Abstract:In a World Bank study that examined the global burden of diseases in working age adults, bipolar disorder (BP) was ranked sixth, with unipolar depression ranked first. However, it is notable that at least 20% of early onset unipolar cases will also ‘convert’ to BP over 5–8 years – further increasing the morbidity and mortality associated with BP. Taken together, these findings highlight that BP is more debilitating and costly than most major medical disorders and also more burdensome to society than schizophrenia (ranked eighth). Part of the explanation of the latter is that, although BP and schizophrenia have similar peak ages of onset, the overall prevalence of BP spectrum disorders is greater than that of schizophrenia. Furthermore, many of the disabilities associated with BP represent a gradual accrual of negative consequences of comorbidities and secondary problems rather than representing an integral part of an underlying neurodevelopmental process (that may have impaired performance even prior to illness onset in psychosis). As such, BP is regarded as the mental disorder associated with the greatest loss of ‘human capital’ – an indicator of the gap between the individual’s potential capacity for professional and personal achievement (based on their premorbid adjustment) and their actual postonset level of functioning. Not surprisingly, such data encouraged exploration of the role of early intervention and treatment in BP. Learning from the evidence assembled to support the rationale for early intervention in psychosis, researchers in mood disorders began by examining pathways to treatment for individuals with early onset BP and delineating its course and outcome. This work highlighted the long duration of untreated illness in BP (median DUI 6–10 years) with adolescent onset cases especially prone to an unacceptably protracted DUI. Indeed, delayed diagnosis and late introduction of appropriate treatment for early onset BP appeared to be a universal unmet need, whatever the country, the mental healthcare system or its accessibility. (N. Drancourt et al., unpubl. data) Observational studies also provided a stark warning of the consequences of the failure to intervene early. For example, it is consistently reported that, compared to adult onset cases, adolescent onset BP is characterized by lower rates of symptomatic and functional recovery with evidence of greater severity of manic and depressive episodes, more frequent relapses, fewer days of euthymia and higher prevalence of rapid cycling. In addition, like their adult onset counterparts, adolescents with BP demonstrate high rates of medical comorbidities even when compared to adolescents with other mental disorders (36% vs. 8%). Very importantly, suicidal behaviour and accidents reportedly occur more frequently during the early phase of BP, whilst substance misuse is more often a secondary phenomenon in adolescent BP (BP occurring first in 55–83% cases) than in adult onset BP. Such aspects represent clear additional targets for the ‘primary prevention of secondary problems’. Raising the awareness of mental health professionals to the need for more timely and accurate diagnosis of BP has been an important first step in promoting the philosophy of early intervention in mood disorders. However, this cannot be an isolated strategy, divorced from a carefully constructed and coherent plan of how to manage detected cases. Crucially, as in psychosis, the treatment of the mood disorder is only one component of the package required for individuals with early onset BP. Any proposed interventions must be modified to be rendered ‘age-appropriate’, provide support to the young person (and their family) as they try to adjust to the implications of the diagnosis and address the unique developmental needs of the individual. The paper by Macneil et al. is therefore pertinent and important. Building on the lessons learned from working with young people with psychosis, Macneil and colleagues explore the range and complexity of factors that must be considered when devising interventions that meet the emotional, cognitive, psychological and social needs of individuals whose personal development and life goals have been temporarily or permanently derailed by the onset of BP. Three components appear to provide the essential foundations on which the therapeutic interventions are superimposed-stable engagement, shared decision-making and personalized care.
