Abstract:脉络丛肿瘤(choroid pluxus tumor,CPT)是一种少见的颅内原发肿瘤,多位于侧脑室和第三脑室.少数肿瘤可发生于脑室以外,如桥小脑角区;发生于脑实质的CPT罕见.笔者报道3例发生于幕下的CPT,并结合文献复习其影像表现,以提高对本病的认识。

Abstract:Abstract Rationale: Intrasellar cavernous hemangiomas (ICHs) are rare vascular lesions that arise in the sellar region. ICHs are usually misdiagnosed and treated as pituitary adenomas. Therefore, a preoperative diagnosis is particularly important, especially when the goal is complete resection. Patient concerns: A 55-year-old woman presented with a 1-month history of intermittent dizziness. Magnetic resonance imaging (MRI) revealed a well-demarcated abnormal ellipsoid signal in the sellar region (size: 2.7 cm × 1.7 cm), with a mulberry-like enhancement after gadolinium injection. Computed tomography revealed an intrasellar mass without calcification that extended into the left cavernous sinus and was faintly contrast-enhanced. Angiography revealed a tumor with mildly delayed staining fed by the C5 segment of the right internal carotid artery. Diagnosis: An intrasellar cavernous hemangioma based on neuroradiological examinations. Interventions: The patient underwent surgery with an endoscopic endonasal transsphenoidal approach to debulk the lesion and obtain tissue for the pathological diagnosis. Outcomes: Blood spurting was observed after puncture, and the capsule was stained blue. Lesion removal was stopped, and the patient underwent gamma knife surgery 1 week later. She remained in good condition during the follow-up. Lessons: Sponge-like or mulberry-like lesions can be identified on MRI after gadolinium injection and can facilitate a preoperative diagnosis of ICH. Currently, surgical debulking with cranial nerve decompression during the acute stage and subsequent gamma knife radiosurgery are considered to be a safe and effective treatment.

Abstract:Objective To study medical imaging features of tumors in cerebellopontine angle(CPA) and its differential diagnosis. Methods CT,MRI finding in 15 cases of tumors in CPA approved by pathology were retrospectively analyzed. Results 12 cases showed a round or oblong masses with diameter of 2.5～5 cm in CPA. 3 cases of tumors, dumbbell form, located between middle and posterior cranial fossae. On T\-2WI the fibrous meningioma showed iso\|intensity or hypo\|intensity, the schwannoma displayed hyper\|intensity; and the metastatic lesion slight hyper\|intensity. The MRI features of the epidermoidomas distinguish may from the arachnoid cysts. Conclusion Differential diagnosis of CPA tumor can be made by combining morphologic and its enhancement characteristies.\;

Abstract:PICCC is the rarest, most malignant primary intracranial GCT. The purpose of this study was to describe and characterize the MR imaging findings in a series of 7 patients (6 males and 1 female; mean age, 11.9 years) with pathologically proved PICCC in our institution from 2004 to 2009. All tumors were located within the pineal (n = 6) or suprasellar (n = 1) regions. On T2-weighted MR imaging, the lesions appeared markedly heterogeneous with areas of both hypointensity and hyperintensity reflecting the histologic heterogeneity, including hemorrhage, fibrosis, cysts, or necrosis. Heterogeneous (n = 7), ringlike (n = 4), and/or intratumoral nodular (n = 3) enhancement was noted on T1-weighted images with gadolinium. These MR imaging findings, combined with patient age and serum β-HCG levels, may prove helpful in distinguishing PICCC from the more common primary brain tumors, thereby avoiding biopsy of this highly vascular tumor.

Abstract:Purpose:To study the CT and MR features of tumors in cerebellum pontine angle(CPA) and explore their diagnostic value.Methods:CT and MRI findings in 115 cases of tumors in CPA verified by operation and pathology were analyzed.Seventy - two cases were performed transverse plane scan with Siemens Somatom Sensation64 or GE Ultra16 CT,some of them were also performed coronal CT scan. Eighty - four cases were performed SE or FSE sequence with GE Signa VH/ i 3.0T or GE Signa Excite HD 1.5T TWINSpeed MR to get sagital T_1WI and transverse T_1WI、T_2WI and FLAIR.Sixteen cases were performed DWI.Sixty - one were performed contrast enhanced MR.Results:Seventy - eight of 115 cases were acoustic neuromas,15 were meningiomas,7 were trigeminal neuromas,5 were cholesteatomas,5 were arachnoid cysts,1 was choroid papilloma,1 was pendymoma,1 was hemangioblastoma,1 was astrocytoma and 1 was cavernoma.Eighty - four of 115 cases were performed MR scan and definite diagnosis was made in 81 cases(96.4%).Seventy - five cases(89.3%) were diagnosed accurately.Six cases were misdiagnosed and 3 cases were missed(3.6%).Seventy - two cases performed CT scan,definite diagnosis was made in 28 cases(38.9%).Twenty - five cases(34.7%) were diagnosed accurately.Three cases were misdiagnosed and 44 cases were missed(61.1%).Conclusion:There are apparent advantages to observe the change of the os petrosum and acoustic meatus by CT and make the diagnosis of tumors by MR.Combing with CT and MR will improve the diagnosis of CPA tumors.

Abstract:Objective To analyze the MRI features of cerebellopontine angle(CPA)medulloblastoma and increase the rate of diag-nosis of this disease.Methods MRI features in five patients with pathologically proven CPA medulloblastoma were retrospectively reviewed.Results In all patients,masses were predominantly solid.Some cystic or necrotic components were demonstrated in four tumors,and the cystic components were on the edge of the tumors.The other one case was totally solid tumor.The solid component of five tumors were hypointense on T1-weighted imaging.Five tumors were mild and mixed hyperintense on T2-weighted imaging.In two tumors,there were focal areas of hyperintensity on DWI.The pons and fourth ventricle were compressed in all patients.Basilar meningeal enhancements were found in two patients.Two tumors were associated with peritumoral and parenchymal edema.Conclusion MRI can provide the informations of extent of invasion,secondary changes and relation with surrounding structures of medulloblastoma.

Abstract:Objective To discuss and investigate the MRI features of cerebellum cystica tumours in order to improve the accuracy of diagnosis.Methods Twenty-three cases of cerebellum cystica tumours with MRI proved by surgery and pathology were analyzed retrospectively.The main evaluation index are cystic extent, diseased region, size, periphery, surrounding edema, mass effect, noncommunicating hydrocephaly and so on.Results Among the 23 cases of cerebellum cystica tumours, there were 12 cases of medulloblastoma(53%), 8 cases of hemangioblastoma(35%), and 3 cases of pilocytic astrocytoma(13%);among these tumours, medulloblastoma with cystic was in eight cases, but the scope was small, hemangioblastoma cystic became large but with small nodules in 8 cases, and astroastrocytoma had larger cystic components and larger nodules than hemangioblastoma.Enhancement scanning showed cystic part of medulloblastoma was not enhanced, solid part showed moderate to severe enhancement, cystic part of hemangioblastoma and pilocytic astrocytoma was not strengthened, nodules were obviously improved, hemangioblastoma capsule wall had no enhancement, while pilocytic astrocytoma capsule wall was strengthened in two cases.Three kinds of tumor boundary was clear, with no or only one degree edema, the placeholder effect was lighter, and the larger tumor size easily caused obstructive hydrocephalus.Conclusion MRI images have important diagnostic value since they can clearly show the tumor location, cystic change degree and the placeholder effect, and so on.Medulloblastoma can be seen more in cerebellar vermis, capsule occurs more but in smaller scale.Hemangioblastoma and pilocytic astrocytoma can be seen more in cerebellar hemispheres, with more cystic change and bigger area;hemangioblastoma has typical MRI feature, with large cystic and small nodule, and rare reinforcement in capsule wall;pilocytic astrocytoma has a typical big cystic and big nodule and frequent reinforcement of capsule wall.

Abstract:Posterior fossa craniopharyngiomas (PFCP) constitute 1.6-4% of all craniopharyngiomas and have long been neglected. The purpose of our study was to investigate neuroimaging features of this unique modality in a cohort of 7 cases.CT and/or MR imaging features (with and without contrast enhancement) of 7 patients who underwent craniotomies for histologically proved PFCP were reviewed and analyzed retrospectively. Surgical management was also reviewed.All PFCPs arised from sellar/suprasellar region and extended into unilateral cerebellopontine angle in 6 cases, and bilateral in 1 case. Seven tumors were of retrochiasmatic origin and 5 of 7 were of retrostalk growth pattern with location in the ventral area of brain stem. On CT scans, tumors were isodense in solid component and hypodense in cystic component with (4/7) or without calcification (3 /7) and destruction in sellae turcia (2/7). Tumors demonstrated cyst formation (7/7) with hypointense on T1-weighted imaging and hyperintense on T2-weighted imaging in 5 cases, hyperintense both on T1- and T2-weighted imaging in 2 cases. The solid components and capsule revealed mild to moderate inhomogeneous enhancement after administration of contrast agents. Total tumor removal was accomplished in 5 cases, subtotal removal and partial removal in 1 case respectively.PFCPs are well demarcated, contrast-enhanced tumors, typically with cystic parts or purely cyst. Most of PFCPs demonstrate a retrostalk growth pattern and characteristic connection. Tumor with cystic component arises from sellar region and then extends to posterior fossa, which should be strongly suspected as a PFCP. The combined supra- and infratentorial approach is an ideal choice for surgical management of PFCP.

Abstract:Objective To analyze the MRI features of rare intracranial germ cell tumors of the pineal region compared with pathologic findings.Methods The MRI and pathologic data of 32 patients with rare intracranial germ cell tumors of the pineal region were reviewed retrospectively.All cases were proved by surgery and pathological examinations.Results Fourteen teratomas with malignant transformations were cyst-solidary which were mixed signals on plain MRI and the solid components showed obvious enhancement.The adiposity signals could be seen in teratomas.The level of serum or CSF AFP was high.Microscopically,many tissue components could be seen.The signal of 6 yolk sac tumors were homogeneous,and they had great enhancement after enhanced scanning.Increasing of preoperative serum AFP level could be seen in all patients.The 5 embryonal carcinomas were well-defined margin masses and heterogeneous with cystoid variation and necrosis and moderate to severe edema.The solid components showed obvious enhance.The signals of 7 mixed germ cell tumors were atypical,which depended on the components of germ cell tumors.Conclusion The MRI findings of the rare intracranial germ cell tumors of the pineal region(teratomas with malignant transformations,yolk sac tumors endodermal sinus tumor,embryonal carcinomas and mixed germ cell tumors)show some feature which are correlated with pathology.Suggestive diagnosis could be made combined with clinical data and MRI features.

Abstract:Objective:To investigate MRI characteristics and pathology of intracranial neuron and mixed neuronal-glial tumor.Methods:Clinical data,histopathology and MRI findings in 26 patients with intracranial neuron and mixed neuronal-glial tumor proved by pathology were analysed retrospectively.Results:Eleven dysembryoplasia neuroepithelial tumor(DNT),mainly seen in the supratentorial brain and beneath cerebral cortex,some were seen as cystic masses,appearing as low signal intensity on T1WI and high signal intensity on T2WI.DNT had a well-demarcated and no or only slight edema,no enhancement or slight inhomogeous enhancement;11 gangliogliomas mainly seen in the supratentorial brain appearing as low signal intensity on T1WI and high signal intensity on T2WI.Two showed mixed signal intensity,margin of lesion was well-demarcated and no edema or slight edema,no enhancement or slight uneven enhancement,3 central neurocytomas,which were located in the lateral cerebral ventricle,all showed mixed signal intensity on T1WI and T2WI,with cystic components.Enhancement was inhomogeneous;1 desmoplastic gangliogliomas seen as cystic masses,located in fronto-temporal-apical lobe,had septation,appearing as low signal intensity on T1WI and high signal intensity on T2WI.Conclusion:Intracranial neuronal and mixed neuronal-glial tumours had no characteristic imaging features.It should be closely referred with clinical manifestations.
Intraventricular pituicytoma: illustrative case

Abstract:Background: Pituicytoma is a rare glial neoplasm from pituicytes of the neurohypophysis or infundibulum. It occurs in the sella and suprasellar area, and it is extremely uncommon to observe intraventricular pituicytoma without affecting the infundibulum or infundibular recess. Observations: A 69-year-old man had suffered progressive dementia for 6 months. Magnetic resonance imaging revealed a solid, homogeneously enhancing mass with flow voids within the anterior third ventricle. The sella, suprasellar area, infundibulum, and infundibular recess were unaffected. The patient underwent a transcallosal transchoroidal approach, which ended in partial removal of the tumor due to significant tumoral bleeding. A second surgery resulted in its subtotal removal. The tumor had bipolar cells, and their nuclei were immunoreactive for thyroid transcription factor-1. A DNA methylation analysis corresponded to the methylation class of pituicytoma, granular cell tumor, and spindle cell oncocytoma. Pituicytoma was the diagnosis based on these results. A systematic review identified 5 intraventricular pituicytoma cases. Lessons: Intraventricular pituicytoma can grow without involvement of the infundibulum or infundibular recess. The current case suggests that pituicytes of the hypothalamic tuber cinereum can also give rise to pituicytoma. Because of the hypervascular nature of intraventricular pituicytomas, it is imperative to control intraoperative bleeding with attention to the adjacent hypothalamus. https://thejns.org/doi/10.3171/CASE24247.
The Imaging Findings of Infratentorial Primitive Neuroectodermal Tumour: A Case Report

Abstract:Central primitive neuroectodermal tumour (cPNET), a rare malignant neoplasm of embryonal origin, often occurs in children younger than 15 years. This is the first case report of the imaging findings of an infratentorial cPNET to be reported in a patient. Here, is reported the case of a 6-year-old boy presenting with symptoms of diplopia for 14 days. Magnetic resonance imaging revealed a solid mass in the fourth ventricle. The postoperative pathological diagnosis was cPNET. To conclude, whenever a child is diagnosed to have an infratentorial solid tumour in the fourth ventricle, cPNET should always be considered despite its rarity.
Case report: Rare case of multinodular and vacuolar neuronal tumors in the cerebellum

Abstract:Multinodular and vacuolar neuronal tumor (MVNT) is a rare and benign neuroepithelial tumor. Most reports describe tumors located in the cerebral hemisphere. A literature review found that 15 cases were located in the posterior cranial fossa, but all lacked pathological evidence. In this case, a patient sought medical attention due to insomnia and irritability. Neuroepithelial tumors were found in the imaging, and the patient underwent radiation therapy. Three years later, malignant tumors were found upon imaging examination. After surgical resection and pathological testing, MVNT occurring in the cerebellum was diagnosed. MVNT is rare in the cerebellum, and direct imaging diagnosis becomes difficult after treatment. Therefore, our report of this case helps to further accurate understanding of the imaging, pathological, and molecular genetic changes occurring before and after MVNT treatment, and will improve the accuracy of pre-treatment diagnosis and reduce the likelihood of overtreatment.

Tentorial Meningioma: Comparative Study of Pathological Features with Imaging Findings

Abstract:Objective:To study the pathological features of the tentorial meningioma and the imaging features in CT and MRI.Methods:Thirty-two patients with pathologically proved tentorial meningioma were examined by CT or MRI.Four men and twenty-eight women,range from 34～75 years old with average age 55.3.All patients were examined by MRI (twenty-two with enhanced scan),eighteen patients by CT (thirteen with enhanced scan).Compare the image features with pathological findings.Results:Pathologically,most were fibrous type,but epitheliated type,angiomatous type,or papillary type may also be seen.Fifteen were supratentorial,nine were supra/infratentorial,and eight were infratentorial.Tentorial meningioma had typical imaging features as common meningioma.The signal intensity related with the pathological type on T_2WI.Commonly marked and homogeneous enhancement could be seen,and dural tail sign presented in most cases.Some had ring-like or inhomogeneous enhancement.A few had calcification,necrosis or hemorrhage.Conclusion:Tentorial meningioma has typical finding in imaging.CT and MRI are valuable examinations in diagnosis,and MRI is much better than CT.

Abstract:Objective:To summarize the imaging features of subtentorial solitary fibrous tumor,in order to improve its diagnostic accuracy.Methods:Imaging manifestations and clinicopathological data of 5 subtentorial solitary fibrous tumor confirmed by operation and pathology were analyzed retrospectively.There were CT plain scan in one case,MRI plain and enhancement scan in three cases,only MRI enhancement scan in one case.Results:All of the 5 cases(4 fe-males,1 male)were dural based single lesion,the lesions were located in right hemisphere(n=2),left cerebellum convex(n=1),across supratentorial and infratentorial(n=1)and left cerebellar pontine angle(n=1).The maxi-mum diameter of tumor is approximately 4.1~5.2 cm,the boundary of 5 cases was clear,4 cases were multilobular and 1 case was round.The tumor showed slightly high density on plain CT scan,isointense or slightly hypointense sig-nal on T1WI,slightly hyperintense signal or hypointense signal on T2WI.There were hypointense signals area in 3 ca-ses on T2WI,cystic portion in 2 cases,flow void vascular signals in 2 cases,mild edema around tumor in 4 cases and cerebrospinal fluid surrounded tumor in 3 cases.The solid and the wall of cystic of tumor were enhanced obviously, dural tail was found in 2 cases.Conclusion:Imaging manifestation of subtentorial solitary fibrous tumor has its own features.If the subtentorial mass showed multilobular and signs of extracerebral tumors,slightly high density on plain CT scan,isointense or slightly hypointense signal on T1WI and slightly hyperintense signal or hypointense signal on T2WI,obvious enhancement,flow void vascular and low signal area,diagnosis of subtentorial solitary fibrous tumor could be considered.But its confirmation still depends on histopathology and immunohistochemistry.
CT and MRI Diagnosis of Intracranial Cavernous Angioma

Abstract:Objective To improve the understanding of intracranial cavernous angioma(CA) by analyzing its CT and MR images.Methods Forty-three patients with intracranial CA,including 37 intracerebral CAs and 6 extracerebral CAs,were analyzed retrospectively.Results The intracerebral CA presented as slightly-high density with a granulo mixed center(22/25) or high density indicating hemorrhage(3/25) on plain CT.It could be manifested as type Ⅰ(11.9%),Ⅱ(83.4%) or Ⅲ(4.7%) on the basis of Zabramskin classification.Most of the intracerebral CAs appeared as hyperintense or isointense signal on T1WI and a mixed-hyperintense core surrounded by a hypointense halo on T2WI.Both on contrast-enhanced CT and Gd-DTPA enhanced MRI,the intracerebral CA demonstrated mild,slight enhancement or no enhancement.All six extracerebral CA located in parasellar region,which presented as isointensity(2/5) or slightly-hyperintensity(3/5) on plain CT and isointense signal or slightly hypointense signal on T1WI,but markedly hyperintense on T2WI.All extracerebral CA lesions demonstrated intensive enhancement on enhanced CT or GdDTPA enhanced MRI,which appeared as a knead dough.Some of extracerebral CAs demonstrated typical peripheral enhancement on early phase and followed by centripetal fill-in of the contrast media with persistently strong enhancement on delayed-scan.Conclusion Most of the intracranial CAs have their characteristic features on CT or MR images,no matter of its intracerebral or extracerebral location.It should be emphasized that some intracerebral CAs could present as hemorrhage.
Radiologic features of the chordoid glioma: a report of three cases

Abstract:Objective To study the radiographic features of the chordoid glioma and its differential diagnosis,and increase neuroradiologists'awareness of this newly described tumor,facilitating prospective diagnosis.Methods Three patients with chordoid glioma were reviewed retrospectively to determine whether any characteristic radiographic findings and clinical-pathologic findings would emerge,and the relevant literatures were reviewed.Routine CT(2 cases)and MR(3 cases),contrast-enhanced MR(3 cases)were performed.Results The masses were well circumscribed and located in the region of the hypothalamus/third ventricle-suprasellar region(2 cases)and intrasuprasellar region(1 case).Tumors were isointense to gray matter on CT scans and were isointense or hypointense on T_1WI and hyperintense on T_2WI.In two patients,vasogenic edema extended into the optic tracts and cystic or necrotic area was seen.All cases were remarkably enhanced following contrast administration.Conclusions Radiologic findings of the chordoid glioma has some unique features.Tumor,in the region of the hypothalamus-third ventricle-suprasellar region,if it is well circumscribed and remarkably enhancing following contrast administration,should be included in the differential diagnosis.
Metastasis of a histologically benign choroid plexus papilloma: case report and review of the literature

Abstract:Cerebrospinal metastases of benign choroid plexus papillomas (CPPs) are extremely rare. We report a case of 32-year-old woman who presented with a 6-month history of vertigo and intermittent headache. Plain CT scan of the cranium revealed a partly calcified tumor filling the fourth ventricle and its right recess. Cranial MRI showed an inhomogeneously contrast-enhancing tumor and leptomeningeal enhancement encasing the brain stem. Complete resection of the tumor was carried out, and seedings to the floor of the fourth ventricle and cervico-medullary junction were found during the operation. While intraoperative frozen section suggested pathology of papillary ependymoma or CPP, to our surprise, final histological examination revealed a benign choroid plexus papilloma. One month after the first operation, a metastatic nodule was found in the spinal subarachnoidal space at the level of T8-9. Two months after the first operation, on follow-up MRI of the cranium, the leptomeningeal enhancement encasing the brain stem had resolved spontaneously. This special case helps increase our understanding of benign CPPs and expands our differential diagnostic consideration of lesions with similar manifestations.

Abstract:Objective To study the CT and MRI features of intracranial Hemangiopericytomas (HPC).Methods 5 cases of intracranial were proved by pathology;All the HPC located outside the brain.All patients were underwent MRI,CE MRI;of 3 cases were underwent CT,CE CT.Results 5 cases of HPC located the brain.On plan CT scan appeared and-iso,iso-and-hyper insensity,calcification was not observed.3 cases with mainly adjacement bony destruction.on T1W images and,iso-and hyperintensity.T2W images high-and insensity . all cases were hetergenous enhanced.3 cases with perifocal edema.Conclusion Intracranial hemangiopericytoma have certain characteristic CT and MRI features,it is helpful to get a correct diagnostic with features.
