Value of Spiral CT Angiography in Operation of Carcinoma of Pancreatic Head

Abstract:Objective To determine the role of spiral CT angiography (SCTA) for predicting vascular involvement from carcinoma of the pancreatic head and its value in choosing intraoperative ex-amination mode and operative means of the pancreatic head carcinoma. Methods SCTA was per-formed in 92 patients who had adenocarcinoma of the pancreatic head. Vessel integrity of the superior mesenteric vein/portal vein, superior mesenteric artery, and celiac artery/common hepatic artery was assessed. Then different intraoperative examination modes and operative means were chosen. Results Forty-five patients with tumor invasion of superior mesenteric vein/portal vein graded less than 2 un-derwent classic pancreaticoduodenectomy. Among them, 12 invaded SMV/PV graded 1 or 2 were con-firmed chronic inflammation and adherent by intraoperative examination. Thirteen patients with invasion of SMV/PV was graded 3 or 4 and shorter than 2 cm underwent pancreaticoduodenectomy in combination with segmental portal vein resection (PVR) and direct portal vein reconstruction. Five of 34 patients with invasion of SMV/PV was graded 4 and longer than 2 cm underwent tumor resection (1 total pancreatectomy and 4 pancreaticoduodenectomy) in combination with PVR and portal vein reconstruction with Gore-Tex artificial blood vessel. Being confirmed to be unresectable by intraoperative examination, another 29 patients received palliative choledochojejunal anastomosis and 6 of them also underwent gastrojejunostomy anastomosis for duodenal obstruction. Conclusion SCTA provides use-ful information about peripancreatic vascular involvement from adenocarcinoma of the pancreatic head. Based on this, surgeons can choose different intraoperative examination mode and operative means.
