Abstract:Objective To evaluate the diagnostic value of new contrast agent and real time gray-scale contrast ultrasonography in focal liver lesions.Methods Thirty nine cases with focal liver lesions were enrolled in the study.Contrast agent SonoVue,Technos DU8 and Logiq9 and their contrast technology were used.The contrast agent was delivered through bolus injection.Results The diagnostic sensitivity,specificity,positive and negative rate and accuracy were 95.83%(23/24),86.67%(13/15),92.00%(23/25),92.86%(13/14) and 92.30%(36/39),respectively.Conclusion Contrast-enhanced gray-scale ultrasonography could play an important role in differential diagnosis of focal liver lesions.

Abstract:目的探讨新型超声造影剂与实时灰阶造影匹配成像技术观察肝肿瘤的灌注过程及回声变化规律,探讨其对肝恶性肿瘤的诊断价值.方法 35例超声不能完全明确诊断或漏诊的肝占位患者,26例经手术或穿刺病理确诊,9例为造影CT、磁共振等临床资料证实,恶性肿瘤占28例.采用第二代新型造影剂SonoVue以及Technos DU6实时超声造影匹配成像技术.造影剂注射方法分别采用静脉快速团注法和慢注法两种.首先观察了正常肝、肝硬化注射造影剂后各个时相出现的时间及峰值,在此基础上观察了肝占位病变的造影剂灌注过程.结果显示典型的原发性肝癌23例均发生动脉早期强化,21例(91%)呈快速消退即\"快进快出\"型,另2例＜2 cm的高分化小肝癌表现为动脉早期强化,但消退缓慢.肝转移性肿瘤5例表现多样,呈动脉期或门脉期环状强化或不同程度强化,消退可快可慢.肝血管瘤3例动脉期瘤内无强化,门静脉期呈向心性填充增强,持续时间长,数分钟后消退.对原发性肝癌23例进行了两种不同注射方法的比较,发现团注法使肝癌病灶更快达到增强峰值,更有利于肝癌特征的显示.本组中31例进行了超声造影前、造影后、增强CT的诊断结果比较,肿瘤病灶的显示分别为76灶、99灶、75灶.11例恶性肿瘤病灶数目较造影前增多,其中87%(20/23灶)为3～10 mm小病灶.另5例显示原病灶范围较前增大.结论新型超声造影技术对肝占位病变的定性诊断明显优于常规超声;在显示肿瘤数目尤其发现微小病灶方面更优于增强CT,从而极大地提高了超声对肝占位病变的诊断价值。

Abstract:目的探讨超声造影鉴别诊断胆囊良、恶性病变的临床应用价值.方法对56例常规超声诊断为或疑为胆囊癌的病例,采用超声造影(声诺维)和对比脉冲序列成像技术行超声造影检查,分析良、恶性病变的增强特点.结果经手术病理证实胆囊癌26例,良性病变30例.超声造影显示病变早于肝实质增强者胆囊癌为80.8%(21/26),良性病变为36.7%(11/30)(P＜0.01).造影开始呈高增强者胆囊癌为80.8%(21/26),良性病变为46.7%(14/30)(P＜0.01);表现为增强不均匀者胆囊癌为57.7%(15/26),良性病变为16.7%(5/30)(P＜0.01).在注射造影剂60s内病变由高或等增强减退为低增强者胆囊癌为95.8%(23/24),良性病变为21.4%(6/28)(P＜0.01).结论胆囊良、恶性病变具有特征的增强表现.超声造影有助于病变性质的鉴别诊断。

Abstract:目的探讨实时超声造影在肝肿瘤诊断中的价值.方法对29例患者共34个肝内实质占位病变进行实时谐波超声造影检查,其中16个原发性肝癌,4个转移性肝癌,9个肝血管瘤,5个局灶性结节性增生.结果经超声造影后,34个病灶均有不同程度的增强并有不同的增强特点.肝癌表现多为整体快速增强,呈不均状,并在门脉期快速消退;肝血管瘤多呈周围向心性增强,呈环状或周围结节状,并持续较长时间;肝局灶性结节性增生表现为中央扩散型快速均匀增强,并充盈整个门脉期.如以肝肿瘤在动脉期增强而在门脉期减退呈低回声作为诊断肝癌的指标,则超声造影诊断肝癌的敏感性、特异性及准确性分别为95%(19/20)、100%(14/14)和97%(33/34).结论实时超声造影能反映不同肝肿瘤在不同时相上的不同表现,可帮助提高肝肿瘤超声诊断的准确性。

Abstract:目的探讨各种不同肾肿瘤的超声造影表现. 方法 124例肾肿瘤(平均直径3.4 cm),应用超声造影观察肿块回声增强时间和造影剂消退时间(皮质相与延迟相),观察肿块的假包膜(与二维声像图对比),所有患者均行手术和病理证实. 结果透明细胞癌和乳头状癌98例,移行细胞癌5例,肾盂鳞癌6例,血管平滑肌脂肪瘤7例,肾盂上皮细胞癌1例,肾母细胞瘤5例,嗜酸细胞瘤2例(潜在恶性).透明细胞癌98中超声造影有58例出现假包膜(58/98,73.3%),而二维声像图显示假包膜31例(31/98,17%);造影剂呈快进快退76例,快进慢退11例,慢进慢退7例,肿块局部回声增强4例.而血管平滑肌脂肪瘤回声增强程度明显低于肾细胞癌. 结论超声造影有助于鉴别肾肿瘤,有助于显示出肾细胞癌的假包膜。

Abstract:With the development of contrast agents and new contrast-specific ultrasound techniques, the ultrasound imaging of the liver has been dramatically improved. Contrast agents are based on gas microbubbles stabilized with different substances. When injected intravenously, microbubbles pass through the pulmonary bed and not only enhance the Doppler signal and improve the gray-scale imaging using specific imaging sequences, but also the microvessels and the tissue perfusion can be evaluated at real time. The new techniques provide crucial information for the diagnosis and treatment of liver lesions, which can not be achieved by the regular ultrasonography. The article also serves as an overview of the applications of the contract-enhanced ultrasound in diagnosis and treatment of liver lesions.

Abstract:目的探讨转移性肝癌的超声造影特征及其诊断价值.方法对怀疑肝内实质占位病变并经手术、病理及或临床资料证实的74例转移性肝癌进行实时谐波超声造影,造影剂选用sonoVue.在实时超声造影状态下观察肝内病灶的增强情况,分别观察增强的方式、开始增强时间、达峰时间、峰值表现、低回声出现的时间等.结果 74例转移性肝癌造影后均表现为快速增强,其增强方式主要为环状增强和整体增强两种,分别为52.7%(39/74)和45.9%(34/74).峰值时的表现主要为环状高回声、高回声及低回声团块等,所有病灶均在门脉期呈低回声改变,其中86.5%(64/74)的病灶在动脉晚期或门脉早期即呈低回声.超声造影诊断符合率93.2%(69/74).结论超声造影对提高转移性肝癌超声诊断的准确性及检出率有很大价值。

Abstract:目的探讨实时超声造影对肝癌血供情况判断的准确性.方法对15例经手术病理证实的原发性肝细胞肝癌患者进行实时超声造影检查,造影剂为SonoVue,经肘部静脉弹丸注射.结果注射SonoVue后15个病灶均快速增强,增强达峰值时,有8个病灶内部出现大小不一或不规则形的造影剂非填充区(低回声),与病理对照显示该低回声区为肿瘤坏死区域,并且大小及位置与标本显示相一致.结论实时超声造影能反映肝癌的血供情况,对判断肿瘤有无坏死有很大帮助。

Abstract:Objective To assess the value of contrast-enhanced ultrasound(US)in differential diagnosis of hyperechoic hepatic lesions when compared to conventional US.Methods Eighty-four hyperechoic liver lesions in 66 patients were examined using both conventional and contrast-enhanced US techniques.Results Receiver operating characteristic analysis revealed a significant improvement in the discrimination of malignant versus benign hyperechoic hepatic lesions (area under the receiver operating characteristic curve,0.867±0.045 at conventional US,0.997±0.003 with contrast-enhanced US(P＜0.0001).For correct diagnoses types of hyperechoic liver lesions,contrast-enhanced US improved from 64.3% to 92.9%,comic)ared with conventional US (P＜0.001).Conclusions Contrast-enhanced US showed a significant improvement in characterization and differentiation of hyperechoic hepatic lesions when compared to conventional US.
Evaluation of treatment response in hepatocellular carcinoma with contrast-enhanced harmonic sonography

Abstract:目的评价超声造影谐频成像在原发性肝癌治疗后疗效判断方面的作用.方法使用Levovist对27例33个肝癌结节治疗前后分别行超声造影能量多普勒谐频成像和灰阶谐频成像数字减影检查,结果与动态增强CT相比较.结果以动态增强CT结果为金标准,治疗前33个肝癌结节,超声造影增强的阳性率为93.9%(31/33);而对治疗后肝癌,能量多普勒谐频成像显示肿瘤内血流信号的敏感性、特异性和准确性均为100%,灰阶谐频成像数字减影则分别为93.9%、100%和96.8%.结论超声造影谐频成像可以敏感而直观地显示治疗后肿瘤内残存的血流信号,即时评估肿瘤的治疗效果,创伤小,值得临床推广.

Abstract:目的探讨超声造影对肾细胞癌诊断的临床价值.方法对65例肾细胞癌患者分别进行实时超声造影和增强CT检查,比较两种成像方法在肾细胞癌中的灌注特征及敏感性、特异性和准确性.所有病例均经手术和病理结果证实,其中肾透明细胞型肾细胞癌54例,肾嫌色细胞型肾细胞癌4例,肾乳头状肾细胞癌7例.结果肾细胞癌在超声造影和增强CT中的灌注特点表现为\"快进快退\"者分别为76.9%(50/65)和83.1%(54/65).超声造影鉴别诊断肾细胞癌的敏感性、特异性和准确性分别为100%(60/60)、60.0%(3/5)、92.3%(60/65);增强CT鉴别诊断肾细胞癌的敏感性、特异性和准确性分别为96.7%(58/60)、40.0%(2/5)、78.5%(51/65).超声造影对肾细胞癌诊断的准确性高于增强CT(P＜0.05);而两者对肾细胞癌诊断的敏感性和特异性差异均无统计学意义(均P＞0.05).结论超声造影可作为诊断肾细胞癌的一种重要方法。

Abstract:目的评价超声造影在检测肝移植术后肝动脉并发症中的应用价值.方法选择肝移植术后患者经彩色多普勒超声检查疑肝动脉血流异常者53例,用SonoVue及实时超声造影匹配成像技术行超声造影检查,观察肝动脉及肝实质的血流灌注.结果 25例经DSA或CT血管造影(CTA)/MR血管造影(MRA)证实,其中肝动脉狭窄22例,血栓闭塞3例.与DSA或CTA/MRA比较,超声造影诊断血栓闭塞符合率为100%(3/3),判断肝动脉狭窄部位诊断符合率为 90.9%(20/22),显示血管狭窄程度的符合率为 86.4%(19/22).结论超声造影可明确诊断血栓闭塞,可较准确地判断肝动脉狭窄部位及程度,有利于提高肝移植术后肝动脉并发症的诊断准确性。

Abstract:目的探讨超声造影后即刻基波超声对改善肝病灶特征显示的作用. 方法应用SonoVue行谐波超声造影检查(CEUS).前瞻性比较105例129个肝局灶性病变的造影前基波(Pre-CEUS)和造影结束后3 min内即刻基波(Post-CEUS)的图像特征.病灶包括62个肝细胞癌(hepatocellular carcinoma,HCC),29个肝转移癌(metestatic liver carcinoma,MLC),32个血管瘤,4个局灶性结节增生(focal nodular hyperplasia,FNH),2个肝包虫病灶.声像图评价包括肿瘤大小、形态、边界、内部回声、结构以及后方回声等特征变化.最后比较应用Post-CEUS前后诊断率的改变. 结果与Pre-CEUS比较,Post-CEUS产生的\"爆破成像\"现象明显改善了图像特征的显示.对62个HCC病灶,Post-CEUS改善肿瘤边界显示,边界清晰者增加(59灶对37灶,P＜0.001),\"晕征\"增加(36灶对23灶,P=0.031);改善内部结构显示,瘤内结构不均者增多(39灶对26灶),其中16灶(25.8%)内部新显示镶嵌征或结中结征;Post-CEUS还使后方回声增强者增多(30灶对19灶,P=0.034).对29个MLC病灶,Post-CEUS改善肿瘤边界显示,边界清晰者增加(28灶对19灶,P=0.003).对32个肝血管瘤病灶,Post-CEUS改善边界及内部结构显示,边界清晰者增加(31灶对24灶,P=0.026),其中13灶(40.6%)新出现强回声边,4灶新出现内部筛孔征.2个肝包虫病灶Post-CEUS新发现多发微小囊及小钙化点.4个FNH中2个显示瘤内瘢痕样结构而Pre-CEUS仅显示1个.Pre-CEUS的诊断正确率为52.7%(68/129灶),CEUS为86.0%(111/129灶),CEUS结合Post-CEUS诊断率达93.0%(120/129灶). 结论高机械指数基波超声使残留造影剂产生的\"爆破成像\"可明显改善肝局灶病变内部结构及边界特征的显示,有助于提高CEUS的诊断率。

Abstract:目的探讨肝局灶性炎性病变超声造影的增强表现.方法采用造影剂SonoVue和对比脉冲序列成像技术对肝脓肿、炎性假瘤、胆管周围炎症、肝结核和真菌感染共58个肝局灶性炎性病灶施行了超声造影检查.观测病灶的增强水平、形态以及随时相变化的特点.结果32个肝脓肿病灶在动脉期呈现高增强、等增强和低增强分别为26个、5个和1个;30个病灶为周边环状增强伴中央无增强,其中22个见内部分隔状增强;20例显示动脉早期病灶所在区域肝组织楔形高增强;26个高增强病灶至门脉期14个、至延迟期共21个消退至低增强;5个等增强病灶至门脉期3个、至延迟期共4个消退至低增强.5例胆管周围炎症者动脉期表现为不均匀高增强(3例)或等增强(2例),门脉期及延迟期所有病灶减退至低增强.19个炎性假瘤病灶动脉期高增强、等增强和低增强分别为11个、3个和5个,其中12个具有周边环状增强的特点,门脉期及延迟期所有病灶均表现为低增强.1个肝结核病灶和1个真菌感染炎性病灶动脉期均呈不均匀高增强,门脉期及延迟期亦减退至低增强.结论肝脓肿病灶大多数在门脉期为低增强,与欧洲指南所描述的典型表现有所不同.胆管周围炎和局灶性非化脓性炎症病变则表现出在动脉期高增强或等增强后,门脉期及延迟期消退为低增强,与恶性肿瘤的增强模式相似。

Abstract:目的比较动态灰阶超声造影和增强CT显示肝肿瘤血流信号的特点.方法对49例病理证实的肝内实质性占位病变(原发性肝癌28例,肝血管瘤9例和肝局灶性结节增生12例)进行超声造影和增强CT检查,比较两种影像技术在注射造影剂后不同时相的增强表现.结果超声造影显示原发性肝癌中 85.7% (24/28)的肿块增强早于肝实质,门脉相和延迟相则多呈低回声;与增强CT显示 84.0% (21/25)的肿瘤动脉期高密度、门脉期多呈低密度相一致.血管瘤中 67.7% (6/9)在超声造影的动脉相无明显增强,尔后逐渐向内部填充;与CT显示的造影剂逐渐填充呈结节状强化极为相似.肝局灶性结节增生在超声造影和增强CT上均显示早期快速增强,而消退较慢.3种肿瘤在超声造影和增强CT不同时相的增强情况比较差异无显著性意义(χ2检验,P\u003e0.05).结论动态灰阶超声造影和增强CT都能敏感地显示不同肝肿瘤的血供特征,为肝肿瘤的鉴别诊断提供依据.

Abstract:Objective:To investigate the value of real-time contrast enhanced ultrasound in discrimination benign from malignant tumors of liver occupying lesions.Methods: Totally 207 patients with liver occupying lesions were examined by contrast enhance ultrasound,all cases were confirmed by histopathology or other image methods.Results: After injection of SonoVue 2.4 ml,of the 92 cases of hepatic cell carcinoma(HCC),71 presented typically with early artery enhancement and immediate wash-out;8 presented with quick wash-in and slow wash-out;9 presented with slow wash-in and quick wash-out;another 4 presented with slow wash-in and slow wash-out.The 29 cases of metastatic hepatic carcinoma were divided into 2 groups: less blood supply metastatic carcinoma group(13 cases),which presented with circular enhancement in low echo in arterial phase and low echo in portal and late phases;and more blood supply metastatic carcinoma group(16 cases),which presented with enhancement in totally high echo in arterial phase and low echo in portal and late phases.In the 37 cases of hemangioma,25 lesions were enhanced centripetally and slowly in portal phase.A pattern of a radial centrifugal enhanced in the very early arterial phase and a central stellate fibrous scar in portal phase was observed in FNHs.A pattern of enhancement similar to the surrounding parenchyma was observed in regenerative nodule.Conclusion: Contrast enhanced ultrasound can show microvascular organization and parenchymal perfusion and it is very useful for diagnosing liver occupying lesions.

Abstract:BACKGROUND: To investigate the accuracy of contrast-enhanced ultrasonography, CT-enhancement and MRI in the diagnosis of liver-occupying lesions. METHODS: 176 patients with suspected liver lesions in our hospital were retrospectively studied from July 2014 to July 2016. All of the 176 patients were diagnosed by contrast-enhanced ultrasonography, enhanced CT and MRI, and the pathological examination was performed. The results of pathological examination were regarded as the results of the diagnosis. The diagnostic accuracywas then compared among contrast-enhanced ultrasound, enhanced CT and MRI of these patients. RESULTS: The results of contrast-enhanced ultrasonography showed that 164 of the 176 patients had liver-occupying lesions, and the accuracy of the diagnosis was 95.35%, which was significantly higher than that of CT enhancement and MRI (80.23% 84.30%). The accuracy of contrast-enhanced ultrasonography, in the diagnosis of primary liver cancer was significantly higher than that of CT enhancement and MRI (P < 0.05), and the difference was significant difference (P < 0.05). CONCLUSIONS: The examination of contrast-enhanced ultrasonography is relatively simple, and the patients can get duplicateexamination, so we should choose the contrast-enhanced ultrasonography as the preferred method of diagnosis in liver mass, especially primary liver cancer.

Abstract:目的探讨适宜中国肝局灶性病变的超声造影诊断指标.方法以注射造影剂后肝动脉开始显影作为动脉相的起始时间,以门静脉开始显影作为门脉相起始时间,以肝实质增强达峰值为实质相起始时间;分析682例肝局灶性病变超声造影肝动脉、门静脉的始增时间及肝实质的增强峰值时间、肝内病灶的开始减退时间,分析增强模式.结果肝硬化与非肝硬化组肝动脉、门静脉及肝实质始增时间有显著差异,肝内良恶性病灶增强退出时间有显著差异.85%的原发性肝癌及99%肝转移癌在注射造影剂后180s内退出,故以180 s作为延迟相起始时间及与实质相的划分点,由此将超声造影分为四个时相.结论由于肝背景不同造成血流动力学差异,病灶增强时相以自身背景对比更为适宜;根据新的时相定义及病灶增强退出特征,总结出中国人肝癌及其他肝局灶性病变的超声造影诊断标准。

Abstract:目的研究经周围静脉彩色多普勒超声造影在肝内小占位性病变的鉴别诊断中的作用和意义.方法选择 0.05);肝癌造影前后血流显示率分别为53%(18/34)和 94%(32/34)(P\u003c0.01),造影后血流显示明显提高.结论 Levovist造影后,肝内病灶血流信号得到明显增强,彩色多普勒血流显像充分显示了肿瘤内血流分布状况,有利于克服肿瘤大小及深度对血流显示率的影响,提高小肝癌的诊断符合率.

Abstract:Background: With the rapid development of contrast agents and ultrasound imaging technique, ultrasonography is now a mature technique. Its clinical use provides a new method for the diagnosis and differential diagnosis of focal hepatic lesions. Aims: To analyze various features of focal hepatic lesions in contrast-enhanced ultrasonography and to appraise its diagnostic. Methods: A total of 22 patients with different kinds of focal hepatic lesions were examined by contrast-enhanced ultrasonography using SoneVue as the contrast agent and recorded with video CD. Results: Different features were shown during the arterial phase, portal venous phase and lag phase, which were important for the diagnosis and differential diagnosis of focal hepatic lesions. Primary hepatocelluar cancer had the characteristics of "rapid-in and rapid-out", metastatic liver cancer presented areas of "filling defect" during the lag phase, hemangiomas had an increased uptake of contrast agent at the portal venous phase and lag phase, liver abscess showed immediate enhancement and kept until the lag phase, while the enhancement of cirrhotic nodule was of the same intensity as the liver parenchyma. Conclusions: This new contrast-enhanced ultrasonography is significantly superior to conventional ultrasonography in the diagnosis of liver neoplasm and also significantly superior to contrast CT in detecting minute lesions, thus it greatly improves the diagnostic value of ultrasonography in liver neoplasms.
