Abstract:Objective To investigate the incidence of intercalary fragment in adult ankle fractures and analyze the morphological characteristics of intercalary fragment as well as its relationship with the type of posterior malleolus fractures.Methods Data of 247 cases with posterior malleolus fractures from 369 cases of adult ankle fractures who were treated from January 2014 to January 2016 were retrospectively analyzed.The pre-operative Ⅹ-ray and CT data of the group were analyzed,and the incidence of intercalary fragment was measured.The intercalary fragment was classified by position,size and shape of its morphological features.The posterior malleolus fracture was classified according to Bartonícek classification system.The relationship between type of posterior malleolar fracture and the intercalary fragment was studied.The incidence of malreduction was also studied using the criterion of more than 2 mm separation or step.Results One hundred and six cases had intercalary fragment,accounting for 42.91％ (106/247) of the posterior malleolus fracture,and 28.73％ (106/369) of all ankle fractures.There were 81 cases with intercalary fragment larger than 2 mm,accounting for 32.79％ (81/247) of the posterior malleolus fracture.There were 33 cases with intercalary fragment larger than 5 mm,accounting for 13.36％ (33/247) of the posterior malleolus fracture.The incidence of intercalary fragment occurred in the posterolateral side was 64.15％ (68/247).According to Bartonícek classification,the incidence of type Ⅰ,Ⅱ,Ⅲ,and Ⅳ posterior ankle fracture was 8.91％ (22/247),49.39％ (122/247),21.86％ (54/247),and 19.84％ (49/247),respectively.The incidence of intercalary fragment was 4.54％ (1/22) in type Ⅰ posterior ankle fractures,40.16％ (49/122) in type Ⅱ,70.37％ (38/54) in type Ⅲ,and 36.73％ (18/49) in type Ⅳ.The incidence in type Ⅲ posterior malleolus fracture was significantly higher than that of other types of posterior malleolus fracture.CT scan after operation was conducted in 43 cases in which 19 cases had their intercalary fragment malreducted,accounting for 44.19％(19/43).Conclusion Intercalary fragment has high incidence in the posterior malleolus fracture,and the highest incidence occurs in Bartoníeek Ⅲ type posterior malleolus fracture.The study helps to further understanding and treatment of the posterior malleolus fracture.

Abstract:Objectives The objective of this study was to evaluate the morphological characteristics of the posterior malleolus fragment (PMF) based on 3-D computed tomography scans, and evaluated the variability in different types of injuries (ankle fracture, spiral tibial shaft fracture and pilon fracture). Methods A retrospective analysis of 3-D computed tomography data of PMF from June 2011 to February 2012 was conducted in a level 1 trauma centre. The cross angle (α), fragment length ratio (FLR), fragment area ratio (FAR), sagittal angle ( θ ), and fragment height (FH) were measured as morphologic assessments in the Picture Archiving and Communication System (PACS). Results Based on the definition of the PMF, 144 cases were brought into this study. And the PMF was described with a mean α of 25.0°, a mean FAR of 17.66 %, a mean θ of 16.1° and a mean FH of 23.06 mm. Besides, this study showed a significant difference on FAR amongst the three injuries with the FAR was 30.31 % ( P < 0.05) in pilon fracture group. Respectively, although the mean θ (16.1°) indicated a vertical fracture pattern, yet no significant difference was shown amongst the three groups. Conclusions Although the PMFs appear to be highly variable, most of the PMFs were located on the posterolateral of the distal tibia, and showed features with vertical nature. The information obtained from this study will be helpful for fracture models in a future biomechanical study and for determining appropriate surgical approaches.
Morphological Analysis of Pilon Variant Posterior Malleolar Fracture and Three-Dimensional Finite Element Analysis of Injury Mechanism

Abstract:Objective To analyze the computed tomography (CT) morphological and injury mechanism of Pilon variant posterior malleolar fracture.Methods The CT images of the patients with posterior malleolar fracture were observed in our Department of Orthopedics.A total of 22 cases of Pilon variant posterior malleolar fracture were included in the study.The ratio of the fragment on the axial image,the maximum height of the fragment and the angle between the fracture line and the horizontal plane on the sagittal image were measured.A three-dimensional finite element model of a healthy adult male foot and ankle was developed using CT scan.The injury process of posterior malleolar fracture that may lead to Pilon variant posterior malleolar fracture was simulated and analyzed.Results When the1.5 times body weight vertical violence was applied on the ankle joint plantar flexion 30°,the stress was mainly concentrated on the posteromedial of the distal tibia and the maximum stress was 78.68 mPa;when the 2.5 times body weight vertical violence was applied,the maximum stress was 151.42 mPa.When the 1.5 times body weight vertical violence was applied on the ankle joint plantar flexion 40°,the stress wss mainly concentrated on the posterior side of the distal tibia and the maximum stress was 111.78 mPa;when the 2.5 times body weight vertical violence was applied,the maximum stress was 165.63 mPa.When the rotational force was applied,the stress was mainly concentrated on the lateral malleolus and the posterolateral of distal tibia.Conclusion The Pilon variant posterior malleolar fracture has a unique mechanism of injury,which is mainly related to the position of the ankle joint,and is caused by vertical violence combined with rotational violence when the ankle joint plantar flexion is about 30°.
Morphological Analysis and Classification of Posterior Malleolar Fractures Based on CT Scans.

Abstract:OBJECTIVES:The aim of the study was to propose a classification system of posterior malleolar fractures by fracture lines with the use of CT scans, including 3D CT reconstruction, which can better understand morphological characteristics, analyze the mechanism and guide the surgeon to choose the optimal approach and fixation.METHODS:Patients with OTA/AO type 44 fractures involving the posterior malleolus and preoperative CT scans were included. We retrospectively analyzed 128 consecutive patients with posterior malleolar fractures from January 2013 to December 2019 at our institution. CT data were loaded into Mimics software (V20.0, Materialize), in which 3D CT reconstruction, morphological analysis and data measurements were made.RESULTS:Based on the number of fracture lines in 128 consecutive patients, posterior malleolar fractures were classified into three types: type 1 with a single fracture line, type 2 with double fracture lines and type 3 with multiple fracture lines. According to the distribution of the fracture line, type 1 was divided into types 1A, 1B and 1C, and type 2 was divided into types 2A, 2B and 2C. The fracture line from the fibular notch to the posterior rim of the distal tibia was defined as type 1A, and the fracture line to the medial malleolus was defined as type 1B. Type 1C was a small fragment in the posterior rim of the distal tibia. Type 2A was regarded as type 1A with type 1C. It was considered type 2B because another fracture line started from the fracture line of type 1A and extended to the medial malleolus. In type 2C, we could see that the double fracture lines were all from the fibular notch to the posterior rim of the distal tibia and did not cross. Type 3 fractures were comminuted fractures with multiple fracture lines.CONCLUSION:The morphology of posterior malleolar fractures, involvement of the fibular notch, or the medial malleolus can be obviously assessed by our classification system. We found the relation of the injury mechanism between type 1 and type 2 by comparing the area of the fragment. We have indicated that each type of fracture corresponds to its associated injury mechanism and which surgical approach and fixation can be chosen.

Abstract:Intra-articular impacted fragment (IAIF) was considered as the articular surface fragment resulting from impact and compressive forces. Malreduction of IAIF in posterior malleolar fractures was associated with the cause of talus subluxation and long-term arthritis. The effect of IAIF on ankle pressure and stress has not been studied, and it's difficult to do this in cadaver bone. So we established IAIF defect finite element model in posterior malleolar fractures and explored the effect of IAIF defect in ankle joint. We also discussed the relation between IAIF defect and post-traumatic arthritis.
Comparison of Radiographs and CT Features Between Posterior Pilon Fracture and Posterior Malleolus Fracture: a Retrospective Cohort Study

Abstract:Objectives: This study explored the morphological differences between posterior Pilon fracture and posterior malleolus fracture from radiographs and CT to provide detail for diagnosis and treatment of them. Methods: Radiographs and CT imaging data of 174 patients with distal posterior tibial fractures who were treated from January 2013 to January 2019 were retrospectively analyzed. Based on the operation and imaging examination, the fractures were classified into posterior Pilon fractures and posterior malleolus fractures. Radiographic parameters including the width, height, depth, α angle, β angle, γ angle, fragment area ratio 1 (FAR1), δ angle and fragment area ratio 2 (FAR2) of ankle mortise were measured. Results: There were 96 posterior Pilon fractures (Type I: 30, Type II: 22 and Type III: 44) and 78 posterior malleolus fractures (Type I: 40 and Type II: 38). The ankle depth, α angle, γ angle, FAR1 and FAR2 of posterior Pilon fractures were larger than these of posterior malleolus fractures (p < 0.05). In addition, FAR1 and FAR2 of Type II and Type III posterior Pilon fractures were significantly larger than these of Type I (p < 0.05). FAR1 and FAR2 of Type I posterior malleolus fractures were significantly smaller than these of Type II (p < 0.05). Conclusion: Radiographs combined with CT analysis is an effective method to accurately distinguish morphological features between posterior Pilon fracture and posterior malleolus fracture. Advances in knowledge: Radiographs combined with CT distinguished the fracture of posterior malleolus and posterior Pilon rapidly and accurately, instead of operation.

Abstract:BACKGROUND:The purpose of the study is to investigatethe prevalence of posterior malleolus intercalary fragment (ICF) in adult ankle fractures; to analyze the morphological characteristics of ICF; and its mechanism.METHODS:Retrospectively, 247 cases of posterior malleolus fractures (PMF) were included in 369 cases of adult ankle fractures. The prevalence of ICF, relationship between type of ankle fracture, posterior malleolar fracture and the ICF was studied.RESULTS:Among 369 cases, the ICF was found in 43% of the PMF and 29% of the hospitalized unstable ankle fractures. Position of ICF is posterolateral in 68 (64%), mid posterior in 20 (19%) and posteromedial in 18 (17%) cases. The prevalence of ICF was 70% in Bartoníček classification type III fractures, which was significantly higher than type I, II and IV.CONCLUSIONS:The highest incidence occurs in Bartoníček III type fracture. The study helps in further understanding and treatment of the PMF.LEVEL OF EVIDENCE:Level IV, case series.

Abstract:Background: The morphology and classification of posterior malleolus (PM) fractures remain controversial. An increasing number of studies have found that merely focusing on the fragment size does not lead to a satisfactory prognosis. This study aimed to demonstrate the fracture line and comminution zones of PM fractures using computed tomography (CT), in order to provide insights into the injury mechanism of PM fractures. Methods: We retrospectively reviewed the data of 95 patients with PM fractures between 2013 and 2018 at a level 1 trauma center. The CT data of the PM of the patients were reconstructed using software. Images of all patients were superimposed together, and drawn as a heat map of the fragments and line distributions of PM fractures. Results: Our study included 66 type I, 19 type II, and 10 type III PM fractures according to the classification of Haraguchi. In the single-fragment fracture group, the fracture lines were mainly concentrated in the posterolateral tibial tubercle area (Volkmann’s tubercle) and a larger tubercle area involving the tibialis posterior groove. In the multifragment fracture group, there were 10 (43.5%) patients with a 2-fragment pattern and 13 (56.5%) patients with a compressive-fragment pattern. Conclusion: According to the fracture map and previous studies on the syndesmosis, our study provides a different understanding of the pathomechanisms of ankle injuries compared with previous classifications of PM fractures. Abstract:IntroductionThe aim of this study was to analyze the pathoanatomy of the posterior fragment on the basis of a comprehensive CT examination, including 3D reconstructions, in a large patient cohort.Materials and methodsOne hundred and forty one consecutive individuals with an ankle fracture or fracture-dislocation of types Weber B or Weber C and evidence of a posterior tibial fragment in standard radiographs were included in the study. The mean patient age was 49 years (range 19–83 years). The exclusion criteria were patients below 18 years of age, inability to provide written consent, fractures of the tibial pilon, posttraumatic arthritis and pre-existing deformities. In all patients, post-injury radiographs were obtained in anteroposterior, mortise and lateral views. All patients underwent CT scanning in transverse, sagittal and frontal planes. 3D CT reconstruction was performed in 91 patients.ResultsWe were able to classify 137 cases into one of the following four types with constant pathoanatomic features: type 1: extraincisural fragment with an intact fibular notch, type 2: posterolateral fragment extending into the fibular notch, type 3: posteromedial two-part fragment involving the medial malleolus, type 4: large posterolateral triangular fragment. In the 4 cases it was not possible to classify the type of the posterior tibial fragment. These were collectively termed type 5 (irregular, osteoporotic fragments).ConclusionIt is impossible to assess the shape and size of the posterior malleolar fragment, involvement of the fibular notch, or the medial malleolus, on the basis of plain radiographs. The system that we propose for classification of fractures of the posterior malleolus is based on CT examination and takes into account the size, shape and location of the fragment, stability of the tibio-talar joint and the integrity of the fibular notch. It may be a useful indication for surgery and defining the most useful approach to these injuries.

[Fracture of the posterior malleolus : A paradigm shift]

Abstract:Ankle fractures with involvement of the posterior malleolus have a poor prognosis. The traditional concept of addressing the posterior fragment as the final step in the surgical reconstruction depending on the fragment size manifest in the lateral X‑ray, does not do justice to the anatomic and biomechanical relevance of the posterior malleolus. Intra-articular step-offs and fragment displacement ≥ 2 mm, impacted intercalary fragments, involvement of the fibular notch and instability of the syndesmosis, represent parameters relevant for the surgical reconstruction and the functional and radiomorphological outcome independent of fragment size. A valid assessment of these parameters can only be achieved via computed tomography (CT) including multiplanar 2D and 3D reconstruction. This is the foundation for the classification of posterior malleolar fractures according to Bartoníček et al., it forms the basis of the preoperative analysis of the fracture components and represents a decision-making tool for the indications for surgery. The individual fracture pattern guides the selection of the suitable approach or a combination of approaches and the surgical strategy. Making use of the posterior approaches enables the surgeon to have a direct view of the fracture components of the posterior malleolus as a precondition for an anatomic reduction. This central surgical step is frequently done as the first measure of the surgical reconstruction of the ankle joint. Furthermore, direct reduction and posterior stabilization is more accurate and stable than indirect reduction from an anterior approach with indirect fixation using lag screws. An additional temporary transfixation of the tibiofibular syndesmosis is often dispensable.

Abstract:The treatment of posterior malleolar fractures is changing rapidly, and the evidence base is still catching up. This study aimed to assess the mid-term prognosis of posterior malleolar fractures based on different morphological types and provides evidence for the treatment of posterior malleolar fractures. We retrospectively analyzed the data of inpatients with posterior malleolar fractures from 1 January 2012 to 31 December 2019 at one high-volume tertiary trauma center. Fracture morphology was classified into small-shell fragment, single-fragment (small-fragment and large-fragment) and multifragment (double-fragment and compressive-fragment) by computed tomography according to our previous study. All patients were followed up at an average of 5.06 (range, 2.21–8.70) years. The Olerud-Molander Ankle Score (OMAS), EuroQol-5 Dimensions (EQ-5D) and American Orthopedic Foot and Ankle Society (AOFAS) score were recorded. Seventy-nine patients were included, and 7 patients were classified into the small-shell group, 52 patients into the single-fragment group and 20 patients into the multifragment group. Of all the patients, the average OMAS, EQ-5D and AOFAS scores were 85.9, 82.8 and 92.5, respectively. In the single-fragment group, patients who underwent surgical fixation in the posterior malleolus had significantly better scores (P = 0.037, 0.033 and 0.027). Among the patients with small fragments, the surgical fixation group also had higher OMAS (93.1 ± 7.5 vs. 83.5 ± 19.5, P = 0.042) and AOFAS scores (98.1 ± 3.1 vs. 91.0 ± 14.1, P = 0.028). The mean OMAS, EQ-5D and AOFAS scores were 85.5, 85.7 and 91.7, respectively, in patients with multiple fragments who underwent surgical fixation. This study shows that in fractures with a single fragment, surgical fixation of the posterior malleolar fragment led to a better prognosis in the midterm. All single fragments should be fixed regardless of size. Fixation of the posterior region in all single- and multi-fragments in posterior malleolar fractures led to satisfactory outcomes. A Retrospective Study of Posterior Malleolus Fractures

Abstract:PURPOSE:In this retrospective study, we evaluated the treatment effect of ankle joint fracture surgery involving the posterior malleolus, and discuss relevant factors influencing the occurrence of traumatic arthritis of the ankle joint.METHODS:A total of 102 cases of ankle joint fractures involving the posterior malleolus in five large-scale skeletal trauma centres in China, from January 2000 to July 2009, were retrospectively analysed in terms of surgical treatment and complete follow-up. Ankle joint mobility, posterior malleolus fragment size, articular surface evenness, Ankle-Hindfoot Scale of the American Orthopedic Foot and Ankle Society (AOFAS) score, and imaging scale score for arthritis were recorded. The degree of fracture pain during rest, active movement, and weight-bearing walking, and satisfaction with treatment were evaluated using a visual analogue scale (VAS).RESULTS:The average AOFAS score was 95.9, excellence rate was 92.2 %, and average VAS scores for degree of fracture pain during rest, active movement, and weight-bearing walking were 0.15, 0.31, and 0.68, respectively. Thirty-six cases showed arthritic manifestations. Ankle joint mobility along all directions on the injured side was lower than that on the unaffected side. There was no obvious difference in treatment effect between the fixed and unfixed posterior malleolus fragment groups for all and for fragment size of < 25 %; between fixing the posterior malleolus fragment from front to back or from back to front; or between elderly patients (≥ 60 years old) and young patients (< 60 years old). There was a distinct difference in the treatment effect between articular surface evenness and unevenness for all and for fragment size of ≥ 25 %.CONCLUSIONS:For all 102 cases of ankle joint fracture involving the posterior malleolus, the treatment effect was satisfactory. Restoration of an even articular surface, especially when fragment size ≥ 25 %, should be attempted during treatment.
Effect of the Posterior Malleolar Fracture on the Loading Area of the Ankle

Abstract:Objective To understand the effect of the posterior malleolar fracture on the loading area and image of the loading contact area in the ankle so as to determine the operative indication. Methods Seven lower legs from fresh cadaver were loaded and their loading contact areas in the intact ankle, and respectively at one-fourth, one-third and one-half of the total articular surface removed were determined by pressure sensitive films inserted into the gaps of the joints. The characteristics of the image of the film coloured were analysed. Results At the intact ankle the loading area was (3.90.30) cm2 calculated by the computer; at one-fourth articular surface removed, it was (3.420.38) cm2. There were significant differences (P0.01) between the data of the two groups. With increasing size of the fragment of the posterior malleolus removed, the loaded contact area of the joint surface was significantly diminished. The two wings of the butterfly in the image of the pressure sensitive film were gradually joined together. Between two wings, the hollow was becoming shallow and the joint site was widened. Conclusions It is necessary that the internal fixation be performed to recover the intact loading area of the ankle while the fracture fragment reaches at one-fourth articular surface. The image of the pressure sensitive film showes that the normal flow of the synovial fluid in the middle of the loading area and the nutrition of the cartilage from it are prevented with the increased size of the posterior malleolar fragment. This can be one of the etiologies that cause the early osteoarthritis.
A Retrospective Study on the Morphology of Posterior Malleolar Fractures Based on a CT Scan: Whether We Ignore the Importance of Fracture Height

Abstract:Objective. The aim of this study was to investigate the respective correlation between the height (H) of a posterior malleolar fracture (PMF) and the involved area (S) of an articular surface and the presence of "die-punch."Methods. Patients with closed posterior malleolar fractures admitted to our hospital from January 2015 to December 2017 were selected, with complete X-ray and 3D reconstruction CT imaging data. The gender, age, injured side, and surgical fixation methods of the patients were recorded. A preoperative ankle CT scan was performed, and the images were viewed through the PACS (Picture Archiving and Communication Systems). Simultaneously, the involved joint surface area (S) by the posterior malleolar fracture was measured, as well as the proportion of the fracture area to the total ankle joint area. On the sagittal reconstruction CT images, the height (H) of the posterior malleolar fracture was measured to compare the correlation between the height of the fracture and the area of the fracture, as well as the area ratio. Besides, according to the presence or absence of "die-punch," patients were divided into two groups: A and B. And each group was further divided into three subgroups according to age (16-39 years old, 40-59 years old, and >= 60 years old). The statistical differences in the height of fracture between the subgroups were compared.Results. A total of 48 patients, aged 16-82 years, with an average age of 48.9 years, were included in this study, including 13 males and 35 females. There were 20 cases of left ankle injury and 28 cases of right ankle injury. The average height of the posterior malleolar fractures was 18.19 mm, the average area of the fracture was 202.28 mm(2), and the average ratio of the fracture area to the total articular surface area was 17.84%. Besides, die-punch was seen in 27 cases and not in 21 cases. The average height of fractures was21.33 +/- 5.38 mmin group A1,14.38 +/- 9.01 mmin group B1,18.30 +/- 7.95 mmin group A2,14.48 +/- 5.37 mmin group B2,26.26 +/- 6.73 mmin group A3, and12.77 +/- 3.07 mmin group B3.Conclusion. The height (H) of the posterior malleolar fractures is positively correlated with the fracture area (S) and the fracture area ratio (FAR). The posterior malleolar fractures with "die-punch" tend to have a greater average height than that without "die-punch." In clinical work, orthopedic surgeons should not only pay attention to the size of the posterior malleolus fracture but also value its height, which hopefully could provide insight into the treatment and prognosis of PMF patients.
Advancing treatment strategies for posterior malleolar malunion: The ankle dislocation method

Abstract:Background: This study aimed to assess the radiological and clinical outcomes of treatment using the ankle dislocation method for posterior malleolar malunion. Method: Thirty-one patients with posterior malleolar malunion who underwent treatment using the ankle dislocation method from May 2015 to October 2021 were retrospectively analyzed. Key outcome measures were radiographic parameters (articular step-off, tibiofibular clear space, fibular length, tibial lateral surface angle, and ankle osteoarthritis), clinical scores (American Orthopaedic Foot and Ankle Society ankle-hindfoot scale and Visual Analogue Scale), and patient satisfaction rate. Result: Preoperative computed tomography revealed that Bartoní ček types 3 and 4 accounted for 64.5 % (n = 20) of total cases. Most posterior malleolar malunions were accompanied by depressed intercalary fragments (61.2 % [n = 19]). At the final follow-up, radiographic parameters and clinical scores showed significant improvements postoperatively (P < 0.05), with a high patient satisfaction rate of 77.4 %. Subgroup analysis revealed that the posterior malleolar fracture morphology significantly affected postoperative pain, particularly in more complex fractures (P < 0.001). Conclusion: The ankle dislocation method effectively exposes the distal tibial articular surface and facilitates the anatomical restoration of joint congruity under direct vision. This approach substantially improves the clinical and imaging outcomes in patients with complex posterior malleolar malunion. Levels of evidence: Level IV, retrospective case series.

Abstract:Several operative approaches are available at present for the exposure and fixation of distal fibular fractures combined with posterior malleolus fractures. The present study was designed to study the anatomical characteristics of the distal fibula and to thereby evaluate the advantages and limitations of various operative approaches, as well as their indications for specific conditions. Ten leg specimens from below the knee joint were dissected using posterior, lateral and posterolateral approaches to the fibula. The adjacent vulnerable structures, including nerves, blood vessels, tendons and ligaments, were carefully examined and their distances from the posterior malleolus were recorded. The distance was 7.2±4.1 mm between the sural nerve and the posterior section of the fibula, 79.2±23.5 mm between the lateral malleolus tip and the point where the shape changes in the lower fibula and 66.4±17.4 mm between the lateral malleolus and the jointed tendon of the peroneal and flexor hallux longus muscles. The widest anteroposterior diameter of the distal fibula was 27.3±3.5 mm. Various approaches have certain advantages and limitations when these anatomical factors are taken into account. The choice should be based on the height of the fibular fracture line, the type of posterior malleolus fracture, the effect of the fracture on the stability of the ankle joint and the materials used for internal fixation.
Incidence and surgical fixation-related factors of posterior malleolus fracture in treatment of ankle fractures

Abstract:Objective To investigate the incidence and surgical fixation-related factors of posterior malleolus fracture in the treatment of ankle fractures.Methods A retrospective study was conducted of the 703 inpatients with ankle fracture at Department of Foot and Ankle Surgery,Beijing Jishuitan Hospital from June 1,2017 to May 31,2018.Of them,464 suffered posterior malleolus fracture.The incidence and surgical fixation rate of posterior malleolar fracture were calculated.Of the patients with posterior malleolus fracture,the gender,age,injury energy,Lauge-Hanse classification,Bartonícek classification,talar backward subluxation,fragment area ratio (FAR) of posterior malleolus fracture on X-ray film and CT cross section,fragmental dislocation,and Die-punch fragment were analyzed to screen out the risk factors related to surgical internal fixation for posterior malleolus fracture.Results In this study,the incidence of posterior malleolus fracture was 66.00％ (464/703) and the surgical fixation rate 43.97％ (204/464).The gender,age,injury energy,Lauge-Hanse classification,direction of talar subluxation and displacement of Die-punch fragments were not the risk factors related to the surgical internal fixation for posterior malleolus fracture (P ＞ 0.05) while the Bartonícek classification,talar backward subluxation,FAR of posterior malleolus fracture on X-ray film and CT cross section,lateral fragmental displacement and Die-punch fragment were the risk factors related to the surgical internal fixation for posterior malleolus fracture (P ＜ 0.05).Logistic regression analysis indicated that the talar backward subluxation (OR =5.580,95％ CI:1.623 ～ 19.181,P =0.006) and the FAR ≥ 15％ on CT cross section (OR =9.103,95％ CI:3.342 ～ 24.800,P =0.000) were independent risk factors for surgical internal fixation of posterior malleolus fracture.Conclusions The incidence and surgical fixation rate of posterior malleolar fracture are very high.The talar backward subluxation and the fragment area ratio ≥ 15％ on CT cross-section are independent risk factors for surgical internal fixation of posterior malleolus fracture.

Abstract:Objective To explore the morphological characteristics of posterior Pilon fractures based on CT.Methods The cases diagnosed as posterior Pilon fractures,which were included in this retrospective study,were based on the “double contour sign”and “Step-off sign”shown on X-rays.On the transverse CT view,the cross angleαbetween the posterolateral fragments and the bimalleolar line and the angleβbetween the posteromedial fragments and the bimalleolar line,fragment length ratio (FLR)of the anteroposterior length of either posterior fragment to that of the anterior plafond,and fragment area ratio (FAR)of the area of either posterior fragment to that of the anterior plafond were measured.On the sagittal CT view,the cross angleγandθbetween the fracture lines and the tibial axis,and the maximal fragment heights (FH)of the posterior fragments were measured in the picture archiving and communication system.Results Based on the diagnosis of posterior Pilon fractures,36 cases were brought into this study.Both posterolateral and posteromedial fragments were found in these cases,among which 28 had talar subluxation or dislocation,and 26 had fracture lines extending to the medial mallelous.The FLR of posterolateral and posteromedial fragments were 29.8%±4.6% and 24.3%±5.2%,respectively;FH of them were 24.6±6.2 mm and 21.9 ±8.4 mm,respectively.Both parameters were found significantly different between the posterolateral and posteromedial fragments.But no significant difference was observed in the fragment area ratio between the two fragments.There were significant differences between angleαandβas well as between angle γandθ(P<0.01 ).The statistics indicated that the direction of major fracture lines of posteromedial and posterolateral on the axial plane was different,and the sagittal fracture lines of the posteromedial fragments were more vertical than that of the posterolateral fragments.Conclusion Posterior Pilon fractures diagnosed according to the X-ray criteria had a high rate of talar subluxation. The posterolateral and posteromedial fragments were different in the fragment length,height and fracture lines.
Retrospective cohort study of operatively treated ankle fractures involving the posterior malleolus

Abstract:Background: Direct fixation of the posterior malleolus through a posterolateral approach is increasing in popularity. However, data is limited, and no studies have stratified results by posterior malleolus size. Methods: A retrospective cohort study of patients with posterior malleolus (PM) fractures undergoing internal fixation over an 18-month period was conducted. Radiographic and patient reported outcomes measures (PROMs) were assessed. 75 patients were included, and 31 returned Manchester-Oxford Foot and Ankle (MOXFQ), EQ-5D-3 L, 5-point Likert satisfaction questionnaires. Mean age was 56.2 years, 68% were female, mean ASA was 1.72, 6.7% were diabetic, and average surgical time was 75.7 min (±40.9). Results: Overall, the MOXFQ summary score was 26.9 (±25.8), with the pain domain showing the most severe score 32.4 (±24.0). The EQ-5D VAS index was 0.75 (±0.24). Larger PM size was associated with more anatomical reduction (58.8%, n = 20 of <10% vs. 90.9%, n = 10 of >30%) and larger sized fragments were more likely to be fixed posteriorly. Best PROMs were observed in PM fragments <10%, and the worst in fragments 10-20%. A tendency towards more secondary surgery was observed with posterior subluxation 21.9% (n = 7) vs. 7.0% (n = 3) (p = 0.06). Conclusions: Patients with fragments >10% have an intra-articular injury. We recommend direct anatomic reduction and rigid internal fixation. Level of evidence: Level III, retrospective comparative study.
Establishment of a finite element model of supination-external rotation ankle joint injury and its mechanical analysis

Abstract:By establishing a three-dimensional finite element model of ankle injury arising from supination and external rotation, the stress characteristics of the posterior malleolar surface can be obtained, and analysis of the corresponding stress on the lateral ankle can be conducted. Thin-layer computed tomography images of normal ankle joint in the supination and external rotation nonweight-bearing states was selected, to construct a three-dimensional data model of each ankle joint. A load was applied to examine different ankle joint stress values and pressure distributions on the surface of the posterior ankle joint. A 600 N vertical compressive and 10 Nm internal rotation load was applied in Stage III (removing the anterior tibiofibular ligament and the posterior tibiofibular ligament) of SER (supination-external rotation). When the lateral malleolar was intact, the maximum stress (132.7 MPa) was located at the point of attachment of the posterior tibiofibular ligament to the fibula, and the maximum pressure of the posterior malleolar surface was lower than 4.505 MPa. When a lateral malleolar fracture was present, the maximum stress (82.72 MPa) was located on the fibular fracture surface, and the maximum pressure of the posterior malleolar surface was 8.022 MPa. This study shows that reconstruction of the lateral malleolus in supination-external rotation ankle injuries significantly affects the stress distribution at the posterior malleolar joint surface. Through this reconstruction, the pressure distribution of the posterior malleolar joint surface can be significantly reduced.
