Abstract:OBJECTIVES:Dysphagia is common in patients with acute ischemic stroke patients (AIS) and it increases the risk of aspiration pneumonia after AIS. The volume-viscosity swallow test (V-VST) is a useful dysphagia screening tool. Therefore, we aim to explore the association between dysphagia (using the modified V-VST) and aspiration pneumonia in acute ischemic stroke patients.PATIENTS AND METHODS:All patients with acute ischemic stroke (AIS) admitted during 1 January 2018 and 31 December 2018 were screened in this study by completing the modified V-VST within 24 hours after admission. The information of aspiration pneumonia was obtained from electronic medical system (EMS) and discharge notes.RESULTS:Among 624 AIS patient, there were 152(24.36%) patients who suffered from aspiration pneumonia (95% CI, 20.65% - 27.35%) during hospitalization. Among all of them, patients with both impaired safety and impaired efficacy (HR, 7.53, 95%CI, 3.42-16.58, P < 0.001)) had a higher risk of aspiration pneumonia than those with only safety impaired (HR, 2.38, 95%CI, 1.40-4.04, P < 0.001) or only efficacy impaired (HR, 2.47, 95%CI, 1.33-4.57, P = 0.004). The risk of aspiration pneumonia was also associated with their completed volume at each viscosity. The patients with all impairment of three viscosities were also found to be related to the higher risk of developing aspiration pneumonia (P = 0.029).CONCLUSIONS:The Modified V-VST is an easy-hand on, reliable screening tool for dysphagia among patients with AIS. Patient subgroup analyses based on the modified V-VST might be able to predict the risk of aspiration pneumonia during hospitalization.

Abstract:BackgroundDysphagia is a common complication after stroke. Water swallowing test (WST) is a recognised but limited tool in providing details about dysphagia, including severity and how to adjust the diet based on the test results.MethodsWe performed a prospective observational study of comparing WST and volume–viscosity swallow test (V-VST) in patients with acute stroke within 14 days. All patients had WST and if failed would have a V-VST. The primary outcome was to compare the dysphagia levels assessed by these two test tools. The secondary outcome was to explore the predictive capability in patients who were at high risk of pneumonia by these two swallowing tests.ResultsConsecutively 276 patients with stroke were enrolled in our study, and 197 had normal WST. Among 79 patients who had both WST and V-VST, 20 showed swallowing safety and effectiveness by V-VST. The chance of being on tube feeding was strongly related to the positive results of failed WST (p<0.001). Both tests showed good predictive ability in patients with stroke for pneumonia even some of them were placed on tube feeding (p=0.001 in WST and p<0.001 in V-VST).ConclusionsV-VST performed better as a clinical screening test for dysphagia in patients with acute stroke at the bedside.Trial registration numberChiCTR1800016442.

Abstract:Objective: This paper focuses on the research and discussion of the efficacy of electromyographic biofeedback combined with swallowing training on post-stroke dysphagia. Methods: This study randomly sampled and analyzed 68 patients with post-stroke dysphagia from January 2023 to December 2023, 34 cases of swallowing training intervention were grouped as the control group, and 34 cases of electromyography biofeedback combined with swallowing training intervention were grouped as the study group, and the clinical therapeutic effects of the two groups of patients after receiving the two different modes of intervention were compared. Results: The swallowing function of patients in both groups improved, and the VFSS score of patients in the seminar group was significantly higher than that of the control group, indicating that the clinical efficacy of the seminar group was more significant. The nasal feeding tube extraction rate, extraction time, and quality of life scores of the seminar group were better than those of the control group (P < 0.05), which is of research value. Conclusion: For patients with post-stroke dysphagia, treatment with electromyography biofeedback combined with swallowing training mode can significantly improve their swallowing function. This effective intervention can not only shorten the time for patients to remove the nasal feeding tube but also help to improve the quality of life of patients, which is worth using.

Abstract:Background: Dysphagia is a common complication after stroke, which could lead to aspiration. Early and reliable bedside detection and intervention of dysphagia is necessary to prevent dysphagia-related aspiration pneumonia. Compared to water swallow test (WST) , modified volume-viscosity swallow test(V-VST) provides more detail information of management. To predict the risk of aspiration pneumonia from dysphagia tool in acute stroke patients, we developed modified V-VST and validated its predictability of risk of aspiration pneumonia. Methods: This was a prospective observational study enrolled acute stroke within 14 days. Only patients with Glasgow Coma Scale score higher than 10 were included into analyze. All patients had bedside WST and if failed would have a V-VST after admission was documented. Modified V-VST assessment, includes tongue movement to V-VST, including tongue rolling, extending to front, left and right. The primary outcome was the rate of pneumonia, which was diagnosed by either chest CT or chest X-ray. Multiple logistic regression models were performed in patients who failed the tests. The area under curves (AUCs) were compared among all tests. Results: From 7/2017-6/2018, 562 hospital-based acute stroke patients were enrolled. The overall incidence of pneumonia after acute stroke was 15.9 % (95%CI 13.0-19.2%). Of 157 patients who failed WST, 101 had a positive V-VST, and 173 had a positive modified VVST. The modified VVST was the most powerful to predict the risk of pneumonia (OR 6.156 in WST, 3.058 in VVST, 18.076 in modified VVST, P Conclusions: Bedside modified V-VST is an easy and sensitive tool to predict post-stroke aspiration pneumonia. A large prospective interventional study is needed to confirm our findings.

Abstract:Background: Dysphagia is a common and clinically significant complication of ischemic stroke. The prevalence and risk factors for dysphagia may vary at different stages following an ischemic stroke. Methods: This study included patients with acute and chronic ischemic stroke who were treated at the Department of Rehabilitation, First Affiliated Hospital, Zhejiang University School of Medicine from 2019 to 2022. Various demographic, clinical, and laboratory parameters were collected, and statistical analyses were performed to investigate their association with dysphagia. Results: Among the 399 ischemic stroke patients included in the study, 165 (41.4 %) experienced dysphagia, with 72 (38.7 %) in the acute phase and 93 (43.7 %) in the chronic phase. Univariate analysis revealed significant associations (p < 0.05) between dysphagia and factors such as pulmonary infection, aphasia, NIHSS score, ADL score, NLR score, lower extremity Brunnstrom's stages, and sit-to-stand balance. Multiple logistic regression analysis, after adjusting for confounding factors, identified the ADL score as an independent predictor of dysphagia. These findings were consistent across three time-windows: the acute phase, the chronic phase, and 180 days after stroke onset. Additionally, the lymphocyte count and pulmonary infection were identified as potential independent indicators. Conclusions: This study investigated the prevalence and risk factors for dysphagia in ischemic stroke patients at different time-windows. A low ADL score (<40) may serve as a valuable and reliable predictor for poststroke dysphagia in clinical settings.

Abstract:Objective: The purpose of this study was to compare the clinical signs in patients with acute ischemic stroke(AIS) and identify which of them was associated with the severity of dysphagia. Methods: This was a prospective observational study enrolled AIS within 14 onset days. All patients had swallowing evaluations by the modified Volume Viscosity Swallowing Test (the modified V-VST). Five clinical signs suggestive of swallow function impaired were directly observed in the patients who failed the test. We compared the performance of clinical signs between different completing volumes at three viscosity series. The area under curves (AUCs) were made to show the ability of specific clinical signs in predicting the dysphagia severity in patients with AIS. Results: 184 hospital-based AIS patients who failed the modified V-VST were enrolled from June 2017 to December 2019. 123 patients were identified as moderate swallow function impaired and 61 patients were identified as serious. Larynx movement and tongue movement were significant different clinical signs at all of three viscosities (p≤0.001). The AUC of larynx movement in predicting severity of dysphagia was 0.733( 95% CI 0.658-0.808,P<0.001). Conclusions: Larynx movement and tongue movement were easy clinical signs for medical staffs to assess the dysphagia severity quickly. Additionally, larynx movement showed stronger ability to predict the severity of dysphagia in AIS patients.

Abstract:Objective To explore the effect of feeding management based on the volume-viscosity swallow test for dysphagia patients with neurological diseases.Methods A quasi-experimental research design was utilized, and the dysphagia patients with neurological diseases were selected and divided into a control group (n=47) and an observation group (n=61) chronologically. The control group received routine nursing, the patients with grade 2 in Water swallow test were given dietary guidance, and the patients with grade 3 or above were indwelled with gastric tube and were given tube feeding, while the observation group were additionally assessed for the risk of dysphagia according to the volume-viscosity swallow test, then received individualized feeding based on the assessment results. Results The implementation rate of tube feeding in the observation group was significantly lower than that of the control group (24.59％ vs.44.68％, P<0.05), the tube retention time was significantly shorter than that of the control group (10.13 days vs.14.05 days, P<0.05), and the incidence of aspiration signs for the patients with oral feeding was significantly lower than that of the control group (17.39％ vs. 42.13％, P<0.05).Conclusion The feeding management based on the volume-viscosity swallow test could provide a precise feeding plan for dysphagia patients, decrease unnecessary tube placement, shorten tube feeding time, and ensure feeding safety.

Abstract:OBJECTIVE: To investigate the characteristics and risk factors of dysphagia using the videofluoroscopic dysphagia scale (VDS) with a videofluoroscopic swallowing study (VFSS) in patients with acute cerebral infarctions.METHODS: In this retrospective study, the baseline VFSS in 275 stroke patients was analyzed. We divided patients into 8 groups according to lesion areas commonly observed on brain magnetic resonance imaging. Dysphagia characteristics and severity were evaluated using the VDS. We also analyzed the relationship between clinical and functional parameters based on medical records and VDS scores.RESULTS: In comparison studies of lesions associated with swallowing dysfunction, several groups with significant differences were identified. Apraxia was more closely associated with cortical middle cerebral artery territory lesions. Vallecular and pyriform sinus residue was more common with lesions in the medulla or pons. In addition, the results for the Korean version of the Modified Barthel Index (K-MBI), a functional assessment tool, corresponded to those in the quantitative evaluation of swallowing dysfunctions.CONCLUSION: A large cohort of patients with cerebral infarction was evaluated to determine the association between brain lesions and swallowing dysfunction. The results can be used to establish a specific treatment plan. In addition, the characteristic factors associated with swallowing dysfunctions were also confirmed.

Abstract:Introduction: The increasing incidence of stroke globally has led to dysphagia becoming one of the most common complications in stroke patients, with significant impacts on patient outcomes. Accurate early screening for dysphagia is crucial to avoid complications and improve patient quality of life. Methods: Included studies involved stroke-diagnosed patients assessed for dysphagia using bedside screening tools. Data was sourced from Embase, PubMed, Web of Science, Scopus, and CINAHL, including publications up to 10 December 2023. The study employed both fixed-effect and random-effects models to analyze sensitivity, specificity, positive predictive value (PPV), and Negative Predictive Value (NPV), each with 95% confidence intervals. The random-effects model was particularly utilized due to observed heterogeneity in study data. Results: From 6,979 records, 21 studies met the inclusion criteria, involving 3,314 participants from 10 countries. The analysis included six assessment tools: GUSS, MASA, V-VST, BSST, WST, and DNTA, compared against gold-standard methods VFSS and FEES. GUSS, MASA, and V-VST showed the highest reliability, with sensitivity and specificity rates of 92% and 85% for GUSS, 89% and 83% for MASA, respectively. Heterogeneity among studies was minimal, and publication bias was low, enhancing the credibility of the findings. Conclusion: Our network meta-analysis underscores the effectiveness of GUSS, MASA, and V-VST in dysphagia screening for stroke patients, with high sensitivity and specificity making them suitable for diverse clinical settings. BSST and WST, with lower diagnostic accuracy, require more selective use. Future research should integrate patient-specific outcomes and standardize methodologies to enhance dysphagia screening tools, ultimately improving patient care and reducing complications. Abstract:Objective: Dysphagia is common in patients with acute ischemic stroke patients (AIS). Feeding tubes are often placed to prevent aspiration pneumonia and supply nutrition when swallow function impaired are detected. However, little is known about when and how to start oral feeding in AIS. The purpose of this study is to establish a feasibility and effective intervention for early oral feeding following removal of feeding tube in patients with AIS. Methods: A total of 37 patients with AIS who were removed feeding tubes and start oral feeding in our hospital from 2017 to 2021 were retrospectively analyzed. An intervention program for early oral feeding, which consisted of the modified volume-viscosity swallow test (V-VST) and mechanical soft diet, was fully operationalized from October 2020 to July 2021. Outcomes were compared between an early oral feeding intervention group of 17 patients and a historical control group of 20 patients from July 2017 to September 2020. We compared the baseline characteristics between the groups. To analyze aspiration pneumonia probability, the Kaplan-Meier method were used. Results: The length of stay in hospital was significantly longer in the control group compared with the early oral feeding group (16.70±5.24 vs 12.88±4.46; P=0.024). Patients in the control group tolerated tube feeding longer days ( 9.45±2.93 vs 6.59±3.57; P=0.011 ). A log-rank test found no significant difference in aspiration pneumonia between the two groups ( P =0.111). Conclusions: Our data suggest that the intervention for early oral feeding can be used safely and possibly.
Assessment of the effects of dysphagia therapy in patients in the early post-stroke period: a randomised controlled trial

Abstract:Aim: This study aimed to evaluate the effects of dysphagia therapy in patients in the early post-stroke period. Clinical rationale: Dysphagia can be one of the consequences of stroke. This problem often leads to complications in the early stages of stroke, including aspiration pneumonia. Although individual reports on dysphagia exist in the literature, no comprehensive guidelines for dysphagia therapy are available. Material and methods: This randomised controlled trial included 60 patients with swallowing difficulties after ischaemic stroke (30 each in the study and control groups; age range: 55-65 years) who were admitted to a hospital stroke subunit. Patient rehabilitation period covered 15 days (seven days a week), with therapy for the first 10 days provided during patient hospitalisation and then outpatient physiotherapy during the subsequent five days. The procedure, including providing education about safe food and liquid consumption to patients and their caregivers, was performed in both patient groups. An original dysphagia treatment method was employed in the study group. Statistical average, standard deviation, and statistical error before and after therapy were determined in the study and control groups. The significance of differences in results between the study and control groups was assessed using the Mann-Whitney U test. Results: The applied therapy for dysphagia improved the swallowing function and selected motor functions of patients after stroke. In the study group, ineffective (delayed) and absent swallowing reflex was observed in 13 patients (44%) before therapy and in one patient (3%) after therapy. The rate of effective swallowing reflex increased from 57% to 97%. Statistically significant differences in swallowing reflex were observed between the study and control groups after therapy (p = 0.00001). Conclusions: A comprehensive therapy for dysphagia is effective and can reduce serious complications of swallowing disorders in clinical practice. Clinical implications: The results of this study could improve the clinical treatment of dysphagia.

Abstract:Background/Objectives: The impact of stroke location and volume on the development of post-stroke dysphagia is not fully understood. The aim of this study is to evaluate the relationship between acute ischemic lesions and the severity of dysphagia. Methods: Brain MRIs were obtained with a 1.5 Tesla MRI system (Magnetom Avanto B13, Siemens, Erlangen, Germany). The brain MRI protocol included axial echo planar diffusion-weighted imaging (DWI). The acute ischemic volume was obtained using DWI by drawing regions of interest (ROIs). The diagnosis and assessment of the severity of dysphagia was carried out by a multidisciplinary team and included the Dysphagia Outcome and Severity Scale (DOSS), the Penetration–Aspiration Scale (PAS), and the Pooling score (P-score). The threshold for statistical significance was set at 5%. Results: Among all the patients enrolled (n = 64), 28 (43.8%) were males and 36 (56.2%) were females, with a mean age of 78.8 years. Thirty-three (51.6%) of them had mild dysphagia and thirty-one (48.4%) had moderate–severe dysphagia. The total ischemic volume was negatively correlated with the DOSS (r = −0.441, p = 0.0003) and positively with the P-score (rs = 0.3054, p = 0.0328). Conclusions: There are significant associations between the severity of dysphagia and the quantitative DWI-based data of the acute ischemic volume and anatomical location.

Samples were selected by using purposive sampling technique and samples were assigned into the experimental group(N=30) and the control group (N=30). The samples are selected by using Gugging swallowing screen scale to pre-test assessment both experimental and control group. The experimental group receivedthe swallowing exercises for 10 minutes per day before breakfast in the morning for five consecutive days. The control group received hospital routine care. The post assessments were done by using Gugging Swallowing Screen at the end of 5th day of intervention to both the experimental and control group. Results:Out of 30 samples in experimental group the pretest mean score of dysphagia among dysphagia was 14.0±3.21 and the posttest mean score was 17.0±3.25. The mean difference score was 3.0. Abstract:Objective: Swallowing examination is crucial in patients with dysphagia. We aimed to compare qualitative and quantitative videofluoroscopic swallowing study (VFSS) results to provide reference for standardizing quantitative parameters. Materials and methods: In total, 117 patients with dysphagia were included, 38 with Parkinson's disease and 39 and 40 in convalescence following cerebral hemorrhage and infarction. VFSS was both qualitatively and quantitatively analyzed. Results: A significant difference of Oral transit time was found between the oral motor function grades ( p < 0.001), also was swallowing reaction times found between swallowing reaction duration grades ( p < 0.001), and soft palate lift duration between the soft palate lift grades ( p < 0.001). Superior hyoid bone movement ( p < 0.001), anterior hyoid bone movement ( p < 0.001), hyoid pause time ( p < 0.001), and hyoid movement duration ( p = 0.032) had significant differences between the hyoid laryngeal complex movement grades, as did the pharyngeal cavity transit time among the cricopharyngeal muscle opening duration grades ( p < 0.001). The laryngeal vestibule closure duration differed among the glottic closure grades ( p < 0.001). No statistically significant difference in upper esophageal sphincter opening diameter ( p = 0.682) or duration ( p = 0.682) among the cyclopharyngeal muscle opening duration grades. The pharyngeal area at rest did not significantly differ among the different vallecular residue ( p = 0.202) and pyriform sinus residue ( p = 0.116) grades. Conclusion: Several quantitative parameters can reflect the swallowing assessment process well. Further optimization of quantitative parameters is recommended.

Abstract:Objective: To evaluate the efficacy of adjuvant electrical stimulation for patients with dysphagia during acute phase of ischemic stroke. Methods: Twenty-three patients with acute ischemic stroke complicated with dysphagia were divided into a control group (n = 10) and a treatment group (n = 13). The control group only received conventional treatment, and the treatment group received adjuvant electrical stimulation on the basis of conventional treatment. The efficacy evaluation was conducted by using Watian water swallowing test, visual assessing scale (VAS) score, functional oral intake scale (FOIS), National Institute of Health Stroke Scale (NIHSS) score, and modified Rankin scale (mRS) score. Results: Circled digit oneWatian water swallowing test, VAS score, and NIHSS score were all decreased after the treatment in the two groups. There were statistical significances (P <0.05) ; FOIS score were increased after the treatment in both groups. There was no statistical significance (P <0.05); although the FOIS score was increased in the control group, there was no statistical significance (P >0.05). Circled digit twoAfter comparing the different values of each score before and after the treatment in the treatment group with those in the control group, there were no statistical significances. Circled digit threeThe patients were followed up for mean 8.7 months, the swallowing function almost returned to normal in both groups. There was no statistical significance in FOIS and mRS scores between the treatment group and the control group. Conclusion: The swallowing function of the patients was improved significantly after being treated with the two treatment modalities, however the adjuvant electrical stimulation did not have more benefits than the conventional treatment.

Abstract:Background: Patients with stroke frequently experience dysphagia. One of the stroke sequel that poses the greatest risk toone's life is post-stroke dysphagia, which can result in negative outcomes like pneumonia, starvation, dehydration, and even death.Guidelines for treatment of post stroke dysphagia include a variety of physical therapy and precautions to prevent complicationsrelated to dysphagia. Swallowing exercises for post-stroke dysphagic patients appear to be a secure and efficient treatment that canhave a substantial influence on patients' clinical outcomes. Aim: the aim of this study is to investigate the effect of swallowingexercises on improving dysphagia among stroke patients in neurocritical care unit. Method: the study has a quasi-experimentalresearch design with a convenience sample of 76 adult critically ill patients who admitted to neurocritical care unit of a NewGeneral Mansoura Hospital in Egypt. Data were collected using patient’s dysphagia assessment tool and 30ml water, swallowingtest tool. Results: there was a statistically significant increase in the average total score of dysphagia screening questionnaire afteswallowing training in comparison to its value before training program (P<0.001). Conclusion and recommendation: swallowingexercises is an effective method for improving post stroke dysphagia, therefore this intervention should be included in theroutinaly nursing care for post stroke patient. Further large scale studies are required to strengthen the evidence to use this programin dysphagia rehabilitation.
Dysphagia screening post-stroke: systematic review

Abstract:Background/scope: Stroke is one of the main causes of death, especially when associated with dysphagia. Hence, the assessment of nutritional status and aspiration risk is important to improve clinical outcomes. The aim of this systematic review is to identify which are the most suitable dysphagia screening tools in chronic post-stroke patients. Methodology: A systematic literature search was conducted for articles published from 1 January 2000 to 30 November 2022 in the Cochrane Library, PubMed, Embase, CINAHL, Scopus and Web of Science databases, including primary studies providing quantitative or qualitative data. Additionally, a manual search was conducted scanning the reference lists of relevant articles and Google Scholar was searched to retrieve additional records. The process of screening, selection and inclusion of the articles, as well as the assessment of risk of bias and methodological quality, were conducted by two reviewers. Results: Out of the 3672 records identified, we included 10 studies, mostly (n=9) cross-sectional, evaluating screening for dysphagia in 1653 chronic post-stroke patients. Volume-Viscosity Swallow Test was the only test applied in multiple studies with adequate sample size, demonstrating high diagnostic accuracy (sensitivity=96.6%-88.2%; specificity=83.3%-71.4%) compared with the videofluoroscopic swallowing study. Conclusions: Dysphagia is an important complication in chronic post-stroke patients. Early identification of this condition through screening tools with adequate diagnostic accuracy is of paramount importance. The limited number of studies available and their small sample sizes may be a limitation to this study. Prospero registration number: CRD42022372303.

Abstract:BACKGROUND Dysphagia, or swallowing disorder, is a common complication following stroke, significantly impacting patients' quality of life. Electromyographic biofeedback (EMGBF) therapy has emerged as a potential rehabilitation technique to improve swallowing function, but its efficacy in comparison with conventional treatments remains to be further explored. AIM To investigate the effects of different treatment intensities of EMGBF on swallowing function and motor speed after stroke. METHODS The participants were divided into three groups, all of which received routine neurological drug therapy and motor function rehabilitation training. On the basis of routine swallowing disorder training, the EMGBF group received additional EMGBF training, while the enhanced EMGBF group received two additional training sessions. Four weeks before and after treatment, the degree of swallowing disorder was evaluated using the degree of swallowing disorder score (VGF) and the Rosenbek penetration-aspiration scale (PAS). RESULTS Initially, there was no significant difference in VGF and PAS scores among the groups (P > 0.05). After four weeks, all groups showed significant improvement in both VGF scores and PAS scores. Furthermore, the standardized swallowing assessment and videofluoroscopic dysphagia scale scores also improved significantly post-treatment, indicating enhanced swallowing function and motor function of the hyoid-bone laryngeal complex, particularly in the intensive EMGBF group. CONCLUSION EMGBF training is more effective than traditional swallowing training in improving swallowing function and the movement rate of the hyoid laryngeal complex in patients with post-stroke dysphagia.

Abstract:Objectives: In the current era, clinicians working in areas with limited and/or without facilities are only able to manage their patients based on clinical signs to detect dysphagia. This study assesses the performance of 5 simple clinical signs for predicting the severity of dysphagia. Methods: We systematically documented 5 targeted bedside clinical signs within 24 h after admission in 737 consecutive patients with acute stroke. We examined the median onset of each sign and calculated their ratios associated with severe dysphagia. The performance and significance of these special clinical signs were evaluated by further computation. Results: In total, 184 of 737 AIS patients were screened positive dysphagia by the modified V-VST. The 5 targeted bedside clinical signs were differed among the patients with and without dysphagia. Patients with serious dysphagia (n = 61,33.15%) showed higher positive percentages of larynx movement disorders than those classified as moderate (73.77% [60.99%-83.50%] vs 29.27% [21.82%-38.03%], p < 0.001). Logistic regression analyses conducted in the subsets confirmed larynx movement disorders as an independent predictor of dysphagia severity. Conclusions: Larynx movement disorders is correlated with a higher probability of severe dysphagia as measured by the modified V-VST in AIS patients.
