Abstract:BACKGROUND:The treatment and management of patients suffering from hypophosphatemic rickets (HR) remain a major challenge for dental practitioners and affected patients.OBJECTIVES:To report a case of HR presenting with specific dental findings and to review the dental manifestations and treatment of HR patients.METHODS:Case: A 32-year-old male presented with multiple dental abscesses and short stature. A thorough history was taken followed by clinical oral examination, and relevant radiological investigation was done. Literature research: In 2020, electronic literature searches were carried out in PubMed and complemented by a careful assessment of the reference lists of the identified relevant papers. Articles and reports fulfilled the inclusion criteria: indexed reviews, case series and case reports in English and restricted to human studies were considered.RESULTS:The intraoral examination revealed multiple dental abscesses and general periodontal disease; the radiographic examination showed poorly defined lamina dura, large pulp chambers and periapical lesions. Based on the contents of the 43 articles identified in the search, the current knowledge of dental manifestations, treatment and management of HR was summarized.CONCLUSIONS:As HR is a multisystem disease, multidisciplinary care is needed. By summarizing current evidences, we proposed an evidence-based dental management and provided recommendations on diagnosis and treatment of the disease. It is of profound clinical significance to acquire knowledge of the dental manifestations and provide optimal treatment options for patients.

Abstract:Congenital absence of permanent second molar is an extremely rare condition, which may appear as part of a syndrome or as a non-syndromic form. Non-syndromic second molar agenesis combined with other types of teeth agenesis has occasionally been described in the literature but isolated cases are rarely observed. It might be caused by a number of complex interactions between genetic, epigenetic and environmental factors during the process of dental development. This report presents an isolated case of hypodontia with absence of bilateral maxillary and mandibular permanent second molars in a healthy 25-year-old male patient and a literature review on the prevalence, etiology, and restoration of the missing teeth.

Abstract:Hallermann-Streiff syndrome (HSS), also known as oculo-mandibulo-facial syndrome or François dyscephalic syndrome, is an extremely rare congenital developmental disorder, first described by Aubry in 1893 and later further delineated as a distinct entity by Hallermann (1948) and Streiff (1950) [1]. Its prevalence is estimated to be around 0.96 – 1.28 per 10 million in Japan, which can be converted to a presumptive incidence of 1 in 3 – 4 millions live births per year according to the relevant Japanese demographic statistics and a putative average HSS mortality of 30% [2]. Boys and girls are equally affected and usually diagnosed during the first months of life based on the presence of 7 cardinal signs described by François, namely, dyscephalia with birdlike facies, dental abnormalities, proportionate short stature, atrophy of skin (especially on the nose), alopecia and hypotrichosis (including eyebrows and eye lashes), bilateral microphthalmia, and bilateral congenital cataract [1]. Abstract:Objective To discuss the clinical features, diagnosis and treatment of Pallister-Hall syndrome. Methods A 27 year old man, who presented with epilepsy, combined with polydactyly, nail dysplasia, bifid epiglottis and hypothalamic hamartoma was reported. Restults The tumor was partly removed via pterional approach and the hypothalamic hamartoma were confirmed by pathology. The final diagnosis was Pallister-Hall syndrome. Abstract:Shwachman-Diamond syndrome (SDS) is a rare autosomal recessive disorder mainly caused by mutations in the Shwachman-Bodian-Diamond syndrome gene on chromosome 7q11. Although skeletal abnormalities are a feature of SDS, no reports have focused on the craniofacial morphology of patients with SDS. Moreover, the detailed dental characteristics of SDS remain unknown. In the present case report, we evaluated the craniofacial morphology and dental findings of two patients with SDS. A Japanese adolescent sibling pair with SDS had the chief complaint of excessive overjet. Cephalometric analysis revealed similar craniofacial morphology in both patients: skeletal class I malocclusion with a hypodivergent pattern and labial inclination of the maxillary and mandibular incisors. A panoramic photograph showed the tendency of delayed permanent tooth eruption and replacement in both patients. These cases suggest that malocclusion requiring orthodontic treatment might be a feature of patients with SDS.

Abstract:OBJECTIVE:To classify the clinical symptoms of oligodontia and summarize the characteristics of the diseases.METHODS:33 patients with congenitally permanent teeth missing (6 or more than 6) were taken clinical examination, diagnosis, prosthetic treatment and made records of the status of tooth agenesis, systemic symptoms, and family history.RESULTS:The symptoms of oligodontia were clinically classified into two groups: oligodontia with or without systemic syndromes. And the former has two typical kinds of diseases: anhidrotic/hypohidrotic ectodermal dysplasia (EDA) and Rieger syndrome.CONCLUSIONS:Correct diagnosis and early treatment for these patients are of very importance.

Abstract:Congenital dental deficiency can lead to aesthetic and masticatory dysfunction, which have a great effect on the quality of life of patients. Genetic inheritance is the determining factor of congenital dental deficiency, which may follow the pattern of autosomal dominant, recessive, or sex chromosome inheritance. However, the specific genetic mechanisms are not clear. The prevalence of congenital absence of permanent teeth in the general population worldwide are not high, and familial oligodontia in clinically healthy patients is rare. This paper reported two cases of familial non-syndromic congenital teeth loss and included a review of literature referring to the etiology, clinical manifestations, and treatment of congenital dental deficiency.

Abstract:Heimler Syndrome 2 (HS-2) is a rare, autosomal recessive mild form of a peroxisomal biogenesis disorder. Though knowledge regarding the disorder is limited, emerging research has found that sensorineural hearing loss, occasional or late onset pigmentation, amelogenesis imperfecta and nail abnormalities are clinical characteristics representative of HS-2.A school-aged male presented to the dental department with a chief complaint of a lack of enamel on multiple teeth. The patient's medical history was significant for patent ductus arteriosus, bilateral sensorineural hearing loss and biallelic mutation of the PEX6 gene. The clinical exam revealed dental crowding, hypoplasia, hypo-calcification of multiple teeth and enlarged pulp chambers of maxillary molars. This case report details the clinical findings associated with HS-2, the comprehensive dental treatment to be rendered to the patient, and critical information to paediatric dentists and general dentists so that they can make proper referrals to medical specialties.

Abstract:Ectodermal dysplasias (ED) are a group of rare genetic disorders characterized by congenital defects involving two or more ectodermal structures. Christ-Siemens-Touraine syndrome or hypohidrotic ectodermal dysplasia is the commonest form of ED. Hypohidrotic ectodermal dysplasia (HED) is an X-linked disorder characterized by excessively dry skin due to the absence of sweat glands (anhidrosis), sparse body hair especially on the scalp and eyebrows (hypotrichosis), brittle nails, absence of sebaceous glands (asteatosis) and malformed or absent teeth. Oral manifestations include oligodontia or complete anodontia, conical teeth, underdeveloped alveolar ridges, generalized spacing and delayed eruption of permanent teeth. This case report discusses a classical case of HED and the options for rehabilitation in a growing patient. A thorough knowledge about the clinical manifestations of ED will lead to proper diagnosis and appropriate treatment plan thereby leading to significant improvements in esthetics, phonetics and masticatory function in ED patients, which in turn leads to improved quality of life in these individuals. Clinical and genetic evaluations of Zimmermann-Laband syndrome with gingival fibromatosis: a rare case report

Abstract:Zimmermann-Laband Syndrome (ZLS; MIM 135500) is a rare genetic disorder with the main clinical manifestations of gingival fibromatosis and finger/toe nail hypoplasia. KCNH1 (potassium channel, voltage-gated, subfamily H, member-1), KCNN3 (potassium channel, voltage-gated, subfamily H, member-3) and ATP6V1B2 (ATPase H+ transporting V1 subunit B2) genes are considered causative genes for ZLS. However, there are limited reports about the diverse clinical presentation and genetic heterogeneity. Reporting more information on phenotype-genotype correlation and the treatment of ZLS is necessary. This case reported a 2-year-old patient with gingival enlargement that failure of eruption of the deciduous teeth and severe hypoplasia of nails. Based on a systemic examination and a review of the relevant literature, we made an initial clinical diagnosis of ZLS. A novel pathogenic variant in the KCNH1 gene was identified using whole-exome sequencing to substantiate our preliminary diagnosis. The histopathological results were consistent with gingival fibromatosis. Gingivectomy and gingivoplasty were performed under general anesthesia. After surgery, the gingival appearance improved significantly, and the masticatory function of the teeth was restored. After 2-year follow-up, the gingival showed slightly hyperplasia. Systemic examination and gene sequencing firstly contribute to provide information for an early diagnosis for ZLS, then timely removal of the hyperplastic gingival facilitates the establishment of a normal occlusal relationship and improves oral aesthetics.
Orthodontic Treatment of a Patient with Dentin Dysplasia Type I and Bilateral Maxillary Canine Impaction: Case Presentation and a Family-Based Genetic Analysis

Abstract:Dentin dysplasia is a rare hereditary disorder, transmitted by autosomal dominant mode, affecting both dentin and pulp. In Type I crown morphology is normal, but root dentin organization loss leads to shorter roots. Mutations in the SSUH2, VPS4B and SMOC2 genes have been reported as responsible for this condition. Orthodontic treatment was conducted on an 11-year-old female patient presenting the disorder along with bilaterally impacted permanent maxillary canines, in close proximity to the roots of the lateral and central incisors. Treatment plan included lateral incisors extraction, surgical exposure and traction of the impacted canines. Light forces were applied from a custom-made trans-palatal arch. Comprehensive orthodontic treatment was performed using edgewise appliances. After 3 years and 2 months, group function occlusion was achieved. The canines underwent composite resin restorations. At one year post-retention, the dentition remained stable. Family-based genetic analysis did not reveal any mutations in the aforementioned genes pointing to further genetic heterogeneity of this disorder. As dental medicine becomes more sophisticated and personalized, the association between mutation type/function and orthodontic treatment response may provide useful therapeutic insights. The positive treatment response of the presented case could be attributed to a more "benign" mutation awaiting to be identified.

Abstract:Cleidocranial dysplasia (CCD) is a rare congenital disorder, typically characterized by persistently open skull sutures, aplastic or hypoplastic clavicles, and supernumerary teeth. Mutations in the gene encoding the runt-related transcription factor 2 (RUNX2) protein are responsible for approximately two thirds of CCD patients. We report a 20-year-old CCD patient presenting not only with typical skeletal changes, but also complex dental anomalies. A previously undiagnosed odontoma, 14 supernumerary teeth, a cystic lesion, and previously unreported fused primary teeth were discovered on cone-beam computed tomography (CBCT) scans. Mutation analysis identified the causal c.578G > A (p. R193Q) mutation in the RUNX2 gene. At 20 years of age, the patient had already missed the optimal period for dental intervention. This report describes the complex dental anomalies in a belatedly diagnosed CCD patient, and emphasizes the significance of CBCT assessment for the detection of dental anomalies and the importance of early treatment to achieve good outcomes.

Abstract:Purpose: We describe the management of Hallermann-Streiff syndrome in monozygotic female twins with congenital cataracts, exudative retinal detachments, and 1 case of corneal descemetocele with associated dellen and subsequent perforation. Methods: This study was a case report and review of the literature. Results: Twins 1 and 2 exhibited all 7 cardinal characteristics of Hallermann-Streiff syndrome, presenting with spontaneous lenticular resorption, anterior uveitis, and glaucoma. They underwent bilateral cataract extraction with near total capsulectomy. Both twins experienced recurrent glaucoma, for which twin 1 underwent successful endocyclophotocoagulation in both eyes and twin 2 in the left eye alone. The fellow eye developed 2 sites of perilimbal corneal descemetoceles with associated dellen at the inferotemporal limbal corneal junction leading to spontaneous perforation of 1 site, requiring a full-thickness corneal graft. Both twins developed recurrent bilateral exudative retinal detachments unresponsive to oral prednisolone. Twin 1's last best-corrected visual acuity with aphakic spectacles was 20/260 in the right eye and 20/130 in the left eye at age 4 years and 8 months. Twin 2's last best-corrected visual acuity was 20/130 in each eye at age 4 years and 11 months, over a year after right eye penetrating keratoplasty. Conclusions: We describe 2 rare cases of Hallermann-Streiff syndrome in monozygotic twins complicated by corneal perforation requiring penetrating keratoplasty in 1 eye of 1 twin. Although corneal opacities have been described in this condition, this is the first case of corneal descemetocele in Hallermann-Streiff syndrome. The cornea was stabilized with a relatively favorable visual outcome over 1 year later.
Prenatal diagnosis of Treacher Collins syndrome: A case report and literature review

Abstract:Treacher Collins syndrome (TCS) should be suspected if the triad of micrognathia, glossoptosis, and posterior cleft palate, and deformed external ears are observed during prenatal ultrasonography, excepting Pierre Robin sequence. Visualization of the fetal zygomatic bone and down-slanting palpebral fissures are conducive to differentiation. Molecular genetics testing can establish a definite diagnosis. A 28-year-old pregnant Chinese woman was referred for systematic ultrasound examination at 24 weeks. Two-dimensional and three-dimensional ultrasound showed polyhydramnios, micrognathia, absence of nasal bone, microtia, secondary cleft palate, mandibular hypoplasia, glossoptosis, and normal limbs and vertebrae. Pierre Robin sequence was misdiagnosed with the triad of micrognathia, glossoptosis, and posterior cleft palate. Final diagnosis of TCS was confirmed by whole-exome sequencing. Visualization of the fetal zygomatic bone and down-slanting palpebral fissures can facilitate a differential diagnosis between Pierre Robin sequence and TCS, with the triad of micrognathia, glossoptosis, and posterior cleft palate.

Abstract:OBJECTIVE:To study gene mutation in Chinese patients with cleidocranial dysplasia.METHODS:A three generation family with the clinical diagnosis of cleidocranial dysplasia was investigated in present study. Genomic DNA was extracted from peripheral blood samples of each of the family members. Direct sequencing of the PCR products of the coding region of CBFA1 gene was used to identify the mutations.RESULTS:In each patient of the family, a heterozygous missense mutation, cDNA 674 G > A (R225Q), was detected in CBFA1 exon 3. The mutation changed the sequence in runt domain of the protein.CONCLUSIONS:Our findings indicate that mutation in CBFA1 is responsible for the tooth agenesis and other phenotypes of cleidocranial dysplasia in this Chinese family. The mutation detection could be applied in prenatal diagnosis for the family.

Abstract:Objective:To investigate the clinical manifestation and MRI of a patient with HallervordenSpatz Disease(HSD).Methods:The clinical manifestation and examination were studied with a retrospective method in one childhood onset HSD.Results:The extrapyramidal sign was prominent,and the patient might have the pyramidal signs,dementia and retinitis pigmentosa,slow onset and progressive course.The brain MRI of HSD showed abnormalities signal to both sides of the globus pallidum,the typical eyeofthetiger sign,suggestive of increased iron deposition.Conclusion:The antemortem diagnosis of HSD may be given on the detailed basis of clinical manifestations and MRI features,but other neurodegeneration disorders should be ruled out.

Abstract:Objective: To detect mutation of one patient with dyschromatosis symmetrica hereditaria (DSH) and to identify another patient with atypical clinical features in this family. Methods: PCR and direct sequencing were performed respectively for the family members to screen the mutations in ADAR1 gene. Then reverse transcription-polymerase chain reaction (RT-PCR) was used to investigate the corresponding mRNA. Results: One non-canonical splice mutation (c.2886-5T>C) was identified in one patient with DSH. In addition, for another patient, whose clinical phenotypes did not accord with typical clinical manifestation of DSH, the same mutation was found. Conclusion: One novel splice mutation was identified. We made the diagnosis of atypical DSH by direct DNA sequencing. Furthermore, his phenotype was observed by Wood's light.
Suspected Endodontic Failure in a Patient with Cleidocranial Dysplasia: A Case Report

Abstract:A patient's medical history and related dental manifestations can significantly contribute to confounding signs and symptoms leading to a diagnostic challenge. An 18-year-old female patient presented with persistent radiographic radiolucency associated with the apex of a previously treated tooth (tooth 9); asymptomatic apical periodontitis and endodontic failure were suspected. This report presents how a patient's condition of cleidocranial dysplasia had a profound effect on her dental history, which included the presence of multiple supernumerary teeth. Extensive surgical intervention during the patient's childhood was required to remove the supernumerary teeth, which resulted in an endodontic misdiagnosis in her adult life. After clinical and radiographic examination, the patient was diagnosed with a periapical scar. Periapical fibrous scars have a prevalence of between 2.5% and 12% and are a rare healing process with fibrous tissue after surgical and nonsurgical interventions. This report describes the diagnosis and pathophysiology of fibrous scars, including their risk factors and long-term monitoring approaches.
