Abstract:Objective To evaluate the clinical application of T-SPOT.TB(enzyme-linked immunospot assay in detection of Mycobacterium tuberculosis infection)in diagnosis of HIV/AIDS and TB patients.Methods We used the T-SPOT.TB to detect T cells with sectetion of IFN-r specific for M.tuberculosis antigents in blood samples from 60 HIV/AIDS patients suspected with tuberculosis,30 healthy persons and 20 patients with other diseases.Results Among 60 HIV/AIDS patients suspected with tuberculosis,35 cases were diagnosed with tuberculosis.26 of 35 tuberculosis patients had detected M.tuberculosis antigen-specific T cells,whereas 2 of 30 healthy persons and 20 patients with non-tuberculosis illnesses responded,This assay had a sensitivity of 74.3%,specificity of 86.7%.Conclusions T-SPOT.TB is a rapid,sensitive and specific method for detecting M.tuberculosis specific T cells.It also can provide objective evidence for the diagnosis of HIV/AIDS and TB patients.

Abstract:Objective To evaluate the application of a new enzyme-linked immunospot assay (ELSIPOT)based on Mycobacterium tuberculosis-sepeific T cells detection in the diagnosis of active pulmonary tuberculosis in China.Methods Using the ELlSPOT assay.T cells specific for early secreted antigenic target 6-kDa protein(ESAT-6)and culture filtrate protein 10(CFP-10)in the peripheral blood from 76 active pulmonary tuberculosis patients were measured.Thirty healthy people were recruited as healthy controls,which had no tuberculosis contacting history and were normal for chest X ray examination as well as negative for tuberculosis skin test(TST).The data were analyzed using Chi-square test and Mann-Whitney test.Results The positive rate of ELISPOT was 71.0%(54/76)and 6.7%(2/30)in the patients with active pulmonarytuberculosis and healthy controls respectively.Furthermore,the positive rate was as high as 87.5% in patients with antituberculosis treatment course less than 1 month,and decreased to 74.1% and 52.0% in the patients with the treatment course varying from 1 to 6 months and more than 6 months respectively(X2=35.63,P=0.000).In the patients with treatment course less than 1 month,the sensitivity,specificity,positive predictive value and negative predictive value of ELISPOT were 87.5 oA,93.3 oA,91.3% and 90.3%,respectively.Conclusion ELISPOT assay is a sensitive assay for detecting tuberculosis infection,especially in the patients with anti-tuberculosis treatment course less than 1 month.
Clinical Application and Antigenicity of ESAT-6/CFP-10 Fusion Protein in Diagnosis of Tuberculosis

Abstract:OBJECTIVE To analyze the specificity of ESAT-6/CFP-10 fusion protein as an antigen of tuberculosis and explore the clinical application of ESAT-6/CFP-10 enzyme-linked immunospot(ELISPOT) assay in diagnosis of tuberculosis.METHODS ESAT-6/CFP-10-ELISPOT assay was performed for detection of IFN-γ-secreting T cells which were specific for M.tuberculosis antigens.Blood samples were from 65 patients only with tuberculosis,16 patients with co-infection of HIV and TB,32 patients with other respiratory diseases,and 30 healthy individuals.RESULTS As compared with ESAT-6/CFP-10-ELISPOT assay and TST for 81 patients with tuberculosis,the positive rate of ESAT-6/CFP-10-ELISPOT was 98.8%,and the positive rate of TST was 56.8%;the positive rates of ESAT-6/CFP-10-ELISPOT in smear positive tuberculosis group,smear negative tuberculosis group,and co-infection HIV and TB group were 100.0%,97.1% and100.0% respectively,suggesting significantly higher sensitivity than that of sputum smear test,and the differences were not statistically significant.There was one positive case in HIV infection group,both the non-tuberculosis respiratory tract disease group and the control group were negative,which indicated that the specificity of ESAT-6/CFP-10-ELISPOT was 98.8%.CONCLUSION ESAT-6/CFP-10 fusion protein excellently stimulated T cells to secret IFN-γ.It can serveas specific antigen which is suitable for the diagnosis of tuberculosis and be applied for the detection of ELISPOT.ESAT-6/CFP-10-ELISPOT is valuable for the diagnosis of tuberculosis.

Abstract:Objective To evaluate a new enzyme-linked immunospot assay (TSPOT-TB) for the diagnosis of latent tuberculosis infection in patients with rheumatic diseases.Methods The rapid TSPOT-TB assay was applied to detect ESAT-6 and CFP-10 specific T cells in blood samples from 126 rheumatic disease patients.A PPD skin test was performed on all patients simultaneously.Results The positive rate of TSPOT assay was 23.8% and that of PPD skin test was 34.9%.The overall agreement between the 2 tests was 71.4%.Among PPD (-) patients (n=82),11 were TSPOT (+) ( 13.4% ).Among PPD (+) patients (n=44),25 were TSPOT(-) ( 56.8% ).The patients who got BCG vaccination showed a significantly higher rate of positive results of PPD skin test than those who did not(41% vs 19%,P＜0.05).While in TSPOT assay,the BCG vaccination did not show any influence on TSPOT results (22% vs 27%,P＞0.05).Conclusion BCG vaccina-tion affects the results of PPD test in patients with rheumatic diseases,but has no influence on TSPOT results.The infection rate of latent tuberculosis in patients with rheumatic diseases in our study is 23.8% detected by TSPOT.

Abstract:Objective To evaluate the application of enzyme-linked immunospot assay (T SPOT) in diagnosis of Mycobacterium tuberculosis (MTB) infection for AIDS patients. Methods The rapid T SPOT assay was employed to detect ESAT-6 and CFP-10 specific T cells in blood samples from 35 AIDS patients with active tuberculosis. The results were compared with those of PPD skin test. Results The positive-MTB rate with T SPOT assay in all patients (n = 35 ) was 65.71%, that in patients with CD4 < 200/μ (n = 21) was 66.67%, in patients with CD4 > 200/μl (n = 10) was 60.0% , in patients with anti-tuberculosis treatment < 2 months ( n = 15 ) was 83.33% and in patients with treatment > 2 months ( n = 6 ) was 25.00%. The positive rates of PPD skin test were 24.24%, 25.00%, 33.33%, 9.09% and 25.00% in these groups, respectively. The positive rates of T SPOT assay were significantly higher than those of PPD skin test in all patients, in those with CD4 < 200/μl and with anti-tuberculosis treatment < 2 months ( P < 0.05 ). Conclusion T SPOT assay was more sensitive than conventional PPD skin test, and can be applied in the diagnosis of MTB infections for AIDS patients.

Abstract:Enzyme-linked immunospot(ELISPOT)assay is used in the diagnosis of tuberculosis as a new immunology detection method.The main principle is application of mycobacterium tuberculosis specific antigen ESAT-6,CFP-10 to stimulate peripheral blood T lymphocytes,make T-cell activated and release γ-interferon,to determine whether there are tuberculosis according to activated T-cell number.This technology is with higher sensitivity and specificity than the TST experiment,for it is not affected by body immunity and BCG vaccination,and here is to make a review on ELISPOT technology principle,brief development history,operation procedure and the application.

Abstract:Objective. To determine the prevalence of latent tuberculosis infection (LTBI) in college students.Patients and methods. Four hundred and twenty newly admitted college students were enrolled. The Enzyme-Linked ImmunoSpot assay (ELISPOT) was used. Overall, 171 students with ELISPOT assay+/TST+ were monitored for three years to detect active TB disease.Results. The overall positive rate of ELISPOT assay was 40.7% among TST+ students. The ELISPOT positive rates were 36.9%, 45.4%, and 64.3% in groups of TST induration of 10-14 mm, 15-20 mm, and >= 20 mm, respectively, with a significant difference (chi(2) = 10.136, P<0.01) but no significant difference between BCG scar and no scar (41.2% vs. 38.8%; P>0.05). None of the 171 untreated students contracted active TB within the three-year monitoring period.Conclusion. The LTBI rate might be overestimated by TST compared with interferon-gamma release assays. On the basis of a close monitoring, few students developed active TB despite a positive result to the TST and ELISPOT assay. (C) 2016 Elsevier Masson SAS. All rights reserved.
[The Comparison of Different Antigens in Enzyme-Linked Immunospot Assay for the Diagnosis of Tuberculosis.].

Abstract:OBJECTIVE To compare different Mycobacterium tuberculosis antigens in enzyme-linked immunospot assay (ELISPOT) for the auxiliary diagnosis of active tuberculosis. METHODS The peripheral blood mononuclear cells (PBMC) from patients with tuberculosis and controls were co-cultured with the following antigens: purified protein derivative (PPD), early secretory antigenic target-6 (ESAT-6) and early secretory antigenic target-6/culture filtrate protein-10 fusion protein (ESAT-6/CFP-10). Spot forming cells (SFC) were enumerated by ELISPOT. RESULTS PPD-ELISPOT, E/C-ELISPOT and ESAT-6-ELISPOT showed significantly higher SFC counts in active tuberculosis [255(93-526), 148(40-354) and 28(10-116) respectively] as compared to the controls [10(5-41), 10(0-20) and 5(0-15) respectively], u values were 1479.5, 1390.5 and 2510.5 respectively, all P < 0.01. Compared with the sensitivity of ESAT-6-ELISPOT (62.1%), that of E/C-ELISPOT and PPD-ELISPOT was higher (90.3% and 84.8%), chi(2) = 17.496 and 28.541, all P < 0.01. Compared with the specificity of PPD-ELISPOT (68.9%), that of E/C-ELISPOT and ESAT-6-ELISPOT was also higher (84.4% and 88.9%), chi(2) = 6.807 and 10.808, P < 0.05 and P < 0.01 respectively. CONCLUSIONS E/C-ELISPOT is a promising approach to the auxiliary diagnosis of tuberculosis, but its specificity could be affected by latent tuberculosis infection.
Comparison of an in-house tuberculosis-specific IFN-γ release assays with T-SPOT TB in latent tuberculosis infection diagnosis among HIV-infected individuals

Abstract:Objective To evaluate the diagnostic value of two tuberculosis-specific IFN-γ release assays in latent tuberculosis infection among HIV-infected individuals. Methods The levels of tuberculosis antigen-specific IFN-γin 102 HIV patients from AIDS Outpatient Clinic of Shenzhen Third People's Hospital were detected by in-house tuberculosis-specific IFN-γ ELISpot assay and commercial T-SPOT TB kit, and tuberculin skin test (TST) were done at the same time. There were 66 males and 36 females,and the average age was 35. Results Seventeen HIV infected patients were positive in both IFN-γ ELISpot and T-SPOT TB methods, the sensitivity, specificity positive predictive value(PPV), negative predictive value(NPV) and compliance rates of ELISpot were 94. 4% ,94. 0% ,77. 3% ,98. 8% and 94. 1% ,respectively. Three patients were positive in both IFN-γELISpot and T-SPOT TB methods, the sensitivity, specificity, PPV, NPV and compliance rates of TST were 16. 7%, 98. 8%, 75.0%, 84. 7% and 84. 3%, respectively. The average number of spots using three kinds of antigen ESAT-6, Pool A,Pool B obtained were 26. 89 ±5. 77,18. 96 ±4. 75 and 14. 51 ± 3.77, respectively. Only ESAT-6 and Pool B have a statistically significant difference (H=7.557,P = 0.022 9), no significant difference was shown between other groups. There was no significant difference between the positive rate and the CD4+ T cellls number(x2 =0. 860 8 ,P =0. 650 2) ,as the same as the T-SPOT TB (x2 = 1. 396 4, P = 0. 497 5 ). Conclusions The performance of this in-house tuberculosis-specific IFN-γ ELISPot assay was comparable to T-SPOT assay in diagnosis of latent tuberculosis infection, and the sensitivity and specificity of both these two assays were all much higher than TST. They canbe recommended in diagnosing latent tuberculosis infection in HIV infected patients.

Abstract:Objective To compare the efficacy of the conventional PPD skin test and a new enzymelinked immunospot assay(TSPOT-TB)for diagnosing latent tuberculosis infection(LTBI)in patients with rheumatic diseases.Methods Two hundred and sixty rheumatic patients were enrolled,and all were screened for LTBI based on clinical history,chest X-ray,PPD skin test or TSPOT.Results The positive rate of TSPOT assay was 24.1%and that of PPD skin test was 39.4%.The overall concordance rate between the 2tests was 61.0%.Among PPD negative patients (n=149).29 were TSPOT(+)(19.5%).Among PPD(+)patients(n=98),69 were TSPOT(-)(70.0%).The patients who got BCG vaccination or had history of tuberculosis infection showed a significantly higher rate of positive result of PPD skin test than those who did not (P<0.05 or P<0.01).While in TSPOT assay,the BCG vaccination or history of tuberculosis infection did not show influence on TSPOT results(P>0.05).Of the 127 patients who received biological agents after screening for LTBI,9 patients were pretreated with isoniazide.Twenty-seven patients stopped biological agent treatment because of the positive results of PPD or TSPOT.Twenty three patients who had positive PPD but negative TSPOT results received biological agent treatment without isoniazide,and none of them developed active tubereulosis after 6 to 18 months of follow-up.Conclusion BCG vaccination affects the result of PPD test in rheumatic patients,but has no influence on TSPOT results.The infection rate of latent tuberculosis of rheumatic patients in our research is 23.8%detected by TSPOT.

Abstract:BACKGROUND:Tuberculosis (TB) is an infectious disease and its mortality rate ranks first. Latent tuberculosis infection (LTBI) means that a patient is infected with Mycobacterium tuberculosis, but has no relative clinical symptoms. It has been estimated that approximately 10% of patients with LTBI would develop into active tuberculosis. Therefore, it was urgent to search for more efficient biomarkers to discriminate LTBI from healthy population. METHODS:The Luminex assay was employed to detect the quantity of cytokines secreted by mononuclear cells from peripheral blood stimulated with the ESAT6 protein among TB, LTBI and healthy controls. The cytokine profile was analyzed by principal components analysis and the receiver operating characteristic curve analysis. RESULTS:The principal components analysis indicated that LTBI and TB were clearly separated from healthy controls, and that LTBI was also successfully differentiated from healthy controls. The cytokine profiling method to distinguish LTBI from healthy controls has a sensitivity and specificity of 100%. Nine potential biomarkers, including IL-23, IL-21, HGF, Bngf, IL-27, IL-31, IL-1β, IL-22 and IL-18, were identified, and these cytokines were considered as a potential cytokine complex for more effectively discriminating LTBI from healthy controls. CONCLUSION:IL-23, IL-21, HGF, Bngf, IL-27, IL-31, IL-1β, IL-22 and IL-18 were demonstrated to be the potential cytokine complex for the assessment between LTBI and healthy controls.
Diagnostic value of T cells spot test of tuberculosis infection on blood and cerebrospinal fluid for tuberculous meningitis

Abstract:Objective To evaluate the diagnostic value of T cells spot test of tuberculosis infection (T-SPOT .TB) on blood and cerebrospinal fluid for tuberculous meningitis (TBM ).Methods One hundred and fifteen adult patients with suspected tuberculous meningitis were retrospectively enrolled from March 2013 to March 2017 in Huashan Hospital affiliated to Fudan University .Among them ,30 were diagnosed with TBM (7 definite ,19 highly probable and 4 possible) ,37 with other infectious meningitis and 29 with non-infectious meningitis .The diagnostic sensitivity ,specificity ,positive predictive values (PPV) and negative predictive values (NPV) of T-SPOT .TB on peripheral mononuclear cells (PBMC) and cerebrospinal fluid mononuclear cells (CSF-MC ) were analyzed using Fisher exact test , and the diagnostic performance was evaluated by using receiver operating characteristic (ROC ) curve and area under the curve (AUC).Results When including the 30 TBM cases and 66 non-TBM cases into analysis , the sensitivities and specificities , PPV and NPV of PBMC and CSF-MC for diagnosing TBM were as follows :93 .1% and 66 .7%,77% and 87 .7%,65 .9% and 71 .4%,95 .9% and 85 .1%,respectively . When including the 30 TBM and 37 other infectious meningitis into analysis , the sensitivities and specificities ,PPV and NPV of the PBMC and CSF-MC for diagnosing TBM were as follows :93 .1% and 66.7%,68 .6% and 86 .5%,71.1% and 80 .0%,92 .3% and 76 .2%,respectively .By ROC curve analysis ,the AUC of blood and CSF were 0 .882 (95% CI :0 .795－0 .969) and 0 .814 (95% CI :0 .704－0 .925) ,respectively .Using a cut-off value of 32 spot forming cells (SFC) per million CSF-MC for T-SPOT .TB on CSF-MC showed a sensitivity of 66 .7%,a specificity of 91 .9%,PPV of 87 .0% and NPV of 77 .3% .The positive likelihood ratio and negative likelihood ratio were 8 .22 and 0 .363 respectively . Conclusions T-SPOT .TB on CSF-MC has a role in diagnosing TBM .And 32 SFC per million CSF-MC might be the optimal cut-off value to differentiate TBM and non-TBM .
Enzyme-linked Immunospot Assay Combined with Serum Latex Agglutination Test for Diagnosis of Pulmonary Tuberculosis and Concomitant Pulmonary Cryptococcosis

Abstract:Objective To evaluate the value of enzyme-linked immunospot assay(TB ELISPOT) combined with serum latex agglutination test(LA) for diagnosis of pulmonary tuberculosis plus pulmonary cryptococcosis.Methods Serum and biopsy specimens of 76 patients,who were suspected of pulmonary tuberculosis and/or pulmonary cryptococcosis based on clinical and imaging features,were collected from March 2006 to September 2008 in Shanghai Public Health Clinical Center.TB ELISPOT assay. LA and histopathological examination were performed in all the patients.Results Histopathological and pathogenic examination confirmed pulmonary cryptococcosis in 15 cases and pulmonary tuberculosis in 22 cases,pulmonary tuberculosis plus pulmonary cryptococcosis in 8 cases.The sensitivity and specificity of TB ELISPOT were 91%and 94.4%.The sensitivity and specificity of LA were both 100%.TB ELISPOT assay and LA test were both positive in the 8 cases of pulmonary tuberculosis plus pulmonary cryptococcosis.Conclusions The value of enzyme-linked immunospot assay combined with serum latex agglutination test is high for diagnosis of pulmonary tuberculosis plus pulmonary cryptococcosis.

Abstract:To explore the application of ELISPOT(Enzyme Linked Immunospot Assay) in the diagnosis of TB(tuberculosis) infection,TB-ELISPOT assays were performed in PBMC(peripheral blood mononuclear cell) isolated from patients who had already been clinically diagnosed as Mtb infection(54 cases) or non-TB infection(52 cases).Other diagnostic approaches,including sputum smear,sputum culture,PPD(Purified Protein Derivative Tuberculin) and DIGFA(dot immuno-gold filtration assay),were also employed for comparison of sensitivities and specificities.The data of TB-ELISPOT showed the sensitivity(Se) and specificity(Sp)were 90.7% and 96.15% respectively.The positive predictive value(PPV) and the negative predictive value(NPV) were 0.96 and 0.91 respectively.The positive likelihood ratio(+LR) and the negative likelihood ratio(-LR) were 23.55 and 0.1 respectively.The data of DIGFA was Se 59.26%,Sp 88.46%,PPV 0.84,NPV 0.68,+LR 5.14 and-LR 0.46.The data of PPD were Se 64.81%,Sp 82.69%,PPV 0.80,NPV 3.74,+LR 0.69 and-LR 0.43.The data of sputum smear were Se 40.74%,Sp 100.00%,PPV1.0,NPV 0.62,+LR10 and-LR 0.52.The data of sputum culture was Se 25.93%,Sp 100.00%,PPV 1.0,NPV 0.57,+LR10 and-LR 0.77.The sensitivity of TB-ELISPOT in sputum smear negative and sputum culture negative patients were 84.38% and 87.5% respectively.Results demonstrated that ELISPOT is a specific and sensitive approach in detecting Mtb active infection and latent Mtb infection.ELISPOT is a less time-consuming assay(usually 24-30 h) and could be useful in TB especially in sputum negative pulmonary tuberculosis diagnosis.

Abstract:Tuberculosis (TB) is still a huge burden of health in the world.It is not satisfactory to diagnose TB with the traditional diagnostic methods.T-SPOT.TB diagnostic test uses the early secretory antigenic target-6 and culture filtration protein-10 as antigen. It has been confirmed that T-SPOT.TB diagnostic test performs better than traditional diagnostic methods in the diagnosis of latent tuberculosis infection and active tuberculosis.T-SPOT.TB diagnostic test also has a potential in assessing the prognosis of tuberculosis. With the help of further study,it will play a more important role in TB diagnosis and treatment in the new century.

Abstract:Objective To evaluate the clinical application of a new enzyme-linked immunospot assay(T-SPOT.TB assay) for the diagnosis of tuberculosis.Methods The tuberculin skin test(TST) and T-SPOT.TB assay were performed in blood samples in 13 intestinal tuberculosis patients,20 the Crohn's disease patients and 20 colon polyps patients.Results Among the three kinds of patients,the positive results of TXT were 7 cases,3 cases and 3 cases,which were 12 cases.5 cases and 4 cases of T-SPOT. TB.The sensitivity and specificity of TST were 53.85%(7/13) and 85%(34/40),which were 92.31%(12/13) and 77.5%(31/40) by T-SPOT.TB assay.The significant difference was found(P<0.01).Conclusions T-SPOT.TB assay is an effective way for the diagnosis of intestinal tuberculosis.
Investigation of the Latent Mycobacterium Tuberculosis Infections in Different Populations in Shenzhen Area Using a Tuberculosis Antigen-Specific IFN-γ Elispot Assay.

Abstract:Objective To investigate the latent Mycobacterium tuberculosis infection in different populations in Shenzhen area using the established method of tuberculosis-specific antigen-specific IFN-γ Elispot assay for diagnosis of tuberculosis infections at early stage.Methods The levels of the tuberculosis antigen-specific IFN-γ(interferon gamma) in 5 different populations including 34 college students in Shenzhen City,18 medical staffs working at tuberculosis ward or outpatients department of Shenzhen Hospitals,19 IT(information telecom) crews of some Shenzhen Company,36 member staffs of some Shenzhen Biotechnology Company,29 security guards of Shenzhen Company for logistics service were determined by Elispot assay.PPD Results were in all different groups as a control were analyzed.Results The positive rates for IFN-γ detection in 5 groups by Elispot assay were as follows: 5.88% in college students,55.6% in medical staffs at TB ward and outpatient department of some Shenzhen Hospital,31.57% in IT company staffs,31.03%,in safety guards,and 19.44% in staffs of Shenzhen biotechnology,respectively;PPD(purified protein derivative) tests results were: college 20.58% in students,88.90% in medical staffs int TB ward and outpatient department 42.11% in IT staffs,41.38% in safety guards staffs and 41.67% in Shenzhen Biotechnolog. Conclusion Tuberculosis-specific antigen-specific IFN-γ Elispot assay is spurerior to PPD test for early diagnosis of tuberculosis infections at early stage.
ELISpot in the diagnosis of tuberculous meningitis

Abstract:Objective To evaluate the diagnostic value of enzyme-linked immunospot(ELISpot) assay in diagnosing tuberculous meningitis.Methods Twenty-six patients suspected of tuberculous meningitis were enrolled in this study.ELISpot assay was employed to detect ESAT-6 and CFP-10 specific IFN-γ-secreting T cells in blood samples.Results According to diagnostic criteria,15 cases were confirmed to suffer from tuberculous meningitis,and 11 were diagnosed with nontuberculous mycobacterial infection of central nervous system.The sensitivity and specificity of ELISpot assay were 80.00% [95% confidence interval(CI) 51.9%～95.7%] and 100%(95% CI 71.5%～100.0%);the positive predictive value and negative predictive value of ELISpot assay were 100%(95% CI 73.5%～100.0%) and 78.57%(95% CI 49.2%～95.3%),respectively.Conclusion ELISpot assay is of great value in diagnosing tuberculous meningitis.

Abstract:Background: The clinical challenge of differentiating suspected tuberculosis with positive T-SPOT.TB results persist. This study aims to investigate the utility of the Systemic Immune-Inflammation Index (SII), Fibrinogen, and T-SPOT.TB in distinguishing between active pulmonary tuberculosis (PTB) and non-tuberculous lung diseases. Methods: A retrospective analysis included 1,327 cases of active PTB with positive T-SPOT.TB results and 703 cases of non-tuberculous lung diseases from May 2016 to December 2020 at Meizhou People's Hospital. These were designated as the case group and the control group, respectively. The detection indicators of T-SPOT.TB: Early Secreted Antigenic Target 6 (ESAT-6), Culture Filtrate Protein 10 (CFP-10), as well as SII and Fibrinogen levels—were compared and analyzed for association and joint diagnostic value between the two groups. Results: The case group showed higher values of ESAT-6, CFP-10, SII, and Fibrinogen compared to the control group (all p < 0.001). In the case group, SII and Fibrinogen did not correlate with ESAT-6 and CFP-10 (∣rs∣ all 0.3). SII and Fibrinogen values in smear-positive pulmonary tuberculosis were higher than in smear-negative cases (all p < 0.05). The optimal diagnostic thresholds for ESAT-6, CFP-10, SII, and Fibrinogen in differentiating between active PTB and non-tuberculous lung diseases were 21.50 SFCs/10 6 PBMC, 22.50 SFCs/10 6 PBMC, 2128.32, and 5.02 g/L, respectively. Regression logistic analysis showed that ESAT-6 < 21.5 (OR: 1.637, 95% CI: 1.311–2.043, p < 0.001), CFP-10 < 22.5 (OR: 3.918, 95% CI: 3.138–4.892, p = 0.025), SII < 2128.32 (OR: 0.763, 95% CI: 0.603–0.967, p < 0.001), and FIB < 5.02 (OR: 2.287, 95% CI: 1.865–2.806, p < 0.001) were independent risk factors for active PTB. The specificity for ESAT-6 + CFP-10, ESAT-6 + CFP-10 + SII, ESAT-6 + CFP-10 + FIB, and ESAT-6 + CFP-10 + SII + FIB was 82.5%, 83.2%, 95.8%, and 80.1%, respectively, while sensitivity was 52.6%, 53.0%, 55.8%, and 44.7%, and positive predictive values were 85.0%, 85.6%, 84.1%, and 89.6%, respectively. Conclusion: SII and Fibrinogen are positively correlated with the degree of tuberculosis inflammation and the bacterial load of Mycobacterium tuberculosis . The combined detection of SII, Fibrinogen, and T-SPOT.TB is significant in distinguishing between active PTB with positive T-SPOT.TB results and non-tuberculous lung diseases.

Abstract:Abstract Background To establish the Mycobacterium tuberculosis (MTB) specific IFN-γ/IL-2/TNF-α-FluoroSpot assay, and preliminarily evaluate its accuracy of differential diagnosis of active tuberculosis (ATB) and latent tuberculosis infection (LTBI). Methods Patients with pathologically confirmed and clinically diagnosed ATB in Peking Union Medical College Hospital and Beijing Chest Hospital from April 2020 to May 2021 were enrolled as case group, while patients with LTBI in the same period were enrolled as control group. The FluoroSpot assay was used to simultaneously detect the secretion of IFN-γ, IL-2 and TNF-α in T cells stimulated by the MTB specific antigens ESAT-6 and CFP-10 at the single-cell level. A binary logistic regression model was used to fit the combined diagnostic parameters, and the sensitivity, specificity, predictive value and likelihood ratio of the differential diagnosis of ATB and LTBI were calculated. Results 62 patients with ATB (37 pathogen-confirmed ATB, 25 clinical diagnosed ATB), 87 patients with LTBI were included. There was significant correlation of the frequencies of total IFN-γ-secreting T cells detected by IFN-γ/IL-2/TNF-α-FluoroSpot assay compared with T-SPOT.TB after stimulation of MTB-specific antigen (r=0.829 for ESAT-6, P< 0.001, r=0.804 for CFP-10, P< 0.001). ROC curve was drawn for both T-SPOT.TB and Fluorospot. For T-SPOT.TB, the AUROC was 0.669 (95%CI 0.574-0.765), the sensitivity and specificity of differentiating ATB from LTBI were 70.97% (95%CI 58.05%-81.80%) and 56.32% (95CI 45.26%-66.94%) respectively. While for Fluorospot, the AUROC was 0.906 (95 CI 0.856-0.957), the sensitivity and specificity of differentiating ATB from LTBI were 80.65% (95%CI 68.63% - 89.58%) and 88.51% (95%CI 79.88% - 94.35%) respectively.
