Abstract:Objective To determine whether real-time three-dimensional transesophageal echocardiography(RT-3D TEE) is an accurate non-invasive technique for defining the morphology of atrial septal defects(ASD). Methods In 20 patients with secundum ASD, mean age 42.2 years (7 male, 13 female) ,live three dimensional surgical views of ASD were acquired,qualitative morphology of ASD such as the shape, presence of fenestrations and the defect margins were noted during the cardiac cycle. The measurements obtained from 2-dimensional transesophageal echocardiography(2D-TEE) and RT-3D TEE were compared to those obtained from stretched balloon diameter(SBD) or surgery. Measurements of the size and area change of ASD were validated during the cardiac cycle by RT-3D TEE,stepwise multiple linear regression analysis was performed to test the correlation between the maximum change ratios of area and diameter of ASD and age, atrial septal length, ASD shunting velocity, the maximum diameter of ASD and right ventricular systolic pressure. Results ① The morphology of ASD was circle-like or elliptical in systole,irregular or elliptical in diastole. ②The area and diameter of ASD measured by RT-3D TEE was minimal in isovolumetric contraction phase, maximal in isovolumetric diastole phase. The correlation coefficient of maximum diameter measured by 3D with SBD was 0.962 , greater than that by 2D and SBD (0.820). ③The change ratio of area and maximum diameter of ASD acquired by RT-3D-TEE was 11.48%-71.12% and 2.80%-43.87% respectively,and the correlation coefficient of them was 0.921. Conclusions RT-3D TEE using live 3D-Zoom mode accurately displayed the varying morphology,dimensions and spatial relations of ASD. RT-3D TEE can offer visualization morphological changing of ASD in different periods of cardiac cycle,providing more information for percutanous catheter intervention and open heart surgery.
Quantitative Evaluation of Left and Right Ventricular Function by Serial Measuring Tei Index in Patients with Atrial Septal Defects Before and After Amplatzer Closure

Abstract:Objective This study attempted to quantitatively assess left and right ventricular function by serially measuring Tei indices in patients with atrial septal defects before and after Amplatzer closure.Methods Fifteen patients with secondary atrial septal defect undergoing transcatheter Amplatzer closure were prospectively investigated.Serial Tei indices were obtained by measuring the ratio of isovolumic time (isovolumic contraction time IVCT and isovolumic relaxation time IVRT) to the ejection time of the left ventricle (LV) and right ventricle (RV) before,during and 30 days after Amplatzer device implantation.Results The LV Tei index was significantly decreased in the patients of secondary atrial septal defects between 30 days after and before Amplatzer device implantation (0.30±0.07 vs 0.22±0.09,P<0.05).The RV Tei index has no obvious change in the patients of secondary atrial septal defects before,during and 30 days after Amplatzer device implantation.Conclusions The LV function has a tendency of improvement after Amplatzer device implantation in the patients with secondary atrial septal defects.The changes of volume load has no significant influence to the RV Tei index.
Utility of Real-Time Three-Dimensional Transesophageal Echocardiography in the Intraoperative Assessment of Mltral Valve Repair

Abstract:Objective To determine the usefulness of real time three-dimensional transesophageal echocardiography(RT-3D TEE) in the preoperative assessment of mitral valve (MV) pathology by comparing images with surgical findings and to evaluate the function of MV postoperatively. Methods Nineteen consecutive adult patients with established diagnosis of mitral regurgitation(MR) scheduled for surgical correction were enrolled. Intraoperative 2D and 3D transesophageal echocardiography(TEE) were performed. All the 3D images were compared with findings obtained from direct surgical inspection. Postoperative RT-3D TEE was performed immediately to evaluate outcomes of mitral annuloplasty. Results Superb 3D-TEE en face views of the MV were obtained in all patients. Correct diagnoses of MV pathology in agreement with the surgical findings were made in 16 of 19 patients (84. 2%), however in the remaining 3 patients, the diagnoses were incorrect despite good image quality. In one, RT-3D TEE diagnosed prolapse of A2 segment and P2 scallop,which was not confirmed at surgery, but MV degeneration and annular dilatation were observed. In another one, RT-3D TEE revealed prolapse of P3 scallop, while at surgery prolapse involved P2 and P3 with ruptured chordae. In the rest one,surgically detected prolapse of P2 was missed by RT-3D TEE. On the other hand with 2D TEE,the diagnoses correlated poorly with surgical findings,only 10 patients were accurately diagnosed (52.6%). In some patients,2D TEE was able to identify the MVP, but it had difficulty in defining the exact location of the prolapsed segment or scallop. The severity of MR decreased significantly after surgery. MV repair was successful in all patients except one(5.1%), in whom moderate to severe MR was still present and MV replacement was conducted subsequently as an alternative.Conclusions RT-3D TEE provides excellent imaging of MV components, including the anterior and posterior leaflets, as well as annulus and subvalvular structures, which can be especially helpful in planning the most appropriate surgery strategy. RT-3D TEE offers exact anatomic characteristics of mitral annuloplasty rings and bands, providing additional information for the evaluation of surgical outcomes.
Assessment of Left Atrial Appendage Volume and Function Using Real-Time Three-Dimensional Transesophageal Echocardiography in Patients with Nonvalvular Atrial Fibrillation

Abstract:Objective To evaluate the volume and function of left atrial appendage (LAA) in patients with nonvalvular atrial fibrillation using real-time three-dimensional transesophageal echocardiography (RT-3D TEE).Methods Eighty-seven patients underwent RT-3D TEE were divided into 4 groups:patients with normal sinus rhythm group,paroxysmal atrial fibrillation groups with sinus rhythm and with fibrillation,persistent atrial fibrillation group.RT-3D TEE data of left atrial appendage volume and function parameters were derived and analyzed offline with Qlab analysis software.LSD of One-way ANOVA was used to analysis the same parameters among these groups,and Pearson correlation coefficient between ejection fraction (EF) with the other parameters in the fibrillation group were obtained respectively.Results ①The correlation analysis indicated that there was significantly negative correlation between LAA-EF and endsystolic volume (ESV),and there were lower negative correlation among LAA-EF and end-diastolic volume (EDV),LAA maximal area (Amax) and the patients ages.Otherwise,LAA-EF correlated positively with LAA area change percentage (A％),LAA diameter change percentage (D％),and had medium positive correlation with LAA peak emptying velocity(Vep).There was no correlation between LAA-EF and LAA maximal diameter (Dmax).②The change rate of spontaneous echo contrast (SEC) and thrombi of LAA were increasing with duration of atrial fibrillation.③In patients with short-time atrial fibrillation and cardioversion autonomously or after drugs,the enlargement of EDV of the LAA occurred firstly,then ESV,A％ and D％ diminished when drugs was ineffective,lastly,the parameters of volume and contraction declined in patients with persistent atrial fibrillation.Conclusions Quantitative study of the volume and function of LAA by RT-3D TEE,and the relationship between the characteristics of LAA and the duration of atrial fibrillation may offer important information for the evaluation of atrial fibrillation and the choose of treatment protocol.

Abstract:Objective:To investigate the clinical application of real-time three-dimensional echocardiography (RT 3DE) in the diagnosis and treatment of atrial septal defects(ASD).Methods:Thirty-two patients with ASD were studied with RT 3DE of Philips Sonos 7500. 12 patients underwent surgery therapy; 12 patients underwent transcatheter closure, and 4 patients were guided by RT 3DE. 3D image database was post-processed using 3D work-station. The results were compared with those measured by two-dimensional transthoracic echocardiography (2D TTE) ,transesophageal echocardiography (2D TEE), stretched balloon diameter (SBD) and (or) surgical results.Results:The defects were delineated objectively frommany unique directions and angles by RT 3DE that would not be available with other current techniques, even simulated intraoperative visualization. The stereoscopic views of ASD including the shape, location, size, rims of the ASD, and its spatial relation to the surrounding structures could be appraised in their nature. For maximal ASD size measurement, the best correlation was found between RT 3DE measurements and the SBD, whereas 2DE was smaller than the other two methods(r=0.97vs,r=0.88). RT 3DE guided the transcatheter closure procedure successfully.Conclusion:Compared with the routine 2DE, RT 3DE provides en face stereoscopic views of ASD; thus,it can accurately measure the size of ASD. Meanwhile RT 3DE can provide a rapid and accurate method for monitoring and guiding the transcatheter closure procedure. RT 3DE has important applied potentiality in clinical field.
Clinical Implication of Real-Time Three-Dimensional Transesophageal Echocardiography in Percutaneous Transcatheter Left Atrial Appendage Closure with Amplatzer Cardiac Plug(ACP) Device for Non-Valvular Atrial Fibrillation Patients

Abstract:Objective To evaluate the application value of real-time three-dimensional echocardiography (RT-3D TEE) in nonvalvular atrial fibrillation patients after transcatheter left atrial appendage closure (LAAC) with the Amplatzer Cardiac Plug (ACP).Methods The two-dimensional transesophageal echocardiography (2D TEE) and RT-3D TEE were performed in selected patients to measure the diameter of left atrial appendage ostium and landing zone,to observe left atrial appendage morphology and lobular distribution.These were also performed to guide the whole process of transcatheter LAAC with ACP,which included the atrial septal puncture,sheathing canal cruise,occluder device implantation,and verifying the stability of occluder device and releasing the device.It involved observing ACP occluder morphology,location,stability,surrounding residual shunt,whether influencing the mitral valve and left superior pulmonary vein flow,and pericardial effusion.Results A total of 15 patients (100％) successfully underwent LAAC with the ACP.The maximum diameter of left atrial appendage ostium by 2D-TEE measurement during operation was (20.5 ± 2.9)mm,located at 135°.The diameter of left atrial appendage landing zone was (17.1 ± 2.8) mm,located at 45°;(18.0 ± 4.0) mm,located at 90°;(22.1 ± 4.7)mm,located at 135°,respectively.The left atrial appendage morphology:2 had one leaf and 13 had two leaves or more,including 2 cases of bifoliate short neck shape.In 15 patients,6 cases of cauliflower type,2 cases of wind sock type,3 cases of chicken wing type,2 cases of cactus type and 2 cases of complex type.The proximal left atrial appendage morphology:3 cases of boot type,2 cases of wide mouth type,6 cases of narrow mouth type,2 cases of straight tube type,and 2 cases of bifoliate short neck type.There was no obvious residual shunt in all the patients at immediately post-operation.Conclusions In the transcatheter LAAC with the ACP,RT-3D TEE has important application value in the preoperative selection of patients,the choice of appropriate occluder,guidance of full-process monitoring during operation,the postoperative effect evaluation and so on.
Real-time Three-dimensional Echocardiography in Assessment of Ventricular Septal Defects

Abstract:Objective To evaluate whether real-time three-dimensional echocardiography (RT-3DE) can accurately confirm the anatomical positions,shapes and sizes of ventricular septal defects (VSDs) in patients undergoing surgery or catheter-based closure.Methods 24 patients with VSDs were studied by RT-3DE,and the results were compared with those measured by surgery or catheter.Results Optimal diagnosis of the VSDs were obtained in all the patients,accurately permitting detailed assessment of the anatomical positions,sizes,and shapes of the VSDs.Good agreement was demonstrated between measurements made by 3DE and those obtained at surgery or catheter-based closure.Conclusions RT-3DE provides excellent visualization of various types of VSDs,and the positions,sizes,and shapes of VSDs can be accurately determined.Such precise imaging will be beneficial for surgery and catheter-based closure.
Diagnosis of intermediate type of atrioventricular septal defect using color Doppler echocardiography

Abstract:Objective To evaluate the diagnostic value of intermediate type of atrioventricular septal defect using color Doppler echocardiography.Methods Fifteen patients with intermediate type of atrioventricular septal defect were studied by color Doppler echocardiography and the echocardiographic characteristics were analysed.Results All 15 cases were accurately diagnosed by color Doppler echocardiography. The most important echocardiographic characteristics of intermediate type of atrioventricular septal defect included:A large atrioventricular defect consisted of a premium atrial septal defect and an inlet ventricular septal defect.②Two separate atrioventricular valves were at the same level. Nearly half of the patients had cleft mitral or/and tricuspid valves.③CDFI demonstrated all patients had significant atrioventricular valvular regurgitation.Conclusion Color Doppler echocardiography has important value in diagnosis of intermediate type of atrioventricular septal defect.
Application of Transthoraic Three-Dimensional Echocardiography in the Diagnosis of Congenital Cardiac Septal Defects in Children-Compared with Two-Dimensional Echocardiography

Abstract:Objective: To assess the reliability and advantage of transthoracic three-dimensional echocardiography (3DTTE) in the diagnosis of congenital cardiac septal defects. Methods: 52 patients with VSD and 20 patients with ASD were diagnosed by using 3DTTE and transthoracic two-dimensional color Doppler echocardiography (CDE), and the results were compared with surgical findings. 3DTTE was carried out by using HP/SONOS5500 with 5MHz transthoracic Omniplane rotational probe and Tomtec/4D Cardio-View RT software. Results: ①The types of VSD defined by surgery included perimembranous VSD (35) and doubly committed VSD (17), which were consistent with both CDE and 3DTTE findings. Mean diameter at major axis of VSD measured on surgery was 9.28±3.19mm, which was excellently correlated with that measured by 3DTTE ( 8.82±2.92mm) (r=0.88, p0.01) and well correlated with that measured by CDE (8.69±3.42mm) (r=0.65, p0.01). However, CDE could not define abnormal tricuspid chordae crossing the defect that happened in 4 patients with perimembranous VSDs, in whom 1 was misdiagnosed as double VSDs and 1 was misdiagnosed as diaphragm infra aortic valve. CDE only detected 2 out of 4 prolapse of aortic valve in those doubly committed VSDs. In contrast, 3DTTE displayed all the problem described above. Moreover, 3DTTE permitted detailed assessment of the positions, sizes, shapes of VSDs. ②Types of ASD defined by surgery included secundum (17), inferior veno caval (2) and superior veno caval (1), which conformed 3DTTE findings. However, CDE failed to differenciate secundum ASD from other types in this group. The diameters of ASDs at major and minor axis measured on surgery were 19.5±4.2mm and 14.8±3.7mm, which were highly correlated with those measured by 3DTTE (18.8±3.7mm and 14.0±3.3mm, respectively) (r=0.93 and 0.92,respectively, p0.01). Mean diameter of ASD measured by CDE was 18.6±4.1mm, which was also well correlated with surgical measurement(r=0.86,p0.01). Conclusion: 3DTTE provides better information on spatial recognition of the VSD and ASD and their relationship to the surrounding anatomic structures as compared with CDE, and it may become an optimal tool for clinical diagnosis in congenital cardiac septal defects.

Abstract:Objective To investigate the value of real-time three-dimensional echocardiography in transcatheter clousure of atrial septal defect (ASD). Methods Philips Sonos 7500 with an X4 probe was used. Four patients(1 male, 3 female, mean age 60.5±8 years) with ASD underwent transcatheter closure under the guidance of real-time three dimensional echocardiography. Results Real-time three-dimensional echocardiography displayed clearly morphology, size, position of ASD and its spatial relation to the surrounding tissue, and guided the transcather closure procedure. Conclusions Real-time three-dimensimal echocardiography provides rapid and accurate method monitoring the transcather closure procedure.
Assessment of atrial septal defect using 2D or real-time 3D transesophageal echocardiography and outcomes following transcatheter closure

Abstract:BACKGROUND: The assessment of interatrial septum (IAS) requires a standardized, systematic approach, including two-dimensional transthoracic echocardiography (2D TTE), 2D transesophageal echocardiography (2D TEE), and three-dimensional (3D) TEE. Although 2D TEE has been widely used for the preoperative assessment of atrial septal defect (ASD), its ability to provide reliable information is often limited due to the structural characteristics of IAS. The introduction of 3D TEE provides a unique "en face" view of IAS, which allows the visualization and accurate measurements of diameters, area, and rims of ASD. Hence, appropriate ASD imaging information is particularly important in successful transcatheter closure.METHODS: In this retrospective study, 2D TTE/TEE, and 3D TEE were performed before ASD closure, with 2D minimal and maximal diameters, areas, and residual rims being recorded. Adequate 3D TEE imaging data sets were collected and then analyzed. ASD related parameters were compared using different echocardiography. Patients who underwent ASD closure completed a clinical follow-up.RESULTS: The mean defect maximal diameter and aperture area by 3D TEE was significantly larger than that of the corresponding 2D TEE (P<0.05). There was no statistical difference in the minimal and maximal diameter or area by TEE for circular-shaped ASDs. For oval ASDs, mean minimal diameter on 2D TEE was larger than that on 3D TEE. The mean maximal diameter measured using 2D TEE was smaller than the 3D TEE measurement (16.0±7.1 <i>vs.</i> 19.8±8.6; P<0.05). For complex-shaped defects, there were statistical differences in minimal and maximal diameter between TEEs. Furthermore, 2D and 3D TEE had a longer superior vena cava (SVC) residual rim than did 2D TTE (P<0.05). The 3D TEE residual rims of the inferior vena cava (IVC) was significantly larger than the corresponding 2D TEE. There was a very strong correlation between the residual rim measurements using 3D and 2D TEE. However, the limits of agreement between 2D and real-time 3D TEE measurements were more apparent in the IVC rim group than in the other groups.CONCLUSIONS: Our study confirms the value of 3D TEE in assessing ASD shape and size reported by previous studies, and is also the first to accurately and systematically characterize ASD residual rim in complex ASDs.

Methodological Study on Real-Time Three-Dimensional Echo Cardiography and Its Application in the Diagnosis of Complex Congenital Heart Disease

Abstract:Background Real-time three-dimensional echocardiography (RT-3DE) has made revolutionized improvements of cardiac imaging during the past few years. However, there is no standard examination method for RT-3DE so far. This study aimed to establish the diagnostic method of RT-3DE and evaluate its application in the diagnosis of complex congenital heart diseases (CHD). Methods Fifty patients with complex CHD were examined by RT-3DE with modes of Live 3DE and Full Volume. A series of novel volumetric views combined with Van Praagh sequential segmental approach were introduced to reveal the pathological morphology of the hearts, which were compared with the findings of two-dimensional echocardiography (2DE), angiography and cardiac surgery. Results In 50 patients, 190 image acquisitions of Full Volume were performed at several acoustic windows including subcostal, apical and parasternal regions. Among them, 94.2% (179/190) of image acquisitions were successful. Most sectional volumetric views could be clearly displayed in 92.6% of the successful image acquisitions. However, sectional volumetric views could not be clearly displayed in 7.4%, which was mainly due to poor perspective conditions of examination location, improper instrument multi-parameter setting and insufficient information of whole heart captured in Full Volume acquisitions. As compared with surgical findings and angiography, RT-3DE made correction to the diagnoses in 2 cases including 1 with corrected transposition of the great arteries and the other with single atrium and mitral cleft. The diagnoses initially made by 2DE for these 2 patients were double outlet right ventricle with transposition of the great arteries and complete atrio-ventricular septal defect. Conclusions RT-3DE can clearly display the pathological morphology of complex CHD by a series of novel volumetric views combined with sequential segmental approach through providing more spatial informative cardiovascular structures, which provides a practical method for RT-3DE diagnosis.
Assessment of atrial septal defect size and residual rim using real-time 3D transesophageal echocardiography

Abstract:BackgroundAccurate preoperative determination of defect location and size is important for successful transcatheter closure of atrial septal defects (ASD). Real-time 3D transesophageal echocardiography (3DTEE) has the potential to delineate the shape of ASD in 3D space.MethodsFull volume and 3D zoom datasets by 3DTEE were acquired in 17 ASD patients. Using quantitative software, maximal/minimal diameter, defect area and residual rim length were measured and compared to the standard 2D measurements.ResultsReal-time 3DTEE allowed delineation of the en-face view of the ASDs. The defect typically had an oval shape, and its size changed dynamically, having its minimal size at end-diastole and maximal at end-systole. A good correlation was noted between the maximal defect area by 3DTEE and 2DTEE (r = 0.93, p < 0.001). Successful delineation of rim length to the specific cardiac structure was 100% by 3DTEE and 88% by 2DTEE. There was a fair correlation of residual rim length between 3DTEE and 2DTEE (r = 0.69, p < 0.001). Eight patients underwent transcatheter closure of the ASD. Excellent correlation was noted between 3D-derived maximal defect diameter and device diameter (r = 0.97, p < 0.001).ConclusionsReal-time 3DTEE allows measurements of the temporal and spatial changes of ASD size and shape. This methodology provides detailed information on defect dynamics.

Abstract:Background: The aim of this study was to demonstrate the utility of real-time three-dimensional (3D) transesophageal echocardiography (RT3D-TEE) using a matrix array 3D transesophageal echocardiographic probe for morphologic evaluation and guidance of transcatheter closure of atrial septal defects (ASDs). Methods: Forty-eight consecutive patients scheduled for the intervention were included. Two-dimensional (2D) transesophageal echocardiography (2D-TEE) and RT3D-TEE were performed before and during the procedures. Measurements of maximal ASD diameter and surrounding rims obtained on RT3D-TEE were compared with those obtained on 2D-TEE. Results: In 46 patients (96%), optimal 3D images for the morphologic evaluation of ASDs were obtained. RT3D-TEE facilitated the evaluation of ASD morphology and surrounding rims and was able to provide intraprocedural information clearly. A Bland-Altman plot showed a mean maximal diameter difference of -0.12 mm between the means (95% limits of agreement, -2.2 to 2.5 mm). Conclusion: RT3D-TEE is a clinically useful, complementary option to 2D-TEE for evaluation of ASD morphology and for interventional guidance.
Real-time three-dimensional transesophageal echocardiography in patients with secundum atrial septal defects: outcomes following transcatheter closure

Abstract:Background: Successful transcatheter closure of atrial septal defects (ASDs) requires the accurate assessment of defect size and morphology. Assessment of ASD anatomy may be difficult by two-dimensional (2D) echocardiography. The aim of this study was to test the hypothesis that real-time three-dimensional (3D) transesophageal echocardiography (TEE) may provide more accurate morphologic assessment of ASDs than multiplane 2D TEE. Methods: Twenty-four patients with ASDs were imaged using 2D and real-time 3D TEE. ASD shape and size were assessed using 3D TEE retrospectively. Maximal ASD dimensions obtained by 3D TEE were compared with unstretched and balloon-stretched dimensions on 2D TEE. Planimetered defect area by 3D TEE was compared with area calculated using the ellipse formula from 2D imaging. Twenty of the 24 patients underwent transcatheter ASD closure. Closure device size was based on findings on 2D TEE. Follow-up was conducted by 2D transthoracic echocardiography. Results: Of the 24 ASDs, 6 (25%) were circular, 10 (42%) were oval, and 8 (33%) were complex in shape. The mean maximal dimension was larger by 3D TEE compared with 2D TEE (1.8 ± 0.8 vs 1.5 ± 0.6 cm; P < .05). There was no difference in the mean area measured by either modality, but for complex-shaped defects, area measured by 3D TEE was larger than that by 2D TEE (2.8 ± 1.3 vs 1.7 ± 1.4 cm(2); P < .05). Follow-up transthoracic echocardiography was available for 19 of the 20 patients undergoing transcatheter closure. Nine patients had residual right-to-left shunting 1 to 6 months after ASD closure, and the majority of these were complex in shape. In patients with residual shunting, ASD area by 3D TEE was 27% larger than by 2D TEE, whereas in patients without residual shunting, there was significantly less discrepancy between 3D and 2D areas (19%; P = .0027). Conclusions: Three-dimensional TEE can identify ASD shape. Maximal dimensions on 3D TEE were well correlated with balloon-stretched 2D dimensions. Two-dimensional TEE can underestimate the area of complex-shaped ASDs, which may result in residual right-to-left shunting.
Clinical advantage of real-time three-dimensional transesophageal echocardiography for transcatheter closure of multiple atrial septal defects

Abstract:This study sought to evaluate the usefulness of real-time three-dimensional (3D) transesophageal echocardiography (TEE) to guide the repair of multiple atrial septal defects (ASDs). Of 212 consecutive patients with secundum ASD who were scheduled for transcatheter closure of their ASDs, 27 had multiple ASDs. These patients underwent two-dimensional (2D) transthoracic echocardiography, 2D TEE, and 3D TEE. Overall, 18 patients had two defects, and 9 patients had three or more defects. The latter group included three patients with multi-fenestrated defects. Optimal 3D images were obtained in 93 % of the patients. In patients with two defects, information on the positional relation of the defects was obtained using 2D TEE and 3D TEE in 71 and 94 % of patients, respectively (P = 0.22). The positional relations of the defects could not be evaluated with 2D TEE in patients with three or more defects, whereas it could be evaluated with 3D TEE in all of these patients (0 vs. 100 %, P = 0.008). In all patients, 3D TEE proved superior to 2D TEE for providing sufficient information (96 vs. 48 %, P = 0.002). Procedural success was obtained in 26 patients (96.3 %), without complications. Transcatheter closure of multiple ASDs under 3D TEE guidance is effective and safe. Real-time 3D TEE can provide useful information regarding complex ASD morphology. It can thus contribute to developing a successful treatment strategy, especially in patients with three or more defects.

Abstract:Objective The current study was the first in China to investigate the value of single-beat real-time three-dimensional echocardiography (sRt-3DE) in assessing the shape and function of right ventricle in Patients with Atrial Septal Defect (ASD). Methods 33 healthy volunteers (11 male, mean age (41.15±17.49 years)) and 18 ASD patients (six male, mean age (34.67±19.50 years)) were enrolled. All the participants received routine echocardiography. sRt-3DE by SIEMENS SC2000 was performed in all to evaluate parameters concerning morphology and systolic function of participants’ right ventricle. All the parameters were indexed by body surface area. Further correlative analysis was calculated between right ventricular parameters and right heart pressures by cardiac catheter as well as right ventricular parameters and body surface area-indexed maximum diameter of ASD (IASD). 21 participants (15 controls and six ASD patients) were randomly selected to explore the inter-observer agreement of this novel technique. Results Rt-3DE was successful in all participants, even in patients with large right ventricle volume up to 250 ml. Statistic analysis showed the technology has good inter-observer correlation, (EDVRV: ICC 0.891 (95% CI 0.731 to 0.956); ESVRV: ICC 0.737 (95% CI 0.731 to 0.893)) Indexed right ventricular end diastolic volume (IEDVRV) and end systolic volume (IESVRV), indexed right ventricular systolic volume (ISVRV) and right ventricular ejection fraction (RVEF) were greater in the ASD group than in controls (p<0.001, p<0.01, p<0.001, p<0.05). IEDVRV, IESVRV and ISVRV had positive relations with pulmonary artery pressures, especially pulmonary artery diastolic pressure measured by cardiac catheter (r=0.61, r=0.79, r=0.83, all p<0.05) and IASD (r=0.57, p<0.05; r=0.6, p<0.01; r=0.55, p<0.05 respectively). At the same time, they had negative relations with LVEF (r=−0.47, r=−0.5, r=−0.52; all p<0.05). Conclusions Left-to-right shunt in ASD patients may lead to an aggressive increase of pulmonary artery pressure and right ventricular volume. The enlargement of right ventricle increases right ventricular contraction and affects the morphology and function of left ventricle. sRt-3DE is a unique new modality to precisely and stably evaluate right ventricular changes in ASD patients.
Three-Dimensional Echocardiography in the Evaluation of Right Ventricular Global and Regional Systolic Function in Patients with Atrial Septal Defect Before and after Percutaneous Closure.

Abstract:OBJECTIVE:To evaluate right ventricular (RV) global and regional systolic function in patients with atrial septal defect (ASD) before and after percutaneous closure using real time three-dimensional echocardiography (RT3DE).METHODS:RT3DE was performed in 81 patients with ASD within 24 hours before and after percutaneous closure to obtain RV global and regional ejection fraction (EF) in three compartments (inflow, body, and outflow). RV fractional area change (FAC), tricuspid annular plane systolic excursion (TAPSE), peak tricuspid systolic velocity (S), and pulmonary vascular resistance (PVR) were recorded. Forty matched normal adults were included as controls.RESULTS:When compared with controls, RV global and regional EF were decreased in preclosure patients (P < 0.001). FAC was lower while TAPSE and S were higher in preclosure patients than in controls (P < 0.05). After closure, RV systolic function parameters were all reduced (P < 0.001). Regional EF in the body compartment was the lowest among the three compartments in ASD patients (P < 0.05). Procedural percentage changes of RV global EF and regional EF in the inflow compartment were lower than those of two-dimensional systolic function parameters (P < 0.05). RV global and regional EF in the inflow compartment were negatively correlated with PVR in patients after closure (r = -0.601, -0.543, P < 0.001).CONCLUSIONS:RV global and regional systolic functions are impaired in open and closed ASD. RT3DE-derived systolic function parameters are negatively correlated with RV after load. RT3DE has potential value in the evaluation of RV systolic function in patients with ASD.
En face view of atrial septal defect by two-dimensional transthoracic echocardiography: comparison to real-time three-dimensional transesophageal echocardiography

Abstract:Background: It is generally believed that three-dimensional (3D) echocardiography can provide unique en face views of atrial septal defects (ASDs), whereas conventional two-dimensional (2D) transthoracic echocardiography (TTE) cannot. The purpose of this study was to develop a special 2D TTE-based en face view of ASDs. Methods: En face views of ASDs on 2D TTE were obtained in 415 consecutive adult patients and compared with the results of real-time 3D transesophageal echocardiography in 25 of these patients. Results: Acceptable en face views of ASDs on 2D TTE were obtained in 80% of patients, in whom secundum ASDs could be adequately imaged in more standard 2D imaging planes. The ability to visualize en face views of ASDs on 2D TTE was inversely related to their sizes (P<.05). The differences in the major and minor dimensions of ASDs between 2D TTE and real-time 3D transesophageal echocardiography were not statistically significant (P>.05). Conventional 2D transthoracic echocardiographic views significantly underestimated ASD size with superior-inferior directional major axis. Conclusion: Two-dimensional TTE can provide en face views of ASDs and their spatial orientations with respect to neighboring structures. This method may provide incremental information to 3D echocardiography in patients with ASDs.
Real-time three-dimensional echocardiography in evaluating left ventricular systolic synchrony in patients with atrial septal defect before and after interventional occlusion

Abstract:Objective:To investigate the dynamic change of left ventricular systolic synchrony in patients with atrial septal defect (ASD) before and after interventional occlusion using real-time three-dimensional echocardiography (RT-3DE).Methods:A total of 52 patients undergoing interventional occlusion in Zhongshan Hospital from September 2015 to January 2016 were enrolled in this study.All subjects were assigned to take RT-3DE one day before,one day and one month after the interventional occlusion.TomTec 4D LV-Analysis CRT 1.0 software was used to analyze volume-time curves (VTC) of the 16 segments of the left ventricle.Parameters including time to minimal segmental volume (Tmsv),time to minimal segmental volume-standard deviation (Tmsv16-SD),and time to minimal segmental volume-difference (Tmsv16-Dif) were analysed.These three parameters were standardized by dividing cardiac cycle to generate other three parameters:Tmsv％,Tmsv16 SD％,and Tmsv16-Dif％.Results:VTC before the interventional occlusion suggested dyssynchronation of the left ventricular systole;VTC of one day after the interventional occlusion suggested synchronization of the left ventricle.So did the results of one month after the interventional occlusion.Accordingly,parameters including Tmsv,Tmsv％,Tmsv16 SD,Tmsv16-SD％,Tmsv16-Dif,and Tmsv16-Dif％ were significantly improved one day after occlusion as compared with one day before occlusion.These parameters were not significantly changed one month after occlusion as compared with one day after occlusion.In terms of the contraction amplitude,only a few segments showed a significant improvement one day after the interventional occlusion,most segments did not improve until one month later.Conclusions:Interventional occlusion significantly improved left ventricular systolic synchronization in patients with ASD one day after occlusion.Additionally,the contraction amplitude was significantly improved one month after the interventional occlusion.
