Study on Tonic Accommodation in Early Onset and Late Onset Myopes

Abstract:Objective Previous studies have shown that a relationship exists between tonic accommodation and myopia. It was once thought that myopes have low tonic accommodation. This study investigated TA in late onset and early onset myopia. Their differences and parasympathetic innervation mechanisms were analyzed.Methods Fifty volunteers were enrolled in this study: 16 emmetropes (-0.25~+0.75 D), and 24 myopes, which included 12 early onset myopes [mean RE (-6.25±2.30)D] and 22 late onset myopes [mean RE (-3.41±2.05)D]. Tonic accommodation was measured under dark field conditions using a modified objective infrared autorefractor.Results Eyes were dark adapted in a resting position in the absence of visual stimulation and showed a mean value of (0.63±0.67)D for tonic accommodation. There was no direct linear relationship between the degree of refractive error and tonic accommodation. However, tonic accommodation for early onset myopia was significantly higher than that for emmetropia and late onset myopia.Conclusion Compared to emmetropia and late onset myopia, early onset myopia has a greater amount of parasympathetic innervation to the ciliary muscle that induces stretching and causes the eyes to become myopic.
