A RETROSPECTIVE STUDY ON COMBINED TREATMENT OF CONTINUOUS AMBULATORY PERITONEAL DIALYSIS AND IMMUNOSUPPRESSIVE THERAPY OF ANCA-ASSOCIATED VASCULITIS COMPLICATING RENAL FAILURE

Abstract:Objective: Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV) are a group of pawl-immune small vessel vasculitides that often affect the kidneys and manifest as rapidly progressive glomerulonephritis. This study mainly aimed to explore the long-term follow-up of pauci-immune renal vasculitis with peritoneal dialysis. Methods: A total of 20 patients with AAV complicating renal failure, who were admitted in hospital from January 2005 to January 2015, were investigated in present study. The mean age was 52 +/- 14 years old at the beginning of the dialysis. Among these patients, 12 patients were male and eight patients were female. All patients received peritoneal dialysis combined with glucocorticoids and immunosuppressant treatment. Results: The kidney was the most common organ involved in vasculitis. The distribution according to ANCAs was 14 p-ANCA/MPO (70%) associated with five anti-GBM-positive and six c-ANCA/PR3(30%) pauci-immune vasculitis. This was followed up for five years, and the five ANCAs and three GBMs became negative. The mean follow-up after first peritoneal dialysis was 63 +/- 23 months (range: 6-96 months). Furthermore, five patients converted to hemodialysis and three patients converted to kidney transplantation. The survival rate for year 1, 3 and 5 was 95%, 90% and 80%, respectively. Birmingham Vasculitis Activity Score (BANS) significantly decreased after treatment for three months (P<0.05). Four patients died in this group, which resulted from infection, pulmonary embolism, upper gastrointestinal bleeding and heart failure. Conclusion: CAPD combined with immunosuppressive treatment and glucocorticoids exhibited a good effect, and improved the survival rate. The complications were mainly associated with the long-term use of corticosteroids and immunosuppressants.
