Abstract:Retinopathy of prematurity (ROP) is a relatively late manifestation in preterm and very low birth weight (VLBW) survivors; thus, increasing survival rates among extremely preterm infants will necessarily result lead to increased incidence and more severe ROP. 1,2 ROP incidence is strongly and inversely linked to gestational age suggesting that a more immature landscape and/or prolonged exposure to noxious agents and environmental stimuli such as excess oxygen and inflammation underlie this relationship. 3 However, some extremely preterm infants escape the neonatal intensive care unit without ROP despite abundant exposure to oxygen and repeat episodes of sepsis/inflammation while other preterm infants with rather uneventful courses reach threshold ROP. With these observations, we seek other explanatory variables for the exceptions to the rules. Gene loci that infer susceptibility to or protection from ROP have long been sought with much of this focus on candidate genes linked to capillary formation. Somewhat surprisingly, no variants of vascular endothelial growth factor (VEGF), the putative driver of angiogenesis, or its receptor have been identified in genome wide association studies with ROP although candidate gene approaches have identified SNP variants in VEGF signaling associated with ROP. 4 Several candidate genes have been proposed as "risk factors" for ROP but these associations often fail to be confirmed in separate populations. In this issue of Pediatric Research , Gimenez and colleagues selected nine candidate genes linked to ROP in a preterm population in the United States for validation studies in their own preterm population from Argentina. 5 In both populations, the rs1868085 polymorphism in the EPAS1 gene associated with ROP. Not surprisingly, EPAS1 encodes the hypoxia inducible factor-2α (HIF-2α) transcription factor, which is closely linked to capillary formation through its regulation of VEGF and other oxygen-sensitive angiogenic genes. The convergence of these linkage studies on EPAS1 is encouraging that a "universal" genetic susceptibility locus for ROP may be identified. The authors identify other SNPs in their sample that associate with ROP including the angiotensin II receptor gene but fail to recognize some SNPs that appear to be similarly linked including the rs699947 polymorphism in VEGF (OR 0.42, CI: 0.15–1.18). Although this variant does not reach "statistical significance", VEGF is directly linked to HIFs and angiotensin II signaling and should be considered as a candidate SNP in future studies.
