Abstract:OBJECTIVE To observe the effects of Xuezhikang on left ventricular hypertrophy and myocardial interstitial fibrosis in spontaneously hypertensive rats(SHR) and the possible mechanism.METHODS Thirty male SHRs were randomly treated with Xuezhikang(300 mg?kg?1?d?1)(XZK group,n=10),simvastatin(5 mg?kg?1?d?1)(SIM group,n=10) or 0.9% normal saline(SHR group,n=10).Age-matched Wistar-Kyoto rats(WKY)(WKY group,n=10) were also gavaged by 0.9% normal saline for 12 weeks.Left ventricular weight index(LVWI) was calculated.In addition,the serum lipid concentrations,the blood pressure levels,the myocardial collagen volume fraction(CVF) and perivascular collagen area(PVCA),the serum levels of procollagen typeⅠC-terminal peptide(PⅠCP),the levels of myocardial superoxide dismutase(SOD) and malondialdehyde(MDA) were also measured in this study.RESULTS After 12 weeks of treatment,the LVWI(P<0.05),CVF(P<0.01),PVCA(P<0.01) and serum PⅠCP concentrations(P<0.01) of XZK group and SIM group were significantly lower than those of SHR group.Myocardial SOD activity and MDA concentrations had no significant changes among four groups.CONCLUSION This study demonstrats that Xuezhikang significantly inhibited left ventricular hypertrophy and myocardial interstitial fibrosis was independent of its antihypercholesterolemic role and exhibited the same effects as simvastatin.The levels of myocardial SOD activity and MDA concentrations seem to be uninvolved in the reverse effects of Xuezhikang.

Abstract:Objective To compare the effects of Xuezhikang and Simvastatin in patients with primary hyperlipidemia. Methods 108 patients with primary hyperlipidemia were randomly divided into two groups. Group 1,53 patients, took 4 Xuezhikang capsules per day （1.2g/day） for 8 weeks and Group 2, 55 patients took Simvastatin 10 mg per day for 8 weeks. At the end of 8 weeks, the lipid levels were compared with those of the baseline in each group. Results In Group 1, the serum levels of total cholesterol （TC）, low-density lipoprotein （LDL） cholesterol and trigly cerides （TG） decreased by 23.0％, 28.0％ and 28.1％（P＜0.001）, and in Group 2 reduced by 23.3％, 29.5％ and 29.5％（P＜0.001） respectively. The serum levels of high-density lipoprotein （HDL） cholesterol after taking Xuezhikang increased by 5.0％（P＞0.05） and after taking Simvastatin increased by 14.3％（P＜0.01）. No significant differences, however, were found between the two groups in TC, LDL cholesterol, TG, and HDL cholesterol. The side effects of Xuezhikang were less than those of Simvastatin. Conclusion Xuezhikang made in China is a safe, effective and well toleraled lipid modulator.

Abstract:Objective:To evaluate the dose-effect and the safety of Rosuvastatin in patients with primary hypercholesterolemia.Methods:We performed a multi-center clinical investigation of a randomized,double-blind and double-dummy positive drug parallel comparison.The screened eligible patients with primary hypercholesterolemia were randomized as(1∶1∶1)for the medication of Rosuvastatin(10 mg/day)group,n=110,Rosuvastatin(5 mg/day)group,n=101,and Atorvastatin(10 mg/day)group,n=104,for 8 weeks of oral administration.Results:After 8 weeks treatment,LDL-C decreased significantly in both Rosuvastatin groups(10 mg/day and 5 mg/day)than those of Atorvastatin group(-43.6±18.7)% and(-42.5±17.0)% vs.(-36.5±17.1)%,both P0.05.There was no statistic difference between Rosuvastatin(5 mg/day)and(10 mg/day)groups in LDL-C reduction.The side effect occurrence was similar in three groups.Conclusion:Rosuvastatin(5 mg/day and 10 mg/day)presented the better effect on decreasing the blood lipid level compared with Atovastatin(10 mg/day)in patients with primary hypercholesterolemia.The medication safety was similar in three experimental groups.
Effect of Xuezhikang Combined with Silibinin on Serum Lipids of Fatty Liver Patients

Abstract:OBJECTIVE:To study the effect of xuezhikang combined with silibinin on serum lipids of fatty liver patients.METHODS:208 cases of fatty liver in our hospital were randomly divided into 2 groups:106 cases of xuezhikang control group and 102 cases of xuezhikang combined silibinin group.Total cholesterol,triglycerides,low density lipoprotein,high density lipoprotein,alanine aminotransferase,aspartate aminotransferase,γ-glutamyl peptidase,albumin-globulin ratio and total bilirubin was tested before and after treatment.RESULTS:After treatment,both 2 groups in all parameters were significantly improved(P< 0.05).Compared with the xuezhikang control group,total cholesterol,triglycerides,low-density lipoprotein protein,alanine aminotransferase,aspartate aminotransferase,γ-glutamyl peptidase,albumin-globulin ratio and total bilirubin in treatment group were significantly lower high-density lipoprotein was significantly increased(P<0.05).There were no serious ADRs found in 2 groups.CONCLUSION:Xuezhikang combined with silibinin can improve fatty liver lipid levels significantly with good safety.
Combined effects of atorvastatin and soybean isoflavones on hypercholesterolemia and lipoprotein metabolism

Abstract:AIM: To study the combined effects of atorvastatin and soybean isoflavones on hypercholesterolemia and lipoprotein metabolism to provide scientific basis for the combination therepy. METHODS: The Sprague-Dawley rats were fed with high fatty diet for 12 consecutive weeks as hyperlipidemia model. The content of lipoprotein in serum, pathological process and mRNA expression of low density lipoprotein receptor and apoA-1 were detected by enzymatic analysis, frozen section, RT-PCR and Western blot respectively. RESULTS: The combination of atorvastatin and soybean isoflavones decreased the contents of total cholesterol (TC), low density lipoprotein-C (LDL-C) and increased the content of high density lipoprotein-C (HDL-C), superior than using atorvastatin alone (P < 0.05). It inhibited the deposition of lipoprotein in liver. The results of RT-PCR and Western blot test showed the combination atorvastatin and soybean isoflavones (40 mg • kg -1 + 120 mg • kg -1, 20 mg • kg -1 + 60 mg • kg -1) were better than atorvastatin 20 mg • kg -1 in regulating the expression of LDL receptor (LDLR) and apoA-1 mRNA and LDLR protein expression (P < 0.05), combination of atorvastatin and soybean isoflavones (10 mg • kg -1 + 30 mg • kg -1) upregulated apoA-1 mRNA expression better than atorvastatin 20 mg • kg -1 (P < 0.05). CONCLUSION: Combination of atorvastatin and soybean isoflavones can prevent lipid metabolism disorders through upregulation of LDLR and apoA-1 transcription level and LDLR protein level.
The effect of simvastatin on C reactive protein in patients with coronary heart disease

Abstract:Objective To investigate the changes of C reactive protein (CRP) in coronary heart disease (CHD) and the effect of simvastatin on CRP. Methods The plasma CRP levels were evaluated in patients with CHD and age-matched normal control group. Patients with CHD were randomized to simvastatin group and non-simvastatin group, and the CRP levels were measured before and after treatment. Results The plasma CRP levels in patients with CHD were significantly higher than those of normal controls (9.87±2.32)mg/ml vs (4.28±1.23)mg/ml(P0.01) . In simvastatin group the plasma CRP levels were significantly decreased after treatment (7.31±2.87)mg/ml, while those of non-simvastatin group remained higher level (9.22±2.54)mg/ml. The latter was significantly higher than former (t=2.34, P0.05). Conclusion Patients with CHD have significantly higher plasma CRP. Simvastatin can decrease the CRP level in patients with CHD.

Abstract:OBJECTIVE:To observe the effects of Xuezhikang and simvastatin on cerebral ischemia/reperfusion injury in rat, as well as the influences after intervention with L-NAME.METHOD:Rats were given orally with Xuezhikang and simvastatin or vehicle for 2 weeks, and then subjected to middle cerebral artery occlusion for 120 min using intraluminal filament model. L-NAME were injected into the lateral ventricles in half of the rats treated with Xuezhikang and simvastatin 45 min before the ischemia. The neurological deficits examinations were performed at 2, 24, 48 h after reperfusion. After the last examination the animals were sacrificed, the infarct volumes were determined by TTC staining, and MDA levels were also measured.RESULT:Xuezhikang and simvastatin both significantly reduced the infarct volume and improved the functional recovery when compared to vehicle. Xuezhikang and simvastatin both significantly decreased the MDA accumulation after reperfusion. L-NAME partially inhibited the protective effect of Xuezhikang but nearly completely abolished the protective effect of simvastatin.CONCLUSION:Xuezhikang has protective effects on ischemic brain damage in rats, which the beneficial effects are partly due to the statins components. The other components in Xuezhikang may also account for the neuroprotective effects, which is worth further investigations.

Reassessment of Lipid Regulating Effect of Xuezhikang on Primary Hyperlipidemia Patients: A Systematic Review

Abstract:Objective To assess the effect of Xuezhikang 1.2 g/d after 4 weeks' treatment on primary hyperlipidemia patients' lipid contents including total cholesterol(TC), triglyceride(TG), low density lipoprotein cholesterol(LDL-C) and high density lipoprotein cholesterol(HDL-C) by literature analysis. Methods A literature research was carried out in CNKI, Wanfang and Weipu full-text database to collect articles published from January 1st, 1994 to August 30 th, 2014, concerning the self-controlled trials on the lipid regulating ef ects of Xuezhikang 1.2 g/d for 4 weeks in primary hyperlipidemia patients. Statistical analysis was performed with Review Manager 5.2. Results Fit yi ve independent trials were included in this review. All of the independent studies showed no homogeneity(P 0.01, I2 70%), and had publication bias. The contents of TC, TG, and LDL-C after a 4-week treatment with 1.2 g/d Xuezhikang were significantly lower than those before treatment [WMD=1.14 mmol/L, 95%CI(1.02, 1.25) mmol/L, P 0.000 01; WMD=0.72 mmol/L, 95%CI(0.59, 0.85) mmol/L, P 0.000 01; WMD=0.87 mmol/L, 95%CI(0.74, 1.00) mmol/L,P 0.000 01] and the contents of HDL-C was signii cantly higher than that before treatment [WMD=-0.16 mmol/L, 95%CI(-0.26,-0.12) mmol/L, P 0.000 01]. Conclusion Xuezhikang can reduce the primary hyperlipidemia patients' TC, TG and LDL-C, and increase HDL-C at er 4 weeks' treatment.

Comparison on the efficacy of atorvastatin, simvastatin and rosuvastatin in the treatment of hypercholesterolemia

Abstract:Objective To compare the efficacy of atorvastatin,simvastatin and rosuvastatin in the treatment of hypercholesterolemia.Methods Selected 120 cases of patients with hypercholesterolemia who were treated in our hospital from January 2014 to December 2016,divided into atorvastatin group,simvastatin group and rosuvastatin group.The blood lipid levels and the incidence of adverse reactions were compared between the three groups before treatment,after treatment for fourth weeks and after treatment for eighth weeks.Results The serum lipid level were significantly improved in three groups after treatment (P ＜ 0.05),and rosuvastatin group was significantly better than atorvastatin group and simvastatin group (P ＜ 0.05);the incidence of adverse reactions of the three groups showed no significant difference.Conclusion Atorvastatin,simvastatin and rosuvastatin can effectively improve the blood lipid levels in patients with hypercholesterolemia,and the effect ofrosuvastatin is the best.
Efficacy and safety of simvastatin and Xuezhikang in newly diagnosed elderly type 2 diabetic patients with dyslipidemia

Abstract:Objective To observe the incidence and awareness of dyslipidemia in newly diagnosed elderly type 2 diabetic patients,and to determine the efficacy and safety of simvastatin and Xuezhikang in the treatment of dyslipidemia.Methods Totally 255 newly diagnosed type 2 diabetic patients aged 60 to 75 years in CDCPS research were included and the incidence of dyslipidemia were retrospectively analyzed.Patients were divided into 3 groups:the group 1 was given simvastatin (20 mg/d); the group 2 was given Xuezhikang (0.6～ 1.2 g/d); the group 3 was given no lipid-lowering drugs.All the three groups were given lifestyle intervention and blood pressure and blood sugar control.All patients were followed up monthly and TG,TC,LDL-C,BUN,ALT and creatinine were examined at 7th,14th,and 20th months.Results The incidence of dyslipidemia and the rate of awareness in the study cohort was 62％ and 55.7％.Hypertriglyceridemia was the most common type of dyslipidemia (29％).Among 88 patients with dyslipidemia,25 (28.4％) patients had been treated with lipid-lowering drugs before our study,in whom,8(32％) patients had normal serum lipid levels and only 3 (12％)patients reached to the control standards.20 months after the treatment,the decrement scales of TG,TC and LDL-C were 1.8％,10.5 ％ and 20 ％ respectively in group 1;5.5 ％,15.0％ and 15.7％ respectively in group 2;2.7％,8.7％ and 4.5％ respectively in group 3.The long-term lifestyle intervention and blood pressure and blood sugar control reduced serum lipid to some degree.In the patients with dyslipidemia,lipid-lowering drugs had a better effect on serum lipid reduction than did the lifestyle intervention (P=0.0047,0.0433).There was no significant difference between simvastatin and Xuezhikang.The function changes of liver and kidney had no difference before and after drug intervention (P＞0.05).Conclusions Serum lipid should be monitored and early medicine intervention should be taken in newly diagnosed elderly type 2 diabetic patients.Medicine intervention has a better effect on serum lipid reduction than lifestyle intervention,and there are no significant differences in efficacy and safety between simvastatin and Xuezhikang.
Comparison of the Reductions in LDL-C and Non-HDL-C Induced by the Red Yeast Rice Extract Xuezhikang Between Fasting and Non-fasting States in Patients With Coronary Heart Disease

Abstract:Background: Xuezhikang, an extract of red yeast rice, effectively lowers fasting blood lipid levels. However, the influence of Xuezhikang on the non-fasting levels of low-density lipoprotein cholesterol (LDL-C) and non-high-density lipoprotein cholesterol (non-HDL-C) has not been explored in Chinese patients with coronary heart disease (CHD). Methods: Fifty CHD patients were enrolled and randomly divided into two groups ( n = 25 each) to receive 1,200 mg/d of Xuezhikang or a placebo for 6 weeks as routine therapy. Blood lipids were repeatedly measured before and after 6 weeks of treatment at 0, 2, 4, and 6 h after a standard breakfast containing 800 kcal and 50 g of fat. Results: The serum LDL-C levels significantly decreased, from a fasting level of 3.88 mmol/L to non-fasting levels of 2.99, 2.83, and 3.23 mmol/L at 2, 4, and 6 h, respectively, after breakfast ( P < 0.05). The serum non-HDL-C level mildly increased from a fasting level of 4.29 mmol/L to non-fasting levels of 4.32, 4.38, and 4.34 mmol/L at 2, 4, and 6 h post-prandially, respectively, and the difference reached statistical significance only at 4 and 6 h after breakfast ( P < 0.05). After 6 weeks of Xuezhikang treatment, the patients had significantly lower fasting and non-fasting serum levels of LDL-C and non-HDL-C ( P < 0.05) than at pretreatment. The LDL-C levels were reduced by 27.8, 28.1, 26.2, and 25.3% at 0, 2, 4, and 6 h, respectively, and the non-HDL-C levels were reduced by 27.6, 28.7, 29.0, and 28.0% at 0, 2, 4, and 6 h, respectively, after breakfast. No significant difference was found in the percent reductions in the LDL-C and non-HDL-C levels among the four different time-points. Conclusions: Six weeks of Xuezhikang treatment significantly decreased LDL-C and non-HDL-C levels, with similar percent reductions in fasting and non-fasting states in CHD patients, indicating that the percent change in non-fasting LDL-C or non-HDL-C could replace that in the fasting state for evaluation the efficacy of cholesterol control in CHD patients who are unwilling or unable to fast.

Isoflavones enhance pharmacokinetic exposure of active lovastatin acid via the upregulation of carboxylesterase in high-fat diet mice after oral administration of Xuezhikang capsules

Abstract:Xuezhikang capsule (XZK) is a traditional Chinese medicine that contains lovastatin (Lv) for hyperlipidemia treatment, although it has fewer side effects than Lv. However, the pharmacokinetic mechanisms contributing to its distinct efficacy and low side effects are unclear. Mice were fed a high-fat diet (HFD) for 6 weeks to induce hyperlipidemia. We first conducted the pharmacokinetic studies in HFD mice following oral administration of Lv (10 mg/kg, i.g.) and found that HFD remarkably decreased the active form of Lv (the lovastatin acid, LvA) exposure in the circulation system, especially in the targeting organ liver, with a declined conversion from Lv to LvA, whereas the Lv (responsible for myotoxicity) exposure in muscle markedly increased. Then we compared the pharmacokinetic profiles of Lv in HFD mice after the oral administration of XZK (1200 mg/kg, i.g.) or an equivalent dose of Lv (10 mg/kg, i.g.). A higher exposure of LvA and lower exposure of Lv were observed after XZK administration, suggesting a pharmacokinetic interaction of some ingredients in XZK. Further studies revealed that HFD promoted the inflammation and inhibited carboxylesterase (CES) activities in the intestine and the liver, thus contributing to the lower transformation of Lv into LvA. In contrast, XZK inhibited the inflammation and upregulated CES in the intestine and the liver. Finally, we evaluated the effects of monacolins and phytosterols, the fractional extracts of isoflavones, on inflammatory LS174T or HepG2 cells, which showed that isoflavones inhibited inflammation, upregulated CES, and markedly enhanced the conversion of Lv into LvA. For the first time, we provide evidence that isoflavones and Lv in XZK act in concert to enhance the efficacy and reduce the side effects of Lv.

Abstract:OBJECTIVE To investigate whether statins have effect on diastolic function of both left and right ventricles in hypertensive patients. METHODS This is a randomized, mono-blind, placebo-controlled study. 120 systemic hypertensive (HT) patients with normal or slightly elevated cholesterol were randomized to placebo or Xuezhikang (1200 mg/d) for 24 weeks. Extended-release nifedipine was administrated to the HT patients. 30 healthy volunteers served as controls. Plasma were obtained at baseline and 24 weeks after Xuezhikang therapy. Cholesterol and carboxy-terminal peptide of procollagen type I (PIP) were measured. Early diastolic velocity (Em) and late diastolic velocity (Am) were obtained from right atrioventricular ring and left atrioventricular ring with pulsed wave tissue Doppler imaging. RESULTS The levels of plasma PIP were higher in HT patients. After 24 weeks, the levels of plasma LDL-C, TC and PIP were significantly lower in Xuezhikang group than those in placebo group; Systolic and diastolic pressure were decreased both in placebo group and Xuezhikang group meanwhile pulse pressure was decreased and Em/Am ratio at left atrioventricular ring was higher in Xuezhikang group as compared with those in placebo group. CONCLUSION In systemic hypertensive patients, Xuezhikang exerts a beneficial effect on diastolic function of left ventricule via controlling blood pressure, lowering blood lipid and inhibiting myocardial fibrosis.
Effect of Xuezhikang on hepatitis and oxidative stress in rats with nonalcoholic fatty liver disease

Abstract:Objective To investigate the effect of Xuezhikang on hepatitis and oxidative stress in rats with nonalcoholic fatty liver dis-ease,and analyze its related mechanism. Methods A total of 32 Wistar rats were assigned randomly into 4 groups:the control group, model group,lovastatin group and Xuezhikang group,with 8 rats in each group.Basic chow was provided for control group,the rats in the other three groups were fed with a high fat diet. On the first day,the rats were injected with a single dose of vitamin D3(6×105U/kg), the rats in the control group were injected with the same dose of saline. After 6 weeks of high-fat diet,the rats in the lovastatin group were treated with lovastatin 2.5 mg/(kg·d) and the rats in the Xuezhikang group were treated with Xuezhikang 300 mg/( kg·d) by gastric tube. After 12 weeks of high-fat diet,the blood lipid and liver function of rats in each group were detected. Serum TNF-α and IL-6 levels were detected by ELISA;immunohistochemistry was used to detect the expression of TGF-β in liver tissues;morphological changes of liver tissue were observed. Results The TG,TC,LDL-C,TNF-α,IL-6,ALT and AST levels of the model group were higher than those of the lovastatin group and the Xuezhikang group;HDL-C level was lower than that of the lovastatin group and the Xuezhi-kang group.TNF-α and IL-6 levels in the lovastatin group were higher than those in the Xuezhikang group,and the difference between the two groups was statistically significant(P<0.05). Comparison of serum lipid level,liver function and pathological changes of liver tissue between Xuezhikang group and lovastatin group had no statistically significant difference(P>0.05). The hepatic steatosis and collagen hyperplasia were the most serious in the model group and the least in the control group. In the model group,the positive ex-pression of TGF-β was the highest in the liver tissue of rats,while that of the control group was the least. Conclusion Xuezhikang can regulate blood lipids,has anti-inflammatory effect,and can exert protective effect on liver tissue through anti-inflammation mechanism, and its effect is superior to lovastatin.
Xuezhikang Capsule for Hyperlipidemia: A Systematic Review

Abstract:Objective To assess the efficacy and safety of Xuezhikang capsule (XZK) for hyperlipidemia. Methods MEDIINE,EMBASE, Cochrane Central Register of Controlled Trials(CCTR), Cochrane Metabolic and Endocrine Disorders Group data bank, and Chinese Biomedical Database(CBM), China Hospital Knowledge Database(CHKD) were searched, and the published/unpublished information was handsearched (updated to Dec., 2005) to identify randomized controlled trials (RCTs) or quasi-RCT of XZK versus statins or other lipid lowering drugs in treating hyperlipidemia patients. The quality evaluation, data extraction and analysis were conducted by the method recommended in Cochrane Reviewer’s Handbook 4.2.2. Data were analyzed using Review Manager (Version 4.2). Results Eleven trials (conducted in China) were identified, including 1 073 patients with hyperlipidemia met the inclusion criteria. All the included RCTs were graded as B or C. ① The effect of XZK in reducing TC: There were no significant differences between XZK and statins ( WMD 0.06, 95%CI-0.09 to 0.20 and RR1.02, 95%CI 0.93 to 1.12), XZK and fibrates (WMD 0.05, 95%CI -0.22to 0.31) and XZK and probucol(WMD 0.42, 95%CI -0.01 to 0.85). XZK was superior to hexanicit (WMD 0.96, 95%CI 0.73 to 1.18). ② The effect of XZK in reducing TG: XZK had the same effect as statins (WMD 0.02, 95%CI -0.10 to 0.14 and RR 1.17, 95%CI 0.92 to 1.49) and hexanicit (WMD 0.28, 95%CI -0.17 to 0.73 ).Meanwhile, XZK was superior to probucol (WMD 0.71, 95%CI 0.22 to 1.20), but it was not as good as fibrates (WMD -0.81, 95%CI -1.40 to -0.23). ③ The effect of XZK in increasing HDL-C:XZK had the same effect as that of statins (WMD -0.03, 95%CI -0.10 to 0.04 and RR 1.03, 95% CI 0.80 to 1.33). The effect of XZK was better than that of hexanicit (WMD 0.15, 95%CI 0.01 to 0.29). XZK had the same effect of fibrates (WMD 0.03, 95%CI -0.08 to 0.15) and probucol (WMD 0.04, 95%CI -0.16 to 0.24). ④ The effect of XZK in reducing LDL-C:The effects of XZK and statins were the same (WMD -0.23, 95%CI -0.61 to 0.15 and RR 1.15, 95%CI 0.75 to 1.77). The effect of XZK was better than that of hexanicit (WMD 0.53, 95%CI 0.16 to 0.90). Meanwhile, XZK had the same effect as those of fibrates (WMD 0.19, 95%CI -0.12 to 0.50) and probucol (WMD 0.35, 95%CI -0.03 to 0.73). ⑤ The adverse reactions of XZK were mainly the gastrointestinal tract reaction, while liver function abnormality and myalgia were scarcely found. Conclusion Xuezhikang capsule have the same effects as those of statins in reducing the levels of TC, TG, LDL-C and raising HDL-C in patients with hyperlipidemia. No obvious adverse reactions are found during the short-term treatment. But further confirmation with clinical randomized controlled trials of high quality, large sample and long-term follow-up is needed.

Effect of Simvastatin on Acute Coronary Syndrome Complicated with Atrial Fibrillation

Abstract:Objective: To investigate the effect of simvastatin on acute coronary syndrome complicated with atrial fibrillation.Methods: A total of 112 cases with acute coronary syndrome complicated with atrial fibrillation were randomly divided into treatment group(60 cases) and control group(52 cases).Besides conventional treatment,simvastatin was used at 40 mg/d for 4 weeks,then 20 mg/d.Therapy of simvastatin was irregular in control group with total course less than 8 weeks,and the total course was 12 months.Hepal and renal function,high senstive C-reaction protein,blood lipid,reoccurrence of paroxysmal atrial fibrillation,incidence of sustained or permanent atrial fibrillation and status of cardiovascular diseases.Results: There was insignificant difference in blood lipid and C-reactive protein before and after treatment in control group(P0.05).After treatment,the levels of total cholesterol,low-density cholesterol and C-reactive protein were significantly decreased(P0.05),and high-density lipoprotein was significantly increased(P0.05) in treatment group.6 cases(10%) had reoccurrence of paroxysmal atri-al fibrillation or had paroxysmal transforming into sustained or permanent one in treatment group,which was significantly lower than control group(17/52,36.5%)(P0.05).And 8 cases had cardiovascular problem,significantly lower than control group(22/52,42.3%)(P0.05).Conclusion: Simvastatin can regulate blood lipid,anti-atrial fibrillation,and decrease the incidence of cardiovascular diseases.
Meta-analysis of the Efficacy and Safety of Simvastatin vs. Pravastatin in the Treatment of Hyperlipidemia

Abstract:OBJECTIVE:To systematically evaluate the efficacy and safety of simvastatin vs. pravastatin in the treatment of hy-perlipidemia,and provide evidence-based reference for the clinical treatment. METHODS:PubMed,Medline,EMBase,Cochrane Library and CJFD were retrieved to collect the randomized controlled trial(RCT)of efficacy and safety of simvastatin(test group) and pravastatin (control group) in the treatment of hyperlipidemia. The methodological quality of included studies was evaluated. The Rev Man 5.2 software was chosen for data analysis. RESULTS:14 RCT involving 1 019 patients were included. Meta-analysis showed that simvastatin had more significant effect in the decreasing of TC [MD=-0.34,95%CI(-0.52,-0.16),P<0.001] and LDL-C[MD=-0.31,95%CI(-0.45,-0.17),P<0.001] than pravastatin;and simvastatin and pravastatin had the similar effect in TG[MD=-0.06,95%CI(-0.18,0.05),P=0.28)] and HDL-C[MD=0.00,95%CI(-0.04,0.04),P=0.85]. Adverse drug reaction rate results showed they were similar[OR=0.70,95%CI(0.36,1.39),P=0.31]. CONCLUSIONS:Simvastatin is more effective in lipid-lowering than pravastatin with similar safety. Due to the limited number and low quality of included studies,it remains to be further verified with more reasonably designed,multi-center and large-sample studies.
Comparison of the Reduction of LDL-C or nonHDL-C induced by Red Yeast Rice Extract, Xuezhikang, between Fasting and Postprandial States in Patients with Coronary Artery Disease

Abstract:Abstract Background: Xuezhikang, an extract of red yeast rice, effectively lowers fasting and postprandial triglyceride (TG) levels. It was unknown that whether Xuezhikang could contribute the lipid management goals, low-density lipoprotein cholesterol (LDL-C) and non-high-density lipoprotein cholesterol (nonHDL-C) at fasting and postprandial states in patients with coronary artery disease (CAD). Methods: Fifty CAD patients were divided into Xuezhikang (XZK, n=25) group and control (CON, n=25) group randomly to receive red yeast rice exact, 1200mg/d Xuezhikang capsules or not for 6 weeks (6w). Blood lipids were detected repeatedly before and after 6w at 0, 2, 4 and 6 hours (h) after a standard breakfast with 800kcal. Result: When taking all patients as a whole (n=50), serum LDL-C level decreased while TG and RC levels increased significantly at 2, 4 and 6 h after breakfast ( P <0.05). Serum nonHDL-C level mildly but significantly increased at 4h and 6h after breakfast ( P <0.05). Short-term Xuezhikang treatment decreased tAUCs of TC, TG, LDL-C, nonHDL-C and RC whereas increased that of HDL-C significantly ( P <0.05). Serum LDL-C level showed a drop of 27.8%, 28.1%, 26.2%, 25.3% at 0, 2, 4 and 6 h, respectively, after breakfast. Serum nonHDL-C level showed a drop of 27.6%, 28.7%, 29.0% and 28.0% at 0, 2, 4 and 6 h, respectively. There was no significant difference in the percentages of reduction in LDL-C or nonHDL-C level among four time-points. Conclusions: Xuezhikang significantly decreased LDL-C or nonHDL-C level with similar percentages of reduction between fasting and postprandial states in patients with CAD, indicating that postprandial blood lipids detected at the same time point after a daily meal could replace fasting blood lipids to evaluate the efficacy of cholesterol-lowering therapy in CAD patients, unwilling or unable to keep a fasting state.
Long-Term Effects of Xuezhikang on Blood Pressure in Hypertensive Patients with Previous Myocardial Infarction: Data from the Chinese Coronary Secondary Prevention Study (CCSPS)

Abstract:Several previous trials from Western population studies have showed that statins may help reduce blood pressure (BP). However, randomized clinical data is limited. Xuezhikang, a partially extract of red yeast rice, contains a family of naturally occurring statins, and has a marked impact on lipids, but it is unknown whether Xuezhikang has any effect on BP during long-term follow-up in the Chinese population. This is a post-hoc subgroup analysis of a randomized, double-blinded, placebo-controlled, parallel group clinical trial, Chinese Coronary Secondary Prevention Study (CCSPS). A total of 2704 hypertensive patients with previous myocardial infarction (MI) were assigned either to placebo (n = 1341) or to Xuezhikang (n = 1363) daily for an average of 4.5 years. The primary outcome was the unadjusted changes in mean arterial pressure (MAP) from baseline to 6 months. We also assessed systolic blood pressure (SBP), diastolic blood pressure (DBP), and pulse pressure. Analysis of covariance was used to calculate the adjusted effects of treatment on changes in these outcomes at 6, 12, 24, and 48 months post-randomization, after controlling for potential confounders. This analysis included 2704/4870 (55.5%) hypertensive patients for whom BP was measured at baseline and at least one follow-up visit after randomization. Median duration of the follow-up was 4.5 years (54 months), and 25 patients (0.92%) were lost to the last follow-up because of adverse effects. The results showed that the unadjusted and adjusted changes in MAP, SBP, DBP, or pulse pressure from baseline were not significantly different for Xuezhikang or placebo recipients at 6, 12, 24, and 48 months after randomization. In this post-hoc subgroup analysis, we failed to demonstrate any significant reducing effects of Xuezhikang on BP in Chinese hypertensive patients with previous MI, suggesting that further prospective study on the effects of statins on BP would be needed, especially in high-risk patients.
