Acetabular fossa restoration in precise total hip arthroplasty for Crowe type Ⅲ developmental hip dysplasia and its radiological quantitative score

Abstract:Objective:To investigate the role of acetabular fossa restoration in precise total hip arthroplasty (THA) for Crowe type Ⅲ developmental hip dysplasia (DDH) and explore the feasibility of the evaluation of the quality of hip prosthesis installation in pelvic plain film using a quantitative scale.Methods:A retrospective study of 86 patients (92 hips) with end-stage osteoarthritis secondary to Crowe type III DDH who were admitted to the First Affiliated Hospital of Bengbu Medical College from March 2014 to April 2019 was conducted. We collected 16 and 76 hip data from 13 men and 73 women, respectively. The average age of the patients was(50.3±7.2)years (ranged 30-64 years). All the patients underwent THA. 3D printed acetabular models were constructed according to the preoperative 3D CT data of the hip joints of 20 patients. The pathomorphological characteristics of the acetabulum and acetabular fossa were observed. Simulated operations were performed on the models. The original center, reaming depth, anteversion, and inclination of the acetabular prosthesis were determined according to the morphology of the acetabular fossa for restoration. Pelvis plain films were captured at 1 or 4 or 12 or 48 weeks, postoperatively. A quantitative scale was designed from the following indicators: the press-fit of the acetabular prosthesis, the vertical and horizontal distances of the rotation center, the anteversion and inclination of the acetabular prothesis, the press-fit of the femoral prosthesis, femoral offset, femoral stem angulation, and lower limb length discrepancy. Among which, the press-fit and anteversion of the acetabular prosthesis were assigned 15 points, and the other seven indicators were assigned 10 points. The quantitative scale included nine indicators with a total of 100 points. The positions of prostheses in the postoperative pelvis plain films of 86 patients (92 hips) were evaluated according to the quantitative scale. The anatomical data of normal hip were used as the reference for patients with bilateral THA, whereas the anatomical data of the healthy side were used as the reference for patients with unilateral THA. A total score ≥91 points was considered excellent, a total score of 81-90 points was considered good, and a total score ≤80 points or a monomial score ≤5 points was considered poor.Results:All the acetabula of 92 hips in this study presented up-wide low-narrow shell-like shapes with different extent, in which 29, 56, and 7 hips presented straight, anteverted, and retroverted shell-like shapes, respectively. The acetabular fossa presented varied shapes because of hyperplastic osteophytes' coverage, among which 68, 13, 6, and 5 hips presented triangular, closed, crack-like, and irregular shapes, respectively. The hyperplastic osteophytes were the most obvious in the lower part of the acetabular fossa, where the average osteophyte thickness was (14.4±3.83) mm. All acetabular notches in this study were covered by hyperplastic osteophytes, and transverse acetabular ligament was not found. The average score of postoperative pelvis plain films in 92 hips was (94.15±1.94)points (ranged 75-98 points), in which the installations of 79, 8, and 5 hips were categorized as excellent, good, and poor, respectively. The total rate of excellent and good installations was 94.56% (87/92).Conclusions:According to the restored morphology of acetabular fossa, the original acetabular center, reaming depth, and the inclination and anteversion of the acetabular cup can be determined to realize the precise installation of the acetabular prosthesis in precise THA for crowe type Ⅲ DDH. The radiological quantitative scale for THA with severe pathological changes is helpful for surgeons to inspect the shortcomings in the operation and improve the technical level of hip arthroplasty.
