Abstract:Objective To discuss the diagnosis and surgical treatment of multiple endocrine neoplasia 1 (MEN1). Methods The clinical data of MEN1 from 1974 to 2005 were retrospectively analyzed. Results There were 17 cases of MEN1 in our group. Among them, 11 cases were from 4 families. MEN1 gene mutation was detected in 14 cases admitted after 1997. There were 11 cases of pHPT, 6 cases of insulinoma, 8 cases of pituitary adenoma, 6 cases of adrenal adenoma, 2 cases of thymic carcinoid and 2 cases of collagenoma. Two patients had respectively 4 glands involved, 3 patients had 3, 7 patients had 2 kinds of gland involved respectively. Four patients had only one kind of gland involved. A 12 year old girl had no clinical symptom and biochemical change. Six patients presented with nephrolithasis, 6 patients had impaired pancreatic endocrine function. Subtotal parathyroidectomy was performed in 4 patients of pHPT, and one patient received parathyroid adenoma enucleation. Insulinomas were enucleated in 4 patients. Two patients underwent thymus tumor extirpation. Conclusions MEN1 varies in symptoms, even those from the same family. Cermline mutation test helps establish diagnosis. Operations should be aiming at tumor resection as well as the improvement of life quality.

Abstract:Background Multiple endocrine neoplasia (MEN) is relatively rare. But more patients could be found by detailed examination. We discuss the diagnosis and surgical treatment of MEN.Methods The clinical data of 95 MEN cases were retrospectively analyzed. There were 30 cases of MEN1 including 19 cases from 6 families. The MEN1 gene mutation was detected in 81.48% of cases admitted after 1997. There were 22 cases of primary hyperparathyroidism (PHPT), 10 cases of enteropanceatic tumor including 9 cases of insulinoma, 15 cases of pituitary adenoma, 9 cases of adrenal adenoma, 2 cases of thymic carcinoid. Two patients had 4 glands involved, 3 patients had 3 glands involved, 16 patients had 2 glands involved, and 6 patients had only one gland involved. Three patients had neither clinical symptoms nor biochemical changes, and was diagnosed by MEN1 gene mutation. Six patients presented with nephrolithasis and 6 patients had impaired pancreatic endocrine function. There were 60 cases of MEN2a and 5 cases of MEN2b. 58 cases of MEN2a belongs to 19 kindreds. All MEN2a patients but one presented RET gene mutation in codon 634, and all MEN2b cases had mutation in codon 918. 48 cases of MEN2a had thyroid masses with elevated calcitonin levels. 27 patients had pheochromocytoma including 12 cases of multiple foci and 5 malignancy. 13 patients presented with hyperparathyroidism. 5 MEN2b patients had medullary thyroid carcinoma and mucosal ganglioneuromatosis with Marfanoid. Among them, 3 patients had bilateral pheochromocytoma.Results In MEN1, subtotal parathyroidectomy was performed in 12 patients with PHPT and one patient received parathyroid adenoma enucleation. Insulinomas were enucleated in 4 patients. Two patients underwent thymus tumor extirpation. Total thyroidectomy with bilateral dissection of regional lymph nodes was performed in 16 patients with MEN2a and nodule enucleation was performed in 9 patients. Twenty two MEN2a patients underwent pheochromocytoma enucleation including bilateral adrenal resection in 10 cases. 5 MEN2b patients underwent total thyroidectomy with bilateral lymph node dissection. Among them, 3 cases underwent bilateral adrenal operations.Conclusions MEN varies in symptoms. Germline mutation test is helpful in establishing a diagnosis. Surgical management should be aimed at the improvement of life quality in MEN1 and prevention of the fetal tumors in MEN2. Chin Med J 2009; 122(13):1495-1500
The Trends in Early Precision Diagnosis and Treatment Strategies of Multiple Endocrine Neoplasia Type 2

Abstract:Multiple endocrine neoplasia type 2 (MEN2) is an autosomal dominant hereditary neuroendocrine cancer syndrome characterized by medullary thyroid carcinoma, in combination or not with pheochromocytoma, hyperparathyroidism and extra-endocrine features, and two forms subtyped as MEN2A and MEN2B. Based on the correlation between RET proto-oncogene mutation and MEN2 phenotype, MEN2 could be prevented through prenatal diagnosis and preimplantation genetic testing. Integrating the detection of RET mutation with measurement of serum calcitonin, plasma or urinary metanephrine/normetanephrine, and serum parathyroid hormone levels could accurately predict the progression of MEN2, and then facilitating implementation of personalized precision treatment. In addition, increased awareness of MEN2 is needed, which requires participation of physicians, patients/family members, and relevant organizations, supplemented by psychological support, which could promote the comprehensive management of MEN2. The "5P" strategies for MEN2 represents a paradigm of precision medicine, which could effectively avoid or reduce the clinical adverse outcomes, improve the prognosis and quality of life of MEN2 patients.

Abstract:OBJECTIVE:To investigate the prevalence and characteristics of adrenal lesions in Chinese multiple endocrine neoplasia type 1 (MEN-1) patients. METHODS:Adrenal CT scan and clinical manifestations were retrospectively reviewed in 32 consecutive MEN-1 patients who were evaluated at our hospital during January 1986 to December 2009. RESULTS:Adrenal lesions were identified in 16 of 32 (50%) MEN-1 patients. Five (31.3%) patents with adrenal involvement showed bilateral lesions, including bilateral adenoma (n=1), bilateral hyperplasia (n=2) and adenoma and hyperplasia on each side (n=2). Unilateral adrenal lesion was presented in 11 (68.7%) patients. Among which, 63.6% had adenomas with a mean diameter of 2.3 cm (0.8-4.0 cm) and the remainder was of hyperplasia or enlargement. In two patients, functioning adrenal abnormalities were detected including Cushing adenoma (n=1) and aldosterone-secreting adenoma (n=1). CONCLUSIONS:The prevalence of adrenal lesion in MEN-1 patient is similar between China and western countries. These tumors are mostly benign, small and nonfunctioning. Taking into account a high incidence of adrenal carcinoma in previous foreign studies, routine screening and close surveillance are still recommended for adrenal lesions in MEN-1 patients.

Abstract:Multiple endocrine neoplasia type 2 (MEN2) is a neuroendocrine cancer syndrome characterized by medullary thyroid carcinoma, in combination or not with pheochromocytoma, hyperparathyroidism and extra-endocrine features. MEN2 syndrome includes two clinically distinct forms subtyped as MEN2A and MEN2B. Nearly all MEN2 cases are caused by germline mutations of the RET proto-oncogene. In this review, we propose “5P” strategies for management of MEN2: prevention, prediction, personalization, , psychological support, and participation, which could effectively improve clinical outcomes of patients. Based on RET mutations, MEN2 could be prevented through prenatal diagnosis or preimplantation genetic testing. Identification of pathogenic mutations in RET can enable early diagnosis of MEN2. Combining RET mutation testing with measurement of serum calcitonin, plasma or urinary metanephrine/normetanephrine, and serum parathyroid hormone levels could allow risk stratification and accurately prediction of MEN2 progression, thus facilitating implementation of personalized precision treatments to increase disease-free survival and overall survival. Furthermore, increased awareness of MEN2 is needed, which requires participation of physicians, patients, family members and related organizations. Psychological support is also important for patients with MEN2 to promote comprehensive management of MEN2 symptoms. The “5P” strategies for management of MEN2 represent a typical clinical example of precision medicine. These strategies could effectively improve the health of MEN2 patient, and avoid adverse outcomes, including death and major morbidity, from MEN2.

Abstract:Objective To study the clinical characteristics and therapy of multiple endocrine neoplasia (MEN),and to better understand the disease.Methods Clinical data of 6 cases of MEN in our hospital from November,2011 to September,2012 were reviewed.Results There were 3 eases of MENI,2 MEN2A,and 1MEN2B.The clinical manifestations of MEN were caused bv the tumors and their secretions.Operation was still the first-line treatment.Conclusions Early diagnosis and treatment are very important for patients.Group consultation of doctors with specific expertise plays important roles in the management of MEN-associated tumors.

Abstract:The endocrine pancreas-duodenum is often be involved in multiple endocrine neoplasia type 1 (MEN-1) which is an autosomal-dominant syndrome as a result of inactivating mutations of the MEN-1 tumor suppressor gene on chromosome 11q13 encoding for menin. The MEN-1 related pancreas-duodenum endocrinopathies are multicentric with early onset and lead to controversial strategy in management. Gene consequence helps diagnosis. Surveillance and screening for pancreas-duodenum endocrine tumors are recommended in presymptomatic gene carriers. An aggressive surgical approach is intended to control the functional syndromes and malignant potential for nodal or distant metastasis.

Abstract:OBJECTIVE:To summarize the experience on diagnosis and treatment of multiple endocrine neoplasia type 1 (MEN-1) related pancreatic endocrine tumors (PET).METHODS:From January 2004 to December 2007, there were 10 patients of MEN-1 related PET were treated in Shanghai Jiaotong University School of Medicine Affiliated Ruijin Hospital. There were 2 males and 8 females, aged from 11- to 49-years-old. They were diagnosed by laboratory tests, imaging examinations and genes sequencing. Drug therapy, surgery and follow-up were applied on the patients.RESULTS:There were 9 patients having insulinomas including 2 cases of multiple insulinomas and 1 case presenting an insulinoma, multiple nonfunctional PET and malignant duodenum gastrinoma with liver metastasis. The other patient was diagnosed as glucagonoma clinically. Five cases had family history and MEN-1 gene mutation was detected in 9 patients. Seven patients accepted nine operations. Twelve insulinomas, four nonfunctional PET and one duodenum gastrinoma were found in the operations. All patients were followed up from 1 month to 11 years, and 9 patients with good conditions and 1 patient died.CONCLUSIONS:Well recognizing PET and MEN-1, early diagnosing MEN-1 related PET, appropriately surgical intervention will prove patients' life quality and will help for prolonging patients' survival time.

Abstract:目的探讨多内分泌腺瘤病1型的发病与MEN1基因突变的相互关系及其遗传特征.方法从2个家系中的患者及其家庭成员抽提外周血淋巴细胞基因组DNA,运用PCR、DNA测序分析技术检测MEN1基因所有10个外显子,进一步用亚克隆测序鉴定其杂合性.结果发现两个家系中的先证者MEN1基因均在第二号外显子存在杂合突变,分别为372-373insACCTGTCTATCATCGCCG和357-360delCTGT,前一种突变为一种新的突变类型.结论在2个多内分泌腺瘤病1型中国人家系检出2种MEN1基因突变,其中MEN1基因372-373 ins ACCTGTCTATCATCGCCG为一种新的MEN1基因突变。
The clinical and histological study of multiple memingiomas

Abstract:Objective:To study the clinical and histological features of multiple meningiomas. Methods:39 cases of multiple meningiomas were analyzed retrospectively. The expressions of PR (Progesterone receptor) , p53 and MIB-1 were detected by immunohistochemistry in the samples of multiple meningiomas and solitary meningiomas. Results:Female predominance was obvious in cases of multiple meningiomas. The tumors had wide distributions with a main location in brain convexity. The results of immunohistochemistry showed stronger expression of PR and similar expression of p53 and MIB-1 in samples of multiple meningiomas than those in solitary meningiomas. Conclusion:The diagnosis of multiple meningiomas mainly depends on radiological examination. The pathogenesis of multiple meningiomas is probably related to progesterone and its receptor, and it is most likely monoclonal origin.

Abstract:Background: Multiple endocrine neoplasia type 1 (MEN1) is a rare hereditary tumor syndrome inherited in an autosomal dominant manner and presents mostly as parathyroid, endocrine pancreas (such as gastrinoma) and anterior pituitary tumors. At present, papillary thyroid carcinoma (PTC) and nodular goiter are not regarded as components of MEN1. Case summary: A 35-year-old woman presented with MEN1 accompanied by coinstantaneous PTC and nodular goiter. The pathological diagnosis was PTC with cervical lymph node metastasis, nodular goiter, parathyroid cyst and adenomatoid hyperplasia. Genetic testing was performed and a MEN1 gene mutation was detected. The patient underwent unilateral lobectomy of the thyroid gland and surgical removal of the parathyroid tumors. At 18 mo of follow-up, ultrasonic examination of the neck showed no abnormality. Serum calcium and parathyroid hormone levels were normal. No new MEN1-associated tumors were detected. Conclusion: The role of inactivating mutations of MEN1 gene in tumorigenesis of PTC and/or nodular goiter remains to be determined by more case reports and further research.

Abstract:Multiple endocrine neoplasia (MEN) includes MEN1 and MEN2. MEN1 gene mutations have been identified in familial and some sporadic MEN1 patients. Loss of heterozygosity at chromosome 11q13 and menin degradation occur in endocrine tumor cells. MEN1 is characterized by parathyroid adenoma, gastroenteropancreatic (GEP) and anterior pituitary tumors. MEN2A and MEN2B are characterized by medullar thyroid carcinoma, pheochromocytoma, parathyroid hyperplasia and mucosa neurofibroma, respectively. RET gene mutation results in receptor tyrosine kinase activation in endocrine-related tumors. Genotype correlates well with phenotype in MEN2.Advances of the research on mechanism of MEN is briefly introduced in this review.

Abstract:OBJECTIVE: We aimed to provide ideas for clinicians, especially radiologists, for the diagnosis of multiple endocrine neoplasia (MEN) syndromes.BACKGROUND: MEN syndromes include MEN1, MEN2, and MEN4 and usually involve 2 or more endocrine tumors. The MEN syndromes are a group of euchromatic dominant genetic diseases, and the main genes involved include MEN1 (MEN1), RET (MEN2), and CDKN1B (MEN4).METHODS: In this article, involving 8 cases (4 cases of MEN1, 2 cases of MEN2A, 1 case of MEN2B, 1 case of MEN4) from our center, we introduced the disease spectrum, clinical manifestations (especially imaging findings), and related genes involved in each type of MEN syndromes. We also discussed the differential diagnosis between MEN and sporadic tumors and emphasized that MEN should be screened and the relevant required examinations.CONCLUSIONS: Considering that MEN syndromes involve multiple endocrine gland tumors and nonendocrine organ diseases, it is very important to identify potential patients early and perform multiple examinations on them, including biochemical and multitype, and multisite imaging examinations according to the disease spectrum of each type. Considering that this is a group of genetic diseases, both interviewing patients about their family history and genetic testing are also very important. Only in this way can a comprehensive and accurate diagnosis be made, enabling patients to receive appropriate treatment and improve their prognosis.

Abstract:Multiple endocrine neoplasia type 1 (MEN 1) is an inherited tumour syndrome characterized by the development of tumours of the parathyroid, anterior pituitary and pancreatic islets, etc. Heterozygous germ line mutations of MEN1 gene are responsible for the onset of MEN1. We investigated the probands and 31 family members from eight unrelated Chinese families associated with MEN1 and identified four novel mutations, namely 373_374ins18, 822deIT, 259deIT and 1092deIC, as well as three previously reported mutations, such as 357_360deICTGT, 427_428deITA and R108X (CGA > TGA) of MEN1 gene. Furthermore, we detected a loss of heterozygosity (LOH) at chromosome 11q in the removed tumours, including gastrinoma, insulinoma and parathyroid adenoma from two probands of MEN1 families. RT-PCR and direct sequencing showed that mutant MEN1 transcripts remained in the MEN1-associated endocrine tumours, whereas normal menin proteins could not be detected in those tumours by either immunohistochemistry or immunoblotting. In conclusion, MEN1 heterozygous mutations are associated with LOH and menin absence, which are present in MEN1-associated endocrine tumours.

Abstract:Multiple endocrine neoplasia type 1 (MEN 1) is an autosomal dominant disorder characterized by the development of parathyroid hyperplasia, pancreatic endocrine tumors, pituitary adenomas, and adrenal adenomas. We reported 1 case of MEN 1 simultaneous with gastrinoma and insulinoma; meanwhile, insulinomas were ectopic and recurrent. The genetic screening showed the mutation of 427del AT of the MEN 1 gene. Surgical removal is considered the treatment of choice, with limited adverse effects and relatively low morbidity and mortality. She was treated by means of several surgical strategies, resulting in improvement of the frequency and severity of the hypoglycemic episodes and a better quality of life.
p.L105Vfs mutation in a family with thymic neuroendocrine tumor combined with MEN1: a case report

Abstract:Multiple endocrine neoplasia type 1 (MEN1) is a rare autosomal dominant disorder arising from mutations of the MEN1 tumor suppressor gene on chromosome 11q13; MEN1 is characterized by the development of neuroendocrine tumors, including those of the parathyroid, gastrointestinal endocrine tissue and anterior pituitary. Additionally, thymic neuroendocrine tumors in MEN1 are also rarely reported. This case report observed a family that presented with MEN1 p.L105Vfs mutation, and two of the family members had been diagnosed with thymic neuroendocrine tumor combined with MEN1. To the best of our knowledge, this is the first time such a mutation in the MEN1 gene has been reported. The proband presented with thymic neuroendocrine tumor, parathyroid adenoma and rectum adenocarcinoma. The son of the proband presented with thymic neuroendocrine tumor, gastrinoma, hypophysoma and parathyroid adenoma. Genetic testing revealed the frameshift mutation p.L105Vfs, leading to the identification of one carrier in the pedigree (the patient’s younger sister). The proband then underwent parathyroidectomy at the age of 26 years (in 1980) for a parathyroid adenoma. Subsequently, the patient underwent thymectomy, radiotherapy and chemotherapy. The patient is now 64 years old, still alive and still undergoing Lanreotide therapy. Thymic neuroendocrine MEN1 is rare, but it accounts for almost 20% of MEN1-associated mortality. Consequently, we should focus on regular clinical screening of the thymus in MEN1 patients.

Abstract:Objective To explore the clinical features of multiple endocrine neoplasia type 1(MEN1) and the mutation of MEN1 gene in the family.Methods Clinical data from 1 patient with MEN1 and other 7 family members were analyzed retrospectively.Peripheral blood samples(5 mL) were collected from the patient and his uncle,father,brother,sister,nephew,eldest daughter and youngest daughter,and genomic DNA was extracted from leukocytes by phenol/chloroform extraction.The 9 exons of MEN1 gene were analyzed by PCR and DNA sequencing.Results The proband and his father and brother had MEN1.The proband's eldest daughter and nephew were carriers of MEN1 gene mutation.No abnormalities in MEN1 gene were found in the proband's uncle,sister and youngest daughter.In normal subjects,stop codon TGC was located in exon 10.In MEN1 patients,C>A heterozygous mutation occurred at 1 227 nucleotides and resulted in a premature stop codon TGA,which leaded to menin protein truncation and MEN1 gene mutation.Conclusion Genetic testing for MEN1 has important implications for early diagnosis and clinical intervention in MEN1 patients and their families.

Abstract:• MEN syndromes are rare autosomal dominant syndromes that present clinically with a mixture of endocrine and non-endocrine-related tumors. • The most common clinical manifestation of MEN 1 is primary hyperparathyroidism, although pancreatic neuroendocrine tumors are the most common cause of mortality in MEN 1. • While genetic testing is important for disease confirmation of MEN 1, it can be especially impactful for guiding clinical decision-making in MEN 2 because of the genotype-phenotype correlation.
