Progress in Extramedullary Disease in Multiple Myeloma

Abstract:Multiple myeloma (MM) is a malignant plasma cell disease characterized by the production of monoclonal immunoglobulin and its fragment (M protein). Normally, myeloma cells are confined in bone marrow, but extramedullary myeloma (EM) could emerge either in newly diagnosed patients or in the course of disease progression. The prognosis of MM patients with EM is extremely poor. According to different ways of pathogenesis, EM is classified as bone-related extramedullary myeloma (EM-b) and soft tissue-related extramedullary myeloma (EM-s). EM-b refers to tumor mass that originates from the underlying bone marrow through the disruption of the cortical bone and extends into adjacent soft tissue, while EM-s occurs at extramedullary soft tissue or other organs by hematogenous spread. Previous researches have shown that EM-s has a poorer prognosis than EM-b at any time of the clinical course. This review will focus the differences between EM-b and EM-s, including incidences, biological characteristics, clinical manifestations, treatments and prognosis.
