The feasibility of CT in the diagnosis of primary gastric cancer patients with external vascular invasion

Abstract:Objective To explore the feasibility of CT in the diagnosis of primary gastric cancer by imaging diagnosis. Methods 100 cases of primary gastric cancer patients from November 2013 to November 2015 in our hospital were randomly divided into A, B two groups, group A was checked by CT determine whether with invading surrounding blood vessels of pa-tients, B group adopted routine doctor judge whether the risk of infringement of vascular wall patients. Record all the patients of A group CT parameters such as T stage (T1, T2, T3, T4), N stage (N1, N2, N3, N0), M stage (M1, M0), tumor size, tumor growth mode and other data. All patients in the surgery to determine whether the presence of the phenomenon of vascular invasion of the patient and recorded, respectively, the diagnosis of the two groups in the diagnosis of the right wall of the proportion of vas-cular invasion and recorded. Compared the results of A and B groups, the sensitivity, specificity, positive predictive value and negative predictive value of the two groups were calculated. Results Group A patients with T11 cases, T20 cases, T30 cases, T415 cases, N13 cases, N24 cases, N33 cases, N06 cases, M110 cases, M06 cases; group Ⅱ T14 cases, T26 cases, T35 cases, T419 cases, N17 cases, N215 cases, N35 cases, N07 cases, M124 cases, M010 cases.5cm1 patients were less than group Ⅰ of tumor patients in the A group (6.25%), 5cm15(93.75%), patients with tumors larger than 10 cases of distal nodular type (62.5%), proximal nodular type and diffuse type in 6 cases (37.5%); group Ⅱ tumors less than 5cm16 cases (47.06%), 5cm18(52.94%), patients with tumors larger than the distal tuberosity in 23 cases (67.65%), proximal nodular type and diffuse type in 11 cases (32.35%). In group A, 15 cases (93.8%) were diagnosed correctly by the external vascular invasion, and 19 cases (67.9%) were diagnosed as B group. A group sensitivity 1, specificity 0.97, positive predictive value 0.94, negative predictive value 1; B group sensitivity 0.59, specificity 0.50, positive predictive value 0.68, negative predictive value 0.41. Conclusion CT imaging can ac-curately determine whether the patients with primary gastric cancer have the phenomenon of out of wall vascular invasion, and the CT parameters can be more accurate in response to the situation of vascular invasion.
