Abstract:Unilateral thyroidectomy (lobeetomy) plus (or) isthmectomy were performed surgically and all patients recovered satisfactorily. No recurrence was found after a mean of 37 months follow-up(9 months-6 years).

Abstract:OBJECTIVE:To discuss outcome of thyroid tumor patients treated with surgery.METHODS:Total number of patients was 2228. These patients of thyroid tumors from 1992-2004 (2072 cases of benign thyroid diseases and 156 cases of thyroid carcinoma) were recruited. The clinical and follow-up datum were retrospective analyzed.RESULTS:(1) Benign thyroid tumors with near-total thyroidectomy including 1761 thyroid adenoma, 207 nodular goiter and 104 Hashimoto thyroiditis, the incidence of recurrent laryngeal nerve paralysis was 0.2%, 55 cases (2.6%) received secondary surgery. All the patients have no hypocalcemia or hemorrhage after operation. (2) Eighty-one cases of papillary carcinoma of the thyroid ( > 1 cm) and 60 cases of microcarcinoma. Unilateral thyroidectomy, contralateral near-total thyroidectomy and ipsilateral modified neck dissection were performed in unilateral papillary carcinoma of thyroid. Among the 9 cases of follicular carcinoma of thyroid, 7 were performed of near-total thyroidectomy without neck dissection, others were the same as papillary carcinoma. Bilateral total thyroidectomy and bilateral modified neck dissection were performed in 2 cases of the medullary thyroid cancer and 1 case of the undifferentiated thyroid cancer. By direct method the 5-year survival was 95.5% (64/67), and by Kaplan-Meier method, it was 98.0%. The treatment of microcarcinoma are multiple. There is no relapse or metastases in 60 cases of papillary thyroid microcarcinoma. The 5-year survival was 100.0%, 1 cases occurred recurrent laryngeal nerve paralysis in thyroid cancer. No hypocalcemia or hemorrhage. Eight case relapsed in 156 cases of thyroid carcinoma,3 cases died.CONCLUSION:The correct surgical management for the patients with thyroid tumor should benefit for the prognosis and reduce the complications and the recurrence of the operation.

Abstract:Objective To investigate the diagnosis and surgical treatment of thyroid cancer. Methods From January 2003 to Decembher 2007, 221 cases with thyroid cancer were analyzed retrospectively. All cases underwent surgical operations as follows:subtotal thyroidectomy 161 cases, total thyroidectomy 57 cases and palliactive resection 3 cases. Except for 3 cases underwent palliactive operation, the lymph mode dissection of jugular central area was performed routinely in all cases. Results Lymph node metastases were confirmed by postoperative pathological examination in 89 cases(40.3%). The postoperative complications had 45 cases, including transient recurrent nerve paralysis 5 cases, temporary hypocalcemia 37 cases, permanent hypoparathyroidism 1 case and hematoma of neck 2 cases. A solid or cystic solid nodules of low echo of thyroid were found during preoperative ultrasonic examination in all cases and among them, 39 cases(35.7%) were associated with gravel calcified lesion in intra-thyroid nodules. Follow-up was got in 187 cases. The follow-up time was from 6 months 5 yeras. Except for 1 cases with poorly differentiated cancer died on 7 months after operation, the other patients were still survival. Conclusion Affected lateral thyroid plus isthmus, contralateral subtotal thyroidectomy and lymph node dissection of jugular central area are essential operative manner for the treament thyroid cancer. Preoperative high frequency ultrasonagraphy revealed as gravel calcified lesions has an important value for the preoperative diagnosis of thyroid cancer.
Diagnosis and surgical treatment of bilateral thyroid carcinoma

Abstract:Objective To investigate the diagnosis and treatment of bilateral thyroid carcinoma.Methods The clinical records of 72 cases of bilateral thyroid carcinoma from 2000 to 2008 were retrospectively analyzed.Results All cases were confirmed as bilateral thyroid carcinoma by paraffin section post-operatively.They included 17 cases of bilateral micro-carcinoma,41 cases of one lateral micro-carcinoma and other lateral non-microcarcinoma,and 14 cases of non-microcarcinoma.Pathological type was divided into 4 types: papillary carcinoma(67 cases,93.1%),follicular carcinoma(2 cases,2.8%),medullary carcinoma(2 cases,2.8%),poorly differentiated carcinoma(1 case,1.4%).Among them,62 cases were treated with total thyroidectomy,2 cases with near total thyroidectomy,and 8 cases with a lobectomy and a sub-total thyroidectomy.Central region lymph nodes were resected in all cases,and additional unilateral functional lymph nodes resection was performed in 19 cases.Central region lymph nodes metastasis rate was 33.33%(24/72),and for bilateral micro-carcinoma of thyroid it was 17.65%(3/17).There was some relationship between central region lymph nodes metastasis rate and cancer diameter,but no significant difference was shown(P>0.05).Seventy patients were followed up from 3 months to 8 years post-operatively,with tumor-free survival in 67 cases,and cervical lymph nodes metastasis in 3 cases.No permanent hypo-parathyroidism or paralysis of recurrent laryngeal nerves occurred.Conclusions Total thyroidectomy is advised for bilateral thyroid carcinoma.It is necessary to emphasize the importance of resection of the central region lymph nodes.
Clinical analysis of reoperation for differentiated thyroid cancer of 82 cases

Abstract:Objective: To study the results of the reoperation for thyroid cancer. Methods: The clinical data of 82 cases in our hospital during a period of ten years who underwent reoperation for thyroid cancer were analyzed retrospectively. Results: Excision of the affected lobe,isthmus and greater part of the opposite lobe was performed in 58 cases,total thyroidectomy in 17 cases, excision of palliation in 7 cases that neoplasms infiltrate generally, functional cervical dissection in 7 cases that found lymphadenectasis in intraoperative. Results of postoperative pathology:22 cases (62.9%) had lymph node metastasis,52 cases (63.4%) with residual cancer in thyroid. There were 4 patients with recurrent laryngeal nerve injury included 3 cases recovered half a year later, 3 cases had parathyroid gland injury that recovered in the near future, there were no cases of esophageal injury. Conclusion: Because of the high residual cancer rate of local mass resection for thyroid carcinoma and it is difficult to diagnose before reoperation, reoperation is necessary.The modus operandi of ablate unilateral gland should be abolished.

Abstract:To evaluate the results of surgically treated patients with benign thyroid disorders from Department of Otolaryngology- Head and Neck Surgery.The clinical data of 496 patients operated for benign thyroid disorders were retrospectively analyzed in this department from January 2001 to April 2004. The incidence of complications, particularly postoperative permanent recurrent laryngeal nerve palsy and hypocalcemia, duration of surgery, incision length on the neck, hospitalization days and postoperative recurrence were evaluated.Unilateral total thyroidectomy with contralateral partial lobectomy was performed in 314 cases, unilateral total thyroidectomy with isthmectomy in 76, bilateral subtotal thyroidectomy with the remnant left at the upper pole in 29, isthmectomy in 3, total thyroidectomy in 46. The operations on 28 patients with substernal goiter have been successfully performed via cervical collar incision. Of these patients, the incidences of unilateral recurrent laryngeal nerve paralysis and temporary postoperative hypocalcemia were 0.2% (1/496) and 1.8% (9/496), respectively. There were no bilateral vocal cord paralyses and permanent hypocalcaemia. Postoperative hemorrhage requiring reoperation occurred in 3 cases (0.6%). No patients had incision infection. The mean duration of surgery was 66 min. The mean incision length on the neck was 5.2 cm. The incidence of postoperative recurrence was 0.2% (1/496). The mean hospitalization day was 6.3 d.Otolaryngologists who had received stringent training of head and neck surgery seem to have the advantage to avoid injury of recurrent nerve in the thyroid surgery.

Abstract:Objective:To discuss the operation mode of the thyroid papillary carcinoma. Methods: Clinical and histological data of 76 cases with thyroid papillary carcinoma from 1995 to 2005 were analyzed retrospectively. Results:Thirty seven patients have Been performed the unilateral lobectomy,30 patients the unilateral lobectomy plus isthmectomy,9 patients the thyroidectomy,62 of these patients were received follow-up from three months to ten years who all survived.One patients recurred,2 are had lymph metastasis, no distant metastasis. Conclusion: The effect of the surgery is satisfied.

Abstract:OBJECTIVE:To evaluate the results of re-operation of thyroidectomy for patients of thyroid carcinoma.METHODS:268 patients of thyroid carcinoma received completion thyroidectomy were retrospectively reviewed from 1984 to 2000. There were 59 males and 209 female cases. 256 cases had received nodule enucleating or partial thyroidectomy in other hospitals and 12 cases had unilateral subtotal thyroidectomy in this hospital. The types of reoperation were: Total thyroidectomy for bilateral thyroid carcinoma (6 cases); expand isthmectomy for isthmus carcinoma (1 case). Lobectomy plus isthmectomy for unilateral disease (261) Simultaneous neck dissections were performed in 196 cases. Among them, 94 cases had classical neck dissection, 102 cases had modified neck dissection.RESULTS:Pathological results confirmed that there were 78 cases with residual thyroid carcinoma. The rate of residual carcinoma was 29. 1% (78/268). There were 95 cases with lymphnode metastasis. The rate of lymphnode metastasis was 48.5% (95/196). The rate of recurrent laryngeal nerve injury was 1.1% (3/268). The 5-year and 10-year survival rates of patients were 94. 0% (251/267) and 85. 2% (127/149) respectively.CONCLUSIONS:The residual carcinoma of completing thyroidectomy was high. Therefore re-operations of thyroid in selected cases were necessary.
Endoscopic Thyroidectomy:Report of 19 Cases

Abstract:[Objective]To evaluate the feasibility and technique characteristics of endoscopic thyroidectomy. [Method] Between December 2001 and October 2003, nineteen patients were selected to undergo endoscopic thyroidectomy. Patient selection was based on the clinical examination, preoperative untrasonography and fine needle aspiration biopsy were usually performed. Frozen section was examined intraoperatively in all cases, and the cosmetic results were gained from all cases in about 1 month. The details of the surgery were described, the advantage and disadvantage were discussed as well. [Result] Total lobectomy(12 cases), sub-total lobectomy(4cases)and enucleation of thyroid tumor(3 cases) were performed successfully by endoscopic surgery. The mean time of operation was 194 minutes by areola approach and 106 minutes by sternal notch approach. No complications were occurred. All the patients were satisfied with cosmetic results compared with conventional thyroidectomy. [Conclusion] Endoscopic thyroidectomy is a safe and feasible method. We recommended this minimal invasive surgery in benign diseases of thyroid with optimal results.
Alternative Surgical Strategies in Patients with Sporadic Medullary Thyroid Carcinoma: Long-term Follow-Up

Abstract:The extent of surgical resection in patients with sporadic medullary thyroid carcinoma (SMTC) remains controversial. The aim of the present study was to discuss the prognosis of sporadic medullary thyroid carcinoma with different surgical treatments. Of 73 patients with SMTC (mean age of 43.78 years at diagnosis), 70 patients were followed up for 12.0-169.0 months (median 90.0). Having given their informed consent, 12 patients underwent total thyroidectomy with bilateral central neck dissection (group A), 40 underwent subtotal thyroidectomy preserving contralateral thyroid tissue on the entrance point of the recurrent laryngeal nerve into the larynx with ipsilateral modified radical neck dissection (group B), and 18 patients underwent subtotal thyroidectomy preserving contralateral thyroid tissue on the entrance point of the recurrent laryngeal nerve into the larynx with bilateral modified radical neck dissection (group C). The diagnosis was confirmed by a pathology examination. The incidences of hypoparathyroidism and recurrent laryngeal nerve injury, the cancer recurrence rates and survival time were investigated post-operatively. Significant differences were found between groups A, B and C in the incidence of hypoparathyroidism (χ(2)=40.9, P<0.01), as well as that of recurrent laryngeal nerve injury (χ(2)=32.9, P<0.01). The cancer recurrence rates in groups A, B and C were 75.0% (9/12), 2.5% (1/40) and 44.4% (8/18) respectively, (χ(2)=31.1, P<0.01) and the cure rates were 25, 97.5 and 55.6% respectively (χ(2)=31.1, P<0.01). The mean survival times in groups A, B and C were 77.8, 106.1 and 111.0 months respectively, but significant difference was noted (χ(2)=3.2, P>0.05). In conclusion, compared to total thyroidectomy with bilateral central neck dissection, subtotal thyroidectomy with ipsilateral/bilateral modified radical neck dissection showed a lower incidence of hypoparathyroidism, recurrent laryngeal nerve injury and lower rates of recurrence, along with a similar cumulative survival.
The Recovery of Thyroid Function in Low-Risk Papillary Thyroid Cancer After Lobectomy: A 3-Year Follow-Up Study

Abstract:Purpose Management strategies after lobectomy for low-risk papillary thyroid carcinoma (PTC) are controversial. This study aimed to identify the proportion of patients among low-risk PTC patients who do not require hormone replacement therapy and to evaluate the risk factors for postoperative hypothyroidism after lobectomy. Patients and Methods The records of 190 PTC patients who underwent thyroid lobectomy from January 2017 to December 2018 were retrospectively reviewed. Clinicopathological characteristics and follow-up data were collected. Univariate and multivariate analyses were performed to identify the risk factors associated with postoperative hypothyroidism and the recovery of thyroid function. Results In summary, 74.21% of patients (141/190) had normal thyroid function without levothyroxine supplementation, while 40.53% (77/190) developed temporary or permanent hypothyroidism. Multivariate analysis indicated that higher preoperative thyroid-stimulating hormone (TSH) levels (>2.62 mIU/L), Hashimoto’s thyroiditis (HT), and right lobectomy were associated with hypothyroidism (all P<0.05). The Area Under Curve (AUC) by logistic analysis was 0.829. Twenty-eight (28/77, 36.4%) patients recovered to the euthyroid state in the first year after surgery, and this recovery was significantly associated with preoperative TSH level. Forty-nine (49/77, 63.6%) patients developed persistent hypothyroidism. The thyroid function of most patients (11/28, 39.3%) recovered in the third month after surgery. Conclusion Patients with a lower level of preoperative TSH, with left lobectomy and without Hashimoto’s thyroiditis had a higher chance of normal thyroid function within the first year after lobectomy. The recovery of thyroid function was associated with the level of preoperative TSH.

[Surgical Treatment and Long-Term Efficacy on Stage T1-3N0 Differentiated Thyroid Carcinoma].

Abstract:BACKGROUND & OBJECTIVE:Whether to choose total thyroidectomy or lobectomy for the primary lesion of differentiated thyroid carcinoma (DTC) limited in one lobe is controversial. This study was to investigate the patterns of surgical treatment for stage T1-3N0 DTC, and observe the long-term efficacy.METHODS:Clinical data of 153 DTC patients, first treated at Cancer Center of Sun Yat-sen University from 1985 to 2000, were reviewed. The postoperative pathologic stage of these patients was T1-3N0. The multiple centricity of primary lesions, patterns of surgical treatment, and prognosis were analyzed.RESULTS:Of the 153 DTC patients, 21 had cancer multifocality: 12 had multiple cancer foci in one lobe and 9 had cancer multifocality in both lobes. Ninety-two patients received unilateral lobectomy plus isthmectomy; 44 patients received ipsilateral subtotal lobectomy plus isthmectomy. Among the 9 patients who had cancer multifocality in both lobes, 7 received unilateral lobectomy plus isthmectomy with contralateral subtotal lobectmoy and 2 received bilateral subtotal lobectomy. Eight patients, whose primary lesions were at isthmus, underwent isthmectomy plus bilateral partial lobectomy. Regional lymph node metastasis appeared in 7 patients after initial operation: 6 in the neck and 1 in the upper mediastinum. Five patients had distant metastasis after operation. The 5-, 10-and 15-year survival rates of these 153 patients were 98.7%, 96.1% and 86.9%, respectively. Age, pathologic type, tumor size and distant metastasis affected the survival (P<0.05). The patients whose age was less than 45 years, tumor size was less than 4 cm, pathologic type was papillary or mixed carcinoma, and who had no distant metastasis had relatively good prognosis. Age and distant metastasis were independent prognostic factors.CONCLUSIONS:It is feasible to perform unilateral lobectomy or subtotal lobectomy plus isthmectomy for patients with stage T1-3N0 DTC. The long-term outcomes of stage T1-3N0 DTC patients are relatively well. Age, pathologic type, tumor size and distant metastasis affect prognosis.
Unilateral Tnm T1 And T2 Papillary Thyroid Carcinoma With Lateral Cervical Lymph Node Metastasis: Total Thyroidectomy or Lobectomy?

Abstract:OBJECTIVE: The aim of this study was to explore the effect of total thyroidectomy (TT) and lobectomy (LT) on the prognosis of unilateral papillary thyroid carcinoma (PTC) with lateral cervical lymph node metastasis.METHODS: Patients with PTC and lateral cervical lymph node metastasis who underwent lateral cervical lymph node dissection between January 2007 and December 2016 were retrospectively reviewed. To investigate the effect of surgical procedure on the prognosis of lymph node metastasis patients, other high-risk factors such as extrathyroidal invasion and large tumor size were excluded. All patients were in Tumor-Node-Metastasis (TNM) stage T1 and T2. Primary end point was recurrence-free survival (RFS).RESULTS: Among 264 PTC patients, 104 (39.4%) patients received TT and 160 (60.6%) patients received LT. With a median follow-up of 50 months (interquartile range, 34 to 74 months), 7 patients (2.65%) experienced recurrence. The 5-year RFS in the TT and LT groups was 96.1% and 97.7%, respectively, and was not significantly different (P = .765). Similar results were found when excluding patients who received radioiodine ablation, which were 97.7% and 97.4%, respectively (P = .752). Age ≥55 years (hazard ratio, 7.368; P = .018) and multifocality in the ispi-lateral lobe (hazard ratio, 10.059; P =.006) were identified as independent risk factors of recurrence.CONCLUSION: For unilateral TNM T1 and T2 PTC patients with lateral lymph node metastasis, there was no significant difference in the effect of TT and LT for RFS in the absence of other risk factors during the follow-up period. Patient age ≥55 years with multifocality in the unilateral lobe might be independent risk factors for prognosis.

Experience of laparoscopic thyroidetomy for thyroid diseases

Abstract:Objective To discuss the effect of the laparoscopic thyroidectomy for patients with thyroid diseases.Methods The clinical data of 30 patients who underwent laparoscopic thyroidectomy through via areola of breasts approach were analyzed.The operation was performed under a pressure of 8 mmHg within the surgical space at the neck,after the thyroid was completely separated.The lesions were resected with ultrasound knife.Results Laparoscopic thyroidectomy was completed in 28 patients including one case of hyperparathyroidism.The other two patients were converted to the open surgery because of hyperthyroidisms complicated with intraoperative hemorrhage or thyroid carcinoma.No severe complications involving the trachea and parathyroid occurred in these series.1 patient developed transient hoarseness after the operation,who was cured spontaneously.Postoperative hospital stay ranged from three to seven days(mean,5 days).The majority of patients were satisfied with cosmetic effect of the surgery.Conclusions Laparoscopic thyroidectomy via areola of breasts approach is safe and feasible for patients with thyroid diseases.It has good cosmetic results.
Total or near-total bilateral thyroidectomy after subtotal bilateral thyroidectomy

Abstract:Objective To evaluate the safety of total or near-total bilateral thyroidectomy after subtotal bilateral thyroidectomy. Methods Thirty-four cases of subtotal bilateral thyroidectomy because of benign thyroid disease were performed total or near-total bilateral thyroidectomy because of the recurrence of nodules between June 1995 and June 2008 at Ruijin Hospital of Shanghai Jiaotong University School of Medicine. The clinical data of all the cases were analyzed. Results Partial fibers of right recurrent laryngeal nerves were cut at their entrance to the larynges in 2 cases.But the mobility of the vocal cord return to normal and the hoarseness were disappeared 6 months after operation. Hypocalcaemia was occurred in 6 cases.The hypocalcaemia disappeared by calcium supplementing through oral or/and intravenously in 5 of 6 cases 2 weeks after operation. But the righr superior parathyroid was cut inadvertently in one thyroid papillary cancer case,and the case underwent parathyroid autotransplantation. Hypocalcaemia was alleviated gradually 1 year after operation in the case. The levels of calcaemia and PTH were still under the normal until 14 months after operation. Conclusion If choosing the right way to expose the remnant gland and being familiar with the anatomical position of bilateral recurrent laryngeal nerves and parathyroids,it is safety to perform total or near-total bilateral thyroidectomy after subtotal bilateral thyroidectomy.

Abstract:Objective: To investigate the feasibility and safety of total thyroidectomy as the surgical treatment for the recurrent hyperthyroidisim.Methods: A total of 32 patients were operated on for recurrent hyperthyroidisim from January 2008 to December 2011 in our hospital.Total thyriodectomy was performed in all patients with routine Lugol's iodine preparation.Recurrent laryngeal nerves and parathyroid glands were identified and preserved during the surgeries.Parathyroid autotransplantation was performed if necessary.The feasibility and safety of the surgery were assessed by analyzing the clinical information.Results: 32 female patients suffered hyperthyroidisim recurrence for 1 to 7 years with the average of 3.2 years.Total thyroidectomy was performed in all patients within 70 to 150 minutes,without blood transfusion.Recurrent laryngeal nerve could be found and preserved in all 32 patients.One parathyroid gland was found being removed with the thyroid and autotransplanted during the surgery.There was no recurrent laryngeal nerve injuries.Transient hypocalcemia occurred in two patients.Conclusion: Based on being familiar with the anatomy and mastering the surgical procedure skillfully,total thyroidectomy for recurrent hyperthyroidisim can be performed safely with no significant increase of complication.
Comparison of Lobectomy and Total Thyroidectomy in Unilateral Papillary Thyroid Microcarcinoma Patients with Ipsilateral Lateral Lymph Node Metastasis Without Gross Extrathyroidal Extension

Abstract:Background Lateral lymph node metastasis (LLNM) occurs in a few of papillary thyroid microcarcinoma (PTMC) cases by the time of diagnosis. Total thyroidectomy (TT) is recommended in the 2015 American Thyroid Association guidelines as the initial surgical procedure for thyroid carcinoma patients with clinically apparent cervical lymph node metastasis. However, none of the controlled studies have focused on the proper extent of surgery for patients who have PTMC with concomitant LLNM without gross extrathyroidal extension (ETE). Methods A total of 2373 consecutive patients with PTMC were retrospectively reviewed. Finally, 129 unilateral PTMC patients with ipsilateral LLNM without gross ETE were enrolled in this study and classified into two groups: those who underwent unilateral lobectomy (LT) plus lymph node dissection (LND) (Group I) and those who underwent TT plus LND (Group II). Surgical outcomes and recurrence-free survival (RFS) during the follow-up period were compared between the two groups. Results There were 62 patients in Group I and 67 patients in Group II. Cases in Group II had a longer median operation time (150 min vs. 120 min, p < 0.001) and a higher incidence of postoperative hypoparathyroidism ( p < 0.001), especially permanent hypoparathyroidism, than cases in Group I. But the RFS showed no statistically significant difference ( p = 0.6005) between the two groups during a median follow-up period of 60 months. Conclusion Thyroid LT alone plus ipsilateral LND may be an optimum initial procedure for unilateral PTMC patients with ipsilateral LLNM without gross ETE. A long-term follow-up, prospective, randomized controlled trial is warranted.
Total Thyroidectomy Plus Bilateral Central Lymph Node Dissection for Bilateral Thyroid Papillary Cancer: an Analysis of 33 Cases

Abstract:Objective To discuss the rationality of total thyroidectomy plus bilateral central lymph node dissection for bilateral thyroid papillary cancer.Methods The clinical data of 33 cases of bilateral thyroid papillary cancer performed total thyroidectomy plus bilateral central lymph node dissection from January 2007 to June 2010 at Ruijin Hospital and Yuanyang Hospital of Shanghai Jiaotong University School of Medicine were analyzed retrospectively.Results Among 33 cases,3 cases had unilateral central lymph node metastasis and 23 cases had bilateral central lymph node metastasis.The rate of bilateral central lymph node metastasis was 69.70%(23/33) and the rate of the total central lymph node metastasis was 78.79%(26/33).Six cases(18.18%) developed transient hoarseness.The diameter of the recurrent laryngeal nerves was less than 1 mm.The voice of the cases recovered 3 months after operation.Nine cases(27.27%)developed transient hypocalcemia in whom one case(3.03%) developed permanent hypocalcemia.The ages of the 10 cases were all more than 50 years old,and the parathyroid gland was found in central lymph tissue in 6 cases.Conclusion Although there are some complications after total thyroidectomy plus bilateral lymph node dissection,most of them are transient.Because of the higher lymph node metastasis rate,it is necessary to perform total thyroidectomy plus bilateral lymph node dissection in bilateral thyroid papillary cancer.
Clinical outcome of total thyroidectomy for management of benign thyroid diseases: a review of 128 cases

Abstract:Objective To investigate the clinical outcome of total thyroidectomy for management of benign thyroid lesions and discuss the prevention of operative complications. Methods The clinical data of 128 cases of benign thyroid lesions treated with total thyroidectomy were analyzed retrospectively and the postoperative complications were discussed. Results No patient developed permanent recurrent laryngeal nerve palsy and permanent hypoparathyroidism in the 98 patients with primary total thyroidectomy; 1 patient (1.02%) developed temporary recurrent laryngeal nerve palsy; and 1 patient (1.02%) developed temporary hypoparathyroidism. In the 30 recurrent patients,the incidence of temporary palsy of the recurrent laryngeal nerve was 10.00%,and the incidence of temporary hypoparathyroidism was 13.33% (Fisher P=0.040 or 0.011,respectively). The incidence of temporary palsy of the superior laryngeal nerve was low in both groups. Conclusions Total thyroidectomy is an appropriate operation for the management of benign thyroid lesions. Mastering anatomy of the thyroid gland and operating appropriately were the key points to prevent postoperative complications.
A New Method of Subtotal Thyroidectomy for Graves’ Disease Leaving a Unilateral Remnant Based on the Upper Pole

Abstract:Background: The aim of this prospective randomized study was to evaluate the feasibility of subtotal thyroidectomy with leaving a unilateral remnant based on the upper pole. Methods: Patients who underwent the subtotal thyroidectomy and isthmusectomy leaving either a unilateral remnant based on the upper pole (Group I, 79 patients) or the bilateral dorsal thyroid tissue remained (Group II, 89 patients) were compared in operation time, blood loss, recurrence, and postoperative complications. Results: Among 168 patients analyzed, the operation time remained similar, but the blood loss, the reoperation time, and recurrence in Group I were much less than Group II. In addition, no postoperative hemorrhage occurred in Group I. Two patients (2.28%) in Group II underwent recurrent laryngeal nerve damages. Four patients (5.06%) in Group I and 3 patients (3.37%) in Group II experienced transient hypocalcemia. Recurrence only occurred in Group II. Conclusion: In terms of blood loss, reoperation time, postoperative complication, and the recurrence, subtotal thyroidectomy with recurrent laryngeal nerves identification and the unilateral superior pole remnant of the gland provides a better outcome than subtotal thyroidectomy with bilateral dorsal thyroid tissue remnant.

Abstract:BACKGROUND Papillary thyroid microcarcinoma (PTMC) measures less than 10 mm in diameter, is more common in the thyroid lobes, but rarely presents in the thyroid isthmus. This retrospective study aimed to compare patient outcomes following various types of surgery in patients with PTMC of the thyroid isthmus, at a single center in China. MATERIAL AND METHODS We analyzed the clinical data of patients with isthmus thyroid cancer treated at the First Hospital of China Medical University. Patients were divided into 2 groups according to the tumor diameter-PTMC of the thyroid isthmus and papillary thyroid carcinoma >10 mm. The clinicopathological features between the 2 groups were compared, and the effects of various surgical methods on the prognosis of patients were analyzed. RESULTS A total of 70 patients were included in this study: 29 with PTMC of the thyroid isthmus (41.4%) and 41 with papillary thyroid carcinoma >10 mm (58.6%). The rates of lymph node metastasis (10.3% vs. 34.1%) and extrathyroid extension (0% vs. 14.6%) in the PTMC of the thyroid isthmus were significantly lower than those in the papillary thyroid carcinoma >10 mm. The recurrence-free survival (RFS) rate was 97.1%. Survival analysis showed that there was no significant difference in RFS among patients with PTMC of the thyroid isthmus undergoing isthmusectomy, unilateral lobectomy, and total thyroidectomy. CONCLUSIONS These findings from a single center showed that for patients with PTMC of the thyroid isthmus, who had no comorbidities, there was no significant difference in outcome between the 3 types of thyroid surgery.
