Abstract:全身使用糖皮质激素治疗慢性阻塞性肺疾病急性加重(AECOPD)的作用已得到肯定,但使用的剂量如何、使用多长时间、是否需要逐渐减量等还有争议.高剂量、中剂量和低剂量糖皮质激素均可改善AECOPD患者临床症状、肺功能、缩短住院时间,考虑到其安全性和有效性临床应用以低剂量短疗程为宜,如泼尼松40mg口服qd,共10d.临床医师通常采用逐渐减量的停药方法 ,但实际上多数情况下并不需要.糖皮质激素雾化吸入可以替代全身应用治疗AECOPD.有关糖皮质激素治疗AECOPD的最佳用药方案如用药途径、剂量、时间、是否逐渐减量等尚需进一步研究。

Abstract:使急性症状得到尽快控制及长期治疗遏制肺功能进行性恶化是治疗慢性阻塞性肺疾病(COPD)的两个关键问题.急性期糖皮质激素能够更快地改善临床症状.稳定期长期大剂量吸入糖皮质激素不能改变第一秒用力肺活量的下降速率,但短期内大剂量吸入糖皮质激素可改善肺功能.本文旨在探讨COPD对糖皮质激素治疗有反应的决定因素:稳定期COPD对糖皮质激素有反应的原因考虑与嗜酸粒细胞有关;急性期主要与糖皮质激素对抗渗出、水肿相关。

Efficacy and Safety of Glucocorticoids in Community-Acquired Pneumonia: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

Abstract:Objective To evaluate efficacy and safety of glucocorticoids for community-acquired pneumonia (CAP).Methods We searched relevant randomized controlled trials (RCT) from Pubmed,EMBASE,Cochrane Library,Chinese Journal Full-text Database,Chinese Biomedical Literature Database and traced the related reference to obtain the information that had not been found by using key words of steroids,glucocorticoids,cortisol,corticosteroids,community acquired pneumonia and CAP.The quality of RCT included was evaluated.Meta-analysis was performed with RevMan 5.0 provided by the Cochrance Collaboration.Results Seven RCTs involving 944 patients were included in the meta-analysis.Mean length of hospital stay in glucocorticoids treatment group was significant shorter than that in conventional treatment group (WMD =-1.70,95％ CI:-2.01-1.39,Z =10.81,P ＜0.01).No significant differences were found regarding mortality (RR =0.77,95％ CI 0.46-1.27,Z =1.03,P =0.30),mean length of ICU stay (WMD =1.17,95％ CI:-1.68-4.02,Z =0.81,P =0.42),the risk of double infection (RR =1.32,95％ CI 0.66-2.63,Z=0.79,P=0.43),the incidence of hyperglycemia (RR=1.84,95％ CI0.76-4.41,,Z=1.36,P=0.17),the incidence of upper gastrointestinal bleeding (RR =1.98,95％ CI O.37-10.59,Z =0.80,P =0.42) between conventional treatment group and glucocorticoids treatment group.Conclusions Glucocorticoids treatment used in patients with community-acquired pneumonia may shorten the course of illness and possess better safety,but could not reduce the mortality. Abstract:BackgroundSystemic corticosteroids (SCS) are effective in the management of acute exacerbation of chronic obstructive pulmonary disease (AECOPD). However, they are not without adverse effects, especially hyperglycemia. Inhaled corticosteroids (ICS) showed satisfactory outcomes with good safety. These benefits were not evaluated in patients with AECOPD with diabetes mellitus. The aim was to compare the efficacy and safety of high dose of ICS vs SCS in the treatment of AECOPD in diabetic patients.Patients and methodsA total of 126 patients with AECOPD were screened, and thirty of them were found to be eligible and were enrolled into two groups: group 1 (n=15) received 1mg budesonide by jet nebulizer four times daily, and group 2 (n=15) received 40mg prednisolone or equivalent systemically. Postbronchodilator forced expiratory volume in 1 s (FEV1%) of predicted was measured at day 1 and day 7, and random blood sugar (RBG) was measured twice daily in all patients.ResultsThere was a significant increase in the mean FEV1 at day 7 as compared with mean FEV1 at day 1 in groups 1 and 2, with the increase in mean FEV1 being 19.6 and 21% in groups 1 and 2, respectively. There was a significant difference, with higher mean RBG in group 2 when compared with group 1, at day 4 of treatment and continued onward. Interestingly, there was a significant elevation in mean RBG among patients in group 2 (SCS) starting by day 3 of treatment and continued onward, with no significant rise in the first two days, although there was no evident effect of ICS on the mean RBG among patients in group 1 (ICS) during the follow-up days.ConclusionBoth ICS and SCS improve airflow in patients with AECOPD, taking into consideration the existence of diabetes mellitus. ICS may be an excellent substitute to SCS in the treatment of AECOPD in diabetic patients.

Abstract:OBJECTIVE: To assess the safety of inhaled corticosteoids for childhood asthma.METHODS: The pertinent literature during 1998～2008 both at home and abroad were retrieved,and the safety of inhaled corticosteoids for childhood asthma was assessed from the three aspects: adrenocortical function,growth and development,and bone metabolism.RESULTS: Treatment of childhood asthma with short-term inhaled corticosteroids could influence the growth and development of children,yet it was safe in a long run.CONCLUSION: Safety and efficacy of inhaled corticosteoids can maintain equilibrium,and it is advisable to follow GINA plan in the treatment of childhood asthma in which the inhaled corticosteoids should be should for a long term.

Abstract:Background: Corticosteroids have been widely used as adjunct therapy for septic shock for many decades, but both the efficacy and safety remain unclear. The study was designed to investigate overall benefits and potential risks of corticosteroids in immunocompromised patients with septic shock.Methods: The Medical Information Mart for Intensive Care III (MIMIC-III) database was employed to conduct a cohort study. Immunocompromised patients with septic shock were enrolled and categorized by whether exposure to intravenous corticosteroids. Cox Proportional-Hazards models were used to control for confounders and assess the relationship between corticosteroids use and mortality.Results: A total of 866 patients were enrolled in this study, including 395 in the corticosteroids group and 471 in the non-corticosteroids group. Corticosteroids infusion was not associated with improved 30-day mortality in overall immunocompromised population [34.7% vs 32.1%; adjusted hazard ratio (HR) 1.11, 95% confidence interval (CI) 0.87-1.43, p = 0.37]. The mortality effects were similar in 90-day, 180-day, 1-year and hospital mortality. For the subgroup of patients with metastatic cancer, corticosteroids infusion was associated with a statistically significant increase in the 30-day mortality risk (HR 1.58, 95% CI 1.06-2.37; p = 0.02). Corticosteroids had adverse effects on hemodynamic stability, prolonged ICU and hospital duration, and increased risk of hyperglycemia.Conclusions: Corticosteroids therapy for the maintenance of blood pressure was not associated with improved mortality or hemodynamic stability in overall immunocompromised population with septic shock. Future randomized clinical trials are required to validate the effects of corticosteroids for septic shock in the special immunocompromised population.(c) 2020 Elsevier Inc. All rights reserved.
Safety of Long-Term Use of Asthma Controller Drugs

Abstract:AIM To evaluate the safety of long-term use of asthma controller drugs.METHODS This questionnaire survey was conducted in outpatients of Zhongshan hospital affiliated to Fudan university during the period between July,2009 and December,2009.The questionnaire involved base situation,medications, adverse drug reactions(ADR) and so on.The bone mineral density(BMD) was tested by dual energy X-ray absorptiometry in 20 asthmatic patients who used inhaled corticosteroids(ICS) for more than 12 months.The BMD of patients with different ICS daily dose and total accumulation were compared.RESULTS There were 200 patients enrolled in the investigation.The incidence of ADR was 7.5%(15/200),and the main clinical manifestations were hoarse voice,sore throat,headache,palpitate,amyostasia,oral candidiasis and so on. There were no significant difference in BMD between the group used ICS≥800μg·d~(-1) and the group used ICS800μg·d~(-1)(P0.05).BMD of the lumbar spine and trochanter of femur in patients with the accumulation of ICS≥600 mg were lower than that in patients with the accumulation of ICS600 mg(P0.05).CONCLUSION Long-term use of asthma controller drugs is regarded as generally safe.Long time exposure to high doses of ICS could increase the risk of bone mass loss in asthmatic patients.

Abstract:Chronic obstructive pulmonary disease (COPD) is a common clinical respiratory disease with high morbidity and mortality, which not only damages human organs, but also brings psychological anxiety and burden to patients and their families. [ 1–3 ] There are two main classes of drugs commonly used in clinical practice, including inhaled glucocorticoids (ICS), bronchodilators, anti-inflammatory drugs, and expectorants. [ 1 ] In patients with COPD, ICS are helpful in lowering airway inflammation, nonspecific airway hyperresponsiveness, and airway obstruction. [ 4 ] The use of ICS with long-acting bronchodilators in the treatment of COPD has been shown to be more effective than using single agents in reducing patient symptoms, enhancing lung function, and preventing exacerbations. [ 5 ] Commonly used ICS include budesonide, fluticasone, fluticasone combined with salmeterol, and budesonide combined with formoterol. [ 6 ] Although the majority of studies have found that using ICS is generally safe, patients and their families are becoming more worried about ICS-related side effects include pneumonia, [ 7 ] oral Candida infections, [ 8 ] and renal suppression. [ 9 ] The relationship between ICS and specific adverse reactions still has to be further established because different investigations on whether ICS raises the likelihood of adverse reactions in COPD patients have produced contradictory results. ICS can be used effectively and optimally by ensuring the accuracy of the safety indicators in each study. Whereas other meta-studies on adverse effects of ICS for COPD have mostly focused on one type of adverse effects, we included more randomized controlled studies with meta-analysis of multiple adverse effects to assess the safety of ICS use in patients with COPD.

Abstract:Objective To explore the effect and its clinical significance of different dose of glucocorticoids on inflammation mediators in patients with acute exacerbation of chronic obstructive pulmonary diseases.Methods 45 patients admitted to our hospitals from March 2007 to March 2011 were randomly divided into 3 groups:methylprednisolone 40 mg group (methylprednisolone 40mg,iv,qd),methylprednisolone 80 mg group (methylprednisolone 80mg,iv,bid),and control group (without any glucocorticoids).The changes of dyspnea scores,arterial blood gas analysis,clinical symptom scores and serum IL-6,IL-8 and tumor necrosis factor (TNF) levels were detected in patients of each group before and at the 7th day after treatment.The related adverse drug reactions were recorded.Results The improvements in clinical symptom scores,Borg scores,PaCO2,PaO2 after treatment were higher in methylprednisolone 40 mg group and methylprednisolone 80 mg group than in control group (F=3.6747.162 and 42.88,respectively,P＜0.01 or 0.001),and the above improvements was better in methylprednisolone 80 mg group than in methylprednisolone 40 mg group (all P＜0.05).The decreases in levels of serum IL-8,TNF-α,IL-6,C-RP after the treatment were more significant in methylprednisolone 40 mg group and methylprednisolone 80 mg group than in control group (F=12.65,16.17,30.99,respectively,all P＜0.001),and the decrements were more significant in methylprednisolone 80 mg group than in methylprednisolone 40 mg group(all P＜0.05).NO serious adverse drug reactions happened during the course of treatment in the three groups.Conelusions Short-term and moderate dose of glucocorticoid treatment is effective and safe in treating the patients with acute exacerbation of chronic obstructive pulmonary diseases.Methylprednisolone 80 mg injection daily can more obviously improve AECOPD symptoms,and reduce the levels of inflammatory factors better.

Abstract:The commercial availability of high-dose inhaled corticosteroid preparations for the treatment of asthma offers an occasion for reviewing the different mechanisms of action of glucocorticoids notably glucocorticoids, notably their interference with lipocortin protein synthesis and arachidonic acid metabolism. A favourable effect on bronchial hyper-reactivity and on delayed hypersensitivity reactions has also been observed. The problems raised by the various preparations responsible for varying degrees of systemic activity, the difficulties inherent in the use of inhalers and the side-effects of these products are discussed. The effectiveness and safety of the current high-dose inhaled preparations are emphasized.

Abstract:Inflammation is an important phenomenon in allergic disorders and corticosteroid drugs since their anti inflammatory properties have a pivotal role in improvement, control and relieving the symptoms and signs of asthma, allergic rhinitis, atopic dermatitis and many other inflammatory disorders. Nevertheless, the prescription of these drugs might be associated with some unwanted reactions such as hyperglycemia, hypertension, Cushing's syndrome, adrenal insufficiency and growth failure. In recent decades administration of intranasal and inhaled corticosteroids instead of systemic parenteral or oral drugs reduced some of these concerns during treatment of patients and enable them to receive the effective and safer kind of therapy in acute or chronic inflammatory disorders of nose and airways. In this article the properties of inhaler (ICS) and intranasal corticosteroids (INCS) were reviewed.
Safety of Common Inhaled Glucocorticosteroids for Childhood Asthma: A Systematic Analyse

Abstract:OBJECTIVE:To assess the safety of inhaled glucocorticosteoids in the treatment of childhood asthma.METHODS:MEDLINE(1995～2007),PUBMED(1995～2007),Cochrane Central Register of Controlled Trials(Issue 1,2008),CNKI(1995～2007) and Wanfang Data were searched by computer for randomized controlled trials or quasi-randomized controlled trials about the safety of inhaled glucocorticosteoids in the treatment of childhood asthma.A meta-analysis was performed on the included studies.RESULTS:A total of twenty three literatures were included(n = 2 112),of which,16 were written in Chinese and 7 in English.According to Jadad scale,6 literatures were high in quality.In the inhaled glucocorticosteoids groups as compared with the control,(1) the urinary-free cortisol(UFC)(within 3 month) was higher than that for the control group [WMD:-11.67,95% CI(-19.68,-3.66),P=0.004],however,the UFC had no statistical difference over 1 year [WMD:-3.45,95%CI(-9.08,2.18),P=0.23];(2) Within 1 year and over 1 year,the growth velocity(GV) had no statistical difference (P0.05) [WMD:0.02,95% CI(-0.07,0.11),P=0.49 versus WMD:-0.22,95%CI(-0.53,0.09),P=1.40];(3) Similar results were also found in 6 month and 1 year for the bone mineral density(BMD) [WMD:0,95% CI(-0.01,0.01),P=0.95 versus WMD:0.01,95%CI(-0.01,0.02), P=0.24];(4) The C-terminal telopeptide region of type I(ICTP) collagen increased significantly within 6 months [WMD:-3.33,95% CI(-5.23,-1.42),P=0.000 6].Heterogeneity was only noted among literatures of ICTP(Heterogeneity test P0.000 01,I~2=96.4%),but sensitivity analysis showed that the analysis results of ICTP were stable and reliable.The literatures of other indexes were of homogenicity.The finding showed that the UFC of publication bias was existed.CONCLUSION:Although inhaled corticosteroids may have effect on adrenocortical function and growth development in short time(within 6 montsh),the clinical evidences available so far indicate that the treatment of childhood asthma with inhaled glucocorticosteroids had no effects on the growth velocity(over 1 year),adrenocortical function and bone mineral density,suggesting they are safe for children in long-term use.

Abstract:目的观察通过鼻喷及经口吸入糖皮质激素丙酸氟替卡松治疗小儿哮喘合并变应性鼻炎的临床疗效.方法65例合并变应性鼻炎的哮喘儿童随机分治疗组(称联合治疗组)35例,对照组(称舒利迭TM组)30例.治疗组在经口吸入沙美特罗替卡松(舒利迭TM)治疗的同时给予鼻喷辅舒良,对照组仅予经口吸入舒利迭治疗.治疗后1个月、3个月测定最大呼气流速(PEFR),对照观察其疗效.结果联合组在治疗第4周测定肺功能明显改善,有32例(91.4%)PEFR上升至正常预计值90%以上,变应性鼻炎有效率达97.1%.而对照组PEFR上升至正常预计值90%以上仅7例,占23%,变应性鼻炎的有效率仅16.7%.两组疗效差异明显(P＜0.05),但3个月后两组PEFR均已达正常预计值(P＞0.05).结论舒利迭TM联合辅舒良吸入治疗哮喘合并变应性鼻炎能迅速缓解临床症状。

Abstract:目的探讨糖皮质激素(GC)在肺外感染致急性呼吸窘迫综合征(ARDS)综合救治中价值.方法收集长征医院急救科1990年3月至2000年2月和新华医院内科ICU 2000.-05-2003.-04收入因肺外感染诱发ARDS患者临床资料,比较GC与非GC组各项临床指标.结果32例患者中男/女为21/11例,年龄(51.2±19.1)岁,APACHEⅡ评分(28.2±9.7),接受机械通气治疗25例(78.1%),病死率65.6%(21/32);GC与非GC组年龄、APACHEⅡ评分、病死率之间差异无显著性(P＞0.05);GC组PaO2/FiO2改善明显,(P＜0.05,Qs/Qt差异无显著性(P＞0.05),MODS和感染性休克发生率较非GC组低(P＜0.05).结论GC能协同常规治疗纠正缺氧和休克,为原发病治疗赢得时间与机会。
Association between inhaled corticosteroid use and risk of hyperglycemia in patients with chronic obstructive pulmonary disease: A systematic review and meta-analysis

Abstract:Objectives: Chronic obstructive pulmonary disease (COPD) patients have a higher risk of developing diabetes, and studies suggest that inhaled corticosteroids (ICSs) use may be associated with a higher risk of diabetes, particularly at higher doses. This study aims to investigate the effects of ICS use on the risk of diabetes and blood glucose levels in COPD patients. Materials and methods: A systematic search was carried out on the PubMed, EBSCOhost, and ProQuest databases using the terms "Inhaled Corticosteroids," "Diabetes," and "Chronic Obstructive Pulmonary Disease" for the period between 2013 and 2023. The systematic review adhered to the PRISMA 2020 guideline. A meta-analysis was conducted using a random-effects model using the RevMan 5 software. Results: A total of 14 studies were included in the final analysis, with 10 randomized controlled trials (RCTs) and 4 observational studies. Two observational studies investigated the relationship between ICS dose and diabetes risk. A meta-analysis of the RCTs studies showed a nonstatistically significant tendency toward increased blood glucose (odds ratio [OR] 1.07 and 95% confidence interval [CI] 0.88-1.30) after a 52-week follow-up. Whereas the observational studies showed a tendency toward an increased risk of diabetes (OR 1.40 and 95% CI 0.96-2.03). Furthermore, a subgroup meta-analysis of high-dose ICS (>900 μg/day) showed a significant increase in the risk of diabetes (OR 1.20 and 95% CI 1.09-1.32). Conclusion: Short-term use of ICS does not have a significant effect on blood glucose. However, long-term use, especially at higher doses, can increase the risk of developing diabetes.

However, the optimal dose remains controversial.ObjectivesWe performed a meta-analysis to evaluate whether high-dose SCS is better.MethodsWe searched PubMed, EMBASE, CPCI-S and CENTRAL databases, and references of reviews or meta-analyses to identify randomized controlled trials using SCS in AECOPD. We performed a routine meta-analysis to evaluate the effects of SCS on treatment failure rate and forced expiratory volume in 1s (FEV1) improvement compared with placebo in AECOPD. Subgroup analysis was performed by dividing the studies into a high-dose group [initial dose 80mg prednisone equivalent (PE)/day] and a low-dose group (initial dose 30-80mg PE/day) in all patients and in only inpatients. Meta-regression was performed using initial dose as an independent factor. We classified the suspected adverse effects into several groups and combined them separately.ResultsOur search yielded 12 studies involving 1172 patients. SCS use was associated with a significant reduction in the treatment failure rate [risk ratio 0.58; 95% confidence interval (CI): 0.46-0.73] and improvement in FEV1 (0.11L; 95% CI: 0.08-0.14L). The high-dose regimen did not show superiority to the low-dose regimen. No obvious correlation was found between the SCS effect and the initial dose. SCS led to an obvious increase in hyperglycemia risk. However, the high-dose group did not show obviously higher risk of adverse effects.ConclusionSCS can reduce treatment failure rate and improve lung function in AECOPD. The low-dose regimen (initial dose 30-80mg/day PE) is proper for treating AECOPD.

Abstract:严重脓毒症及感染性休克目前仍是ICU中最主要的致死原因,病死率在30%～50%之间.虽然经过清除感染灶、合理应用抗生素、通气及血流动力学支持,病死率并没有明显改善.近年来陆续报道小剂量激素替代疗法能改善严重脓毒症及感染性休克患者血流动力学指标及提高生存率,小剂量激素在此类患者中的应用再次引起讨论.该文就糖皮质激素在严重脓毒症及感染性休克中应用的进展进行综述。
