Blood Loss and Transfusion in Hemiarthroplasty for Femoral Neck Fracture

Abstract:Objective To determine the blood loss and transfusion during treatment of femoral neck fracture with hemiarthroplasty so as to improve the attention to occult bleeding.Methods Clinical data of 147 patients with femoral neck fracture treated with hemiarthroplasty between January 2006 and May 2013 were analyzed retrospectively.Routine preoperative and postoperative blood tests,intraoperative and postoperative blood loss and transfusion,and the incidence of hypertension were analyzed.Results (1) Mean total blood loss was 970.4 ml in postoperative 3 days,with mean intraoperative and postoperative blood loss of 134.4 ml and 236.1 ml respectively,and mean occult bleeding of 599.9 ml (61.8％).(2) No hypertension was observed in 62 patients,with meantotal blood loss of 887.0 ml and mean hidden blood loss of 521.9 ml (58.8％).While hypertension occurred in 85 patients,with mean total blood loss of 1 032.2 ml and mean hidden blood loss of 657.9 ml (63.7％) (P ＜ 0.05).(3) Blood transfusion was done in 79 patients,with mean blood transfusion of 318.2 ml.Proportion transfused among the patients with a preoperative hemoglobin (Hb) level of ＜ 100 g/L,100-110 g/L,110-120 g/L,120-130 g/L,130-140 g/L,and ＞ 140 g/L was 100％,96.9％,53.5％,43.5％,18.2％,and 11.1％ respectively.
