Abstract:In recent years, the incidence rate of Type 2 diabetes mellitus（T2DM） has been increasing. In 2019, there were about 417 million T2DM patients worldwide and 104 million in China, which has been a threat to human health and quality of life. The main symptom of T2DM is insulin resistance（IR） that the body cannot respond normally to insulin. Excessive intake of high fat diet（HFD） is the major incentive of IR. Many studies have proved that HFD can lead to intestinal micro-ecological imbalance, which in turn induces the IR of metabolic tissues. Researches have shown that the intestinal micro-ecology mainly controls the body’s glucose and lipid metabolism by regulating the body’s energy metabolism and inflammation. Further research suggests that bile acid metabolism disorder caused by intestinal micro-ecological imbalance is also another important cause of insulin resistance in the body. Therefore, this article will review the interactions among high-fat diet, intestinal flora and insulin resistance from different aspects, such as high-fat diet and insulin resistance, intestinal flora and insulin resistance, and the ’gut microbiota-bile acid-insulin resistance’. Although there is no evidence to confirm the causal relationship of ’high-fat diet-intestinal flora-bile acid’ in the occurrence and development of insulin resistance, but the mutual regulations between intestinal flora and bile acid metabolism provide enlightenment for insulin resistance, prevention and treatment of T2DM.

Abstract:<p>Remarkable evidence supports the link between type 2 diabetes (T2DM) and fatigue. However, a unifying recommendation to identify and prevent fatigue or to prevent its clinical consequences in T2DM is not available at present. Therefore, this study aimed to conduct a systematic review to summarize the definition, measurement tools, and contributing factors of fatigue in T2DM. Nine articles were included for analysis, and results showed that T2DM fatigue was associated with five major factors, including sociodemographic factors, clinical disease factors, inflammatory factors, psychological factors, and behavior and lifestyle. The contributing factors of T2DM fatigue are reviewed, and clinical benefits provide a theoretical basis for further clinical intervention to prevent the occurrence of fatigue and improve the patient's treatment compliance and self-management ability and may be beneficial to their quality of life.</p>

The Science of Diabetes Self-Management and Care

Abstract:Purpose: The purposes of this study were to (1) examine the relationships between fatigue, its influencing factors, and diabetes self-management and (2) test the mediation effects of fatigue on the link between the influencing factors and diabetes self-management in adults with type 2 diabetes. Methods: This cross-sectional, correlational study was guided by the theory of unpleasant symptoms. Data were collected using structured questionnaires. Fatigue was measured by the Fatigue Symptom Inventory and the Multidimensional Fatigue Inventory. Diabetes self-management was measured by the Summary of Diabetes Self-Care Activities. From March to July 2021, a convenience sample of 150 participants was recruited from 2 diabetes outpatient clinics of a regional hospital in Taiwan. Data were analyzed using structural equation modeling. Results: A more recent diagnosis of diabetes, more depressive symptoms, and lower sleep quality were related to higher fatigue. Higher fatigue correlated with less performance in diabetes self-management. Fatigue mediated the relationship between depressive symptoms, sleep quality, and diabetes self-management. Conclusions: Fatigue had a mediating effect on the link between psychological influencing factors and diabetes self-management. Future development of fatigue interventions integrating depressive symptoms and sleep management will likely increase the performance of diabetes self-management and improve the health outcomes in adults with type 2 diabetes. The study tested the theory of unpleasant symptoms using empirical data and will assist in building theory-guided fatigue interventions to improve diabetes self-management in people with type 2 diabetes.
Fatigue and its related factors in patients with chronic heart failure

Abstract:Aims and objectives: To understand fatigue in chronic heart failure patients and its related factors. Background: Fatigue is a common symptom in patients with chronic heart failure, but little is known about this phenomenon. Design: Correlational study. Methods: This correlational study sampled 107 chronic heart failure patients from a medical centre in northern Taiwan. Data were collected on objective outcomes (ejection fraction, EF; New York Heart Association classification, NYHA; beta-blocker use and haemoglobin, Hb) and subjective outcomes (depression and fatigue). Since the main outcome variable, fatigue, is multidimensional, it was measured by two scales: Lee's Fatigue Visual Analog Scale and a researcher-developed scale, the Tang Fatigue Rating Scale. Results: Subjects experienced a medium level of fatigue. Fatigue was significantly higher (p < 0.05) in females (226.6 SD 49.1) than males (203.0 SD 52.2) and in patients who took beta-blockers than those who did not (p < 0.01). Depression, EF and NYHA as a set explained 73% of the variance in subjects' fatigue. Subjects with greater depression, lower EF and worse NYHA experienced significantly higher fatigue (F = 76.50, p < 0.001). Conclusions: Fatigue for this sample of chronic heart failure patients was significantly predicted by depression, EF and NYHA. Additional research is needed to explore patterns of fatigue and its related factors over time. Relevance to clinical practice: In nursing care, psychological factors are as important as physiological factors to patients' well-being. Health care providers should therefore pay more attention to the psychological status of patients with chronic heart failure, thus helping them control their fatigue and improve their well-being.
Fatigue in Peritoneal Dialysis Patients and an Exploration of Contributing Factors: A Cross-Sectional Study

Abstract:Context: Fatigue is a common and detrimental symptom in dialysis patients; however, our understanding of it and investigation of its contributing factors is still very limited, especially in peritoneal dialysis (PD) patients. Objectives: To assess fatigue in PD patients and identify contributing factors. Methods: One hundred eight PD patients in a comprehensive hospital in China were recruited. The fatigue severity of the participants was assessed using the Chalder Fatigue Scale 11. Demographic factors and results of physiological tests were collected. Quality of sleep, mental health, and social support were assessed with the Pittsburgh Sleep Quality Index, Symptom Checklist 90, and Social Support Rating Scale, respectively. Multiple linear regression models were conducted with candidate variables with a P-value of less than 0.1 on univariate analysis and variables that were clinically relevant to identify contributing factors for fatigue. Results: The fatigue level in PD patients was significantly higher than the community population, and 78.7% of them were suffering from fatigue. The factors that were significantly associated with fatigue were quality of sleep, normalized protein nitrogen appearance, transferrin, alkaline phosphatase, and total cholesterol (adjusted R squared 0.86). Among them, quality of sleep, transferrin, alkaline phosphatase, and total cholesterol were significant contributors for physical fatigue, whereas the quality of sleep and normalized protein nitrogen appearance were contributing factors for mental fatigue. Conclusion: Fatigue is a common symptom in PD patients, suggesting that increased awareness of this symptom is required. The identification of correlates by extensive exploration of multidimensional factors in this study may help practitioners to identify patients at higher risk and to develop a multidimensional and targeted intervention plan.

Abstract:Abstract Patient with diabetes may increase the incidence of frailty. Frailty cause chronic inflammation and insulin resistance. The purpose of this study is to investigate the current situation of frailty and its influencing factors in elderly patients with diabetes. Totally 300 elderly patients with diabetes were selected from a tertiary hospital in Zhengzhou of China via convenience sampling method,and were investigated by self-designed general information questionnaire,TFI (Tilburg Frailty Indicator) and SDSCA(Scale of Diabetes Self-care Activities). Totally 296 valid questionnaires were collected. A total of 137 elderly patients with diabetes suffered from frailty and the prevalence was 46.3%. The mean score of total frailty was (5.26±2.87) and the scores for each dimension were as follows: physical frailty (2.79±2.08), psychological frailty (1.40±0.94) and social frailty (1.07±0.75). Multiple linear regression showed that comorbidity, self-management behavior, glycosylated hemoglobin, educational degree, polypharmacy and smoking were the major influential factors (P<0.05). The prevalence of frailty in elderly patients with diabetes was at a high level, containing different degrees of physical, psychological and social frailty. Medical staff should attach great importance to the assessment of frailty among elderly patients with diabetes, take targeted and holistic interventions timely to prevent or delay the development of frailty.
Fatigue in Persons With Heart Failure: A Systematic Literature Review and Meta-Synthesis Using the Biopsychosocial Model of Health

Abstract:Background: Fatigue is a common and distressing symptom of heart failure (HF) and has important implications for patient-reported and clinical outcomes. Despite being a common and bothersome symptom, fatigue has been understudied in HF. We sought to synthesize existing literature on fatigue in HF through a systematic literature review guided by the biopsychosocial model of health. Methods and results: A systematic search of the literature was performed on March 18, 2020, using Pubmed, Embase, and CINAHL. Full-text, primary research articles, written in English, in which fatigue was a primary symptom of interest in adults with a diagnosis of HF, were included. The search yielded 1138 articles; 33 articles that met inclusion criteria were selected for extraction and synthesis. Biological and psychological factors associated with fatigue were New York Heart Association functional class, hemoglobin level, history of stroke, and depression. However, there are limited HF-specific factors linked to fatigue. Social factors related to fatigue included social roles, relationship strain, and loneliness and isolation. Few nonpharmacologic interventions have been tested by show some promise for alleviating fatigue in HF. Studies show conflicting evidence related to the prognostic implications of fatigue. Conclusions: Important biological correlates of fatigue were identified; however, psychological and social variables were limited to qualitative description. There is need for expanded models to better understand the complex physiologic nature of fatigue in HF. Additionally, more research is needed to (1) define the relationships between fatigue and both psychological and social factors, (2) better describe the prognostic implications of fatigue, and (3) develop more therapeutic approaches to alleviate fatigue with the goal of improving overall quality of life.

Abstract:Fatigue is a common physiological state that affects normal human activities. Prolonged fatigue induces a variety of diseases and seriously affects human health, so it is imperative to discover nutritional dietary supplements and treatments without side effects, among which natural anti-fatigue polysaccharides have shown great potential. Polysaccharides, a class of biomolecules produced by a variety of organisms such as plants, animals, bacteria and algae, have attracted much attention in recent years due to their anti-fatigue activity and fewer side effects. This review summarizes the classification, dosage and experimental models of polysaccharides with anti-fatigue activity obtained from different natural sources. We also review the fatigue-relieving effects of these polysaccharides through mechanisms such as modulating oxidative damage, regulating energy metabolism and influencing intestinal flora, as well as the effects of molecular weights, monosaccharide compositions, structural features and chemical modifications of the polysaccharides on their anti-fatigue activities to support their potential application value in functional foods and pharmaceuticals. New valuable insights for future research on natural polysaccharides are also presented in the field of natural production of bio-based functional materials, functional foods and therapeutic agents.
Bibliometric and visualization analyses of cancer-related fatigue research published worldwide from 2001 to 2023

Abstract:Objective: Cancer seriously endangers human health and represents a global public health issue. Cancer-related fatigue (CRF) is a distressing and persistent sense of exhaustion caused by cancer or cancer treatment, widely prevalent among cancer patients. This study aims to summarize emerging trends and provide directions for future research of CRF through bibliometric and visualization analyses. Methods: A systematic search in the Web of Science Core Collection database from 2001-01-01 to 2023-05-18 were conducted. Only reviews and articles written in English were considered. CiteSpace and the R were used for bibliometric and visualization analyses. Results: The analysis revealed that 2,566 studies on CRF have been published by 1,041 institutions in 70 countries so far. The number of articles published and cited annually have been steadily increasing. Eduardo Bruera published the most articles, and Julienne E Bower is the most co-cited author. The University of Texas System is the leading institution in cancer-related fatigue research. The United States and China have the largest number of publications. Supportive Care in Cancer published the most articles, and Journal of Clinical Oncology is the most co-cited journal. "Comparison of Pharmaceutical, Psychological, and Exercise Treatments for Cancer-Related Fatigue: A Meta-analysis", authored by Mustian KM et al. and published in JAMA Oncology was the most co-cited document. Keyword analysis indicated that research focus had shifted from "epoetin alpha" and "anemia" to "risk factors", "systematic review", "acupuncture", "anxiety", "traditional Chinese medicine" and "guidelines". Conclusion: In conclusion, this analysis provides comprehensive research trends and knowledge network maps of CRF. Clinical physicians should concurrently focus on the anemia, insomnia, anxiety, and depression status of patients when assessing or managing CRF. Improvements in related risk factors also contribute to alleviating fatigue. Furthermore, it is essential to pay attention to authoritative CRF guidelines. Acupuncture and traditional Chinese medicine also have therapeutic potential, which merits further investigation. Researchers should draw attention to the crucial roles of inflammation, hypoxia, and mitochondrial dysfunction, which could be the frontiers.

Abstract:BACKGROUND: Patients with Diabetes Mellitus (DM) are required to have long-term treatment regimen and strict self-management, which thus might lead to the Diabetes Distress (DD). Patients' DD varies in different regions with different levels of medical conditions. For improving the treatment effect of the patients with the Type-2-Diabetes-Mellitus (T2DM), this study explores the influencing factors of the patients' DD in the regions where the medical treatment are at low level.SUBJECTS AND METHODS: In this study, 167 adult patients with T2DM were selected from the People's Hospital of Jinhua city, an A-grade hospital of a small-mid-sized city in Eastern China. Based on these samples, the Diabetes Distress Scale with 17 items (DDS17) was adopted to measure the degree of patients' DD, and then regression analyses were carried out to investigate the influencing factors for their DD.RESULTS: The T2DM patients with moderate and high levels of DD take up 54.5% of the samples investigated in this study. According to the Spearman correlation analysis, sleep time, physical exercise, diet control, treatment regimen, TG and HbA1c significantly affect the patients' DD. Through the multivariate linear regression analysis, this study shows that (1) sleep time significantly influences the T2DM patients' DD (β=-0.213, P=0.034); (2) sleep time also impacts emotional burden (β=-0.379, P=0.027); (3) physical exercise (β=-0.195, P=0.002), treatment regimen (β=0.158, P=0.026) and diet control (β=0.158, P=0.026) are the three major influencing factors for regimen-related distress.CONCLUSION: In the small-mid-sized city where the treatment regimen is not highly advanced, to alleviate the T2DM patients' DD, the medical staff should suggest the patients to improve sleep quality and exercise more, help them positively understand the insulin infusion therapy and take proper diet control.

The Status and Related Factors of Fatigue in Patients with Peritoneal Dialysis

Abstract:Objective To investigate the fatigue in patients with peritoneal dialysis (PD),and analyze the related factors of fatigue. Methods Fatigue was assessed using the SF-36 vitality sub-scale in 116 PD patients. In addition,the quality of sleep,physical activity level,mental health and biochemical variables were collected. Results The mean score of vitality subscale was 54.9±24.0. The prevalence of fatigue was 36.2%. Logistic regression analysis showed that low physical activity level (OR=3.007,P0.05),quality of sleep (OR=1.126,P0.05),and mental health (OR=0.957,P0.01)were predicted factors of fatigue. Conclusions More attention should be paid to fatigue in PD patients. Some interventions aimed to improve the physical activity,quality of sleep,and mental health may decrease the fatigue in PD patients.

Abstract:A third of the world's population suffers from unexplained fatigue, hugely impacting work learning, efficiency, and health. The fatigue development may be a concomitant state of a disease or the side effect of a drug, or muscle fatigue induced by intense exercise. However, there are no authoritative guides or clinical medication recommendations for various fatigue classifications. Traditional Chinese medicines (TCM) are used as dietary supplements or healthcare products with specific anti-fatigue effects. Thus, TCM may be a potential treatment for fatigue. In this review, we outline the pathogenesis of fatigue, awareness of fatigue in Chinese and western medicine, pharmacodynamics mechanism, and substances. Additionally, we offer a comprehensive summary of fatigue and forecast the potential effect of novel herbal-based medicines against fatigue.

Abstract:Vitamin D has historically been associated with bone metabolism. However, over the years, a growing body of evidence has emerged indicating its involvement in various physiological processes that may influence the onset of numerous pathologies (cardiovascular and neurodegenerative diseases, rheumatological diseases, fertility, cancer, diabetes, or a condition of fatigue). This narrative review investigates the current knowledge of the pathophysiological mechanisms underlying fatigue and the ways in which vitamin D is implicated in these processes. Scientific studies in the databases of PubMed, Scopus, and Web of Science were reviewed with a focus on factors that play a role in the genesis of fatigue, where the influence of vitamin D has been clearly demonstrated. The pathogenic factors of fatigue influenced by vitamin D are related to biochemical factors connected to oxidative stress and inflammatory cytokines. A role in the control of the neurotransmitters dopamine and serotonin has also been demonstrated: an imbalance in the relationship between these two neurotransmitters is linked to the genesis of fatigue. Furthermore, vitamin D is implicated in the control of voltage-gated calcium and chloride channels. Although it has been demonstrated that hypovitaminosis D is associated with numerous pathological conditions, current data on the outcomes of correcting hypovitaminosis D are conflicting. This suggests that, despite the significant involvement of vitamin D in regulating mechanisms governing fatigue, other factors could also play a role.

Abstract:Diabetes mellitus is a common chronic metabolic disease characterized by a high incidence and disability rate. Intestinal flora refers to the microbial community that lives in the intestines and plays a crucial role in maintaining intestinal health and the human immune system. In recent years, an increasing body of research has revealed a close relationship between intestinal flora and diabetes. The pathophysiological mechanisms between them have also been constantly uncovered, and the regulation of intestinal flora has shown promising efficacy in the adjuvant treatment of diabetes. This study mainly summarized the characteristics and mechanisms of intestinal flora in patients with diabetes in recent years, as well as the methods of regulating intestinal flora to prevent and treat diabetes, and prospected the future research direction. This will offer a theoretical basis for the clinical adjuvant treatment of diabetes with intestinal flora and the development of new drugs.
Global prevalence of fatigue in patients with multiple sclerosis: a systematic review and meta-analysis

Abstract:Background: Fatigue is one of the most common and burdensome symptoms for patients with multiple sclerosis (PwMS), considerably impacting their quality of life and employment. Numerous reports have described the prevalence of MS-related fatigue, but there is no global consensus on this matter. Objective: To examine the global prevalence of MS-related fatigue and identify sources of heterogeneity in the published literature. Methods: A systematic review and meta-analysis were conducted. A comprehensive search of the PubMed, EMBASE, Cochrane Library, Web of Science, PsycINFO, CINAHL, China National Knowledge Infrastructure (CNKI), and Wanfang database for potential literature from 2000 to January 31, 2024. A random effects model was used to calculate the prevalence of MS-related fatigue. Subgroup analyses and a meta-regression were used to explore the sources of heterogeneity. Results: Sixty-nine studies from 27 countries were included. The global prevalence of MS-related fatigue was 59.1%, and it has decreased every decade since 2000. Fatigue was prevalent among females, those with lower education levels, those who were older, those with greater disability, and those with longer MS durations. The meta-regression revealed that fatigue measurement instruments were the largest source of heterogeneity. Conclusion: The prevalence of MS-related fatigue is quite high. Healthcare professionals should screen for and manage fatigue for PwMS as early as possible and pay attention to populations with a high prevalence of fatigue. The high heterogeneity among the prevalence rates due to differences in the fatigue scales suggests the importance of reaching a consensus on the best screening tools for MS-related fatigue.
Relationship between fatigue symptoms and subjective and objective indicators in hemodialysis patients

Abstract:PurposeThe purpose of the study was to investigate the major factors affecting fatigue symptoms and to examine the relationships between fatigue symptoms and subjective and objective indicators in patients on hemodialysis (HD).MethodsPatients on HD who met the inclusion criteria were chosen from two grade A tertiary general hospitals by convenience sampling and cross-sectional survey methods.ResultsA total of 511 patients were included in the study. The morbidity rate of fatigue in patients on HD was 61.6%, and the median fatigue level was 3.91. Fatigue and subjective indicators showed a mild-moderate relationship (0.3 < all |r| < 0.6, all P < 0.001), whereas a weak correlation was found between fatigue and objective indicators (all |r| < 0.2, all P < 0.05). Multivariate analysis showed that subjective indicators, including employment, exercise time, appetite, the vitality of 36-item Short Form Health Survey, perceived social support, intrafamilial support, cramping, headache, chest tightness, and whole-body pain, as well objective indicator, such as serum intact parathyroid hormone levels, were the influence factors of fatigue in patients on HD.ConclusionThe prevalence of fatigue symptoms was high and the level of fatigue was moderate in patients on HD. Subjective and objective indicators can both affect fatigue symptoms in patients on HD.

Fatigue and Its Contributing Factors in Chinese Patients with Primary Pituitary Adenomas

Abstract:Background. Pituitary adenomas (PAs) refers to a group of benign tumors that develop in the pituitary gland and are often characterized by fatigue. However, fatigue has not been documented in any Chinese research involving people with primary PA. The study sought to examine the prevalence, predictors, and correlation of fatigue with the quality of life (QoL) among PA patients in China. Methods. In total, 203 primary PA patients were included in this cross-sectional study. A series of questionnaires were administered, including the Multidimensional Fatigue Inventory (MFI), M. D. Anderson Symptom Inventory Brain Tumor (MDASI-BT), Short-Form 36 Health Survey (SF-36), Pittsburgh Sleep Quality Index (PSQI), and the Hospital Anxiety and Depression Scale (HADS). Data analysis was accomplished by Pearson or Spearman correlations, linear regression, and simple path analysis. Results. Severe fatigue prior to the initial diagnosis and preparation for surgery affected 50% of PA patients. Depression, sleep disturbance, and MDASI-BT symptom total scores were independently able to predict patient fatigue. Sleep disturbance mediates the influence of depression on fatigue (IE sleep = 0.296, 95% CI: LB = 0.148 to UB = 0.471). Conclusions. Chinese patients with primary PA often report experiencing fatigue. Depression and poor sleep quality were shown to be significant contributors to PA patients' fatigue. Depression affects PA patients' fatigue directly or indirectly. Medical professionals should take a proactive approach to PA patients suffering from fatigue before initial diagnosis and preoperative preparation to determine necessary interventions early, thus reducing fatigue and ultimately enhancing their QoL.

Abstract:Cancer, a significant public health issue, has emerged as one of the leading causes of disability and mortality globally. [ 1 ] According to the 2020 Global Cancer Statistics report, it is estimated that there were approximately 19.3 million new cancer cases and nearly 9.96 million deaths globally. [ 2 ] A majority of these patients, who underwent conventional treatments such as surgery, radiation, or systemic therapy, experienced a range of cancer-related symptoms and treatment side effects. [ 3 ] Cancer-related fatigue (CRF) is recognized as one of the most prevalent symptoms throughout the cancer journey, accounting for 62% of the reported symptoms in cancer patients. [ 4 ] As defined by the National Cancer Comprehensive Network, CRF is "a distressing, persistent, subjective sense of physical, emotional and/or cognitive tiredness or exhaustion related to cancer or cancer treatment, which is not proportional to recent activity and interferes with usual functioning." [ 5 ] Unlike fatigue resulting from excessive exercise or lack of sleep, CRF is more severe and persistent, and is less likely to be alleviated by rest. [ 6 ] Given the high prevalence of these symptoms, it is imperative for nurses, who have frequent contact with patients, to recognize and address CRF in their care.
