Abstract:Objective To investigate the analgesic effect of parecoxib sodium and oxycodone for postoperative pain after total knee arthroplasty. Methods Sixty patients undergoing elective TKA were randomly divided into 2 groups( n =30 each) : parecoxib group( group A),parecoxib and oxycodone group( group B). All the patients were received parecoxib sodium for analgesia at the first 24h after surgery. From then on,if the visual analogue score( VAS) > 5 the supplements( parecoxib sodium,oxycodone) would be infused in turn till the patient felt comfortable or the analgesic effect reached the clinical maximum. The VAS and Ramsay sedation score( RSS) were recorded at 24,32, 40,48,and 72 hours after surgery. Adverse effects related to analgesia and the maximum tolerable degree of passive exercise were investigated and compared between the two groups. Results Compared with group A,the resting and exercising VAS was decreased in group B at 24,32,40,48,and 72 hours after surgery,while the sedation score increased( P < 0. 05). And the maximum tolerable degree of passive exercise was higher in group B at the 48 and 72 hours after surgery( P < 0. 05). No serious adverse effects occurred between the two groups. Conclusions Intravenous injection of parecoxib sodium combined with oral oxycodone is consistent trend of multimodal analgesia. It can realize the basic goal of postoperative analgesia in patients undergoing total knee arthroplasty.

Abstract:Objective To evaluate the effect and safely of a multimodal analgesic program in patients undergoing total knee arthroplasty(TKA).Methods Eighty-two patients undergoing knee arthroplasty under general anesthesia were randomized into two analgesic groups,pain intensity and complications were assessed.Results Obvious pain relieve were observed in both groups with perfect satisfaction and lower complication rate.Conclusions Multimodal analgesic program is of great importance to early re habilitation and patient's satisfaction.

Abstract:Total knee arthroplasty has become one of the effective operation methods on end-stage knee osteoarthritis. However,the postoperative pain has been plaguing the clinicians. The cause of postoperative pain can be divided into iatrogenic, prosthesis and patient. Pain treatment in perioperative period includes preoperative education, analgesia in advance, and the selection and design of reinforcement; during operation mainly includes the appropriate surgical approach, keep the balance of soft tissue around the knee joint, cocktail analgesia pain around the knee joint; after operation mainly includes oral analgesic drugs, femoral nerve tissue and patient controlled analgesia. Abstract:Total knee arthroplasty (TKA) is one of the most common surgeries performed to relieve joint pain in patients with end‐stage osteoarthritis or rheumatic arthritis of the knee. However, TKA is followed by moderate to severe postoperative pain that affects postoperative rehabilitation, patient satisfaction, and overall outcomes. Historically, opioids have been widely used for perioperative pain management of TKA. However, opioids are associated with undesirable adverse effects, such as nausea, respiratory depression, and retention of urine, which limit their application in daily clinical practice. The aim of this review was to discuss the current postoperative pain management regimens for TKA. Our review of the literature demonstrated that multimodal analgesia is considered the optimal regimen for perioperative pain management of TKA and improves clinical outcomes and patient satisfaction, through a combination of several types of medications and delivery routes, including preemptive analgesia, neuraxial anesthesia, peripheral nerve blockade, patient‐controlled analgesia and local infiltration analgesia, and oral opioid/nonopioid medications. Multimodal analgesia provides superior pain relief, promotes recovery of the knee, and reduces opioid consumption and related adverse effects in patients undergoing TKA.

Abstract:[Objective]In this prospective study,we will evaluate the safety and efficacy of multimodal analgesia(MA) in patients undergoing total hip arthroplasty(THA).[Method]In this study,60 patients scheduled to unilateral primary THA were randomized into two groups: MA group and the control group.Both of the 2 groups were were given celecoxib 200mg two times a day,3 days preoperatively.The multimodal analgesia group was given local infiltration analgesia(LIA)(ropivacaine 2.5 g/L,80 ml,with 0.1 mg epinephrine) after closing the joint capsule,the control group didn't have the LIA.All of the patients didn't use the patient-controlled analgesia.VAS scores were recorded at 1,4,24,48,72 h postoperatively and the day checkout;the hip joint range of motion was measured at 24 h,48 h,72 h postoperatively,the day checkout respectively after surgery.The use of opioids and side effects was also recorded.[Result]The mean VAS score at each time of MA group was lower than of the control group,but only the dates within 48h postoperation were significant different.The joint range of movement of the MA group was better than of the control group(P0.05).The control group used more opioids than the MA group,and had a higher rate of vomit and nausea.No serious complications occurred(such as infection,deep vein thrombosis) in our study.[Conclusion]The technical MA can relieve pain after the operation,reduce the opioids use and improve the inhabitation but doesn't increase the rate of complications.MA is one of the safe,effective way to control pain after THA.

Abstract:Objective Patients with knee osteoarthritis undergoing total knee arthroplasty may have a different degree of pain during the perioperative period. This will not only bring a lot of adverse effects to the patients,and directly affect the early postoperative functional exercise and rehabilitation of the knee joint. At present,the commonly methods used to relieve the pain after TKA are:patient con-trolled Intravenous analgesia(PCIA)、patient controlled epidural analgesia( PCEA)、continuous femoral nerve block analgesia(CFNB)、joint peripheral injection analgesia and some methods without using medi-cine. The Perioperative Multimodal Pain Relieve in Total Knee Arthroplasty

Abstract:Objective To evaluate the benefits and safety of a multimodal analgesia protocol that included perioperative injection of morphine and oral analgesic in patients undergoing total knee arthroplasty.Methods Ninety cases of total knee arthroplasty were randomly divided into two groups,then were treated to relieve pain perioperatively.Postoperative pain score and complications were analyzed.Results The positive relieve method take obvious effect,patient satisfaction and function were improved greatly,complication rate were low.Conclusion Aggressive,comprehensive pain relieve method is benefical to total knee arthroplasty.

Abstract:Objective To find an optimal method for postoperative pain management after total knee arthroplasty(TKA).Methods Eighty patients subject to unilateral TKA were randomly divided into 4 groups(n=20 each):control group(C group),parecoxib group(P group),tramadol group(T group),fentanyl group(F group).All the patients were infused with 15 mL of 0.2% ropivacaine via femoral nerve catheter at the first h after surgery and from then on per 6 h postoperatively till 48 h postoperatively.If the visual analogue score(VAS)>5 the supplements(parecoxib,tramadol,fentanyl)would be infused in turn till the patient felt comfortable or the analgesic effect reached the clinical maximum.The VAS and sedation score(SS)were recorded per 8 h after surgery.Side effects and degree of satisfaction were investigated at the 48th h after surgery.Results In C group VAS was significantly higher than in P,T,F groups(P<0.05)and that in P group was significantly higher than in T,F groups(P<0.05).There was no significant difference in VAS between T and F groups(P>0.05).The incidence of side effects was significantly higher in F group than in C,P,T groups(P<0.05)and the T group had a significantly higher degree of satisfaction than in C,P,F groups(P<0.05).Conclusion Continuous femoral nerve block compound with parecoxib and tramadol is the optimal method for postoperative pain management after TKA.

Abstract:Objective To investigate the analgesic effect of oxycodone and acetaminophen tablets for total hip arthroplasty(THA).Methods Sixty-four patients who undergone unilateral THA surgery were randomly divided into two groups evenly.The treatment group took one tablet of oxycodone and acetaminophen(containing oxycodone hydrochloride 5 mg and paracetamol 325 mg)tid and the control group took celecoxib 200 mg bid.Both treatment lasted for 5 days.Resting visual simulation pain score(RVAS),initiative visual simulation pain score(IVAS),perisistent visual simulation pain score(PVAS),the time for active leg lifting up to 30° and for limbs outreaching up to 45° after surgery and the overall hospital satisfaction with analgesia and complications were recorded.Results The RVAS,IVAS and PVAS of the treatment group at 24 h,48 h,and 72 h postoperation were significantly lower than those in the control group(P<0.01).The duration for leg lifting at 30° and for limbs outreaching at 45° of the treatment group[(1.9±0.8) h,(2.9±1.1) d ]were significantly shorter than those of the control [(3.8±1.2) h,(4.5±1.4) d ](P<0.01).The overall satisfaction with analgesia in the treatment group(83.1%)was significantly better than that in the control group(43.8%).Three cases of adverse reactions occurred in the treatment group.Conclusion The oxycodone and acetaminophen tablets show good analgesic effect for THA,which promote early functional recovery,improve the patients' degree of satisfaction with surgery and develop fewer side effects.It could be an ideal postoperative analgesia for THA.

Abstract:Objective To study the effect of multimodal analgesia on postoperative physical exercises in patients with total hiparthroplasty.Methods One hundred and five patients with total hip arthroplasty were selected from March 2010 to June 2010 for the study.They were randomly divided into three groups,observation group(group A and group B) and control group.All patients received the treatments of periarticular injection of ropivaeaine hydrochlorid.Patients controlled group received a postoperative epidural analgesia,with administration of tramcontin and celecoxib.The patients in group A received education of multimodal pain control protocol and intravenous injection of parecoxib,and patients in group B received education of multimodal pain control protocol and intraoperative injection around the joint cavity.The VAS score,functional exercises(range of motion,straight leg raising,postoperative walking)and compliance for patients among the three groups were observed and compared.Results There were significant difference of resting pain VAS score,postoperative limbs situation(range of motion.straight leg raisin,postoperative ambulation)and compliance between observation and control group.But no remarkable differences were observed between group A and group B.Conclusions Multimodal pain control protocol after total hip arthroplasty can significantly reduce the pain after replacement and promote the recovery of hip function,increase the compliance of patients with postoperative functional exercise.

Abstract:BACKGROUND:Analgesia plays an important role in the rehabilitation of total knee arthroplasty(THA),which ensures effective function exercise and improves perioperative life quality.OBJECTIVE:To evaluate the analgesic effect and security of tramadol/acetaminophen combined with patient-controlled intravenous analgesia during perioperative THA.METHODS:Totally 70 patients who received unilateral THA from Affiliated Provincial Hospital of Shandong University from September 2010 to September 2011 were divided into two groups.Patient in group A(multi-model analgesia group) were treated with patient-controlled intravenous analgesia and oral tramadol/acetaminophen for 7 days after operation.Patients in group B(control group) were only treated with patient-controlled intravenous analgesia.RESULTS AND CONCLUSION:Visual analogue scale of the group A was lower than that of the group B at the first three days after operation(P < 0.05),but during the period of exercise,there was no statistical difference between the two groups.The range of motion and overall satisfaction with the treatment in the group A were significantly higher than those in the group B at postoperative 7 days(P < 0.05),and there was no statistical difference in the incidence of adverse reactions and severity between the two groups.The results showed that application of tramadol/acetaminophen combined with patient-controlled intravenous analgesia during perioperative THA can improve the efficiency of function exercise and satisfaction with the treatment,and without increasing the risk of adverse drug reactions.
Preemptive multimodal analgesia and post-operative pain outcomes in total hip and total knee arthroplasty

Abstract:Background: Use of multimodal analgesia (MMA) prior to orthopedic surgery has been adopted by many practitioners as a strategy to minimize use of opioid medications. The purpose of this investigation was to quantify the effect of a preemptive three-drug regimen (acetaminophen, celecoxib, and gabapentin) in terms of post-operative opioid consumption and pain control in the field of total joint arthroplasty. Methods: A retrospective chart review was conducted on 1691 patients who underwent total hip arthroplasty (THA) or total knee arthroplasty (TKA) and stratified by whether they received a preemptive three medication analgesic therapy (acetaminophen, celecoxib, and gabapentin) within 30 to 60 min prior to entering the operating room. Post-operative opioid consumption as well as subjectively reported patient pain scores were assessed throughout their hospital stay. Results: A total of 1416 eligible patients were identified with 485 undergoing THA and 931 undergoing TKA. Statistically significant reductions in oral morphine equivalents were shown on post-operative day zero and two within the TKA cohort, and non-significant reductions were demonstrated in other intervals for both procedure types. Statistically significant reductions in patient reported pain scores were shown in nearly every time interval in both procedure types. Conclusion: The receipt of preemptive acetaminophen, celecoxib, and gabapentin 30-60 min prior to total joint arthroplasty demonstrated modest reductions in opioid requirements post-operatively. Patients receiving preemptive MMA reported lower pain scores throughout nearly every time interval during their admission after surgery. Further investigations are warranted regarding optimal preoperative medication therapies to promote adequate post-operative pain control-and ultimately diminished opioid consumption-in the setting of total joint arthroplasty.
Oral Multimodal Analgesia for Total Joint Arthroplasty

Abstract:Background: Oral multimodal analgesia for hip and knee arthroplasty is increasingly used as part of enhanced recovery protocols designed to minimize early postoperative pain and to facilitate early discharge, while minimizing undesirable side effects related to single-agent opioid administration. Methods: This article is a review of previously published data evaluating the use of various oral medications in the management of postoperative pain after lower extremity arthroplasty and was presented as part of a symposium at the November 2016 AAHKS Annual Meeting. Results: Multimodal analgesia has been shown to reduce opioid consumption and side effects, with a positive effect on both early and longer term outcomes for hip and knee arthroplasty patients. Medications directed at multiple points on the pain cascade minimize pain by different mechanisms. Suggested dosing regimens are proposed. Conclusion: Oral multimodal analgesia incorporating a combination of opioid and nonopioid analgesics, selective and nonselective anti-inflammatory drugs, acetaminophen, and gabapentinoids are recommended as a part of a pre-emptive approach to pain management in patients undergoing hip or knee arthroplasty. Reduction of opioid consumption and minimization of side effects are primary outcomes, and prevention of chronic pain can positively affect long-term results.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Study objective</h3><p>This study aimed to evaluate the efficacy of ultrasound-guided transmuscular quadratus lumborum block (QLB) combined with local infiltration analgesia (LIA) for pain management and recovery in patients who have undergone total hip arthroplasty (THA) <em>via</em> a posterolateral approach.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>This was a prospective, randomized controlled trial.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Setting</h3><p>We collected data in the preoperative area, operating room, and bed ward.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Patients</h3><p>A total of 80 patients with American Society of Anesthesiology functional status scores of II-III were included and assigned to two groups, and all 80 patients were included in the final analysis.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Interventions</h3><p>All included patients were randomly assigned to the nerve block (group N) or the control group (group C). Patients in the group N received transmuscular QLB combined with LIA, while patients in the group C received only LIA.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Measurements</h3><p>The primary outcome was postoperative pain during the first active motion: it was measured at six hours after surgery and assessed using a visual analog scale (VAS). Secondary outcomes were the resting VAS scores in the post-anesthesia care unit (PACU) and at 2, 6, 12, 24, 48, and 72 h after surgery; VAS scores during motion at 12, 24, 48, and 72 h after surgery; intraoperative consumption of opioids; postoperative consumption of morphine hydrochloride; frequency of sleep interruption due to pain on the night of surgery; time until the first "walk out of the bed" after surgery; muscle strength of the quadriceps femoris; and postoperative adverse effects.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Main results</h3><p>Compared to the group C, patients in the group N had significantly lower VAS scores during motion at 6, 12, and 24 h after surgery, as well as lower resting VAS scores in the PACU and at 2, 6, 12, and 24 h after surgery. Patients in the group N also consumed significantly smaller amounts of intraoperative opioids and morphine after surgery. Patients in the group N reported significantly fewer interruptions in sleep due to pain on the night of surgery and were able to "walk out of the bed" significantly earlier than those in the group C. There was no significant difference between the two groups in muscle strength of the quadriceps femoris or incidence of postoperative adverse effects.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>Compared to treatment with LIA alone, ultrasound-guided transmuscular QLB combined with LIA can provide better postoperative pain relief and enhance the recovery of THA patients, since it does not cause quadriceps femoris muscle weakness and is associated with significantly lower need for intraoperative opioids.</p>

Multiple-model Pain Control Methods for Perioperative Pain Relief after Unilateral Total Knee Arthroplasty

Abstract:Objective The goal of this study is to evaluate the method and the effect of multimodal analgesia during perioperative period.Methods Sixty patients with the diagnosis of osteoarthritis of the knee joint who underwent total knee arthroplasty,were included in this study.They were randomly divided into three groups.Group A patients (n=20) only received postoperative patient controlled intravenous analgesia (PCIA);Group B patients (n=20) received preoperative multimodal analgesia and received postoperative PCIA;Group C patients (n=20) received periopertive multimodal analgesia but did not receive postoperative PCIA.The consumption of analgesic was measured at different time-points during the twenty-four hours postoperative period.Postoperative visual analog scales (VAS) for pain and satisfaction,the patient gross satisfaction scales (GSS),the verbal rating scale scores of the overall pain intensity and all the side effects,functional rehabilitation and scores according to the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) were recorded.Results (1) The VAS scores,2-8 h for pain at rest,the overall pain intensity were similarly between Group C and Group B (P0.05),but significantly lower than Group A;the GSS and the degree of passive activity 2 and 14 day after operative were similarly between Group C and Group B (P0.05),but significantly higher than Group A (P0.05).(2) The Group C had a significantly lower consumption of mrophine than that of Group A and Group B.(3) There was no difference in the range of motion six weeks after operation and the WOMAC score three months after operation among the three Groups.(4) The incidence of nausea,vomit and urine retention were lower in Group C than in Group A and Group B (P0.05).Conclusions Perioperative multimodal analgesia can reduce the overall pain intensity,enhance the GSS,make PCA unnecessary and it will reduce the side effects of PCIA.it can improve the range of passive activity 2-14 d postoperative,and advance rehabilitation.But the range of passive activity 3 month postoperative and the WOMAC were not different.
Pain management in total joint arthroplasty: a historical review

Abstract:Patients undergoing total hip and knee arthroplasty experience substantial and sustained postoperative pain. Inadequate analgesia may impede recovery and delay hospital discharge. Traditionally, postoperative analgesia following arthroplasty was provided by intravenous patient-controlled analgesia or epidural analgesia, but each technique has distinct advantages and disadvantages. Recently, peripheral nerve blockade of the lumbosacral plexus has emerged as an alternative analgesic approach. An increasing number of studies have reported multimodal analgesia featuring unilateral peripheral block provide pain relief and functional outcomes similar to that of continuous epidural and superior to systemic analgesia but with fewer side effects. This review discusses the indications, benefits, and side effects associated with conventional and innovative analgesic approaches to facilitate rehabilitation and improve outcome following total joint arthroplasty.

Abstract:Objective To evaluate the analgesic effect of parecoxib in total knee arthroplasty (TKA) and total hip arthroplasty (THA).Methods The study was a prospective,randomized and double-blind trial and was operated by the same group of surgeons in 101 patients with TKA and 105 patients with THA.According to analgesic protocol,the patients were divided into three groups:Group One ( intravenous injection with parecoxib),Group Two ( periarticular injection with parecoxib) and Group Three ( the control group).The postoperative visual analog scores (VAS),range of motion ( ROM),the ability of straight leg raising and the incidence of nausea and vomiting complications were examined and compared between the three groups.Results There were no significant differences in VAS (6,12,24,36,48,72 hours after operation),ROM ( 24 hours after operation) and the ability of straight leg raising between Group One and Group Two ( P ＞ 0.05 ),but all of them were significantly higher than those in Group Three ( P ＜ 0.05 ).Nausea,vomiting and other adverse effects did not significandy increase with the use of parecoxib.Conclusions Both intraoperative intravenous injection and periarticular injection with parecoxib have a good analgesia effect on TKA and THA,which are beneficial to the rapid recovery of joint function in patients.The simple and practical method provides an effective adjunct to a multimodal analgetic approach in improving the postoperative course of TKA and THA.

Abstract:Since there is no consensus on the optimal perioperative analgesic method, the present article provides a brief and practical synthesis on current multimodal pre–postsurgery analgesia protocols for primary TKA (excluding intraoperative anesthetic techniques) reported especially in recent meta-analyses and reviews. The focus is not only on the traditional aims (pain scores and opioid sparing), but also the functional recovery and patient-reported outcomes. Multimodal analgesia (pre-emptive analgesia, local infiltration analgesia, peripheral nerve blocks, and specific systemic drugs) is considered the optimal regimen for perioperative pain management of TKA.

Abstract:Background: The ability of intraoperative periarticular drug infiltration (PDI) to control pain after total hip arthroplasty (THA) has been studied for many times, but it still remains controversial. Therefore, we undertook a meta-analysis to evaluate the efficacy and safety of PDI on postoperative pain after THA. Methods: Databases, including Pubmed, Medline, Embase, Web of Science, and Cochrane library, were searched to identify randomized controlled trials concerning PDI for pain management in patients undergoing THA. The primary outcomes included pain score with rest or activity and opioid consumption. Secondary outcomes were length of hospital stay and complications (nausea or vomiting). Results: A total of 666 THA patients from 8 randomized controlled trials were subjected to meta-analysis. The results showed that the PDI group had better pain relief, less opioid consumption, and less length of hospital, when compared with the placebo group (P<0.05). No significant differences were observed in regard to visual analog score with activity and complications between the 2 groups. Conclusion: PDI may be recommended for the pain management after THA. However, due to the variations in the included studies, additional studies are still needed to validate these conclusions.
