Abstract:Cubital tunnel syndrome (CTS) is a chronic condition caused by the compression of the ulnar nerve at the elbow. The ulnar nerve is susceptible to entrapment at multiple sites along its path, with the elbow joint being the most critical. Symptoms typically include numbness in the ulnar nerve distribution area, hand muscle atrophy, and weakness. Patients exhibiting mild symptoms can be treated conservatively, whereas those with moderate to severe symptoms often require surgical intervention. Extensive research has been conducted on the clinical treatment of CTS. The complex etiology of this condition, coupled with the unique anatomical structure of the ulnar nerve, has led to less -than -satisfactory clinical outcomes. Physiotherapy plays an important role in aiding nerve recovery and reducing disability rates. Initiating physiotherapy early can address the cause of the disease, mitigate nerve damage, improve functional impairment due to nerve injury, and enhance the quality of patients ' daily activities. However, definitive guidelines for the treatment and evaluation of cubital tunnel syndrome are lacking. This review compiles the various modalities and advancements in the diagnosis and rehabilitation of CTS, drawing from recent domestic and international literature. It summarizes and compares the diagnostic tools currently employed in clinical practice and offers suggestions for physicians and therapists in selecting personalized diagnostic tools for patient assessment. Additionally, the review describes various rehabilitation methods, providing fresh insights for patients with cubital tunnel syndrome who have received conservative or surgical treatment. Abstract:OBJECTIVE:To identify the overall anatomical characteristics and the clinical value of the lumbar nerves under CT multiplanar reconstruction.METHODS:Fifty normal subjects and 30 patients with LN diseases (51 sides) were selected, including 10 patients with lumber intervertebral disk hernia, eight patients with spinal stenosis, 5 patients with spondylolisthesis, 1 patient with tethered cord syndrome, 1 patient with lumbar scoliosis, and 5 patients with spinal trauma The 16-slice helical CT (Light Speed, GE Company) was used for scanning the lumbar vertebra with multiplanar reconstruction in Workstation (ADW4.1) with UNIX System in DICOM format. The image was set on the same slice for the overall anatomy and manifestations of the lesions.RESULTS:The same-slice imaging showed the strip-like LN slightly lower than the surrounding muscle in density. Each LN went out of the dural sac at an acute angle. The course of the lumbar plexus and its major branches, including the obturator nerve, femoral nerve and reproductive nerve, and their relations to the adjacent structures were clearly revealed. The percentage of the segments displayed was well associated with the reconstruction angle, with the order being oblique coronal > outward-rotated oblique coronal > oblique sagittal > coronal > sagittal section. The major manifestations of abnormal LN included compression and displacement (50 sides, 98.0%), morphological changes (49 sides, 96.1%), adhesion (41 sides, 80.4%).CONCLUSIONS:The CT multiplanar reconstruction is ideal for the imaging of the overall size, shape, running and tension of the LN root; it is valuable in clinical diagnosis.
Deep Fusion Network Based Sparse View CT Reconstructions for Clinical Diagnostic Scanners

Abstract:Sparse view CT scan has the advantage of reducing radiation exposure and scanning time in clinical diagnosis. However, the limited number of x-ray projections can make the reconstruction problem ill posed and result in image artifacts. To tackle the problem, we propose a novel model-based deep fusion network(DFN) satisfying the clinical set-up. It extracts fused features encoded from both the sinogram and the preliminary reconstructed image generated by filtered back projection (FBP) to improve the quality of reconstruction. The preliminary reconstructed image endows fused features with prior knowledge that facilitate the convergence of neural network to high-quality reconstruction images. We design a custom loss for training that enforces the network to learn both the pixel value and the integrity of the tissue structure. A synthetic sparse view breast CT dataset from American Association of Physicists in Medicine(AAPM) is used for training, validation and testing. The qualitative and quantitative evaluations show that the DFN reconstruction algorithm significantly improves in balancing between the image quality and reconstruction speed, hence enables fast and high quality CT reconstruction despite the sparse view limitations.

Abstract:Purpose: Knowledge of microanatomy of the peripheral nerves may help to improve nerve repair after injuries. This study aimed to determine the potential of high-frequency ultrasound (US) in detecting the experimental model-rat sciatic nerve. Methods: We carried out an ultrasonic detection and anatomical dissection of rat sciatic nerve and surrounding tissues. The ultrasonic cross sectional area (CSA) was measured and compared with the histological parameters based on different morphological segments. Results: The rat sciatic nerve can be shown by using high-frequency US at the posterior thigh. US morphological features were related to the fascicle content. Transversely, it appeared oval, flattened, and irregular shape corresponding to one, two, and three fascicles with nerve elongation. There was a decreased tendency of CSA and fiber number at three segments, and the CSA was correlated with the fiber number and area at the proximal segment. Conclusion: The rat sciatic nerve anatomy features can be displayed by US, which may be a non-invasive tool to provide anatomical details to the experimental studies.

Abstract:Aim. To study the application value of ankle fracture classification and diagnosis. In this paper, the clinical data of 100 cases of ankle fracture patients admitted from May 2020 to May 2021 were analyzed by CT 3D reconstruction. All patients received surgical treatment and underwent spiral CT 3D reconstruction and X-ray examination before surgery. The results showed that 20 cases (20.00%) of the 100 cases were PER, 24 cases (24%) of the 100 cases were PAB, 31 cases (31%) of the 100 cases were SER, and 25 cases (25%) of the 100 cases were SAB, respectively. Conclusion. The diagnostic accuracy of CT 3D reconstruction for different types of ankle fracture is higher than that of X-ray, and the differences are statistically significant ( P < 0.05 ). CT 3D reconstruction is applied in the early diagnosis of ankle fracture, which can accurately detect the classification of patients. It has important clinical application value and can be used as the first choice for the early classification diagnosis of ankle fracture.

Three-dimensional Reconstruction of Human Sciatic Nerve from Serial Tissue Sections

Abstract:Objective To study a practical three-dimensional reconstruction method from serial tissue sections of human sciatic nerve. Methods While human hair taken as the localization material,one fresh human sciatic nerve was serially horizontally sliced,and then all the slices were stained using Karnovsky-Roots acetylcholinesterase(AChe)histochemical method. The digital information of each section was acquired by high-resolution scanner and all information was put into computer to made three-dimensional reconstruction. Results The reconstructed results successfully reappeared three-dimensional structure of fasciculus inside the sciatic nerve and showed the topographic anatomy of every nerve fascicle and its relationship in arbitrary sections, and presented patterns of complicated recombination of nerve bundles in the whole length. The reconstructed results could be shown either separately or together and it can be rotated or zoomed in any direction. Conclusions It is a useful and practical way to reconstruct the three-dimensional structure of fasciculus in the peripheral nerve by taking human hair as the registration material and combining with soft-aided registration method.
Identifying the neural marker of chronic sciatica using multimodal neuroimaging and machine learning analyses

Abstract:Introduction: Sciatica is a pain disorder often caused by the herniated disk compressing the lumbosacral nerve roots. Neuroimaging studies have identified functional abnormalities in patients with chronic sciatica (CS). However, few studies have investigated the neural marker of CS using brain structure and the classification value of multidimensional neuroimaging features in CS patients is unclear. Methods: Here, structural and resting-state functional magnetic resonance imaging (fMRI) was acquired for 34 CS patients and 36 matched healthy controls (HCs). We analyzed cortical surface area, cortical thickness, amplitude of low-frequency fluctuation (ALFF), regional homogeneity (REHO), between-regions functional connectivity (FC), and assessed the correlation between neuroimaging measures and clinical scores. Finally, the multimodal neuroimaging features were used to differentiate the CS patients and HC individuals by support vector machine (SVM) algorithm. Results: Compared to HC, CS patients had a larger cortical surface area in the right banks of the superior temporal sulcus and rostral anterior cingulate; higher ALFF value in the left inferior frontal gyrus; enhanced FCs between somatomotor and ventral attention network. Three FCs values were associated with clinical pain scores. Furthermore, the three multimodal neuroimaging features with significant differences between groups and the SVM algorithm could classify CS patients and HC with an accuracy of 90.00%. Discussion: Together, our findings revealed extensive reorganization of local functional properties, surface area, and network metrics in CS patients. The success of patient identification highlights the potential of using artificial intelligence and multimodal neuroimaging markers in chronic pain research.

Abstract:Spinal cord injury (SCI) profoundly affects an individual's ability to move. Fortunately, recent advancements in neuromodulation, particularly the spatio-temporal epidural electrical stimulation (EES) targeting the spinal nerve roots, promoted rapid rehabilitation of SCI patients. Such neuromodulation techniques require precise anatomical modelling of spinal cord. However, the lack of spine imaging datasets, especially high-quality magnetic resonance imaging (MRI) datasets highlighting nerve roots, hinders the translation of EES into medical practice. To address this problem, we introduce an open-access lumbosacral spine MRI dataset acquired in 14 healthy adults, using constructive interference in steady state (CISS) sequence, double echo steady state (DESS) sequence, and T2-weight turbo spin echo (T2-TSE) sequence, with enhanced nerve root resolution. The datas et al so includes the corresponding anatomical annotations of nerve roots and the final reconstructed 3D spinal cord models. The quality of our dataset is assessed using image quality metrics implemented in MRI quality control tool (MRIQC). Our dataset provides a valuable platform to promote a wide range of spinal cord neuromodulation research and collaboration among neurorehabilitation engineers.

Ultrasonographic Reference Values for Assessing Normal Sciatic Nerve Ultrasonography in the Normal Population.

Abstract:Objective: High-resolution ultrasonography (HRUS) has been used recently to characterize median and ulnar nerves but is seldom used to characterize the lower extremity nerves. The reference standard for normal the lower extremity nerves has not been established. Thus, this study measured the cross-sectional areas (CSAs) of the sciatic nerve of 200 healthy male or female volunteers, aged 18-80 using HRUS. These data provide basic clinical data for the use of high-resolution ultrasound for the future diagnosis, treatment, and prognostic evaluation of peripheral neuropathies. Methods: Two hundred healthy volunteers with 400 lower extremities were studied with HRUS. According to their age, the subjects were assigned to young group (18-30 years, n = 75), middle group. (31-60 years, n = 70), and old group(61-80 year, n = 55). Age, sex, height, weight were recorded and CSAs of sciatic nerve were obtained at every predetermined sites. Results: The mean CSAs of sciatic nerves at GS and MGPF were 0.527 +/- 0.028 cm(2) and 0.444 +/- 0.026 cm(2) respectively. Pearson's correlation analysis showed that the mean CSAs were correlated with height and weight. There was no difference in mean CSAs among the three groups (P> 0.05). Women had smaller CSAs of the normal Sciatic nerves than men in two measuring sites (GS, MGPF) (P < 0.05). Conclusion: Peripheral nerve ultrasonography is a reliable and reproducible diagnostic method in the hands of experienced examiners. Normal values for the sciatic nerve nerves are provided by our study. Thus, reference values of Sciatic nerve CSA of the lower extremity can facilitate the analysis of abnormal nerve conditions.

Abstract:Objective To observe the applied anatomy of surface location of SD rats sciatic nerve and related acupoints,such as Huantiao(GB30) and Yanglingquan(GB34).Methods After local anaesthesia,rats were detached their sciatic nerve trunk and its branchs,then the sciatic nerve was located and measured with the vernier caliper and the samples were observed by taking pictures.Results The distance between the upper end of tibia and greater trochanter of femur was(30.56 ±1.26) mm on the left,and(30.30± 0.88) mm on the right;the length of sciatic nerve trunk was(16.89±0.86) mm on the left,and(16.78±1.17) mm on the right;the distance between the midpoint of femur and superior border of sciatic nerve trunk was(4.88±0.51) mm on the left,and(4.98±0.52) mm on the right.The included angle of bilateral sciatic nerve trunk and femur was 30°;the projective point of the branch of sciatic nerve was located in the corresponding surface location of gluteal epithelial veins.The length of femur was 3 cm and the sciatic nerve trunk in the posterior segment was 1.7 cm.Conclusion The beginning of branch of sciatic nerve trunk was located below 0.5 cm of the midpoint of femur.According to the discovery combined with the surface projection of gluteal epithelial vessels,the experimenter might accurately locate and ligate the sciatic nerve trunk.

Abstract:Repair and regeneration of injured peripheral nerves have always been a focus in the field of orthopedics. Traditional nerve atlas contains only two-dimensional information and canpsilat display the real cross and recombination process of nerve bundles in its whole length. To solve this problem, a system, 3D nerve, is proposed for the 3D reconstruction of peripheral nerves. Through image registration, image segmentation, contour correspondence and surface reconstruction process, this system successfully represents the topographic anatomy of every nerve fascicle and its relationship with the nicety localization information of sense fibers and motor ones in arbitrary sections. It is a great help for clinical peripheral nerve repair and 3D peripheral nerve teaching.
Anatomical Investigation Of The Sinus Node Artery Using Dual-Source Computed Tomography

Abstract:Background Few investigators have studied the anatomy of the sinus node artery (SNA) using noninvasive imaging modalities, so the objective of this study was to visulize the in-vivo 3-dimensional anatomical relations of the SNA using dual-siource computed tomography (DSCT).Methods and Results: In the 101 patients included in this study, the visualization rate, anatomical type and diameter of the SNA, the distance between the orifice and coronary artery, and the terminal type of SNA were recorded. The visualization rate was 95.2% (96/101). Of 96 patients, 106 SNAs were detected among which 51 (48.1%, 51/106) originated from the right coronary artery, 52 (49.1%, 52/106) from the left circumflex artery, and 3(2.7%, 3/106) from other branches. There were 3 types of SNA: right (n=52), left (n=45), and posterior (n=9). The distance between the orifice of the right SNA and the right coronary sinus was 14.2 +/- 15.2 mm, for the left SNA it was 5.5 +/- 3.5 mm, and for the posterior SNA, 33.7 +/- 12.8 mm. The average diameter was 1.27 +/- 0.29 mm. The SNA approached the nodal tissue by 1 of 3 routes; retrocaval (51.5%), precaval (25.2%), or pericaval (22.3%).Conclusion The SNA can be visualized in vivo using DSCT, which is important for preoperative knowledge of its origin, course, termination, and anatomical type.
A Preliminary Study on Anatomical Characteristics of the Sinoatrial Nodal Artery in Chinese: Analysis of 156 Cases of CTA

Abstract:Objective To investigate the value of morphologic features of sinoatrial nodal artery(SNA) with 256 slice CT.Methods The anatomic origin,number,course and branches of the SNA were observed and analyzed retrospectively in 156 patients underwent 256 slice CT coronary angiography.Results A single SNA originated from the right coronary artery(RCA) in 98 and from of the LCX artery in 37 patients.Dual blood supply to the sinuatrial nodal was seen in 20 patients.The distance from the origin of right SNA to the RCA ostium was in the range of 0~45 mm and the left SNA to the left circumflex(LCX) artery ostium was in the range of 2~32 mm.SNA was not observed in one patient.Following the SNA originated from the RCA or(and) LCX,small atrial branches were commonly seen supplying the left atrium,the anterior part of the interatrial septum,the left atrial appendage and the right atrium.Conclusion 256 slice CT coronary artery imaging could effectively display SNA,evaluate the anatomic origin,number,the course and branches correctly.
Application of Spinal Subtraction and Bone Background Fusion CTA in the Accurate Diagnosis and Evaluation of Spinal Vascular Malformations.

Abstract:BACKGROUND AND PURPOSE: Accurate pretreatment diagnosis and assessment of spinal vascular malformations using spinal CTA are crucial for patient prognosis, but the postprocessing reconstruction may not be able to fully depict the lesions due to the complexity inherent in spinal anatomy. Our purpose was to explore the application value of the spinal subtraction and bone background fusion CTA (SSBBF-CTA) technique in precisely depicting and localizing spinal vascular malformation lesions. MATERIALS AND METHODS: In this retrospective study, patients (between November 2017 and November 2022) with symptoms similar to those of spinal vascular malformations were divided into diseased (group A) and nondiseased (group B) groups. All patients underwent spinal CTA using Siemens dual-source CT. Multiplanar reconstruction; routine bone subtraction, and SSBBF-CTA images were obtained using the snygo.via and ADW4.6 postprocessing reconstruction workstations. Multiple observers researched the following 3 aspects: 1) preliminary screening capability using original images with multiplanar reconstruction CTA, 2) the accuracy and stability of the SSBBF-CTA postprocessing technique, and 3) diagnostic evaluation of spinal vascular malformations using the 3 types of postprocessing images. Diagnostic performance was analyzed using receiver operating characteristic analysis, while reader or image differences were analyzed using the Wilcoxon signed-rank test or the Kruskal-Wallis rank sum test. RESULTS: Forty-nine patients (groups A and B: 22 and 27 patients; mean ages, 44.0 [SD, 14.3] years and 44.6 [SD,15.2] years; 13 and 16 men) were evaluated. Junior physicians showed lower diagnostic accuracy and sensitivity using multiplanar reconstruction CTA (85.7% and 77.3%) than senior physicians (93.9% and 90.9%, 98% and 95.5%). Short-term trained juniors achieved SSBBF-CTA image accuracy similar to that of experienced physicians (P > .05). In terms of the visualization and localization of spinal vascular malformation lesions (nidus/fistula, feeding artery, and drainage vein), both multiplanar reconstruction and SSBBF-CTA outperformed routine bone subtraction CTA (P = .000). Compared with multiplanar reconstruction, SSBBF-CTA allowed less experienced physicians to achieve superior diagnostic capabilities (comparable with those of experienced radiologists) more rapidly (P < .05). CONCLUSIONS: The SSBBF-CTA technique exhibited excellent reproducibility and enabled accurate pretreatment diagnosis and assessment of spinal vascular malformations with high diagnostic efficiency, particularly for junior radiologists.
High‐Resolution Ultrasound Imaging of Axillary Nerve and Relevant Injury

Abstract:Objective-To evaluate the feasibility of axillary nerve (AN) visualization in healthy volunteers and the diagnostic value of AN injury via high-resolution ultrasonography (HRUS). Methods-AN was examined by HRUS on both sides of 48 healthy volunteers and oriented the transducer according to three anatomical landmarks: quadrilateral space, anterior to subscapular muscle, and posterior to axillary artery. The maximum short-axis diameter (SD) and cross-sectional area (CSA) of AN were measured at different levels, and AN visibility was graded by using a five-point scale. The patients suspected of having AN injury were assessed by HRUS, and the HRUS features of AN injury were observed. Results-AN can be visualized on both sides in all volunteers. There was no significant difference in SD and CSA of AN at the three levels between the left and right sides or in SD between males and females. However, the CSA of males at different levels was slightly larger than those of females (P <.05). In most volunteers, AN visibility at different levels was excellent or good, and AN was best displayed anterior to subscapular muscle. Rank correlation analysis revealed that the degree of AN visibility had correlation with height, weight, and BMI. A total of 15 patients diagnosed with AN injury, 12 patients showed diffuse swelling or focal thickening in AN, and 3 patients showed AN discontinuity. Conclusion-HRUS is able to reliably visualize AN, and it could be considered as the first choice for diagnosing AN injury.
UNSCT-HRNet: Modeling Anatomical Uncertainty for Landmark Detection in Total Hip Arthroplasty

Abstract:Total hip arthroplasty (THA) relies on accurate landmark detection from radiographic images, but unstructured data caused by irregular patient postures or occluded anatomical markers pose significant challenges for existing methods. To address this, we propose UNSCT-HRNet (Unstructured CT - High-Resolution Net), a deep learning-based framework that integrates a Spatial Relationship Fusion (SRF) module and an Uncertainty Estimation (UE) module. The SRF module, utilizing coordinate convolution and polarized attention, enhances the model's ability to capture complex spatial relationships. Meanwhile, the UE module which based on entropy ensures predictions are anatomically relevant. For unstructured data, the proposed method can predict landmarks without relying on the fixed number of points, which shows higher accuracy and better robustness comparing with the existing methods. Our UNSCT-HRNet demonstrates over a 60% improvement across multiple metrics in unstructured data. The experimental results also reveal that our approach maintains good performance on the structured dataset. Overall, the proposed UNSCT-HRNet has the potential to be used as a new reliable, automated solution for THA surgical planning and postoperative monitoring.

Preliminary Study of High Frequency Ultrasound in Sciatic Nerve of Rat with Diabetic Neuropathy

Abstract:Objective To evaluate the value of high frequency ultrasound in sciatic nerve of rat with diabetic neuropathy.Methods Fifty male Sprague-Dawley rats were divided into 4 groups:Diabetic animals were induced by intravenous injection of streptozotocin after high fat feed for 1 month,after which rats were grouped as 3-month (n =12) and 5-month (n =20) diabetes.Control group were received the streptozotocin vehicle and were classified as 3-month group (n =8) and 5-month group (n =10).Ultrasounic study of cross sectional area (CSA) and pathologic study including number and area of nerve fiber were performed at 3 and 5 months according to the disigned.Results The CSA,number and area of nerve fiber in 5 month diabetes were significantly reduced than 5-month control group (t =5.121,P ＜ 0.05; t =7.113,P ＜0.05; t =6.328,P ＜0.05).The CSA were statistically with the number and area of nerve fiber in 5 month diabetic group (r =0.732,P ＜0.05 ; r =0.715,P ＜0.05).However,the CSA had no significant correlation with the number and area of nerve fiber between 3-month diabetes and control group (P ＞0.05).Conclusions The CSA of sciatic nerve can evaluate the diabetic neuropathy of rat,which might be regarded as a potential index in diagnose of diabetic neuropathy.
MR Optimal Technique for Imaging Sacral Plexus and Adjacent Sciatic Nerve

Abstract:Objective To study the MR optimal technique for demonstrating sacral plexus (SP) and adjacent sciatic nerves (SN) as well as its clinical value. Methods The SPs and adjacent SNs of 20 volunteers and 7 patients were imaged by GE-Signa CV/I 1.5 T MR scanner with the sequences of 2-dimension spin echo (2D-SE) T1WI, fast spin echo fat saturation (FSE F/S )T2WI oblique sagittal imaging,axial 3-dimension fast imaging employing steady state acquisition (3D-FIESTA), and additional oblique 3D-FIESTA in 10 of the 20 volunteers. The displaying rate of sacral(S_1) nerves and adjacent SNs were compared between 2D-SE T1WI and reconstructive images of axial 3D-FIESTA. The displaying degree of SPs and adjacent SNs on reconstructive images of axial and oblique 3D-FIESTA was graded, and average scores were analyzed with Wilcoxon signed rank tests. Results The displaying rates of S_1 and adjacent SN were significantly different between 2D-SE and 3D-FIESTA ( P 0.01). Oblique 3D-FIESTA was superior to axial 3D-FIESTA for demonstrating LST and S_1, but there was no significant difference in S_2 and S_3. Sacral nerves of the 7 patients were invaded and/or displaced. Conclusion Reconstructive images of 3D-FIESTA is better than 2D-SE for demonstrating of S_1 and adjacent SN, and can display every sacral nerve. Oblique acquisition is better than axial one.

Abstract:This study was undertaken to implement the three-dimensional (3D) reconstruction of the intraneural structure in the upper limb peripheral nerves. Three fresh cadaver peripheral nerves (one median, one ulnar and one radial nerve) were located with human hair, embedded in OCT, sectioned consecutively every 100 mum at a 20 mum thickness. Then all slices were stained with the acetylcholinesterase histochemical (AChe) method. 2D information of these slices were obtained through high resolution scanner. A special software (3D nerve visualization system) was used for 3D reconstruction of these nerves. Finally, 3D microstructure inside these nerves were reproduced in the computer. 3D nerve visualization system can show exactly the 3D microstructure inside these nerves and the rules of the regrouping and redistribution of the fasciculus. In addition, the reconstructed nerves could rotate at any angle along any axis, which might help us to observe the shape, position and relationships of fasciculus from different positions. It might do a great help to provide a anatomic basis for the approach in treating peripheral nerve injuries.
