Abstract:BACKGROUND:Breast tumor heterogeneity is related to risk factors that lead to worse prognosis, yet such heterogeneity has not been well studied.PURPOSE:To predict the Ki-67 status of estrogen receptor (ER)-positive breast cancer patients via analysis of tumor heterogeneity with subgroup identification based on patterns of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI).STUDY TYPE:Retrospective study.POPULATION:Seventy-seven breast cancer patients with ER-positive breast cancer were investigated, of whom 51 had low Ki-67 expression.FIELD STRENGTH/SEQUENCE:T1 -weighted 3.0T DCE-MR images.ASSESSMENT:Each tumor was partitioned into multiple subregions using three methods based on patterns of dynamic enhancement: 1) time to peak (TTP), 2) peak enhancement rate (PER), and 3) kinetic pattern clustering (KPC). In each tumor subregion, 18 texture features were computed.STATISTICAL TESTING:Univariate and multivariate logistic regression analyses were performed using a leave-one-out-based cross-validation (LOOCV) method. The partitioning results were compared with the same feature extraction methods across the whole tumor.RESULTS:In the univariate analysis, the best-performing feature was the texture statistic of sum variance in the tumor subregion with early TTP for differentiating between patients with high and low Ki-67 expression (area under the receiver operating characteristic curves, AUC = 0.748). Multivariate analysis showed that features from the tumor subregion associated with early TTP yielded the highest performance (AUC = 0.807) among the subregions for predicting the Ki-67 status. Among all regions, the tumor area with high PER at a precontrast MR image achieved the highest performance (AUC = 0.722), while the subregion that exhibited the highest overall enhancement rate based on KPC had an AUC of 0.731. These three models based on intratumoral texture analysis significantly (P < 0.01) outperformed the model using features from the whole tumor (AUC = 0.59).DATA CONCLUSION:Texture analysis of intratumoral heterogeneity has the potential to serve as a valuable clinical marker to enhance the prediction of breast cancer prognosis.LEVEL OF EVIDENCE:4 Technical Efficacy: Stage 3 J. Abstract:Breast cancer, with its high heterogeneity, is the most common malignancies in women. In addition to the entire tumor itself, tumor microenvironment could also play a fundamental role on the occurrence and development of tumors. The aim of this study is to investigate the role of heterogeneity within a tumor and the surrounding stromal tissue in predicting the Ki-67 proliferation status of oestrogen receptor (ER)-positive breast cancer patients. To this end, we collected 62 patients imaged with preoperative dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) for analysis. The tumor and the peritumoral stromal tissue were segmented into 8 shells with 5 mm width outside of tumor. The mean enhancement rate in the stromal shells showed a decreasing order if their distances to the tumor increase. Statistical and texture features were extracted from the tumor and the surrounding stromal bands, and multivariate logistic regression classifiers were trained and tested based on these features. An area under the receiver operating characteristic curve (AUC) were calculated to evaluate performance of the classifiers. Furthermore, the statistical model using features extracted from boundary shell next to the tumor produced AUC of 0.796 +/- 0.076, which is better than that using features from the other subregions. Furthermore, the prediction model using 7 features from the entire tumor produced an AUC value of 0.855 +/- 0.065. The classifier based on 9 selected features extracted from peritumoral stromal region showed an AUC value of 0.870 +/- 0.050. Finally, after fusion of the predictive model obtained from entire tumor and the peritumoral stromal regions, the classifier performance was significantly improved with AUC of 0.920. The results indicated that heterogeneity in tumor boundary and peritumoral stromal region could be valuable in predicting the indicator associated with prognosis.
Intratumor Heterogeneity of DCE-MRI Reveals Ki-67 Proliferation Status in Breast Cancer

Abstract:Breast cancer is a highly heterogeneous disease both biologically and clinically, and certain pathologic parameters, i.e., Ki67 expression, are useful in predicting the prognosis of patients. The aim of the study is to identify intratumor heterogeneity of breast cancer for predicting Ki-67 proliferation status in estrogen receptor (ER)-positive breast cancer patients. A dataset of 77 patients was collected who underwent dynamic contrast enhancement magnetic resonance imaging (DCE-MRI) examination. Of these patients, 51 were high-Ki-67 expression and 26 were low-Ki-67 expression. We partitioned the breast tumor into subregions using two methods based on the values of time to peak (TTP) and peak enhancement rate (PER). Within each tumor subregion, image features were extracted including statistical and morphological features from DCE-MRI. The classification models were applied on each region separately to assess whether the classifiers based on features extracted from various subregions features could have different performance for prediction. An area under a receiver operating characteristic curve (AUC) was computed using leave-one-out cross-validation (LOOCV) method. The classifier using features related with moderate time to peak achieved best performance with AUC of 0.826 than that based on the other regions. While using multi-classifier fusion method, the AUC value was significantly (P=0.03) increased to 0.858 +/- 0.032 compare to classifier with AUC of 0.778 using features from the entire tumor. The results demonstrated that features reflect heterogeneity in intratumoral subregions can improve the classifier performance to predict the Ki-67 proliferation status than the classifier using features from entire tumor alone.

Abstract:In breast cancer, tumor heterogeneity is a reflection of differing tumor subtypes, which may display markedly different genotypes and clinical phenotypes. Although pathological and qualitative (based on contrast enhancement patterns) studies suggest the presence of clinical and molecular predictive tumor subregions, this has not been fully investigated. Our goal is to develop a novel algorithm to utilize the potential information available in different tumor subregions (periphery and core) by extracting textural kinetic features, for the purpose of estrogen receptor (ER) classification. We show that features from different tumor subregions, at appropriate scales and quantization levels, can be used to better classify ER subtypes than features averaged from the whole tumor. We analyzed representative two dimensional (2D) slices from twenty breast tumors with volumetric dynamic contrast enhanced magnetic resonance imaging (DCE-MRI) available by extracting multi-parametric textural kinetic features from the periphery, core and whole tumor. The utility of the features from different subregions are evaluated using six meta-classifiers (feature selector and classifier pairs), formed from two feature selectors and three classifiers. Classification accuracy approached 94%.

Abstract:Breast cancer is one of the most common malignant tumor in females. Adjuvant chemotherapy is a common method of breast cancer treatment, while not all patients will benefit from the treatment. The purpose of this study is to predict prognosis of breast cancer and stratify patients with high risk of recurrence by radiomic analysis based on dynamic-enhanced magnetic resonance imaging (DCE-MRI) and gene expression data. We performed this study in three steps. First, a retrospective single-institution cohort of 61 patients with invasive breast cancer was enrolled. We extracted quantitative imaging features depicting tumor enhancement patterns and screened for those that were potentially prognostic for survival. Multivariate Cox regression analysis showed that image feature of inverse difference was independently associated with recurrence-free survival (RFS) with P value of 0.0371. Second, we built a regression model with 74-gene signature from 87 patients whose MRI and gene expression data were available for associating image features that is related with breast cancer prognosis identified in the first dataset. Finally, we validated the prognostic value of the established signature by applying it on public available genomic data sets. Using the 74-gene signature in the independent validation set, 1010 patients were divided into two groups to stratify patients for RFS (log-rank P=0.011) and overall survival (OS, log-rank P=0.029) among different survival risk levels. Our results showed that imaging features representing tumor biological characteristics would be valuable in predicting prognosis of breast cancer.

Abstract:Human epidermal growth factor receptor-2 (HER2) plays an important role in treatment strategy and prognosis determination in breast cancers. However, breast cancers are characterized by considerable heterogeneity both between and within tumors, which is a key impediment to accurately determine HER2 status for radiomic analysis. To this end, tumor heterogeneity was evaluated by unsupervised decomposition method on breast magnetic resonance imaging (MRI), in which three tumor subregions were generated terms as Input, Fast and Slow. This tumor decomposition was performed by a convex analysis of mixtures (CAM) method, which was designed according to analysis of contrast-enhancement patterns. The study retrospectively investigated 181 patients who underwent dynamic contrast enhancement magnetic resonance imaging (DCE-MRI) examination. Among them, 124 were HER2-negative and 57 were HER2-positive status. Imaging features of texture and histogram were computed in each subregion. Multivariate logistic regression classifiers were trained and validated with leave-one-out cross-validation (LOOCV) method. An area under a receiver operating characteristic curve (AUC) was calculated to assess performance of the classifier. The classifier based on features from Fast subregion obtained an AUC of 0.802 ± 0.067 and was significantly (P = 0.0113) outperformed the classifier based on features from the whole tumors. When the predicted values from the respective classifiers were fused by weighted average, the AUC significantly increased to 0.820 ± 0.063 (P = 0.0011). The results indicate that analysis of intratumor heterogeneity through decomposing method of DCE-MRI has the potential to serve as a marker for predicting HER2 status.

First, principal component analysis (PCA) was used to reduce the dimensionality of the DCE‐MRI data with a high‐temporal resolution. We then partitioned the whole tumor into multiple subregions using k‐means clustering based on the PCA‐defined eigenmaps. Within each tumor subregion, we extracted four quantitative Haralick texture features based on the gray‐level co‐occurrence matrix (GLCM). The change in texture features in each tumor subregion between pre‐ and during‐NAC was used to predict pathological complete response after NAC.Results:Three tumor subregions were identified through clustering, each with distinct enhancement characteristics. In univariate analysis, all imaging predictors except one extracted from the tumor subregion associated with fast wash‐out were statistically significant (p< 0.05) after correcting for multiple testing, with area under the ROC curve or AUCs between 0.75 and 0.80. In multivariate analysis, the proposed imaging predictors achieved an AUC of 0.79 (p = 0.002) in leave‐one‐out cross validation. This improved upon conventional imaging predictors such as tumor volume (AUC=0.53) and texture features based on whole‐tumor analysis (AUC=0.65).Conclusion:The heterogeneity of the tumor subregion associated with fast wash‐out on DCE‐MRI predicted early pathological response to neoadjuvant chemotherapy in breast cancer.

Abstract:Breast cancer histological grade and lymph node status are important in evaluating the prognosis of patients. This study aim to predict these factors by analyzing the heterogeneity of tumor and its adjacent stroma based on dynamic contrast enhancement magnetic resonance imaging (DCE-MRI) and diffusion-weighted imaging (DWI). A dataset of 172 patients with surgically verified lymph node status (positive lymph nodes, n=62; negative lymph nodes, n=110) who underwent preoperative DCE-MRI and DWI examination was collected. Among them, 144 cases had available histological grade information, including 56 cases of low-grade (grade 1 and 2), and 88 samples of high-grade (grade 3). To this end, we identified six tumor subregions on DCE-MRI as well as the corresponding subregions on ADC according to their distances to the tumor boundary. The statistical and Haralick texture features were extracted in each subregion, based on which predictive models were built to predict histological grade and lymph node status in breast cancer. An area under a receiver operating characteristic curve (AUC) was computed with a leave-one-out cross-validation (LOOCV) method to assess each classifier's performance. For histological grade prediction, the classifier using DCE-MRI features in the inner tumor achieved best performance among all the subregions with AUC of 0.859. For lymph node status, classifier based on DCE-MRI features from tumor subregion of proximal peritumoral stromal shell obtained highest AUC of 0.882 among all the regions. Furthermore, the predictions from DCE-MRI and DWI were fused, and the AUC value was increased to 0.895 for discriminating histological grade. Our results demonstrate that DCE-MRI and ADC imaging features are complementary in predicting histological grade in breast cancer.
Prediction of Histological Grade and Ki-67 Expression in Breast Cancer by DCE-MRI and DWI Features

Abstract:The purpose of this study was to predict the histological grade of breast cancer and Ki-67 expression using features extracted from dynamic contrast enhancement magnetic resonance imaging (DCE-MRI) and diffusion weighted imaging (DWI). In this study, 144 cases of breast invasive ductal carcinoma were collected, which have not experienced breast surgery or chemotherapy before MRI examination. DW and DCE-MR images were obtained from preoperative breast MRI examination using a 3 T scanner, and ADC map was calculated from DWI. Breast tumor segmentation was performed on all of the image series. After that, image features of texture, statistic, and morphological features of breast tumor were extracted on both the DW and DCE-MR images. The unsupervised discriminative feature selection (UDFS) and Fisher Score algorithm were used for feature selection. The classification model was established on these images respectively, and the classifiers of single-parametric image were fused for prediction. In order to evaluate the classifier performance, the area under the receiver operating characteristic curve (AUC) were calculated in a leave-one-out cross-validation analysis. The predictive model based on the second postcontrast image series of DCE-MRI generated an AUC of 0.780 with the specificity and sensitivity of 0.647 and 0.934 respectively in histological grade task, while in Ki-67 expression task, the model based on DWI generated an AUC of 0.756 with the specificity and sensitivity of 0.806 and 0.695, respectively. After multi-classifier fusion using features both from the DWI and DCE-MRI, the classification result was increased to AUC of 0.808 with the specificity and sensitivity of 0.706 and 0.895 respectively in histological grade prediction task, and generated an AUC of 0.783 with the specificity and sensitivity of 0.778 and 0.722 respectively in Ki-67 expression prediction task. In conclusion, it was showed that compared with each single parametric image alone, the performance of the classifier could be improved by combining features of DCE-MRI and DWI.

Abstract:The purpose of this study was to investigate the role of features derived from breast dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) and to incorporated clinical information to predict the molecular subtypes of breast cancer. In particular, 60 breast cancers with the following four molecular subtypes were analyzed: luminal A, luminal B, human epidermal growth factor receptor-2 (HER2)-over-expressing and basal-like. The breast region was segmented and the suspicious tumor was depicted on sequentially scanned MR images from each case. In total, 90 features were obtained, including 88 imaging features related to morphology and texture as well as dynamic features from tumor and background parenchymal enhancement (BPE) and 2 clinical information-based parameters, namely, age and menopausal status. An evolutionary algorithm was used to select an optimal subset of features for classification. Using these features, we trained a multi-class logistic regression classifier that calculated the area under the receiver operating characteristic curve (AUC). The results of a prediction model using 24 selected features showed high overall classification performance, with an AUC value of 0.869. The predictive model discriminated among the luminal A, luminal B, HER2 and basal-like subtypes, with AUC values of 0.867, 0.786, 0.888 and 0.923, respectively. An additional independent dataset with 36 patients was utilized to validate the results. A similar classification analysis of the validation dataset showed an AUC of 0.872 using 15 image features, 10 of which were identical to those from the first cohort. We identified clinical information and 3D imaging features from DCE-MRI as candidate biomarkers for discriminating among four molecular subtypes of breast cancer.
Predicting Ki67% Expression from DCE-MR Images of Breast Tumors Using Textural Kinetic Features in Tumor Habitats.

Abstract:The use of Ki67% expression, a cell proliferation marker, as a predictive and prognostic factor has been widely studied in the literature. Yet its usefulness is limited due to inconsistent cut off scores for Ki67% expression, subjective differences in its assessment in various studies, and spatial variation in expression, which makes it difficult to reproduce as a reliable independent prognostic factor. Previous studies have shown that there are significant spatial variations in Ki67% expression, which may limit its clinical prognostic utility after core biopsy. These variations are most evident when examining the periphery of the tumor vs. the core. To date, prediction of Ki67% expression from quantitative image analysis of DCE-MRI is very limited. This work presents a novel computer aided diagnosis framework to use textural kinetics to (i) predict the ratio of periphery Ki67% expression to core Ki67% expression, and (ii) predict Ki67% expression from individual tumor habitats. The pilot cohort consists of T1 weighted fat saturated DCE-MR images from 17 patients. Support vector regression with a radial basis function was used for predicting the Ki67% expression and ratios. The initial results show that texture features from individual tumor habitats are more predictive of the Ki67% expression ratio and spatial Ki67% expression than features from the whole tumor. The Ki67% expression ratio could be predicted with a root mean square error (RMSE) of 1.67%. Quantitative image analysis of DCE-MRI using textural kinetic habitats, has the potential to be used as a non-invasive method for predicting Ki67 percentage and ratio, thus more accurately reporting high KI-67 expression for patient prognosis.

Abstract:The optimal biomarkers for the diagnosis of breast lymphovascular invasion have not yet been found. DCE-MRI features can evaluate the tumor microenvironment which is related to LVI. This study explored the independent predictor of LVI, as well as developed and validated the parametric combined prediction model for the diagnosis of lymphovascular invasion in breast cancer using quantitative analysis of DCE-MRI. The prediction model based on K ep and N stage could improve the performance of LVI prediction, compared to clinicopathological or magnetic resonance parameters.

To DCE-MRI data acquired from several hospitals across multiple provinces, a series of in-silico computational methods were applied for lesion segmentation and identification of breast tumor in this paper. The image segmentation methods include Otsu segmentation of subtraction images, signal-interference-ratio segmentation method and an improved variational level set method, each has its own application scope. After that, the distribution of benign and malignant in lesion region is identified based on three-time-point theory. From the experiment, the analysis of DCE-MRI data of breast tumor can show the distribution of benign and malignant in lesion region, provide a great help for clinicians to diagnose breast cancer more expediently and lay a basis for medical diagnosis and treatment planning.
Assessing the Role of Different Heterogeneous Regions in DCE-MRI for Predicting Molecular Subtypes of Breast Cancer Based on Network Architecture Search and Vision Transformer

Abstract:Breast cancer, the most common female malignancy, is highly heterogeneous, manifesting as different molecular subtypes. It is clinically important to distinguish between these molecular subtypes due to marked differences in prognosis, treatment and survival outcomes. In this study, we first performed convex analysis of mixtures (CAM) analysis on both intratumoral and peritumoral regions in DCE-MRI to generate multiple heterogeneous regions. Then, we developed a vision transformer (ViT)-based DL model and performed network architecture search (NAS) to evaluate all the combination of different heterogeneous regions for predicting molecular subtypes of breast cancer. Experimental results showed that the input plasma from both peritumoral and intratumoral regions, and the fast-flow kinetics from intratumoral regions were critical for predicting different molecular subtypes, achieving an area under receiver operating characteristic curve (AUROC) value of 0.66-0.68.Clinical Relevance— This study reduces the redundancy in multiple heterogeneous subregions and supports the precise prediction of molecular subtypes, which is of potential importance for the medicine care and treatment planning of patients with breast cancer.

Abstract:Objective: To evaluate the value of the malignant subregion-based texture analysis in predicting Ki-67 status in breast cancer. Materials and methods: The dynamic contrast-enhanced magnetic resonance imaging data of 119 histopathologically confirmed breast cancer patients (81 patients with high Ki-67 expression status) from January 2018 to February 2023 in our hospital were retrospectively collected. According to the enhancement curve of each voxel within the tumor, three subregions were divided: washout subregion, plateau subregion, and persistent subregion. The washout subregion and the plateau subregion were merged as the malignant subregion. The texture features of the malignant subregion were extracted using Pyradiomics software for texture analysis. The differences in texture features were compared between the low and high Ki-67 expression cohorts and then the receiver operating characteristic (ROC) curve analysis to evaluate the predictive performance of texture features on Ki-67 expression. Finally, a support vector machine (SVM) classifier was constructed based on differential features to predict the expression level of Ki-67, the performance of the classifier was evaluated using ROC analysis and confirmed using 10-fold cross-validation. Results: Through comparative analysis, 51 features exhibited significant differences between the low and high Ki-67 expression cohorts. Following feature reduction, 5 features were selected to build the SVM classifier, which achieved an area under the ROC curve (AUC) of 0.77 (0.68–0.87) for predicting the Ki-67 expression status. The accuracy, sensitivity, and specificity were 0.76, 0.80, and 0.68, respectively. The average AUC from the 10-fold cross-validation was 0.72 ± 0.14. Conclusion: The texture features of the malignant subregion in breast cancer were potential biomarkers for predicting Ki-67 expression level in breast cancer, which might be used to precisely diagnose and guide the treatment of breast cancer.

Abstract:Breast cancer is one of the most common malignant tumor with upgrading incidence in females. The key to decrease the mortality is early diagnosis and reasonable treatment. Molecular classification could provide better insights into patient-directed therapy and prognosis prediction of breast cancer. It is known that different molecular subtypes have different characteristics in magnetic resonance imaging (MRI) examination. Therefore, we assumed that imaging features can reflect molecular information in breast cancer. In this study, we investigated associations between dynamic contrasts enhanced MRI (DCE-MRI) features and molecular subtypes in breast cancer. Sixty patients with breast cancer were enrolled and the MR images were pre-processed for noise reduction, registration and segmentation. Sixty-five dimensional imaging features including statistical characteristics, morphology, texture and dynamic enhancement in breast lesion and background regions were semiautomatically extracted. The associations between imaging features and molecular subtypes were assessed by using statistical analyses, including univariate logistic regression and multivariate logistic regression. The results of multivariate regression showed that imaging features are significantly associated with molecular subtypes of Luminal A (p=0.00473), HER2-enriched (p=0.00277) and Basal like (p=0.0117), respectively. The results indicated that three molecular subtypes are correlated with DCE-MRI features in breast cancer. Specifically, patients with a higher level of compactness or lower level of skewness in breast lesion are more likely to be Luminal A subtype. Besides, the higher value of the dynamic enhancement at T1 time in normal side reflect higher possibility of HER2-enriched subtype in breast cancer.

Abstract:Background and Objective. Breast cancer is a common malignant tumor that seriously threatens the health of women in my country and even around the world. The proliferation marker Ki-67 has been utilized to distinguish luminal B from luminal A tumors and is a reliable indicator of more aggressive breast cancer growth. If a reliable prediction method for breast cancer patients to avoid invasive damage can be found to predict Ki-67 before pathological examination, it will be very beneficial for doctors to formulate later treatment plans and provide more useful treatment options. Methodology. This paper proposes a tumor segmentation and prediction framework based on the combination of improved attention U-Net and SVM. The framework first improves on attention U-Net by introducing coefficients for learning multidimensional attention. Make the attention mechanism more aware of the main situation in the segmentation process. At the same time, the segmented breast MRI results and corresponding labels were input into the SVM classifier to accurately predict the expression of Ki-67. Results. The DSC, PPV, and sensitivity of our combined model are 0.94, 0.93, and 0.94, respectively, with better segmentation performance. And we compare with the segmentation frameworks of other papers and find that our combined model can make accurate segmentation of breast tumors. Conclusion. Our method can adapt to the variability of breast tumors and segment breast tumors accurately and efficiently. In the future, it can be widely used in clinical practice, so as to help the clinic better formulate a reasonable diagnosis and treatment plan for breast cancer patients.
Breast Cancer Ki67 Expression Preoperative Discrimination by DCE-MRI Radiomics Features

Abstract:To investigate whether quantitative radiomics features extracted from dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) are associated with Ki67 expression of breast cancer. In this institutional review board approved retrospective study, we collected 377 cases Chinese women who were diagnosed with invasive breast cancer in 2015. This cohort included 53 low-Ki67 expression (Ki67 proliferation index less than 14%) and 324 cases with high-Ki67 expression (Ki67 proliferation index more than 14%). A binary-classification of low- vs. high- Ki67 expression was performed. A set of 52 quantitative radiomics features, including morphological, gray scale statistic, and texture features, were extracted from the segmented lesion area. Three most common machine learning classification methods, including Naive Bayes, k-Nearest Neighbor and support vector machine with Gaussian kernel, were employed for the classification and the least absolute shrink age and selection operator (LASSO) method was used to select most predictive features set for the classifiers. Classification performance was evaluated by the area under receiver operating characteristic curve (AUC), accuracy, sensitivity and specificity. The model that used Naive Bayes classification method achieved the best performance than the other two methods, yielding 0.773 AUC value, 0.757 accuracy, 0.777 sensitivity and 0.769 specificity. Our study showed that quantitative radiomics imaging features of breast tumor extracted from DCE-MRI are associated with breast cancer Ki67 expression. Future larger studies are needed in order to further evaluate the findings.
