches 0.16%, and most of these patients have poor prognosis. HCV RNA detection should be performed for patients with immunodeficiency to avoid missed detection.
Monitoring of human cytomegalovirus infection in bone marrow and liver transplant recipients by antigenaemia assay and enzyme-linked immunosorbent assay.

Abstract:Human cytomegalovirus (HCMV) infection is a common complication in transplant recipients. Sensitive, specific and timely diagnostic tests for the detection of HCMV infection remain essential for successful therapy. The results of three tests to detect HCMV in bone marrow and liver transplant recipients were compared: a pp65 antigenaemia assay, an immediate-early (IE) antigenaemia assay and an anti-HCMV immunoglobulin M (IgM) antibody enzyme-linked immunosorbent assay (ELISA). Of 1344 samples, 911 (67.8%) and 917 (68.2%) samples were positive for pp65 and IE, respectively. The coincidence level was 85.1%. There was no statistical difference after transplantation to the first positive detection of HCMV (mean first checkout time) between the pp65 and IE antigenaemia assays. Moreover, the levels of HCMV detected by the pp65 and IE antigenaemia assays were significantly correlated. The HCMV-positivity rate as detected by the anti-HCMV IgM ELISA was 11.1%, which was significantly different from the IE and pp65 antigenaemia assays. We suggest that the IE antigenaemia assay could replace the pp65 antigenaemia assay for monitoring active HCMV infection and early detection of HCMV infection.
Next Generation Sequencing-Based Investigation of Potential Patient-To-Patient Hepatitis C Virus Transmission During Hemodialytic Treatment

Abstract:We investigated potential patient-to-patient transmission of hepatitis C virus (HCV) in two hemodialysis centers in Beijing, China. Approximately 8.25%(32/388) hemodialysis patients were HCV antibody positive, and 4.90% (19/388) were HCV RNA-positive, which consisted of 2a genotype (1/19) and 1b genotypes (18/19). Using next generation sequencing (NGS) approach, MiSeq platform, we sequenced HCV, targeting hypervariable region 1 (263 base-pairs) of genotype 1b specimens and obtained 18 to 243 unique HCV variants. Analysis of phylogenetic tree, viral epidemiology signature pattern (VESP) and Shannon entropy indicated no obvious HCV similarity for most HCV infections but limited HCV variants from Patient 31 (P31) were closer with respect to evolutionary relationship with Patient 24 (P24). However, it was unlikely that HCV was transmitted directly from P24 to P31 in the hemodialysis center. Otherwise, their genetic distance (3.92%-8.92%), would have been much less. Moreover, P31 was infected less than two years before specimen collection, and other external high risk factors existed for these two patients. Thus, our data indicated no evidence of patient-to-patient transmission of HCV in the two hemodialysis centers, suggesting that current HCV infection control measures are effective.
Hepatitis C Prevalence, Incidence, and Treatment in Chinese Hemodialysis Patients: Results From the Dialysis Outcomes and Practice Patterns Study-China (2019-21)

Abstract:Background: Prior work from the Dialysis Outcomes and Practice Patterns Study (DOPPS) showed HCV prevalence in China in 2012-2015 being in the upper third and HCV incidence the 2nd highest among 15 different countries/regions investigated. The goal of the present investigation was to: (1) determine if HCV prevalence and incidence has changed, and (2) collect detailed data to understand how HCV is treated, monitored, and managed in Chinese HD facilities and non-dialysis chronic kidney disease (CKD) clinics. Data and methods: Detailed data for 1,700 randomly selected HD patients were reported by 39 randomly selected HD facilities from Beijing, Shanghai, and Guangzhou participating in the DOPPS 7-China study from 2019 to 2021. The study site medical directors completed a survey regarding numerous aspects of HCV treatment and management in HD and ND-CKD patients. Results: In this 2019 to 2021 cohort, HCV prevalence was 7.4%, which was lower than the 14.8 and 11.5% HCV prevalence for the 2009-2011 and 2012-2015 cohorts, respectively. HCV incidence of 1.2 cases per 100 pt-yrs also was lower compared to the incidence of 2.1 for the 2012-2015 cohort. Although the great majority of study site medical directors indicated that all or nearly HCV+ patients should be treated for their HCV, very few HCV+ patients have been treated presumably due to substantial cost barriers for affording the new direct acting antivirals (DAAs). The randomly selected facilities in our DOPPS 7-China study appear to have excellent programs in place for frequent monitoring of patients and staff for HCV, education of staff, and referral of HCV cases to external infectious disease, gastroenterology, and liver disease specialists. Liver biopsies were not commonly performed in HCV+ HD patients. HCV genotyping also was rarely performed in participating units. Conclusions: Our study indicates a 50% decline in HCV prevalence and a >40% decline in HCV incidence in Chinese HD patients over the past 10-12 yrs. Chinese HD facilities and associated specialists appear to be well-equipped and organized for successfully treating and managing their HCV+ HD and CKD patients in order to achieve the WHO goal of eliminating HCV by 2030.
Significance of the signal-to-cutoff ratios of anti-hepatitis C virus enzyme immunoassays in screening of Chinese blood donors.

Abstract:BACKGROUND: The correlation between signal-to-cutoff (S/CO) ratios of a second-generation hepatitis C virus (HCV) enzyme immunoassay (EIA; Abbott) and a third-generation HCV enzyme-linked immunosorbent assay (ELISA; Ortho) and confirmed HCV infection has been reported. The utility of the values for the Chinese anti-HCV EIA kits, however, has not been studied in evaluating test results in Chinese blood donors. STUDY DESIGN AND METHODS: A total of 156 donor samples repeat reactive for anti-HCV at routine screening from five representative regions of China were retested for anti-HCV by the Ortho third-generation HCV ELISA and six Chinese EIA kits and for HCV RNA by a human immunodeficiency virus-1 and HCV assay (Procleix, Chiron Corp.). The HCV RNA-nonreactive samples were further tested for anti-HCV by a third-generation recombinant immunoblot assay RIBA (Chiron Corp.). The positive result by either nucleic acid amplification test or RIBA was interpreted as confirmed HCV infection. RESULTS: The confirmed HCV prevalence rate in donors in five representative regions obtained in this study was 0.20 percent (77/37,900) in 2004. All seven anti-HCV EIA kits had a significant correlation between S/CO ratios and confirmed HCV infection. The threshold S/CO ratios, which predicted more than 95 percent of confirmed HCV infections for the Ortho, SABC, BGI-GBI, InTec, GWK, KHB, and WANTAI kits, were 3.8, 6.0, 7.0, 8.6, 10.0, 10.0, and 14.0, respectively. CONCLUSIONS: Anti-HCV EIA kits commonly used in Chinese donors screening demonstrate good correlation between S/CO ratios and the confirmed infection. For the Ortho third-generation HCV ELISA, the S/CO ratio of 3.8 determined by the US Centers for Disease Control and Prevention is applicable to Chinese blood donors. The Chinese domestic EIA kits evaluated show a diverse range of threshold S/CO ratios.

Abstract:Objective To study the prevalence of hepatitis C virus(HCV)in different subjects in Qidong area.Methods The sera from the patients with liver cancer(LC),the HBsAg carriers(CR)and general population(GP)were collected to detect anti-HCV by using ELISA assay.HBV,anti TTVIgG,ALT,and other related markers were also tested for comparison.Results The prevalence rates of anti-HCV for the three groups were 5.11% in LC,1.82% in CR,and 2.42% in GP,respectively(P>0.05).No relationship was found between infections of HCV,HBV,and TTV,and age,sex,and blood types.Six out of 9 liver cancer patients with positive anti-HCV were accompanied with HBsAg.One case,with no detectable HBVM,ALT,and AFP;and another one,co-infected with TTV.Conclusion The prevalence rate of anti-HCV is not high in Qidong area.There is no significant difference between patients with liver cancer,carriers of HBsAg,and general population.It seems that the prevalence of anti-HCV,as compared with the past data,has showed a decrease.

Abstract:Background: Hepatitis C virus (HCV) is a major cause of liver disease and a potential cause of substantial morbidity and mortality. This study aims to provide a comprehensive evidence on HCV Infection in Iranian hemodialysis (HD) patients we conducted a systematic review. Materials and methods: In this systematic review and meta-analysis, through a comprehensive search of literature until January of 2016, we estimated the pooled prevalence of hepatitis C infection in Iranian HD patients. Using Medical Subject Headings terms, Emtree, and related equal Persian key words for Iranian databases and also international databases of PubMed and NLM Gateway (for MEDLINE), and SCOPUS. Interest outcome of HCV infection prevalence was confirmed based on positive hepatitis B surface antigen of blood samples. Random effect meta-analysis was used to estimate pooled prevalence of HCV infection in Iranian HD patients, date and language, HD patients, in adult HD patients, Institute of Scientific Information, Iran-doc, irrespective of age, living in Iran. Searches run through main domestic databanks of Iran-Medex, renal transplantation, Scientific Information Database, the relevant literature-searched concentrating on HCV infection. Results: Through searching steps, 305 publications were found from them following the excluding duplicates and overlapping studies 54 studies relevant to HCV prevalence in Iranian HD zcxw patients, with number of 23921 participants, remained in our analyses. The overall results of test of heterogeneity demonstrate sever heterogeneity between reported prevalence (I2 = 96.62%, Chi-square = 1566, P < 0.001). Due to sever heterogeneity results of random effect meta-analysis showed that the estimated pooled prevalence was 11% (95% confidence interval [CI] =10%-13%). The pooled prevalence base on polymerase-chain reaction, recombinant immunoblot assay, and enzyme-Linked Immunosorbant Antibody method were 11% (95% CI = 6%-15%), 9% (95% CI = 5-13) and 12% (95% CI = 10-14), respectively. In line with previous studies, the present finding shows the significant variation in the rate of HCV in dialysis units among the regions in Iran. Conclusion: Present paper is the comprehensive updated systematic review on HCV prevalence in the Iranian HD patients. Our findings provide the reliable evidence for promotion of policies and interventional programs.
[Detection of Serum Anti-F Antibody in Hepatitis C Virus Infected Patients].

Abstract:OBJECTIVE:To assess the prevalence of serum anti-F in patients with hepatitis C virus (HCV) infection and the distribution of anti-F.METHODS:The recombinant protein (HCV-F/GST) was coated onto micro titer plates as antigen. Sera of 120 patients with hepatitis C virus infection, 15 patients with hepatitis B, 3 patients with hepatitis E and 10 normal sera were tested by indirect ELISA for detecting anti-F.RESULTS:82 samples out of the 120 (68%) HCV infected patients exhibited a positive anti-F reaction, showing significant difference from the controls with no HCV infection (P < 0.01). Data from logistic analysis showed that the positive rate of anti-F was higher in patients over 50 year olds (OR = 6.675, 95% CI: 2.407-19.071). Patients of midrange, severe phase and hepatic cirrhosis had higher rate than the others (OR = 2.749, 95% CI: 1.470-5.141).CONCLUSION:Prevalence and distribution of anti-F in Yixing hepatitis C patients was reported and which might be related to the progression of HCV infection.

Abstract:Objective To observe the clinical outcome of maintenance hemodialysis patients with hepatitis C viruses(HCV)infection.Methods A retrospective investigation about hepatitis C viruses infection was performed in our hemodialysis centre on December 31,2007.The clinical data including demography,serum transaminase,bilirubin,albumin,anti-HCV antibody,HCV RNA,infection-control procedures associated hemodialysis and ultrasound of the liver was analyzed.Results The prevalent rate of HCV infection in our hemodialysis centre was 20.2%(20/99),The mean age of maintenance hemodialysis patients with HCV infection was(63.3±10.9)years old,the duration on dialysis was(79.9±38.7)months,and the duration of positive serum anti-HCV was(32.6±22.6)months(the longest duration was 103.5 months).16 patients(80%)had blood transfusion history in the patients with HCV infection.Only four cases had viremia of HCV,the serum HCV RNA became negative in one of them spontaneously and in another one after interferon therapy for half a year,the other two of them have viremia now.Most of patients found their serum transaminase elevated transiently and mildly.The age and duration on dialysis of the patients with positive serum anti-HCV were significantly longer than those of patients with negative serum anti-HCV.After strict infection-control procedures were carried out,the new onset cases of HCV infection was significantly decreased(1.2% vs 5.3%,P0.05).Blood transfusion(P=0.001)and duration on dialysis(P=0.001)are the independent risk fators of HCV infection in hemodialysis patients by logistic analysis.Conclusion Blood transfusion and duration on dialysis are the independent risk factors of HCV infection in hemodialysis patients.Strict infection-control procedures help to decrease the prevalence of HCV infection in hemodialysis unit.

Abstract:Objective To explore the value of hepatitis C virus antibody (HCV Ab) and hepatitis C virus nu-cleic acid (HCV RNA ) for the diagnosis of hepatitis C .Methods Real-time fluorescence quantitative polymerase chain reaction (FQ-PCR) was used to detect HCV RNA ,enzyme-linked immunosorbent assay (ELISA) and colloidal gold labeled method was used to detect HCV Ab ,and the rate method was used to detect alanine aminotransferase (ALT ) in 116 patients with hepatitis C .Results The positive ratios of HCV Ab detected by ELISA ,HCV RNA , HCV Ab detected by colloidal gold labeled method and ALT were respectively 83 .6% ,73 .3% ,66 .4% and 59 .5% . Positive rated of joint determination of HCV Ab and HCV RNA was 100 .0% .Conclusion Combined detection of HCV Ab and HCV RNA could be conducive to the diagnosis of hepatitis C .

Abstract:Development of HCV infection is typically followed by chronic hepatitis C (CHC) in most patients, while spontaneous HCV viral clearance (SVC) occurs in only a minority of subjects. Compared with the widespread application of HCV RNA testing by quantitative RT-PCR technique, HCV core antigen detection may be an alternative indicator in the diagnosis of hepatitis C virus infections and in monitoring the status of infectious individuals. However, the correlation and differences between these two indicators in HCV infection need more investigation, especially in patients coinfected by HIV-1. In this study, a total of 354 anti-HCV and/or anti-HIV serum positive residents from a village of central China were enrolled. Besides HCV-related hepatopathic variables including clinical status, ALT, AST, anti-HCV Abs, as well as the altered CD4+/CD8+ T cell counts, HCV core antigen and HCV viral load were also measured. The concentration of serum HCV core antigen was highly correlated with level of HCV RNA in CHC patients with or without HIV-1 coinfection. Of note, HCV core antigen concentration was negatively correlated with CD4+ T cell count, while no correlation was found between HCV RNA level and CD4+ T cell count. Our findings suggested that quantitative detection of plasma HCV core antigen may be an alternative indicator of HCV RNA qPCR assay when evaluating the association between HCV replication and host immune status in HCV/HIV-1 coinfected patients.

Abstract:Background The accurate assessment of liver fibrosis among hemodialysis patients with chronic hepatitis C (CHC) is important for both treatment and for follow up strategies. Applying the non-invasive methods in general population with viral hepatitis have been successful but the applicability of the aminotransferase/platelet ratio index (APRI) or the fibrosis-4 index (FIB-4) in hemodialysis patients need further evaluation. Materials and methods We conducted a prospective, multi-center, uremic cohort to verify the applicability of APRI and FIB-4 in identifying liver fibrosis by reference with the standard transient elastography (TE) measures. Results There were 116 CHC cases with valid TE were enrolled in our analysis. 46 cases (39.6%) were classified as F1, 35 cases (30.2%) as F2, 11 cases (9.5%) as F3, and 24 cases (20.7%) as F4, respectively. The traditional APRI and FIB-4 criteria did not correctly identify liver fibrosis. The optimal cut-off value of APRI was 0.28 and of FIB-4 was 1.91 to best excluding liver cirrhosis with AUC of 76% and 77%, respectively. The subgroup analysis showed that female CHC hemodialysis patients had better diagnostic accuracy with 74.1% by APRI. And CHC hemodialysis patients without hypertension had better diagnostic accuracy with 78.6% by FIB-4. Conclusions This study confirmed the traditional category level of APRI and FIB-4 were unable to identify liver fibrosis of CHC hemodialysis patients. With the adjusted cut-off value, APRI and FIB-4 still showed suboptimal diagnostic accuracy. Our results suggest the necessary of TE measures for liver fibrosis in the CHC uremic population.

Abstract:Background The GALAD and ASAP scores are two well-recognized algorithms to estimate the risk of hepatocellular carcinoma (HCC) based on gender, age, alpha-fetoprotein (AFP), protein induced by vitamin K absence or Antagonist-II (PIVKA-II) and AFP-L3 (included in the GALAD score but not in the ASAP score). The current study sought to compare the diagnostic performance of each score to detect HCC among patients infected with hepatitis C virus (HCV). Methods A multicenter case-control study was undertaken in which blood samples were collected from HCVinfected patients with and without HCC. Using the area under the receiver operating characteristic curve (AUROC), ASAP and GALAD scores were compared relative to diagnostic performance to detect any stage HCV-HCC and early-stage HCV-HCC. Results The analytic cohort included 168 HCV-HCC patients and a control group of 193 HCV-infected patients. The ASAP score had a higher AUROC to detect any stage HCV-HCC versus the GALAD score, both in the overall group (0.917 vs. 0.894, P=0.057) and in the cirrhosis subgroup (0.909 vs. 0.889, P=0.132). Similar results were noted relative to the detection of early-stage HCV-HCC, whether defined by BCLC staging (stage 0-A: 0.898 vs. 0.860, P =0.026) or 8 th TNM staging (stage I: 0.899 vs. 0.870, P =0.070). In subgroup analysis to detect AFP-negative HCV-HCC, the ASAP score also demonstrated a higher AUROC than the GALAD score to detect any stage HCV-HCC in the AFP-negative subgroup (0.815 vs. 0.764, P =0.063). Conclusions The ASAP score had better diagnostic performance for early detection of HCV-HCC compared with the GALAD score. The ASAP score may be preferrable to the GALAD score for HCC screening and surveillance among HCV-infected patients.
