Eosinophilic Granuloma of the Orbit: Report of Seven Cases

Abstract:Objective To investigate and further understand the presentation, clinical features and diagnosis of orbital eosinophilic granuloma. Methods In this retrospective study, the clinical data of 7 orbital eosinophilic granuloma patients treated in Zhongshan Ophthalmic Center, Sun Yat-sen University from May 2005 to August 2009 was collected and analyzed. Results Among the total 7 cases, 4 cases were male and 3 were female. The right eye was affected in 4 cases and the left one in 3 cases. Six patients were aged from 3 to 9 years, with only other one older than 10 years. All 7 cases showed local soft tissue swelling of the upper eyelid or the temporal periorbital area, accompanied by various degrees of pain, proptosis, ptosis, local skin numbness.Orbital CT revealed low-density orbital lesion and extensive osteolysis, centered at the marrow space of the anterolateral frontal bone. B-type ultrasund showed moderate to low level of echogenic mass with the echo distributed unevenly and ill-defined. Surgical biopsy or subtotal curettage was taken preoperatively for making a definite diagnosis. Postoperative steroid or 1ow-dose radiation revealed good therapeutic outcome. Conclusions Orbital eosinophilic granuioma has a high occurrence in children under the age of 10, with several clinical characteristics which include: usually unilaterally affected; soft tissue swelling of the upper eyelid or the temporal periorbital area and extensive osteolysis, centered at the marrow space of the anterolateral frontal bone in CT scan are the main clinical findings; surgical biopsy or subtotal curettage should be taken in order to make a definite diagnosis; minimal intervention with postoperative steroid or low-dose radiation show good therapeutic outcome.
