Modified Stove Procedure Combined with Sarasola-Klose Hemorrhoidectomy for the Treatment of Vascular Malformation Involving Anorectum and Sigmoid Colon

Abstract:Objective To introduce modified Soave procedure combined with Sarasola-Klose hemorrhoidectomy as the treatment of vascular malformation involving the anorectum and sigmoid colon (VMARS) in children.Methods Retrospective analysis was carried out on 12 pediatric patients who were admitted between 2000 and 2008 with VMARS Diagnostic methods and surgical procedures were analyzed.Results Upon review of medical history and results of electronic colonscopy,barium enema,and abdominal CT and MRI,12 pediatric patients had been diagnosed with VMARS.All 12 patients underwent a modified Soave procedure,in which perianal submucosal and mucosal blood vessels were removed applying the Sarasola-Klose hemorrhoidectomy technique(modified Whitehead procedure).At postoperative follow-up,(three months to eight years),no rectal bleeding or constipation was reported,but intermittent fecal soiling was noted in one case.Conclusions VMARS forills a unique subset of patients with vascular anomalies who suffer from rectal bleeding and hemorrhoid prolapse.This condition can be diagnosed by colonoscopy,CT,and MRI.The modified Soave procedure combined with Sarasola-Klose hemorrhoidectomy is a safe and effective treatment.
