Abstract:Objective To evaluate the effectiveness and efficiency of mesially inclined second molar correction with uprighting spring.Methods Nine patients with 12 impacted second molars were treated with uprighting spring and fixed appliance.The inclination of second molar on panoramic film before and after treatment was measured and the improvement of the tooth inclination was compared with paired t-test.Results Good results were obtained in all patients, the impacted teeth were corrected.The average inclination of second molar was increased by (37.0 ±15.2) degrees (P＜0.05).The treatment time was between 9 to 36 months.Conclusions The cantilever uprighting spring is an easy and reliable way to correct the mesially inclined second molar.
[A new method to orthodontically correct dental occlusal plane canting: wave-shaped arch]

Abstract:Objective: To introduce a technique of second order wave-shaped arch wire to orthodontically treat dental occlusal plane canting (DOPC) with left-right interactive anchorage, and to test its clinical efficacy. Methods: Among the permanent dentition malocclusion patients who showed no obvious facial asymmetry, we screened for patients who showed anterior occlusal plane canting (AOPC) after routine orthodontic examination, treatment planning, MBT fixed appliance installation and serial arch wires alignment. Each patient had been clinically appraised in frontal view by 2 orthodontists and the patient him/herself; if all 3 agreed that the AOPC was obvious, the patient was included. By this means, we included 37 patients, including 10 males and 27 females; the average age was (21.9±5.2) years. To correct AOPC, opposite direction equal curvature second order rocking-chair curve was bent on each side of 0.46 mm×0.56 mm stainless steel edgewise wire. With reference to normal occlusal plane, a curve toward the occlusal surface was made to extrude under-erupted teeth on one side while a curve toward the gingiva was made to intrude over-erupted teeth on the other side, so that the arch wire was made into a wave shape in vertical dimension. Before and after application of wave-shaped arch wire, frontal facial photographs were taken when the patient's mouth was open slightly with lips retracted to show anterior occlusal plane (AOP) clearly. An AOP was constructed by connecting the center of the slot in the medial edge of canine bracket on each side in the photograph. The angles between the bipupillary plane(BPP) and the constructed AOP were measured in ImageJ1-48v software and the angle differences before and after treatment were compared with paired Wilcoxon test in SPSS 10.0 software. Results: The wave-shaped arch could correct AOPC effectively in 3 to 10 months time with an average of 5.5±1.7 months; the angles between AOP and BBP before treatment ranged from 2.90° to 6.12° with a median of 4.01°; after treatment the angles were from -0.17° to 2.57° with a median of 1.87°, the decrease of the angles between AOP and BBP after treatment ranged from 1.08° to 4.15° with a median of 2.21°. Paired Wilcoxon test P was 0.000. Conclusion: The wave-shaped arch can be used independently or in combination with other treatment methods, which can take advantage of left and right interactive anchorage to correct AOPC effectively, so it has certain application value in clinical practice.
Intrusion of Overerupted Upper Posterior Teeth with Modified Transpalatal Arch

Abstract:Objective To evaluate the clinical effect on intrusion of overerupted upper posterior teeth with modified transpalatal arch.Methods Five adult patients with 5 overerupted upper posterior teeth were treated with modified transpalatal arch.Study models were taken before and after orthodontic treatment.The effect on intrusion of overerupted posterior teeth and movement of anchorage teeth were evaluated three dimensionally.The root resorption was assessed on panoramic radiograph.Results All overerupted upper posterior teeth were intruded by (2.29±0.44)mm.The average treatment time was 6 months.Comparison of the intrusion amount in this group using the t-test for paired samples showed a statistically significant difference (P＜0.001),Some of anchorage teeth were extruded and the others were intruded.The movement ranged from -0.24 mm to 0.71 mm (negative value presented extrusion,positive value presented intrusion).No statistically significant difference was found in the movement amout the anchorage teeth (P＞0.05).There was no significant root resorption in the intruded posterior teeth and the anchorage teeth.Conclusions The modified transpalatal arch was an effective method to intrude the overerupted upper molars which were not suitable for intrusion with implant anchorage.

Abstract:Background Maxillary canine impaction has been extensively reported, but studies of mandibular canine impaction are relatively few. Clinical studies and reports of experience treating mandibular canine impaction are of clinical benefit to both orthodontists and dentists. Aims This report introduces a Class II division 2 crowded case with severely impacted, fully transposed, mandibular canines treated by a non-extraction approach and mandibular arch expansion. Methods The completely transposed, impacted, mandibular canines were successfully aligned. The crowding was relieved by arch expansion and incisor proclination without obvious radiographic alveolar bone loss. The two severely labially displaced mandibular canines assisted in the expansion of the narrow mandibular arch. Results The maxillary and mandibular arches were expanded and well aligned, and Class II molar and canine relationships and a normal overjet and overbite were established. The mandibular canines were aligned in advantageous positions. There was no regional alveolar bone resorption around the mandibular canines. Conclusion The treatment approach provided an example of significant bone remodelling. Using the impacted mandibular canines for anchorage, the age of the patient provided an opportunity to adopt a relatively conservative and unique treatment approach which may be applied to relieve crowding.
Orthodontic traction of an impacted maxillary third molar through a miniscrew-anchored cantilever spring to substitute the adjacent second molar with severe root resorption

Abstract:Background: An orthodontic traction technique with a miniscrew-anchored cantilever spring was used for the traction of a deeply impacted maxillary third molar to substitute its adjacent second molar with severe root resorption. Case description: A 30-year-old woman sought multidisciplinary treatment planning for a second molar with severe root resorption caused by a deeply impacted third molar. Panoramic radiographs and cone-beam computed tomographic images showed that the third molar was located on the apical and palatal side of the second molar and that the second molar had severe root resorption. The second molar was removed, and the impacted third molar was orthodontically tractioned occlusally with the aid of a miniscrew-anchored cantilever spring. After 12 months of orthodontic treatment, the maxillary left third molar was successfully tractioned occlusally and well aligned with adjacent teeth for the substitution of the second molar. The patient was satisfied with the treatment outcome. Practical implications: A palatal miniscrew-anchored cantilever spring is an effective and viable technique for the orthodontic traction of deeply impacted third molars to substitute second molars with severe root resorption.
The anterior mandibular expansion with the technique of distraction osteogenesis

Abstract:Objective When clinical correction of the narrowing of mandibular arch, the existing protocol was correcting the teeth axis of mandible. It was not the real expansion of the mandibular arch, the effect of correction is suspected. It is possible to use the technique of distraction osteogenesis to make the real expansion of the mandibular arch. In this paper we want to discuss the surgical procedures, effects, and cautions of the technique. Methods Three cases of patient with developmental narrowing of the mandibular arch, associate with retrognathia of mandible or protrusion of maxilla. The correcting plan was divided into 2 stages: First, expansion the anterior port of mandible or expansion the maxilla simultaneously. Second, orthognathic surgery procedured to correct the associate deformities. The orthodontic techniques were used to align the teeth between the two stages. Results After the expansion, the anterior part of patients' mandible were widened 7mm-10mm. And after the operations of the second stage, the patients' profile and occlusion were satisfied and stable. Conclusion The technique of distraction osteogenesis can be used to correct of the narrowing of the mandibular arch. It is easily to spread with its uncomplicated procedures, little postoperatively reaction and affirmative effects.

Abstract:Objective: To analyze and summarize the therapeutic effects of multiloop edgewise arch wire (MEAW) on open bite. Methods: 12 cases of various open bite were treated by MEAW technique. Reasults: After therapy, every of 12 patients was corrected with satisfactory results. Conclusion: Open bite can be treated successfully by MEAW technique.

Abstract:Objective: To evaluate the effectiveness of the segmental arch technique in fixing replanted traumatically avulsed permanent teeth.Methods: Between 1996 and 2007,we treated 85 traumatically avulsed teeth of 65 patients by fixing the replanted teeth with Edgewise brackets and the segmental arch technique,recorded relevant indexes and observed the clinical effect.Results: We achieved excellent curative effect in 84(98.82 %) of the 85 teeth,failing only in 1(1.18 %),which was extracted 1 year later.Conclusion: The segmental arch technique with Edgewise brackets,with easy operation and unique advantages over conventional arch bar splint fixation,is a reliable method for the stabilization of traumatically avulsed teeth.
A Comparison Study on the Clinic Effects of Segmental Arch and Ni-Ti Closure Coiling in Orthodontic Canine Distalization

Abstract:Objective To compare the clinic effects of segmental arch and Ni-Ti closure coiling in the orthodontic application of canine distalization.Methods The edgewise arch wire fixed appliance technique was applied in the treatment of 36 cases of dental crowding,whose four first premolars were extracted and canine distalization was needed according to the treatment protocol.18 cases were randomly selected as group A,to whom the canines were distalized with segmental arch;the other 18 cases were set up as group B,to which the canines were distalized with Ni-Ti closure coiling.The distance of canine distalization and the angle of anchorage molar inclinization in all cases was measured and compared.Results The mean velocity of canine distalization in the group A was(1.285±0.084) mm/month,it was faster than the(0.987±0.066) mm/month of group B;the mean degree of molar inclinization(1.86±0.12)°in group A was less than the(2.49±0.14)°in group B.Statistic analysis found that the differences were significant.Conclusion The segmental arch can move the canine faster and control the anchorage molar better than Ni-Ti closure coiling,so it is suggested that the segmental arch should be the preferential selection in orthodontic canine distalization

Abstract:Objective The objective of this study is evaluating the effect in open bite treated with multiloop edgewise arch wire and straightwire appliance and to analyse its therapeutic effects and principles.Methods With straight wire appliance,13 openbite cases were treated using Multiloop edgewise arch wire,and the change of jaws,overbite and overjet were observed by cephalometry.Results Satisfactory results have been achieved in all cases.Conclusion The above mentioned technique could be a reliable way to treat open bite patient and it worth to general used in clinic orthopaedic treatment。
[Three-Dimensional Analysis of the Early Correction of Anterior Crossbite Using Eruption Guidance Appliance].

Abstract:OBJECTIVE:To investigate the clinical effect of eruption guidance appliance in the treatment of anterior cross bite in mixed-dentition children.METHODS:In the study,10 mixed-dentition children with anterior cross bite, totally 12 incisors, were selected. Alginate was used to take upper and lower dentition impression and make a hard plaster model,which served as the eruption guidance appliance for treatment. The pre- and post-operative dental casts were digitized with SmartOptics Activity 880 scanner,and the three-dimensional overlapping models were obtained by reverse engineering software,Geomagic Studio 2012,then the three-dimensional movements of the upper and lower incisors were analyzed using Imageware 13.2 software. The overbite and overjet were analyzed using the same methods. Measurement with copper wire was used to analyze the upper and lower arch length. Space analysis was the result by the sum of crown width minus the arch length. The crown width of unerupted permanent teeth was according to X-ray method to predict. The SPSS 17.0 software was used to analyze the pre- and post-operative measurements of the same child. The normality test of the measured data showed that it conformed to the normal distribution. Therefore,the t test and double side test were used,and the significance level was 0.05.RESULTS:The course of treatment was (5.6±2.7) months. During orthodontic treatment, the upper incisors moved mainly labially (P<0.001) in three-dimensional displacement, and the lower incisors moved mainly the same direction (P=0.025). During the treatment of eruption guidance appliance,the average overbite decreased (1.01±0.9) mm (t=-3.531, P=0.006), and the difference was statistically significant. There was no statistically significant difference between the pre- and post-operative average overjet (t=0.771, P=0.460). The severity of crowding in upper arch decreased (1.9±0.99) mm (t=-6.042, P<0.001),and that in lower arch decreased (1.9±0.74) mm (t=-8.143, P<0.001), both of the differences were statistically significant.CONCLUSION:The anterior cross bite in mixed dentition could be corrected by eruption guidance appliance, and at the same time, the normal overjet and overbite were established, and the teeth were aligned.
Effects of modified transpalatal arch on intrusion of overerupted maxillary second molars-a quantitive study

Abstract:Objective To evaluate the clinical effect on intrusion of overerupted upper second molars with modified transpalatal arch.Methods Thirteen adult patients with eighteen overerupted upper second molars were treated with modified transpalatal arch.Record models were taken before and after orthodontic treatment.After models scanning,the movements of overerupted second molars and anchorage teeth were measured three dimensionally by Rapidform 2006 software.The root resorptions were assessed with panoramic radiographs.Results All overerupted upper second molars were intruded successfully.The average treatment time was 5.5 months.The movement of four cusps of overerupted molars were measured at cusp apex of MB,DB,MP and DP.The intrusion distances were 0.90 mm,0.94 mm,1.78 mm,and 1.78 m.The distallization distances were 1.06 mm,1.06 mm,1.03 mm and 0.92 mm.The palatal movement distances were 0.95 mm,0.88 mm,1.10 mm and 0.85 mm.T-test for paired samples showed a statistically significant difference (P＜0.001).The movements of anchorage teeth were not obvious.Conclusions The modified transpalatal arch was an effective and simple method to intrude overerupted upper second molars.

Abstract:This report describes the treatment of a 16-year-old girl with a severe transverse dental arch discrepancy resulting from a mildly constricted maxillary arch and an overexpanded mandibular arch. The patient had a Class I skeletal relationship, a high mandibular plane angle, a Class III molar relationship, bilateral posterior crossbites, and deviated midlines. A 3-dimensional digital setup was used to assist treatment planning. The digital setup allowed us to evaluate multiple treatment options before deciding on the most suitable one for the patient. The final treatment protocol consisted of extraction of upper second premolars and lower first molars due to enamel hypoplasia. Starting with the leveling and alignment stage of treatment, the maxillary archwires were expanded and the mandibular wires were constricted to correct the transverse discrepancy. An auxiliary expansion arch was used to achieve overcorrection. The active treatment period was 24 months. Proper over-bite and overjet, facial balance, and good occlusion were achieved. Abstract:Background Dental implant technology has developed rapidly in recent years. However, the use of implant-supported fixed bridges with cantilevers has been controversial. The purpose of this study was to evaluate the clinical results of the mandibular anterior implant-supported fixed bridges with a cantilever.Method Thirty-three patients (15 males, 18 females; mean age, 42.6 years; range 20-54 years) with two missing anterior mandibular teeth had single implant-supported fixed bridges with a cantilever. Clinical examination was recorded and radiographs were taken. The mean duration of follow-up was 30 months (15-44 months).Results All implants survived. Loosening or fracture of the prosthesis was not observed. All patients were satisfied with the treatment. The mean bone resorption values after 12, 24, and 36 months of implant loading were 0.94, 1.18 and 1.35 mm respectively. The changes of gingival papilla height ranged from 0 to 0.5 mm. There was significant difference between 1-year and 2 or 3 years restoration groups regarding the average gingival height changes (P <0.05).Conclusion After careful and precise selection of patients, restoration with a single implant-supported fixed bridge with a cantilever can be recommended if two anterior mandibular teeth are missing.
Efficacy of a four-curvature auxiliary arch at preventing maxillary central incisor linguoclination during orthodontic treatment: a finite element analysis

Abstract:Background: Correct torque of the incisors is beneficial in the assessment of the effects of orthodontic treatment. However, evaluating this process effectively remains a challenge. Improper anterior teeth torque angle can cause bone fenestrations and exposure of the root surface. Methods: A three-dimensional finite element model of the maxillary incisor torque controlled by a homemade four-curvature auxiliary arch was established. The four-curvature auxiliary arch placed on the maxillary incisors was divided into four different state groups, among which 2 groups had tooth extraction space retracted traction force set to 1.15 N. Initial displacements and pressure stresses of the periodontal tissue in the maxillary incisors and molars were calculated after torque forces (0.5, 1, 1.5, and 2 N) were applied to the teeth at different stable states. Results: The effect of using the four-curvature auxiliary arch on the incisors was significant but did not affect the position of the molars. Given the absence of tooth extraction space, when the four-curvature auxiliary arch was used in conjunction with absolute anchorage, the recommended force value was < 1.5 N. In the other 3 groups (i.e., molar ligation, molar retraction, and microimplant retraction groups), the recommended force value was < 1 N. The application of a four-curvature auxiliary arch did not influence the molar periodontal and displacement. Conclusion: A four-curvature auxiliary arch may treat severely upright anterior teeth and correct cortical fenestrations of the bone and root surface exposure.
The Impact of the Restoration of Invisible Orthodontic Titanium Alloy Implant Without Bracket on Individuals Afflicted with Dental Malocclusion and Arch Deficiency Accompanied by Periodontitis and a Local Periodontal Inflammation

Abstract:Objective: To investigate the impact of the restoration of non-bracket invisible orthodontic titanium alloy implant on individuals with dental malocclusion and arch deficiency accompanied by periodontitis and local periodontal Inflammation. Method: A cohort of 120 patients presenting with dental malocclusion and defects compounded by periodontitis, were treated at our institution between January 2021 and January 2022; these patients were enrolled in a randomized controlled trial.. These patients were allocated into two groups. The control group (comprising 60 cases) underwent titanium alloy implant restoration, while the research group (also with 60 cases) received titanium alloy implant restoration following invisible orthodontic treatment without brackets. A one-year post-treatment follow-up was conducted, during which various parameters, including pain levels, aesthetic improvement, inflammatory response, dental function, oral hygiene, and the incidence of adverse events, were evaluated and compared before and after treatment between the two groups. Results: After six months of treatment, the visual analog scale (VAS) in the study group was lower than that in the control group (P < .05). After 6 months of treatment, the research team observed the changes in gingival crevicular interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), Interleuckin-1 (IL-1), plaque index (PLI), and soft dirt index (DI) were all lower than those in the control group (P < .05). After 6 months of treatment, the research group had higher scores for tooth functions such as chewing, swallowing, speech expression, and occlusion than the control group, as well as higher pink and white aesthetics indexes (P < .05). The difference in the incidence rate of adverse outcomes between the research and control group was not distinct (P > .05). Conclusion: In case of dental malocclusion accompanied by periodontal disease, the utilization of titanium implants for rectifying dental arch deformities without the use of orthodontic brackets, devoid of orthodontic brackets, has demonstrated notable efficacy in alleviating patients' periodontal discomfort, their oral hygiene, and dental functionality. This modality is conducive to augmenting dental aesthetics without incurring heightened rates of unfavorable consequences, thereby enhancing treatment outcomes.

Abstract:A boy, aged 12 years 3 months, sought treatment for a complete unilateral Brodie bite. His maxillary dental arch was asymmetric, and his dentition was 10 mm wider than normal values for his age. The transverse discrepancy was his chief complaint because it caused a chewing dysfunction. We used a special slow maxillary contraction appliance, which contains a screw, connectors, and retainers, to contract the maxillary transverse asymmetric dental arch. This was followed by preadjusted fixed appliances to level the teeth and adjust the occlusion. A symmetric and functional Class I occlusion was achieved.
Two kinds of methods to treat second molar scissors-bite

Abstract:Objective To evaluate the advantages and disadvantages of 2 kinds of methods in treating second molar scissors-bite.Methods 38 cases(22 males,16 females) were divided into 2 groups.One was treated with mutual elastics on the upper and lower teeth,while the other was treated with modified Nance arch.Results After 3 to 4 months′ treatment,the right locked occlusion was relieved.The average time treating with modified Nance arch was less than that treating with mutual elastics.Conclusions Both mutual elastics and modified Nance arth can cure the second molar scissors-bite,while the latter is more effective and can relieve the symptoms in a shorter time.

Abstract:OBJECTIVE:To evaluate the orthodontic treatment outcome in patients with impacted maxillary central incisor in the mixed dentition.METHODS:Nine patients, aged 8 to 11 years, with impacted maxillary central incisor were treated orthodontically. The cone-beam CT(CBCT) was taken before treatment, after treatment and one year out of retention to evaluate the root length, root canal wall thickness, width of the apical foramen, and degree of root bending, alveolar bone height and thickness. The crown-to-root ratio was calculated. The periodontal and endodontic conditions were evaluated. The parameters of the treated incisors and contralateral ones served as controls were compared.RESULTS:Nine impacted teeth were treated successfully. Throughout the treatment, the root of impacted central incisor continued to develop and the alveolar bones also continued to develop and remodel. The condition of the alveolar bone of vertically impacted teeth was better than that of horizontally impacted ones. No periodontal pocket or pulp necrosis was found after treatment. Seven patients were examined one year after treatment. No significant difference was found in root length, palatal alveolar bone level and palatal alveolar bone thickness. The control group root length was (13.07±2.15) mm, the treatment group root length was (12.06±2.00) mm. No further alveolar bone loss, gingival recession and pulp necrosis were found. The control group labial and palatal alveolar bone levels were (0.90±0.62), (0.45±0.52) mm, labial and palatal alveolar bone thickness were (0.85±0.14), (1.21±0.41) mm. The treatment group labial and palatal alveolar bone levels were (2.18±1.59) mm, (0.57±0.71) mm, labial and palatal alveolar bone thickness were (0.48±0.29), (1.43±0.31) mm.CONCLUSIONS:Orthodontic therapy for impacted maxillary central incisor in the mixed dentition could promote root development and alveolar bone remodeling. Good periodontal and endodontic conditions were achieved.
Open surgical exposure of two horizontally impacted maxillary incisors combined with orthodontic treatment: A case report

Abstract:Introduction: Maxillary anterior teeth that have not erupted may substantially alter the appearance of the teeth and face. Orthodontists often encounter a clinical challenge while dealing with an impacted maxillary incisor, which creates space problems in the anterior region. The purpose of this paper is to describe the well-synchronized orthodontic and surgical treatment of a horizontally impacted maxillary central incisors. Case presentation: A male patient, aged 27, presented with a complaint of unerupted two maxillary front teeth. This resulted in the displacement of adjacent teeth into the vacant region. An intraoral examination revealed a Class II molars on both sides, a deep curve of the space with a 2.3 mm overjet, and an edge-to-edge bite of 0.1 mm. The 3D cone beam computed tomography (CBCT) imaging unveiled a labial impacted and a rotation of approximately 90 degrees (horizontal impacted) on both central maxillary incisors. Discussion: The self-ligating bracket was installed and orthodontic traction aligned the affected tooth in the dental arch. To reach the labial surface of the impacted incisor, open surgical exposure by window excision of soft tissues with a laser was preferable due to the large bulge in the sulcus. Because self-ligating bracket systems employed modest pressures to position the maxillary right central incisor in the arch, the window surgical technique did not produce gingival scarring or increased clinical crown length. Conclusion: The impacted upper central incisor was successfully treated using a collaborative interdisciplinary (surgical-orthodontic) approach, which resulted in a favorable aesthetic and functional outcome.
