The use of C-arm X-ray machine delivered radiation hazard to both surgeon and patients. This study was initiated to explore a step-by-step X-ray free balloon dilation surgical approach to treat ureteral stenosis and verify its safety and effectiveness. METHODS: This retrospective study reports clinical data for 31 benign ureteral stricture patients treated with balloon dilation without fluoroscopic assistance at our center from December 2013 to December 2017. All surgical procedures were carried out under direct vision of ureteroscope and decomposed into five standard steps: Assessment of ureteral stricture, ensure proper placement of the balloon, dilute the stricture, re-check the ureter and indwell two double-J stents. RESULTS: All patients received balloon dilation without fluoroscopic guidance. The mean operative time was 59 mins [19.5-125]. No intraoperative and postoperative complications were reported. Thirty-one patients were followed up from 3 months to 44 months with a median time of 14 months. During this period, one-year success rate and two-year success rate were 68.5% and 60.0% respectively, and no recurrence of ureteral stricture and hydronephrosis deterioration were found in successful patients over two years after operation. Longer operative time and longer stenotic lesion length (≥ 5 mm vs. <5 mm) were significantly associated postoperative stenosis recurrence. The study limitations include its retrospective design and small sample size of the study population. CONCLUSIONS: The X-ray free endoscopic balloon dilation is a safe and effective procedure that could be performed on ureteral stenosis patients, its long-term prognosis is comparable to traditional surgical approach. Most recurrent stenosis was found within two years after surgery.
