Abstract:Introduction Pharmacogenetics (PGx) is increasingly being utilized to manage complex medication regimens. Older Americans have the highest utilization of prescription medications, which increases adverse event risk and drug interactions.(1) While PGx-aided prescribing aims to reduce these outcomes, it is only one tool, among several, that can reduce the use of potentially inappropriate medications (PIM).(2) The American Geriatric Society Beers Criteria (AGS Beers) is a widely used guideline that identifies PIM for individuals 65 years (y) of age or older.(3) Combining PGx with AGS Beers guidance can provide a powerful platform to enhance prescribing in geriatric populations. J Am Geriatr Soc. 2023; 71(7): 2052-2081. doi:10.1111/jgs.18372 Methods 8,879 patients aged 65y or older who received PGx testing from 4/25/2017 through 8/30/2023 were identified in the Genomind database. Current or considered medications were provided through requisition forms and compared to the current AGS Beers list. Medications were placed into AGS Beers categories as follows: Avoid Medication; Avoid Medication under Certain Conditions; Avoid Medication due to Drug-Drug Interaction Risk; Caution with Medication or Caution with Medication under Certain Conditions. Conditions include renal insufficiency and select diagnoses as described in the 2023 AGS Beers update. The "Caution" categories, as defined by Beers, are intended to "raise concern without rising to the level of avoid." The four classes of drug-drug interactions evaluated were: 1) Three or more central nervous system (CNS) active medications; 2) Two or more anticholinergic medications; 3) One or more opioids with one or more benzodiazepines; 4) Two or more renin-angiotensin system (RAS) inhibitors. The prescribers who ordered the PGx testing were stratified by specialty which included Psychiatry-General; Family Medicine; Internal Medicine; Integrative/Functional Medicine; Psychiatry-Geriatric; Psychiatry-Addiction or Other. Results 8,879 patients aged 65y through 99y were evaluated with a mean age of 75.4y and a sex distribution of 69.2% female. 53,901 total medications were currently being prescribed or being considered as a next treatment option. The total number of unique medications was 478, with the most common classes of medications being for the treatment of psychiatric conditions (depression, anxiety, bipolar). 50.8% of patients were currently taking or considering a medication with a Beers "Avoid" recommendation and 91.3% had an "Avoid under Certain Conditions" flag (renal insufficiency, heart failure, history of falls/fractures, etc.) [Fig.1] The most common "Avoid" medications were tricyclic antidepressants. All the top 20 most prescribed or considered medications, all of which were psychotropic medications in this population, had some type of AGS Beers guidance, except for bupropion and vortioxetine [Fig.2]. The percent of the population with a potential Beers "Caution" flag was 82.1% with 6.9% in the "Caution under Certain Conditions" category. [Fig.1]The most common potential drug-drug interaction was 3 or more CNS active medications (43.2%), followed by 2 or more anticholinergics (21.8%), an opioid plus a benzodiazepine (1.9%) and 2 or more RAS inhibitors (0.1%). [Fig.3] Analysis of provider specialties indicated that most prescribers were psychiatrists or psychiatric nurse practitioners (71%), followed by family medicine (9%), Internal Medicine (6%) and geriatric psychiatry (2%). The remainder included addiction, integrative/functional medicine and other. [Fig.4] Conclusions Analyzing a population of older Americans' medication usage and their prescribers next potential medication via the Beers Criteria provides an opportunity to better identify drug-age interactions.
