Effects of maternal preeclampsia and eclampsia on neonatal brain damage and neurodevelopments

Abstract:Objective To study the effects of maternal preeclampsia and eclampsia on neonatal brain damage and neurodevelopment. Methods The clinical observation and cerebral image examination were used to detect brain injury of 265 infants born to the mothers complicated with preeclampsia or eclampsia. The infants were followed up and their neurodevelopments were evaluated. Results The incidence of neonatal brain damage were 63.0%. White matter damage and hypoxic-ischemia encephalopathy were the main types of brain damages. The more severe the maternal preeclampsia or eclampsia, the higher the incidence of severe neonatal brain damage. And the incidence of brain damage of the infants born to the mothers with early onset severe preeclampsia （38. 5%） were significantly higher than those born to the mothers with later onset preeclampsia （19.4%） （P〈0.05）. Of the 214 infants followed up more than 3 months, the incidence of neurological abnormalities was 56.1 %, and the incidence of severe, mild and transient neurological abnormalities was 3.3%, 17.3% and 35.5%, respectively. The more severe the maternal preeclampsia and eclampsia, the higher the rate of infants with abnormal neurological developments. And the incidence of abnormal neurological developments with severe brain damage （93. 5%） was significantly higher than those with mild brain damage （66.7%）, （χ^2 =82.5, P〈0.01）. The severe preeclampsia （OR= 4. 37,95% CI 1.67-8.35） and the severe neonatal brain damage （OR= 9.66,95% CI 3.73-21.16）were the risk factors of infant abnormal neurological development. Conclusions The more severe the maternal preeclampsia and ec- lampsia, the higher the incidence of severe neonatal brain damage and abnormal neurological develop- ment. The risk factors of abnormal neurological development were severe preeclampsia and severe neonatal brain damage. It is necessary to take intensive observation on the infants born to the mothers with preeclampsia or eclampsia to make early diagnosis and treatment to improve the prognosis of the infants.\r\n\r\nKey words: \r\nPre-eclampsia;  Eclampsia;  Infant, newborn;  Brain damage, chronic;  Mental retardation
